Agreement Between Telemedicine and In-Person Examination for Neonatal Hypothermia Decisions.

OBJECTIVE: Therapeutic hypothermia within 6 hours of birth is critical for newborns with hypoxic-ischemic encephalopathy (HIE) but is often delayed by transfers to appropriate facilities. Telemedicine (TM) based hypothermia evaluation could expedite these assessments. There is limited evidence on the efficacy of TM compared with the in-person (IP) Sarnat examination. In this study, we aim to compare the TM with the IP examination in infants with suspected HIE. METHODS: Newborns were enrolled from March 2022 to December 2023. All infants underwent IP and TM assessments using the modified Sarna

IGF-1 ameliorates the blood brain barrier disruption induced by the neonatal hypoxia-ischemia.

Neonatal hypoxic-ischemic encephalopathy (HIE) disrupts the blood-brain barrier (BBB) and destroys nascent vessels, thereby amplifying parenchymal loss and chronic neurological disability. Restoring a competent cerebrovasculature is therefore a critical therapeutic goal. Insulin-like growth factor-1 (IGF-1) is already recognized as a neurotrophic factor whose circulating levels correlate with HIE severity and long-term outcome, yet its capacity to drive vascular repair after the insult remains incompletely defined. This study investigated the role of exogenous IGF-1 in BBB repair in neonatal m

Temporal and Spatial Gene Expression Dynamics in Neonatal HI Hippocampus with Focus on Arginase.

BACKGROUND: Hypoxic-ischemic (HI) brain injury triggers a dynamic, multi-phase response involving early microglial efferocytosis followed by extracellular matrix (ECM) deposition and scar formation. Arginase-1 (ARG1), a key enzyme in tissue repair, is implicated in both processes, yet its role in neonatal microglia remains poorly defined. We characterize ARG1-linked pathways in neonatal microglia, identifying distinct efferocytic and fibrotic phases post-HI. METHODS: HI was induced in P9 mice using the Vannucci model, and brains were collected at 24 h (D1) and 5 days (D5). Spatially resolved

Postnatal Azithromycin Is Neuroprotective and Anti-Inflammatory in a Piglet Model of Hypoxic-Ischemic Encephalopathy.

BACKGROUND: There is an urgent need for cytoprotective therapies for hypoxic-ischemic encephalopathy in low- and middle-income countries, where therapeutic hypothermia is not beneficial. Infection and inflammation are important risk factors for hypoxic-ischemic encephalopathy in sub-Saharan Africa, and immunomodulatory therapies might improve outcomes. Azithromycin is neuroprotective in rodent hypoxic-ischemic encephalopathy models but validation in large animal models is needed. Our aim was to assess safety and efficacy of intravenous azithromycin after inflammation-amplified hypoxia-ischemia

Early Detection of Cerebral Palsy Using Standardized Screening Assessments in Neonatal Hypoxic Ischemic Encephalopathy: A Pilot Case Series.

Prechtl's General Movement Assessment (GMA) and the Hammersmith Infant Neurological Exam (HINE) are recommended for early detection of cerebral palsy (CP) in high-risk infants. These tools are well validated in premature infants but less well studied in the high-risk term population. We sought to determine the added prognostic value of incorporating GMA and HINE assessment in term- and near-term infants with hypoxic ischemic encephalopathy (HIE) who underwent therapeutic hypothermia (TH). In this retrospective and prospective pilot case series of 20 neonates with HIE that were treated with TH,

Comparison of the prognosis of asphyxiated infants receiving hypothermia therapy with those receiving erythropoietin: a prospective cohort study.

BACKGROUND: Although Therapeutic Hypothermia in the last two decades promises hope in reducing the problems of asphyxiated babies in developed countries, it is still not available in most parts of the world, and the results available in developing countries are not promising. OBJECTIVE: In this study, we aimed to compare Hypoxic Ischemic Encephalopathy (HIE) infants receiving total hypothermia (TH) and erythropoietin (EPO). METHODS: In a prospective cohort study, we compared the developmental prognosis of infants with HIE receiving TH (control group, N = 26) with those receiving EPO (case grou

miR223, miR30e, and miR30a show associations with perinatal metabolic acidosis in neonates with hypoxic-ischemic encephalopathy.

OBJECTIVE: Hypoxic-Ischemic Encephalopathy (HIE) is a leading cause of neonatal death and long-term disabilities, leading researchers to explore novel candidate biomarkers to improve early diagnosis and guide treatment strategies. miR223, miR30a and miR30e have attracted attention as potential biomarkers for HIE due to its involvement in key cellular processes like inflammation, cell death, and neuroprotection. The rationale of this study was to examine longitudinal maternal and infant circulating miRNA blood levels from T0–T3 to 48 h of life in relation to perinatal metabolic acidosis and neo

Placental histopathology and early childhood neurodevelopment in the Environmental influences on Child Health Outcomes cohort.

BACKGROUND: Prenatal exposures influence childhood neurodevelopment. Placental histopathology has been associated with abnormal early childhood neurodevelopment, albeit often confounded by prematurity and/or fetal growth restriction. Most pregnant people, however, have term births, and some of these children have abnormal neurodevelopment despite the absence of adverse birth outcomes. Leveraging placental histopathology may help distinguish infants at a higher risk of subsequent neurodevelopmental impairment following a term birth. OBJECTIVE: To investigate the association of placental histop

Defining the 'Hypothermia Resistant' phenotype in neonatal HIE: persistent multi-organ dysfunction on day 3 is the hallmark of MRI injury.

OBJECTIVE: Therapeutic hypothermia (TH) reduces mortality in neonatal hypoxic-ischemic encephalopathy (HIE), yet nearly half of treated infants suffer adverse outcomes. This study aimed to define the "Hypothermia Resistance" phenotype by characterizing the 4-day trajectories of multi-organ dysfunction and identifying the physiological signals associated with severe MRI injury. STUDY DESIGN: This retrospective cohort study included 24 term neonates with moderate-to-severe HIE treated with whole-body hypothermia. Patients were stratified into "Responder" (n = 18) and "Resistant" (n = 6) phenoty

End-of-life circumstances and unanticipated deaths in a neonatal intensive care unit: a retrospective analysis.

BACKGROUND: Despite major advances in neonatal intensive care, mortality in neonatal intensive care units (NICUs) remains a persistent reality. Most deaths now occur after withholding or withdrawing life-sustaining therapies (WWLST), yet some remain sudden or unanticipated. Understanding how and under which circumstances infants die is essential to improving anticipatory communication, ethical consistency, and family-centered support in neonatal end-of-life (EOL) care. METHODS: We conducted a retrospective study including all infants who died in the tertiary NICU of Montpellier University Hos

Lactate receptor HCAR1 in neonatal hypoxic-ischemic seizures.

Hydroxycarboxylic acid receptor 1 (HCAR1) is a G-protein-coupled lactate receptor expressed in the brain and plays a role in neuronal excitability and repair after injury. Hypoxic-ischemic encephalopathy (HIE) is the most common cause of brain injury and seizures in term neonates. The goal of this study was to describe HCAR1 expression and function in the neonatal brain and understand its role in HIE-associated seizures. HCAR1 expression was measured using quantitative reverse transcriptase polymerase chain reaction in postnatal day (p)10-50 mice. Neuronal properties and spontaneous excitatory

TrkB Agonist Treatment Decreases Hippocampal Testosterone Contents in a Sex-Dependent Manner Following Neonatal Hypoxia and Ischemia.

Hypoxia-ischemia (HI)-related brain injury impacts millions of neonates worldwide. Male neonates are two times more susceptible to developing HI. We have previously reported that the administration of the neurotrophin receptor tyrosine kinase B (TrkB) agonist 7,8-dihydroxyflavone (DHF) following neonatal HI increases hippocampal TrkB phosphorylation and improves hippocampal-dependent learning and memory in early adult life only in females. We hypothesize that sex differences in HI outcomes are due to alterations in neonatal hippocampal steroid content, mainly the neural testosterone. At postna
