New insights into mechanisms of neuronal damage in the developing brain.

Hypoxia-ischemia, hypoglycemia, and status epilepticus damage specific regions in the developing brain. The factors which determine selective neuronal vulnerability have remained obscure but recent research suggests that the patterns may be related to dysfunction of specific sets of synapses. An important current hypothesis suggests that hyperactivity of excitatory synapses, which use neurotransmitters such as glutamate, may cause excessive transmitter release and lead to damage of adjacent neurons. Excessive stimulation of excitatory neurotransmitter receptors triggers a cascade of biochemica

Positron emission tomography in the newborn: effect of seizure on regional cerebral blood flow in an asphyxiated infant.

Regional cerebral blood flow (CBF) was measured by positron emission tomography (PET) with 15O-labeled water in an asphyxiated infant during a seizure. After intrauterine asphyxia, the infant had a syndrome characteristic of hypoxic-ischemic encephalopathy. During a PET scan on the second postnatal day, the infant had a focal seizure with deviation of eyes to the right and clonic jerking of the right arm. Regional blood flow was highest, about 80 ml/100 g/min, in the left temporal-parietal frontal region of the left hemisphere, the site of origin of the seizure; blood flow in the same region o

Pathogenesis and management of hypoxic-ischemic encephalopathy in the term newborn.

The pathogenesis and management of hypoxic-ischemic encephalopathy in the term infant is reviewed. Specifically, the potential significance of a variety of factors on the pathogenesis of brain injury is discussed including changes in cerebral blood flow, brain swelling, carbohydrate status, alterations in ion homeostasis, and synaptic activity. Management is reviewed in terms of prevention, supportive care, corrections of metabolic derangements, control of seizures, and prevention of fluid overload.

Positron emission tomography in the asphyxiated term newborn: parasagittal impairment of cerebral blood flow.

Hypoxic-ischemic encephalopathy secondary to perinatal asphyxia in the term newborn is the most common recognized cause of the subsequent motor deficits often grouped under the rubric "cerebral palsy." In order to provide insight into the basic nature and pathogenesis of the brain injury in such infants, we studied regional cerebral blood flow (CBF) by positron emission tomography (PET) in 17 asphyxiated term infants during the acute period of illness. A consistent and apparently unifying abnormality was observed, namely, a relative decrease in CBF to parasagittal regions, generally symmetrica

Hypoxic-ischaemic cerebral injury in the term newborn: correlation of CT findings with neurological outcome.

For asphyxiated term infants, prediction of neurological outcome based on clinical criteria alone is not always reliable. We have used changes in tissue density on CT scan (known to represent hypoxic-ischaemic injury) as an adjunct to clinical examination to assess the severity of cerebral injury. The study population comprised 56 term newborns with hypoxic-ischaemic encephalopathy. Scans were performed with a General Electric 8800 scanner and classified as Normal, Patchy, Diffuse or Global according to the extent of areas of decreased density (DD). These were correlated with neurological outc

Focal motor seizures heralding stroke in full-term neonates.

We describe the clinical syndrome, medical management, etiology, and neurologic outcome of stroke diagnosed by computed tomographic scan in 11 full-term neonates encountered during a two-year period. Neonatal stroke is relatively common and may appear in the setting of diverse cerebrovascular disorders such as hypoxic-ischemic encephalopathy, polycythemia, acute severe hypertension, and embolization. Repetitive, persistently unifocal motor seizures heralded localized cerebral injuries in eight infants. The majority of patients did not display any other lateralized clinical neurologic signs. An

[Multicenter study of asphyxia neonatorum].

Out of 14,673 term infants born at seven neonatal centers in the Barcelona area, 215 newborns were identified as being at risk of developing hypoxic-ischemic encephalopathy. They had shown one or more of the following criteria: fetal pH less than or equal to 7.20, abnormal fetal heart rate patterns, umbilical artery pH less than or equal to 7.20, 5 minute Apgar score less than or equal to 6, or the need for immediate neonatal resuscitation. 135 items were investigated in relation to epidemiological, clinical and laboratory parameters. The incidence of HIE was 6.39% and the mortality was 0.55%.

Term infants with hypoxic-ischemic encephalopathy: outcome at 3.5 years.

A total of 167 term neonates with a diagnosis of hypoxic-ischemic encephalopathy (HIE) had detailed neurodevelopmental follow-up at 3.5 years of age. All 66 children with mild HIE were free from handicap; all seven with severe HIE were severely handicapped; and of the 94 with moderate HIE at birth, 21.3 per cent were handicapped. Mean IQ was significantly related to the category of HIE. Within the moderate HIE category, the neurological examination at discharge from the Neonatal Intensive Care Unit was more useful than the presence of neonatal convulsions in identifying children with subsequen

Investigation of cerebral energy metabolism in newborn infants by phosphorus nuclear magnetic resonance spectroscopy.

31P NMR spectroscopy can be safely and successfully used to study cerebral energy metabolism in sick newborn infants, provided a suitable life support and monitoring system is available. Studies of normal infants have shown characteristic spectral peaks assigned to adenosine triphosphate, phosphocreatine, phosphodiesters plus phospholipids, inorganic orthophosphate, and a large peak at the phosphomonoester resonance frequency that is attributable mainly to phosphoethanolamine, a precursor of membrane phospholipids. Both the PCr/Pi and PCr/ATP ratios were lower in normal newborn infants than re

[Hypoxic-ischemic encephalopathy. Clinical considerations].

The Hypoxic-Ischemic Encephalopathy (HIE) is a severe illness of the unborn, respectively of the newborn. About 90 percent of the causes occur in utero, about 10 percent after birth. The risk for HIE arises from anatomical and pathophysiological particularities: little overlapping between the great cerebral arteries, poor periventricular vascularisation, and a loss of the autoregulation of cerebral blood flow during asphyxia. Most important is the early detection of intrauterine asphyxia. After birth the general measures include: thermoneutral temperature, oxygenation, normal pCO2, regular blo

Hypoxic-ischemic encephalopathy: cerebrovascular carbon dioxide reactivity in neonates.

Using noninvasive measurement of cranial blood flow, we previously demonstrated that full-term asphyxiated neonates have decreased cerebral perfusion that can persist up to 5 days of age. In an attempt to test their postischemic cerebrovascular CO2 reactivity, we measured cranial blood flow in ten asphyxiated term (39 +/- 0.8 weeks and 3078 K 400 gm) infants with and without inhaled carbon dioxide (3 percent). The end tidal CO2 (PaCO2) increased significantly, from 28.8 +/- 1.0 mm Hg to 32.3 +/- 2.0 mm Hg after CO2 inhalation (p less than 0.01), whereas the cranial blood flow showed no signifi

Effects of dexamethasone in hypoxic-ischemic brain injury in the neonatal rat.

To clarify the effects of corticosteroids in neonatal hypoxic-ischemic brain injury, 7-day-old rats were subjected to unilateral common carotid artery ligation and hypoxia (Levine procedure) after being injected subcutaneously with saline, low-dose dexamethasone (4 mg/kg) or high-dose dexamethasone (40 mg/kg). Neither low-dose nor high-dose dexamethasone ameliorated the brain edema, lactacidemia, or hypoglycemia associated with hypoxia-ischemia. In addition, dexamethasone did not alter the pattern of neuropathologic damage or reduce the fall in brain high-energy phosphates. Finally, high-dose

Effects of hypoxia-ischemia on monoamine metabolism in the immature brain.

We measured acute changes in monoamine metabolites in corpus striatum of immature rat pups exposed to hypoxia-ischemia, hypoxia alone, or total global ischemia. Carotid ligations and two hours of 8% oxygen environment in 7-day-old pups led to asymmetrical turning behavior, a 70% decrease in endogenous striatal dopamine levels, and a 125% increase in homovanillic acid (HVA) concentrations on the side of ligation. In contrast, hypoxia alone and total global ischemia alone were not associated with HVA level elevation. Elevation of HVA level with hypoxia-ischemia showed a threshold effect between

EEG in premature infants with intraventricular hemorrhage.

Seventy-eight EEGs from 44 premature infants with CT- or autopsy-verified intraventricular hemorrhage (IVH) were reviewed retrospectively. The patient's most abnormal EEG was a reliable predictor of outcome, independent of the estimated gestational age, 1- and 5-minute Apgar scores, and IVH grade. Nine of 12 infants who had all normal or mildly abnormal EEGs had favorable outcomes. The nine infants whose worst EEGs were moderately abnormal experienced mixed outcomes, ranging from normal survival to death. All 23 infants with one or more markedly abnormal EEGs suffered unfavorable outcomes. Pos

Hypoxic-ischemic encephalopathy in term infants: diagnosis and prognosis evaluated by ultrasound.

The results of cranial ultrasonography in 32 term infants who had hypoxic-ischemic encephalopathy (HIE) and in 26 control infants were reviewed. Small or nonvisualized ventricles were present in 94% of the patients with HIE and in 62% of the control subjects. In 20 of the 32 patients (63%) with HIE, ultrasound demonstrated parenchymal abnormalities; the most common finding was periventricular hyperechogenicity (12/32 patients). There was a single false positive result in the control group. Fifty per cent of patients with HIE who had abnormalities demonstrated on ultrasonograms died (within 15
