Study protocol: neonatal colonisation and infection with Ureaplasma in very immature preterm infants born <29 weeks of gestation (NEO-CONSCIOUS) - a prospective multicentre study assessing early life colonisation rates and potentially associated adverse outcomes.

INTRODUCTION: Preterm infants, particularly those born before 29 weeks of gestation, are at increased risk of developing bronchopulmonary dysplasia (BPD) and other complications of prematurity. Substantial evidence suggests that respiratory tract colonisation with Ureaplasma species significantly contributes to pulmonary inflammation, impaired lung function and subsequent lung disease especially in very immature infants. Moreover, Ureaplasma exposure has been implicated in the pathogenesis of other inflammation-related sequelae of prematurity. Although representing a potentially actionable ris

Face mask versus nasal mask device use for initial resuscitation in extremely and very preterm infants (FONDUE): an open-label, single-centre, randomised, controlled trial.

BACKGROUND: More than 85% of very preterm infants (born <32 weeks' gestation) breathe spontaneously within 1 min of birth, however, more than 60% of infants receive positive pressure ventilation. Face mask application soon after birth might suppress breathing through the trigeminal nerve reflex, causing vocal cord closure and hypoxia. We aimed to investigate whether nasal mask continuous positive airway pressure (nCPAP) would improve CPAP success, reducing the need for positive pressure ventilation and intubation at birth, compared with face mask CPAP (fCPAP). METHODS: This open-label, random

Effect of very preterm delivery on outcomes following in utero spina bifida repair.

OBJECTIVE: In utero fetal spina bifida repair decreases the risk for cerebrospinal fluid (CSF) diversion including ventriculoperitoneal (VP) shunt placement, reverses hindbrain herniation, and improves motor function with a higher likelihood of independent ambulation. Unfortunately, in utero surgery can be complicated by preterm delivery. The goal of this study was to compare pregnancy and neonatal outcomes between individuals who delivered very preterm, and those who delivered later in gestation following in-utero spina bifida repair. METHODS: Data from the North American Fetal Therapy Netwo

Vibrating mesh nebulizers vs. less invasive surfactant administration in the treatment of preterm respiratory distress syndrome: a multicenter, open-label, exploratory, randomized clinical trial.

OBJECTIVE: Deficiency of pulmonary surfactant (PS) induces the onset of neonatal respiratory distress syndrome (RDS), which can lead to progressively worsening respiratory failure and even death. However, studies on improving lung function via the use of vibrating mesh nebulizer (VMN) technology to deliver PS to premature infants with RDS are limited. METHODS: This prospective multicenter, open-label, exploratory, randomized clinical trial, including 2 parallel groups and a 1:1 allocation, was conducted in 7 hospitals from Jan. 1, 2023, to Dec. 31, 2024. Premature infants born at less than 32

Intranasally delivered colostrum-derived small extracellular vesicles mitigate acute neuroinflammation in periventricular leukomalacia.

BACKGROUND: Periventricular leukomalacia (PVL) is a predominant white matter injury in preterm infants, leading to lifelong neurodevelopmental disability, and yet disease-modifying therapies are lacking. Breast milk, especially colostrum, contains bioactive components with potential neuroprotective properties, among which extracellular vesicles (EVs) have recently attracted increasing attention. This study aimed to evaluate the neurorestorative efficacy of intranasally administered colostrum-derived small EVs (sEVs) in a lipopolysaccharide (LPS)-induced PVL model. METHODS: sEVs were isolated

Cumulative Hydrocortisone Exposure and Early Brain Volumetrics in Very Low Birth Weight Infants: Associations with Neurodevelopmental Outcomes.

Background/Objectives: Systemic hydrocortisone (HCS) in very low birth weight (VLBW) infants is commonly used to treat early hypotension or prevent bronchopulmonary dysplasia. This study evaluated the associations between postnatal HCS exposure and neurodevelopment in VLBW infants by comparing regional brain volume at term-equivalent age (TEA) with neurodevelopmental outcomes in early infancy. Methods: This retrospective cohort study included VLBW infants admitted to a neonatal intensive care unit (NICU) between 2013 and 2019. The cumulative HCS dose during hospitalization was recorded, and re

Infantile hemiparesis secondary to periventricular leukomalacia: clinical findings and multidisciplinary rehabilitation outcomes.

A middle childhood child with infantile hemiparesis is the subject of this case study. He has left-sided weakness and delays in both gross and fine motor milestones, particularly in the upper limb. The child's delayed cry and need for NICU admission, despite a full term and vaginal delivery, suggested perinatal hypoxia. The results of the MRI showed slight right lateral ventricular asymmetry and periventricular leukomalacia, which are signs of hypoxic-ischaemic encephalopathy. Clinical assessment revealed isolated motor impairment, hypertonia and elevated deep tendon reflexes. The start of phy
