[Morbimortality at a second-level neonatology unit].

In order to detect existing problems and opportunely pinpoint failures, an evaluation was conducted to determine the most frequent causes of morbidity and mortality in a neonatal ward. A retrospective analysis was carried out on the total number of births attended per year (1,003) at the hospital and of those newborns, those transferred to the neonatal ward (213, 21.2%). Of the 213 newborns placed in the neonatal ward, 30 of them (14.08%) were transferred to a Third Level Hospital (Neonatal Intensive Care Unit) while the rest remained in the neonatal ward. The five most frequent pathologies se

Diagnostic and prognostic value of CT brain scan in term neonates with moderate and severe birth asphyxia.

Twenty term neonates with moderate (stage II) and 5 with severe (stage III) hypoxic ischemic encephalopathy (HIE) were prospectively studied to determine diagnostic and prognostic value of CT brain scan. Three neonates expired, 4 were lost to follow up while 18 were followed up to 18 months of age. Cerebral hypodensities were noted in 20 and intracranial hemorrhage (ICH) in 8, of which 6 had both ICH and hypodensity. Twelve of 14 infants with hypodensities and 5 of 6 with ICH who were followed up were handicapped at 18 months. Thirteen of 18 babies followed up were subjected to repeat CT scans

Partial visual recovery in two fullterm infants after perinatal hypoxia.

Behavioural methods were used to assess the visual development of two infants with a history of severe perinatal hypoxia. Both infants were born fullterm and showed hypoxic-ischaemic encephalopathy and subsequent cerebral palsy. Low visual acuity, small and asymmetrical visual fields, asymmetrical binocular and monocular OKN, and absent visual threat response were demonstrated initially. Ophthalmological examination revealed strabismus, but no further abnormalities. Longitudinal assessments up to the age of two years showed impressive (partial) improvements of visual functions. The mechanism f

[The relevance of duplex Doppler sonography in pediatrics].

Since the introduction of duplex Doppler sonography the noninvasive assessment of hemodynamic changes in children has become possible. The clinical application comprises the depiction of cerebral hemodynamics in infants, where characteristic flow patterns can be found in patent ductus arteriosus, hypoxic-ischemic encephalopathy, cerebral hemorrhage, hydrocephalus or in brain death. Moreover, Duplex Doppler Sonography play an important role in the evaluation of renal hemodynamics, especially in kidney transplants. Finally, the examination of the portal venous system can demonstrate thrombosis o

Cerebral blood flow abnormalities in cerebral palsied children with a normal CT scan.

Cerebral palsy (CP) is often a consequence of a hypoxic-ischemic encephalopathy and/or intracerebral hemorrhage secondary to pre- and peri-natal asphyxia. Hypodense lesions on the CT-scan are found in about 70 to 80% of CP-patients. In the present study, regional cerebral blood flow (CBF) was measured in CP-patients having a normal CT-scan. The aim was to correlate the CBF changes with the clinical and the etiologic findings. CBF was measured by xenon-133 inhalation and single photon emission computer tomography. The mean CBF value in 20 CP-patients, age 6-19 years, was 67 +/- 11 (1 SD) ml/100

Hypobaric-ischemic conditions produce glutamate-like cytopathology in infant rat brain.

We present a new animal model of perinatal hypoxic/ischemic brain damage and compare this type of brain damage with the excitotoxic type of damage previously described in the brains of infant rats and monkeys treated systemically with glutamate (Glu). Ten-d-old rats with unilateral occlusion of the common carotid artery were subjected to hypobaric conditions for 75 min and sacrificed 0-4 hr later for light and electron microscopic brain examination. The mortality rate was relatively low (12%), and brain damage was evident ipsilateral to the ligated carotid in 94% of surviving animals 4 hr afte

Hypoxic-ischaemic encephalopathy after near miss sudden infant death syndrome.

Between 1982 and 1985, 14 infants aged 3-26 weeks presented with severe hypoxic episodes as a result of the 'near miss' sudden infant death syndrome (SIDS). They all had metabolic acidosis, cardiovascular instability, acute renal failure, ischaemic colitis, or acute neurological dysfunction. Investigation of the cause excluded infection and trauma, or a primary metabolic, pulmonary, cardiac, or seizure disorder. Seven infants were deeply comatose on admission, never regained consciousness, and died within 60 hours. A characteristic evolution of hypoxic-ischaemic encephalopathy not previously c

Role of excitatory amino acids in brain injury caused by hypoxia-ischemia, status epilepticus, and hypoglycemia.

Glutamate, the major excitatory neurotransmitter of the brain, mediates neuronal injury from hypoxia-ischemia, hypoglycemia, and status epilepticus. Drugs that block glutamate receptors, particularly the N-methyl-D-aspartate (NMDA) receptor, protect neurons from these insults. Noncompetitive antagonists of NMDA receptors have the potential to prevent perinatal neurologic morbidity.

New model of hemorrhagic hypoxic-ischemic encephalopathy in newborn mice.

An easy and inexpensive method is reported for producing hemorrhagic brain damage in newborn mice, involving only exposure to hypoxia. One-day-old mice, Jcl:ICR strain, were subjected to a humidified 5% oxygen, 95% nitrogen mixture for 8 hours. After the hypoxic episode, 34% of newborn mice survived, 59% manifested cerebral parenchymal hemorrhage. Cortical hemorrhage could be detected in live mice; intracranial hemorrhage was observed through the thin skin and skull. Cortical hemorrhage usually affected the bilateral parietal regions symmetrically and neuronal destruction was observed in the d

Sensitivity of the developing rat brain to hypobaric/ischemic damage parallels sensitivity to N-methyl-aspartate neurotoxicity.

The endogenous excitotoxin, glutamate (Glu), acting at the N-methyl-aspartate (NMA) subtype of Glu receptor, is thought to play a major role in hypoxic/ischemic neuronal degeneration. In the present study, the sensitivities of the developing rat CNS to hypoxic/ischemic neuronal degeneration and to the neurotoxic action of NMA were compared at various postnatal ages. In the hypoxic/ischemic experiments, ischemia was produced by unilateral common carotid artery ligation and hypoxia by subjecting the pups to a partial vacuum. Keeping the duration of the hypobaric episode constant at 75 min for al

Regional cerebral glucose utilization in the immature rat: effect of hypoxia-ischemia.

The 2-deoxy-[14C]-glucose (2-DG) method of Sokoloff was used to assess regional cerebral glucose utilization (CGU) in the immature rat. The 7-d postnatal rats received 2.5 muCi 2-DG subcutaneously, after which blood was collected for measurement of plasma glucose and 2-DG activity at intervals up to 90 min. The brains of the 90-min rat pups either were frozen for analysis of glucose concentration and chromatographic separation of 2-DG and 2-DG-6-phosphate or for [14C]-autoradiography. A lumped constant of 0.55 was calculated from plasma and brain glucose levels of 6.4 and 1.62 mmol/L.kg, respe

[Prognosis factors in hypoxico-ischemic encephalopathy in full term infants].

This study analyzes 61 full-term neonates affected by hypoxic-ischemic encephalopathy, who were followed for a minimum of 15 months to determine the factors related to the later appearance of neurological sequels. No severe sequels have been detected, while 8.2% showed moderate sequels and 4.9% slight sequels. The degree of cerebral suffering and the intensity of convulsions influence both the appearance of sequels and their severity, contrary to the scores of the Apgar tests after one minute or 5 minutes and the birth weight. By applying the formula (Sequel Rate = 0.2143* Cerebral suffering +

[Perinatal factors predicting hypoxic-ischemic encephalopathy in asphyxic newborn infants].

Between 1 May 1984 and 30 April 1985, 1,354 term or posterm neonates being white and having asphyxia in controlled delivery were studied to predict development of neurological signs and its seriousness. In each case correlations were estimated between the existence of neurological signs and its grade and 100 gestational, obstetric and neonatal factors of potential predictor. The discriminatory analysis indicated that the more significants predictors were: Apgar at five minutes, prolonged labor, twins, resuscitation, DIP I, cephalhematoma, caput succedaneum, umbilical artery pH, urgent cesarean

State of the art of magnetic resonance (MR) in neonatal hypoxic-ischemic encephalopathy.

Magnetic resonance (MR) has begun to play an important role in neonatal neurology. Several MRI techniques have been applied to the diagnosis of hypoxic ischemic encephalopathy. Cerebral perfusion examined by intravoxel incoherent motion, a non-invasive tool, seems to be opening new inroads for detecting variations (neurophysiological modifications) in cerebral flows during hypoxic ischemic encephalopathy. MR spectroscopy allows identification of specific biochemical alteration of spectra patterns at various moments of hypoxic ischemic distress, including: (1) primary expression of metabolic di

Neurosonographic findings in infants treated by extracorporeal membrane oxygenation (ECMO).

Extracorporeal membrane oxygenation (ECMO) is an approved therapy for some neonates who have respiratory failure that is due to hyaline membrane disease, meconium aspiration, persistent pulmonary hypertension, congenital diaphragmatic hernia, or sepsis. The major complication of this therapy is hemorrhage, with intracranial hemorrhage having the highest morbidity and mortality. Seizures, incisional bleeding and bleeding in the pleural space, hypoxic-ischemic encephalopathy, renal failure, and cardiovascular complications account for most of the other complications. Cranial sonography provides

Echoencephalographic findings in neonates associated with maternal cocaine and methamphetamine use: incidence and clinical correlates.

Term neonates (n = 74) exposed antenatally to cocaine, methamphetamine, or cocaine and a narcotic but without any other known perinatal complications were prospectively examined with cranial ultrasonography to detect the presence of central nervous system injury. These studies were compared with those of a drug-free but clinically ill group of infants (n = 87) at risk for hypoxicischemic encephalopathy, and with those of infants who were well. Cranial abnormalities were detected by ultrasonography in 35.1% of the drug-exposed infants, similar to the incidence in the infants at risk for hypoxic
