Neonatal hypoxic-ischemic encephalopathy: current management practices.

A national survey of training programs in neonatal-perinatal medicine was undertaken to determine neurologic management practices in dealing with the asphyxiated, term newborn. Seventy-two of 105 responses (68.6 per cent) were returned. Commonly used diagnostic tests include routine electroencephalography, cranial sonography, and determinations of arterial blood gases and serum calcium. Fluid restriction and treatment of seizures with phenobarbital are widely used therapies. Many available techniques for both diagnosis and treatment show wide variability in utilization. These results demonstra

Regional cerebral blood flow during hypoxia-ischemia in immature rats.

Immature rats subjected to a combination of unilateral common carotid artery ligation and hypoxia sustain brain damage confined largely to the ipsilateral cerebral hemisphere. To ascertain the extent and distribution of ischemic alterations in the brains of these small animals, we modified the Sakurada technique to measure regional cerebral blood flow using carbon-14 autoradiography. Seven-day-old rats underwent right common carotid artery ligation following which they were rendered hypoxic with 8% O2 at 37 degrees C. Before and during hypoxia, the rat pups received an injection of iodo[14C]an

Seizures in the neonatal intensive care unit of the 1980s. Types, Etiologies, Timing.

A retrospective review was taken of 150 newborns with seizures evaluated at the same medical center and by the same investigator(s) from 1982 to 1987. The aims of the study were to determine in this current population the distribution of clinical seizure types, the distribution of causative etiologies, and the relation of etiology to the timing of onset of the seizures. Seizures were classified as subtle, multifocal clonic, generalized tonic, focal clonic, and myoclonic. Subtle (65% of total) and multifocal clonic seizures (54% of total) were the most common seizure types. Subtle seizures usua

Electronic fetal heart rate monitoring: why the dilemma?

As medical technology becomes more and more complex, both the benefits and risks increase. Perhaps we have misled ourselves and, unwittingly, our patients into expecting the perfect outcome by complication-free care. As we learn more about the pathophysiology of disease it becomes apparent that our expectations are unrealistic. In addition, as clinicians we have become more sophisticated about the utility of clinical testing. We have learned that the predictive value of a test is related to its sensitivity and specificity, and to the prevalence of the disease in the population being tested. We

Intracranial arterial duplex Doppler waveform analysis in infants.

Duplex sonography of the brain was performed on 130 pediatric patients: 53 with normal brains, 22 with intracranial hemorrhage, 15 with hypoxic-ischemic encephalopathy, 29 with hydrocephalus requiring ventricular shunting, and 11 with ventriculomegaly without hemorrhage or shunting. Doppler arterial waveforms from the Circle of Willis showed increased pulsatility in 82% of patients with intracranial hemorrhage, 53% of patients with hypoxic-ischemic change, and in only 31% of patients with hydrocephalus requiring shunt and 36% of patients with ventriculomegaly without shunt or hemorrhage. Mean

Patterns of cerebral glucose metabolism using 18FDG and positron tomography in the neurologic investigation of the full term newborn infant.

18F fluorodeoxyglucose (18FDG) and positron tomography (PT) were used in 20 full term babies with seizures or hypotonia to describe regional cerebral glucose metabolism. Among babies with seizures, birth asphyxia was the most common cause. PT was performed at age 6-17 days. One hour before PT, 18FDG (50-100 microCi/kg) was injected intravenously. Ten or more PT sections were obtained in each infant. The areas of the brain that were metabolically the most active were the cortex and the thalami. Six cortical areas and a white matter reference area were selected for analysis of relative rates of

Selective sparing of NADPH-diaphorase neurons in neonatal hypoxia-ischemia.

Excitatory amino acids have been implicated in ischemic neuronal injury. To test this hypothesis in neonatal hypoxia-ischemia, lesions of the cortex and striatum were induced in 7-day-old rats by unilaterally ligating their carotid arteries and subjecting them to hypoxic conditions for 2 hours. Brains examined 1 week later demonstrated, within the regions of ischemic damage, a striking preservation of neurons that stained histochemically for nicotinamide adenine dinucleotide phosphate diaphorase (NADPH-d) activity. Concentrations of the neuropeptides somatostatin and neuropeptide Y, which colo

Renal injury in the asphyxiated newborn infant: relationship to neurologic outcome.

The relationship of renal and central nervous system injury was prospectively evaluated in 120 asphyxiated infants. Renal evaluation findings were considered abnormal if there was oliguria (urine output less than 1 ml/kg/hr), which was designated transient if present in the first 24 hours only and persistent if present for at least 36 hours, or if the urinary beta 2-microglobulin concentration from first-void urine was elevated: (1) Thirteen infants had persistent oliguria; the urinary beta 2-microglobulin level was elevated in all. The six term infants had clinical signs consistent with hypox

Early high-dose phenobarbital treatment for prevention of hypoxic-ischemic brain damage in very low birth weight infants.

In a randomized prospective trial, we studied the effect of early high-dose phenobarbital treatment on the early (intraventricular hemorrhage) and late (neurodevelopmental abnormalities) manifestations of hypoxic-ischemic encephalopathy in preterm infants weighing 1500 g or less at birth. The first intravenous dose of 15 mg/kg was given at a mean age of 110 minutes, followed by 15 mg/kg after 4 hours and then by 5 mg/kg at 24-hour intervals for 5 days. The overall incidence of intraventricular hemorrhage was 32% in treated and 46% in control infants, a nonsignificant difference. An ultrasound

Neurosonographic changes in newborns treated with extracorporeal membrane oxygenation.

Ten out of 40 near-term neonates on extracorporeal membrane oxygenation (ECMO) therapy developed abnormal neurosonograms. We identified two abnormal patterns. Hyperechoic areas of intracranial hemorrhage were observed in two patients (a significantly lower incidence than previously reported). Diffuse or focal echogenic areas of hypoxia-ischemia resulting from periventricular leukomalacia, cerebral edema or large vessel infarction had not previously been noted in nine of these patients. Three of the ten patients survived with neurological sequelae. Recognition of hemorrhage or a hypoxic-ischemi

The rationale for biochemical monitoring of the fetus.

1. Rates for perinatal mortality and low Apgar scores have not been affected by fetal intensive care (electronic monitoring +/- fetal blood sampling) in any randomized controlled trial to date. 2. Acidosis at birth may be susceptible to improvement by fetal intensive care but has not been tested adequately in more than a few women. 3. Birth asphyxia which is followed by evidence of cerebral dysfunction within the first 48 hours of life can be significantly altered by current technics of fetal intensive care. 4. The evidence suggests that cognitive disorders and cerebral palsy occur as long ter

[Function of the sympathetico-adrenal system and the acid-base equilibrium in newborn infants with a hypoxic lesion of the central nervous system].

The function of the sympathetic-adrenal system was studied by measuring umbilical cord blood catecholamines in 31 full-term newborns with perinatal asphyxia and evidence of hypoxic-ischemic encephalopathy (HIE) and in 20 normal newborns. The results indicate that the full-term newborn responds to asphyxia with graded catecholamine release. The epinephrine concentration in newborns with moderate HIE is lower than in newborns with mild HIE, which may reflect decreased sympathetic-adrenal function due to prolonged asphyxia. Acid-base values in capillary blood and lumbar cerebrospinal fluid were d

Clinical spectrum and outcome of neonatal convulsions.

The clinical spectrum and outcome of neonatal convulsions within an obstetric hospital population were reviewed for the 5 years, 1978-82, inclusive. There were 156 convulsing neonates managed at the Mater Mothers Hospital (110 inborn, 46 outborn). The incidence of early neonatal convulsions for inborn babies was 3.0/1000 live births. Antenatal and perinatal risk factors were compared between the 156 convulsing infants and the 36,082 infants born during the same period who did not convulse. The leading risk factors for convulsions were prematurity, intra-uterine growth retardation, low 5 min Ap

Focal protein synthesis inhibition in a model of neonatal hypoxic-ischemic brain injury.

Cerebral hypoxia-ischemia was produced in 1-week-old rats by exposing them to 8% O2-92% N2 commencing 4 to 6 h after unilateral carotid artery ligation. Protein synthesis rates were measured in cerebral cortex, caudate-putamen, thalamus, and lateral septal nuclei during 2 h of hypoxia and at three times (15 min, 10 h, and 18 h) after a 3.5-h hypoxic exposure. Protein synthesis was inhibited in the ipsilateral but not contralateral forebrain during the brief hypoxic exposure, and in both hemispheres during early recovery after a 3.5 h exposure, which was sufficient to produce brain injury in th

New fontanometer for continuous estimation of intracranial pressure in the newborn.

Intracranial pressure was estimated by a new pneumatic applanation fontanometer in babies in intensive care. A close correlation with cerebrospinal fluid pressure was found on 35 separate occasions. In 12 control babies the mean (SD) fontanelle pressure was 5.2 (2.2) cm H2O, in eight with a hypoxic-ischaemic encephalopathy it was 12.6 (4.3) cm H2O, and in nine with post-haemorrhagic hydrocephalus it was 10.8 (4.3) cm H2O. In four babies monitored continuously over seven days the correlation remained good. The fontanometer enables pressure to be monitored accurately, continuously, and non-invas

Value of intracranial pressure monitoring of asphyxiated newborn infants.

Twenty-three infants suffering the effects of moderate or severe hypoxic-ischaemic encephalopathy were continuously monitored for intracranial pressure (ICP) by means of a subarachnoid catheter for a total of 1083 hours. Cerebral perfusion pressure (CPP) was also continuously monitored for 21 of the infants. The median age at the start of ICP monitoring was 17 hours, and the opening pressure correlated poorly with maximum sustained pressures. Maximum sustained ICP allowed the infants to be divided into three groups: (1) those with no elevation of ICP (nine), of whom two died and five had a nor
