Treatment of eumycetoma and actinomycetoma.

Mycetoma is a chronic disease caused by aerobic actinomycetes and eumycetes which mainly affects the lower extremities. It predominates among farm workers in tropical, subtropical and adjacent zones. Clinically it is characterized by a firm swelling with abscesses and fistulae discharging pus that contains granules or grains of the causal agent. Their color, size, consistency and histopathology contribute to their identification. Cultures and metabolic studies determine the disease's etiology. Eumycete and actinomycete antigens can be used serologically to diagnose and predict prognosis of the

Concurrent chromoblastomycosis caused by Fonsecaea pedrosoi and actinomycetoma caused by Nocardia brasiliensis.

Chromoblastomycosis and mycetoma are chronic infections caused by the repeated traumatic inoculation of organisms into the skin and subcutaneous tissue. Chromoblastomycosis is caused by several dematiaceous (pigmented) fungi, occurs in tropical regions, and usually affects the lower extremities. The lesions may be nodules, plaques, or tumorous masses. Mycetomas are caused by either true fungi (eumycetes) or filamentous bacteria (actinomycetes); they also occur predominantly in tropical areas and most frequently affect the foot or leg. Typically there is a firm swelling with multiple sinus trac

[Mycetomas in Africa].

Mycetoma is the pathological process in which exogenous fungal or actinomycotic etiological agents generate grains. These agents belong to two groups: fungi and aerobic actinomycetes. Eumycetoma (caused by fungi) and actinomycetoma (caused by actinomycetes) must be distinguished as their treatments are different. These causative agents are introduced by traumas. Mycetomas are frequent in the northern tropical zones of America in Mexico and Venezuela, Africa in Senegal, Mauritania and Sudan and Asia in India, but can also be observed beyond these areas. In Africa, a high endemicity has been not

Nocardia brasiliensis: in vitro and in vivo growth response to steroid sex hormones.

As actinomycetoma is more frequent in males than in females, the possibility that hormones might modify the Nocardia brasiliensis growth and the course of experimental actinomycetoma was explored. Five N. brasiliensis strains were grown on Sabouraud agar containing estradiol, progesterone or testosterone, in 3 different concentrations. Colony diameters were measured weekly for 7 weeks. N. brasiliensis strains were also grown in Sabouraud broth containing hormones. Glucose concentration was measured weekly for 6 weeks. Finally, experimental actinomycetoma was produced in male and female hormone

Mycetoma by Nocardia asteroides: a 9 year follow-up.

An extensive and severe actinomycetoma by Nocardia asteroides, a rare etiologic agent of this infection in Brazil, observed during a 9 year follow-up is reported. Unsuitable social and financial conditions led to amputation as the only possible solution for this case, no signs of infection relapse having been observed in three years after his surgery.

Mycetoma: a retrospective study of 41 cases seen in São Paulo, Brazil, from 1978 to 1989.

Clinical and epidemiological aspects of 41 cases of mycetoma seen in São Paulo city from January 1978 to December 1989 are presented. Twenty-eight cases (68%) were caused by actinomycetes, while 13 (32%) were caused by true fungi. In only 22 (78%) actinomycetoma cases was it possible to identify the agent. Nocardia brasiliensis, by far the commonest actinomycete isolated, was responsible for 13 cases. Among eumycetomata, Madurella grisea was isolated from 3 cases, Scedosporium apiospermum from 2 cases, and Madurella mycetomatis from 1 case, and in 7 cases the agent was not identified. Distribu

Amoxicillin and clavulanic acid in the treatment of actinomycetoma.

BACKGROUND: Mycetoma is a relatively frequent disease in tropical countries. Drugs commonly used need a long period of treatment, and some cases are resistant to these drugs, especially those with bone or visceral involvement. The combination of amoxicillin-clavulanic acid has shown effectiveness against strains of Nocardia brasiliensis in vitro. METHODS: We have used this combination in two cases of mycetoma caused by N. brasiliensis, both with bone involvement and resistance to the drugs usually used. A dose of three tablets a day for 5 to 6 months was given (each tablet contains 500 mg of

Mycetoma.

Mycetoma is a chronic granulomatous disease caused by bacteria or true fungi. It affects the skin the underlying tissue and sometimes subjacent bones and organs. The diagnosis of the disease is confirmed by the microscopic identification and by isolating the infecting agent. Actinomycetoma are treated with trimethoprim-sulfamethoxazole and/or diamino-diphenyl-sulfona (DDS). In patients resistant to these treatments adding amikacin cures about 95% of the resistant cases. In true fungi mycetoma, amphotericin B, ketoconazole, itraconazole, and in some cases these combined with surgery is the trea

Mycetoma: infection with tumefaction, draining sinuses, and "grains".

Mycetoma is a tumorous infection of skin and subcutaneous tissue. It is caused by either actinomycotic bacteria or eumycotic fungi. The three cardinal features are tumefaction or the appearance of indolent inflammatory nodules and secondary fibrosis, formation of sinus tracts and fistulas that may have the ability to penetrate deep tissue, and the presence of grains or granules in the affected tissue and discharge. Although mycetoma is relatively uncommon in the United States, increasing mobility and changes in demographic characteristics should lead to a greater awareness of this disease. Cha

Mycetoma.

Mycetoma is a chronic infective condition of tropical and subtropical regions. It is commoner in males, especially those in their third or fourth decade who work on the land. The clinical triad of subcutaneous nodule, sinuses and discharge usually leads to diagnosis; the disease is commonly painless. Treatment is by extensive surgical excision of affected areas and may include limb amputation. Recurrence is common, rates ranging from 20 to 90 per cent. Medical treatment may be used on its own or as an adjunct to surgery. Although such therapy may cure over half of those with actinomycetoma (ca

Actinomycotic mycetoma. Report of a case with diagnosis by fine needle aspiration.

Mycetoma (madura foot, maduromycosis) is present worldwide but more so in the tropics. We report a case of actinomycetoma diagnosed by fine needle aspiration using cell smears. The authors believe it to be the first case so diagnosed. Definitive diagnosis of the etiologic agent is made by culture, leading to a delay in institution of treatment. The authors advocate the use of fine needle aspiration for rapid diagnosis, thus instigating further workup and treatment.

Comparison of antigens from agents of actinomycetoma by immunodiffusion and electrophoresis procedures.

Antigenic extracts were prepared from culture filtrates of the principal agents known to cause actinomycetoma, namely Actinomadura madurae, Actinomadura pelletieri, Nocardia asteroides, Nocardia brasiliensis, Nocardia otitidis-caviarum, and Streptomyces somaliensis. These antigenic preparations were compared by immunodiffusion (ID), counterimmunoelectrophoresis (CIE), line immunoelectrophoresis (LIE) and rocket line immunoelectrophoresis (RLIE), with rabbit antisera prepared against each of the extracts. Cross-reactivity between antigenic extracts from the different actinomycetes, measured by

Experimental actinomycetoma due to Nocardia transvalensis in laboratory mice.

Nocardia transvalensis was investigated for its ability to cause mycetoma in the skin and foot pad of laboratory mice. Freund's incomplete adjuvant appeared to aid the development of mycetoma lesions. The infection manifested itself as palpable nodules under the skin which discharged white to cream coloured pus containing nocardial granules. The lesions in the skin necrotized, while those of foot pad resulted in bone destruction.

In vitro studies of antibiotic sensitivities of Streptomyces somaliensis--a cause of human actinomycetoma.

Ten Streptomyces somaliensis strains isolated from mycetoma patients were tested in vitro against 13 antibacterial agents. Rifampicin was the most effective antibiotic in terms of low minimum inhibitory concentration (MIC) followed by erythromycin, tobramycin, fusidic acid and streptomycin sulphate. The S. somaliensis strains were all resistant to trimethoprim, even though the combination of sulphamethoxazole and trimethoprim is commonly used as treatment.
