Amplitude-integrated electroencephalogram (aEEG): A bedside window into newborn brain function in the NICU.

Amplitude-integrated electroencephalography (aEEG) remains an important tool for bedside neonatal brain monitoring. The rapid application of aEEG offers critical practical advantages for bedside clinicians assessing infants at risk for brain injury. aEEG has a distinct clinical niche in evaluations for hypoxic-ischemic encephalopathy (HIE) and as a first-line tool for seizure detection, alongside conventional video electroencephalogram (cEEG). Ultimately, the integration of aEEG with a broader multimodal neuromonitoring framework enhances neurodevelopmental prognostication and facilitates indi

Endometrial ablation plus levonorgestrel-releasing intrauterine system versus endometrial ablation alone in women with heavy menstrual bleeding in The Netherlands (MIRA2): a multicentre, open-label, randomised, controlled, superiority trial.

BACKGROUND: Endometrial ablation is an established and effective treatment for women with heavy menstrual bleeding, but many patients require further treatment, with approximately 10% ultimately undergoing a hysterectomy within 2 years. The insertion of a levonorgestrel-releasing intrauterine system (LNG-IUS) immediately after endometrial ablation might inactivate residual untreated endometrium or prevent its regeneration, thereby preventing recurrent symptoms necessitating a hysterectomy. The aim of the study was to assess whether LNG-IUS insertion immediately after endometrial ablation in wo

Effect of Adjunctive Mifepristone Pre-treatment on Operative Efficiency and Postoperative Recovery in Cesarean Scar Pregnancy Undergoing Curettage After Uterine Artery Embolization.

AIM: This study aims to evaluate whether short-course mifepristone pre-treatment improves procedural efficiency and early recovery after uterine artery embolization (UAE), followed by dilation and curettage (D&C) for cesarean scar pregnancy. METHODS: This retrospective study recruited 108 patients with cesarean scar pregnancy who underwent dilation and D&C between January 2020 and July 2023. Patients were divided into 2 groups according to the pre-treatment approach. The control group (n = 65) received pre-treatment with UAE alone, while the study group (n = 43) received pre-treatment with mi

[A delayed threat: S. aureus sepsis 4 years post-IUD insertion in a healthy young woman].

BACKGROUND: Intrauterine devices (IUDs) are highly effective long-acting reversible contraceptives with an established safety profile. Infectious complications are rare and usually occur shortly after insertion, typically involving polymicrobial pathogens. Staphylococcus aureus is an uncommon pathogen in this context, but its biofilm-forming ability may contribute to delayed and severe presentations. OBJECTIVES: A healthy 25-year-old primiparous woman developed septic shock four years after the insertion of a levonorgestrel-releasing intrauterine system (Mirena®). She was admitted to the inte

Case Report: Distal ureteral obstruction decades after presumed intrauterine device removal in a woman with uterus didelphys.

BACKGROUND: An ectopic intrauterine device (IUD) can rarely cause ureteral obstruction and hydroureteronephrosis. Diagnosis may be delayed when a prior removal procedure is believed to have been complete. CASE PRESENTATION: A 48-year-old woman presented with a 1-year history of right flank pain and urinary frequency after repeated treatment for presumed urinary tract infection and possible genitourinary tuberculosis without sustained improvement. More than 20 years earlier, an IUD had been inserted before uterus didelphys was recognized. Two to three years later, an unintended pregnancy led t

Renin-Aldosterone Activation and Enhanced Urinary Potassium Excretion in Asphyxiated Newborns Treated with Therapeutic Hypothermia: A Prospective Study.

Hypoxic-ischemic encephalopathy (HIE) is a critical neonatal condition for which therapeutic hypothermia (TH) remains the cornerstone of neuroprotective intervention. Clinical observations and prior studies note that electrolyte imbalances-particularly hyponatremia and hypokalemia-frequently emerge during TH. Therefore, this study explores how activation of the renin-angiotensin-aldosterone system (RAAS) may contribute to sodium (Na) and potassium (K) disturbances in asphyxiated neonates receiving TH. In this prospective study, 38 newborns with HIE treated with TH and 21 healthy term newborns

Neuroprotective Effect of Intranasal Breast Milk-Derived Exosomes in a Neonatal Rat Model of Periventricular Leukomalacia.

BACKGROUND: Periventricular leukomalacia (PVL) is a major cause of neurodevelopmental impairment in premature infants and is characterized by inflammation-associated white matter injury. Breast milk-derived exosomes have regenerative and immunomodulatory properties. METHODS: Experimental PVL was induced in neonatal Sprague-Dawley rats by maternal lipopolysaccharide administration. Animals were assigned to control (n=10), PVL (n=9), PVL + saline (n=10), and PVL + exosome (n=9) groups. Breast milk-derived exosomes were isolated, characterized according to MISEV2018 criteria, and administered in

Comparison of Long-Term Outcomes of Combined Hypothermia and Whole Body Cooling Therapy in Newborns with Hypoxic-Ischemic Encephalopathy.

Therapeutic hypothermia is widely used as a neuroprotective intervention in infants with hypoxic-ischemic encephalopathy (HIE); however, limited data exist regarding the long-term neurodevelopmental outcomes of different cooling strategies. The aim of this study is to compare the long-term neurodevelopmental and cognitive outcomes of combined hypothermia (CH) and whole-body cooling (WBC) treatments in children diagnosed with HIE with each other and with a healthy control group without an HIE diagnosis. This retrospective study included a total of 50 children diagnosed with HIE and treated with

Human chorionic gonadotropin levels of cesarean scar pregnancy compared with those of other ectopic pregnancies: a retrospective cohort study.

BACKGROUND: A cesarean scar pregnancy (CSP) is an abnormal implantation of an embryo within a previous cesarean section scar and considered a relatively rare form of ectopic pregnancy. We aimed to clarify the factors associated with CSP and evaluate whether differences in serum human chorionic gonadotropin (hCG) levels according to implantation site could contribute to early detection of CSP. METHODS: We investigated 27,528 freeze-thawed embryo transfer cycles performed at our hospital between January 2016 and March 2025. RESULTS: Forty-four patients were diagnosed with ectopic pregnancy: 26

Unclassified COL6A1 Variant in a Child With Congenital Hypotonia: A Case Report.

We report a 3-year-old male with congenital hypotonia, proximal contractures, and distal hyperlaxity. Exome sequencing identified a novel homozygous COL6A1 variant (c.2821C>T), classified as a variant of uncertain significance. The distinct phenotype strongly supports Ullrich congenital muscular dystrophy, expanding the gene's mutational spectrum in consanguineous populations.

Case Report: Treatment of subthreshold bipolar disorder in pregnancy: a report on the effectiveness of the Zamar Protocol® in three treatment-resistant depression cases.

INTRODUCTION: Subthreshold bipolar disorder (SBD), also referred to as unspecified bipolar disorder, is frequently encountered in patients with treatment-resistant depression. It presents with depressive and subthreshold manic and mixed symptoms. Sustained remission with full functional recovery is the aim of treatment in mood disorders as residual symptoms may cause disability, while relapse may be associated with increased morbidity or mortality. To date, evidence for achieving sustained remission in SBD is absent, with no studies reporting sustained remission outcomes. The only available ev

Effective combined immunotherapy for severe hemolytic disease of the fetus.

BACKGROUND: Maternal anti-Kell and anti-Rhesus D-alloimmunization cause severe hemolytic disease of the fetus and newborn (HDF) by crossing the placenta, leading to anemia and hydrops fetalis. Early-onset HDF from anti-K and anti-RhD alloimmunization often requires multiple, early intrauterine transfusions (IUTs), with higher complication rates and decreased sensitivity after the first intervention. Mortality remains high. CASE REPORT: We report the successful treatment of three pregnant women with high-titer red cell alloantibodies-two with anti-K and one with anti-RhD-during the first trime

MRI and proton MR spectroscopy in neonatal hypoxic-ischemic encephalopathy: a comprehensive imaging review.

Neonatal hypoxic-ischemic encephalopathy (HIE) remains a leading cause of neonatal mortality and long-term neurodevelopmental disability. Although cranial ultrasound is widely used as a bedside screening tool, brain MRI has become central to diagnosis, injury pattern classification, timing estimation, and outcome prediction. Furthermore, proton MR spectroscopy (1H-MRS) provides complementary metabolic biomarkers that enhance prognostication. This review focuses on MRI and 1H-MRS in neonatal encephalopathy due to hypoxic-ischemic injury, addressing both term and preterm infants. The pathophysio

Annexin A6 in Eye-Lavage Extracellular Vesicles-a Candidate Biomarker for Sex-Specific Differences in Mild Hypoxic-Ischaemic Brain Damage-Findings from an Explorative Neonatal Mouse Model Study.

Extracellular vesicles (EVs) play important roles in cell communication, including via transfer of cargo proteins, and are promising biomarkers for pathobiological processes. Reliable biomarkers for mild neonatal hypoxia ischaemia (HI) are scarce. Wrongly determined HI severity often leads to adverse long-term neurological outcomes, with more detrimental effects in males. Therefore, identification of sex-specific markers for HI is of great interest. We recently reported the potential of EVs from eye lavage as novel non-invasive biomarkers of neonatal HI brain injury, differing in proteomic car

Enteral Feeding During Therapeutic Hypothermia for Hypoxic-Ischemic Encephalopathy in Neonates: A Multicentre Study from Italy and Belgium.

Background: Enteral feeding during therapeutic hypothermia (TH) for hypoxic-ischemic encephalopathy (HIE) remains heterogeneous across centres because of concerns regarding feeding intolerance and the risk of necrotizing enterocolitis (NEC). Evidence regarding optimal feeding initiation and advancement remains limited. Methods: We performed a secondary analysis of a multicentre retrospective cohort of infants with HIE treated with TH at two tertiary neonatal intensive care units in Italy and Belgium between 2011 and 2024. We included survivors with complete nutritional data, aware that this re

Radiofrequency Ablation for Selective Reduction in Complicated Monochorionic Twin Pregnancies: Pregnancy-Level and Neonatal Outcomes.

INTRODUCTION: Monochorionic twin pregnancies may be complicated by shared placental vascular anastomoses and hemodynamic interdependence between fetuses. Radiofrequency ablation (RFA) is a vaso-occlusive option for selective reduction when continuation of the affected fetus is expected to endanger the co-twin. METHODS: This retrospective cohort study included 39 monochorionic twin pregnancies treated with RFA at two tertiary centers between January 2017 and December 2025. Indications were grouped as TTTS/TAPS-related conditions, selective fetal growth restriction (sFGR)-related conditions, tw

Identification of Transcriptome-Derived Candidate Genes Associated with Post-Translational Modification Gene Sets in Neonatal Hypoxic-Ischemic Encephalopathy.

Neonatal hypoxic-ischemic encephalopathy (HIE) is a major cause of neurological injury in newborns. Post-translational modifications (PTMs) precisely regulate protein activity, localization, and interactions during cellular stress. However, transcriptomic changes in genes associated with PTM pathways in HIE remain incompletely characterized. Gene expression data from two mouse datasets (GSE23317 and GSE144456) were acquired from GEO. Candidate screening combined differential expression, WGCNA, and overlap with 21 PTM-related gene sets. Further analyses included PPI networks, expression compari

Integrated single-cell multi-omics analysis delineates neonatal immune dysregulation associated with gestational diabetes mellitus.

INTRODUCTION: Gestational diabetes mellitus (GDM) is a common pregnancy complication which could induce immune dysfunction. Although intrauterine hyperglycemia impairs fetal immune development, its cell-type specific effects on neonatal immune cells, and cell‑cell communication remain unclear. METHODS: In this pilot study, we performed integrated 5' single‑cell RNA sequencing and TCR repertoire analysis of cord blood mononuclear cells from two GDM and one type 2 diabetes mellitus (T2DM) mother-neonate pairs, with public healthy cord blood data (GSE157007) as controls. After quality control, 5
