An integrated model for haemorrhagic and ischaemic lesions in the newborn brain.

The vascular anatomy of the developing brain changes from a predominantly basal ganglia orientated pattern at 24 wk to a cortically orientated pattern by 34 wk. This information, combined with other known data on cerebral vascular anatomy and physiology, allows us to develop a model for haemorrhagic and ischaemic lesions in the newborn brain based on two main series of reactions. One series involves the effects of hypoxia and hypercapnia in leading to breakdown of the blood--brain barrier with resultant oedema or haemorrhage. The other links a fall in perfusion pressure and factors causing vas

Maternal endotoxemia, fetal anomalies, and central nervous system damage: a rat model of a human problem.

Endotoxemia is a common consequence of the gram-negative urinary tract infections that complicate human pregnancies. Only rarely, however, have the effects of maternal endotoxemia been evaluated by animal experiments or by human investigations. Data of the Collaborative Perinatal Study suggest an association between maternal endotoxemia and fetal central nervous system damage. For these reasons we performed controlled studies of the fetal effects of treatment of pregnant rats, at appropriate gestational ages, with E. coli endotoxin. We found a maximum 7 per cent incidence of fetal anomalies in

[Leukomalacia and subsequent brain status in relation to intensive neonatal care].

Leucomalacia or white matter necrosis is one of the major aspects of neonatal encephalopathies, especially in the premature baby. These necroses are found mainly in the deep periventricular white matter of the "semi-oval centre". Characteristically, they are ischaemic and their anatomical and histological progress is rapid: coagulation necrosis with rapid axonal fragmentation followed by polymorphic cellular reaction with glial cells and macrophages. Regeneration in the minor forms consists of glyosis process in severe forms; multiple cavities appear in a few weeks. Physiopathogenic hypotheses

Morphologic variations in periventricular leukomalacia.

Periventricular leukomalacia (PVL) usually is manifested as discrete foci of coagulation necrosis of the deep periventricular white matter in the human neonatal brain. During the examination of the brains of 116 infants utilizing an oil red O technic on gelatin-embedded frozen sections, 25 cases of PVL were found with typical foci of coagultion necrosis. Three morphologic varieties of the lesion could be demonstrated. In the first type, rather than being restricted to the periventricular zone, the discrete necrotic foci extended throughout the entire zone of cerebral white matter, even out to

Glial fatty metamorphosis. An abnormal response of premyelin glia frequently accompanying periventricular leukomalacia.

Within the developing nervous system, there is a complex relationship between the accumulation of sudanophilic lipids in glial cells and gestational and postnatal age. Sudanophilic lipids accumulate to a small degree as a stage of normal premyelin lipogenesis, but large amounts of such accumulations may be considered abnormal. The dividing line between the normal and the abnormal (glial fatty metamorphosis) must be defined for each region of the nervous system, since the time and rate of myelination vary widely. Examination of the brains of premature infants routinely coming to autopsy utilizi

Cerebral damage in neonates resulting from arteriovenous malformation of the vein of Galen.

Seven neonates with arteriovenous malformation of the vein of Galen are described. Six presented with cyanotic heart failure. Two cases had antenatal cerebral infarction, five periventricular leukomalacia, and three recent haemorrhagic infarction. Cerebral damage due to the malformation suggests that surgical correction of the arteriovenous malformation offers little to neonates, though after the neonatal period individuals can be cured by operation.
