Drugs and Cell-Based Therapies for the Prevention and Treatment of Neonatal Brain Injury.

The international burden of neonatal brain injury (NBI) remains significant with enormous potential for therapeutic intervention. Recent large trials have demonstrated no neuroprotection with either erythropoietin or darbepoetin for term or pre-term brain injury. Neuroprotective strategies with strong evidence are currently limited to antenatal steroids, antenatal magnesium, and supportive care for pre-term NBI and therapeutic hypothermia for term hypoxic-ischemic encephalopathy. Of the pharmacologic strategies currently being investigated clinically, the agents with the highest likelihood of

Three-Domain Serial Cranial Ultrasound Phenotypes and Outcomes in Very Preterm Infants with Severe Brain Injury: A Single-Center Cohort Study.

Background/Objectives: Severe brain injury (SBI) in very preterm infants includes heterogeneous lesions with distinct timing, burden and outcomes. We used cranial ultrasound (CUS) to describe SBI entity, documented timing, three-domain burden, deaths following documented withdrawal, withholding or non-escalation of life-sustaining treatment for poor neurological prognosis (neuro-WWLST), and survivor outcomes. Methods: Retrospective single-center cohort (1991-2020) of 2841 very preterm infants (<32 weeks' gestation and/or birth weight ≤ 1500 g) with complete CUS within 48 h after birth. CUS was

A mouse model for cerebral/cortical visual impairment (CVI) impairs vision and disrupts the spatial frequency tuning of neurons in visual cortex.

Cerebral/cortical visual impairment (CVI) is a visual disorder often associated with perinatal hypoxic injury. The pathophysiology of CVI is poorly understood in part because of the lack of an animal model. Here we developed a murine model of CVI from existing rodent early postnatal hypoxia models for periventricular leukomalacia. Exposure to hypoxia during the equivalent to the human third trimester did not perturb gross motor function but caused severe impairments in binocular depth perception and visual acuity measured with behavioral assays. Impaired vision was associated with normal retin

Maternal body mass index and the risk of short-term neurological outcomes: a nationwide Korean cohort.

OBJECTIVES: Maternal obesity is a well-known factor influencing pregnancy outcomes. However, the influence of maternal obesity on neonatal neurological morbidities, such as intraventricular hemorrhage (IVH), periventricular leukomalacia (PVL), and retinopathy of prematurity (ROP), remains unclear. We aimed to evaluate the association between maternal body mass index (BMI) and short-term neonatal neurological morbidities using a nationwide cohort. METHODS: Using the Korean National Health Information Database, we conducted a retrospective cohort study with data obtained between 2014 and 2021.

Correlation analysis of blood metabolites and periventricular leukomalacia in preterm infants two weeks postnatally.

Periventricular leukomalacia (PVL) is a neonatal brain injury disease that may cause neurodevelopmental disorders. In this study, our objective was to compare the differences in blood metabolites between preterm infants with and without PVL. We also aimed to screen for potential metabolic markers related to the early identification of PVL. Preterm infants diagnosed with periventricular leukomalacia (PVL) by cranial imaging were assigned to the PVL group (n = 32), while those without a PVL diagnosis were assigned to the control group (n = 68). Blood metabolite levels were measured at 2 weeks po

Gestational age-dependent risk patterns in small for gestational age preterm infants: a large multivariable cohort study.

BACKGROUND: Small for gestational age (SGA) preterm infants remain at high risk for morbidity and mortality, yet the influence of gestational age (GA) on these outcomes is not fully understood. METHODS: The influence of gestational age on morbidity and mortality in SGA preterm infants (23-34 weeks) was analyzed in a cohort of 11,208 preterm infants. Infants were divided into three groups according to gestational age (23-27 weeks, 28-31 weeks, and 32-34 weeks). RESULTS: Compared with non-SGA infants of the same gestational age group, SGA infants showed higher rates of fetal distress and Apgar

Vigabatrin therapy for infantile epileptic spasms syndrome with periventricular leukomalacia.

BACKGROUND: Infantile epileptic spasm syndrome (IESS) and periventricular leukomalacia (PVL) cause developmental regression and poor neurological outcomes. The primary treatments for IESS include adrenocorticotropin and vigabatrin; however, no study has yet evaluated the efficacy of vigabatrin for patients with both IESS and PVL. Therefore, we investigated the efficacy and safety of vigabatrin in patients with both disorders. METHODS: We retrospectively enrolled patients with IESS and PVL who were treated with vigabatrin. We performed ophthalmological examinations and electroretinography (ERG

Immunization Gaps Among High-Risk Preterm Infants in Kazakhstan.

BACKGROUND: Advances in neonatal intensive care in Kazakhstan have improved the survival of preterm infants, including those with very low birth weight (VLBW) and extremely low birth weight (ELBW). However, after hospital discharge, these medically vulnerable children may remain insufficiently protected against vaccine-preventable infections because of prolonged temporary medical exemptions, clinical caution, parental concerns, and fragmented coordination between specialized follow-up services and primary health care. OBJECTIVE: This study aimed to assess vaccination coverage in a 10-year coh

Twenty-five-year trends in mortality and major morbidity among very low birth weight infants at a Saudi tertiary centre: improving morbidity despite expanding resuscitation at the limits of viability.

BACKGROUND: Very low birth weight (VLBW) infants remain at high risk of mortality and major morbidity, yet longitudinal data from Saudi Arabia spanning several decades are scarce. We examined 25-year trends in survival and morbidity at a single tertiary centre, with particular attention to whether patterns reflect evolving case-mix at the limits of viability. METHODS: This retrospective cohort study included inborn VLBW infants (birth weight <1,500 g, gestational age <33 weeks) admitted to the NICU of King Saud University Medical City, Riyadh, across three periods: 1999-2007 (n = 468), 2011-2

Development and external validation of a prediction model for poor prognosis before discharge in very preterm and/or very low birth weight infants with early-onset sepsis.

BACKGROUND: Clinical early-onset sepsis (EOS) is associated with death and major morbidity in very preterm and/or very low birth weight infants. We aimed to develop and externally validate a model for a composite adverse outcome before discharge. METHODS: This retrospective multicenter study included infants with gestational age < 32 weeks and/or birth weight < 1500 g who met operational criteria for clinical EOS. Infants from two hospitals formed the development cohort (n = 249), and infants from a third hospital formed the external validation cohort (n = 84). The outcome comprised death, gr

Acute Neurological Deterioration in a Child with Shunt-Dependent Post-Hemorrhagic Hydrocephalus: A Case Report.

Post-hemorrhagic hydrocephalus (PHH) is a serious neurological sequela of severe intraventricular hemorrhage (IVH) in premature infants and remains one of the leading indications for ventriculoperitoneal (VP) shunt placement. Evaluating possible VP shunt-related complications can be challenging because clinical manifestations are often nonspecific, neuroimaging may initially appear unchanged, and microbiological cultures may remain negative. We report the case of a 19-month-old male born at 28 weeks' gestation who developed Grade IV germinal matrix/intraventricular hemorrhage with bilateral in

IL-4/IL-4Rα signaling activates PPARγ to promote oligodendrocyte differentiation and remyelination.

The restoration of white matter damage is essential for the functional recovery observed in demyelinating diseases such as multiple sclerosis (MS). Interleukin 4 (IL-4), a type II cytokine, has been acknowledged for its protective role in modulating microglia and immune cells, thus suppressing inflammatory processes. Emerging evidence suggests that IL-4 extends beyond its immunomodulatory function, playing a part in memory, neuronal pruning, protection, and neurodegeneration. Here, our study identified the capacity of IL-4 to stimulate myelinogenesis and remyelination in the central nervous sy

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Molecular Approach of Oxidative Stress and Bronchopulmonary Dysplasia: Relationship of GSTM1 and GSTT1 Genes.

Bronchopulmonary dysplasia (BPD) is a chronic lung disease, with its own clinical, radiological, and histopathological characteristics, which mainly affects premature newborns (NBs), resulting from a combination of factors that include immaturity, inflammation, and lung injury, in addition to therapy with mechanical ventilation and exposure to high concentrations of oxygen. However, even with advances in care for critically ill NBs, BPD continues to be a challenge for the care team and family members. This has been identified as one of the most important causes of morbidity and mortality due t
