Pathogenesis of hypoxic-ischemic brain injury in a perinatal rodent model.

Exposure of immature rats to 8% oxygen after unilateral carotid artery ligation (UCL) causes metabolic, neurochemical and histopathological changes in the ipsilateral forebrain that resemble those in human perinatal hypoxic-ischemic encephalopathy. Regional cerebral perfusion in this model was examined by visual analysis of India ink trapped in cerebral vessels and measurement of [14C]iodoantipyrine [( 14C]IAP) and [3H]flunitrazepam extraction into the brain. UCL alone reduced [14C]IAP accumulation in the ipsilateral hemisphere by 20% and hypoxia superimposed on UCL progressively reduced ipsil

Elevation of plasma concentrations of arginine vasopressin following perinatal asphyxia.

Hypoxic ischemic encephalopathy is a major cause of mortality in neonates. Studies in experimental subjects have shown differing responses of plasma arginine vasopressin to hypoxia. Plasma arginine vasopressin levels, serum osmolality, urine osmolality and fluid intakes were measured in thirteen asphyxiated and nineteen control newborn infants during the first seventy-two hours of life. In the asphyxiated infants plasma arginine vasopressin was found to be elevated as compared to control infants on days one (p less than 0.001) and two (p less than 0.007) but not on day three of life. Urine osm

Neonatal status epilepticus I: Clinical aspects.

Fifteen newborns with neonatal status epilepticus were prospectively identified from a protocol of convulsion in the newborn, at the neonatal intensive care unit. The mean postnatal age was 7.1 days, with fourteen full term babies and one premature. Previous complications during childbirth existed in twelve infants. The principal etiology associated was hypoxic-ischemic-encephalopathy, and four patients died. In thirteen cases the antecedent of subtle seizures existed; only in four cases was status epilepticus suspected. Difficulty in clinical recognition makes it necessary to perform an EEG o

Visual defects in children after cerebral hypoxia.

Visual functions were examined in 18 survivors of perinatal hypoxia/ischemia with mild to severe neurological sequelae, aged between 3 months and 17 years, and in two patients, aged 8 and 13 years, who had suffered postnatal hypoxic events. All but two patients showed clear visual deficits ranging from mild defects in visual acuity, visual field size, and/or optokinetic nystagmus to blindness. In 5 patients, the visual field was restricted to tunnel vision, a finding which appeared to be specifically related to the hypoxic/ischemic nature of the brain damage. The severity of the visual defects

Hypoxic-ischemic encephalopathy and plasma beta-endorphin.

In an attempt to determine whether hypoxic-ischemic encephalopathy in and of itself or its associated pathologic conditions lead to increased concentrations of plasma beta-endorphin (beta-ED), measurements were made in three groups of term infants. Group 1 (control) consisted of 8 infants with a mean gestation of 38.6 +/- (SE) 0.4 weeks, a mean birth weight of 3,420 +/- 150 g, and a mean postnatal age of 1.4 +/- 0.7 days. Group 2 consisted of 10 infants with a mean gestational age, birth weight and postnatal age of 40.1 +/- 0.5 weeks, 3,310 +/- 80 g and 3,9 +/- 1.1 days, and group 3 included 6

[Severe peripartum acidosis of the mature newborn infant (umbilical artery pH less than or equal to 7.00)--early morbidity and early growth].

Early morbidity and neurological development in 40 term infants with UApH less than or equal to 7,00 (6,60-7,00) were investigated. Group 1: 18 infants (45%) responded well to the measures of primary resuscitation--they never revealed any neurological symptoms and their growth and development were normal. Group 2: 15 infants (37,5%) who responded less well to primary resuscitation initially had mild to moderate neurological symptoms subsiding during the first few days of life. There were 2 cases of necrotizing enterocolitis and 4 cases of moderate meconium aspiration syndrome. Apgar scores, pH

Intracranial hemorrhage in the term newborn.

Over a five-year period we identified 22 term newborns with intracranial hemorrhage by computed tomography in an intensive care unit for newborns. Primary subarachnoid hemorrhage (diffuse or focal) was the most common type of hemorrhage. Diffuse subarachnoid hemorrhage was caused either by traumatic delivery or severe hypoxic-ischemic encephalopathy and caused seizures on the first day. Focal subarachnoid hemorrhage was associated with cerebral infarction. Intraventricular hemorrhage was always accompanied by bloody CSF. Somewhat more than half the newborns with intraventricular hemorrhage had

Factors affecting outcome in hypoxic-ischemic encephalopathy in term infants.

Forty-nine term infants were prospectively shown to have hypoxic-ischemic encephalopathy (HIE). All infants survived the neonatal period, and all but two infants (seen at 12 months) were followed up to at least 27 months of age. Factors that significantly correlated with outcome included the Sarnat encephalopathy stage and the occurrence of intractable seizures not controlled by phenobarbital sodium alone. There was no association between the one- or five-minute Apgar score, the need for early ventilation, the EEG, the occurrence of seizures, and the subsequent outcome. There was no significan

Neurotransmitter alterations in a model of perinatal hypoxic-ischemic brain injury.

Vulnerability of neurotransmitter-specific neurons to hypoxia-ischemia was examined in the immature rat corpus striatum. Carotid artery ligation plus 2 hours of 8% oxygen atmosphere at 1 week of age produced ipsilateral striatal injury and reduced hemisphere mass 2 and 6 weeks later. Striatal injury was always more severe than damage to overlying cortex. Over half the animals had status marmoratus, a neuropathological change seen in the basal ganglia and thalamus after hypoxic-ischemic injury in full-term human infants. Two weeks after the insult, markers for cholinergic, dopaminergic, and gam

Brain maturation and types of perinatal hypoxic-ischemic encephalopathy.

The degree of brain maturation and the location of hemorrhage in the germinal layer and of periventricular leukomalacia were analyzed in 85 newborn brains. The average age of the premature brains with hemorrhage was 31 weeks and of those with leukomalacia 37 weeks. Hemorrhage was more frequently found in the frontal regions, while periventricular leukomalacia predominated in the parieto-occipital regions. This study confirms that the type and the location of the hypoxic-ischemic perinatal cerebral damage are dependent on the rate of regression of the embryonic type of periventricular arterial

Hypoxic-ischemic encephalopathy in the newborn.

Many term newborns suffer some degree of perinatal asphyxia, but few become permanently brain damaged as a consequence, The newborns at risk for major neurologic handicaps have evidence of derangement in many organs, depressed cerebral function at birth that continues for days or weeks, and in many cases, convulsions soon after birth. The pathophysiologic mechanism of brain damage in hypoxic-ischemic encephalopathy remains contested, and no specific therapy is clearly beneficial. The immediate treatment of the asphyxiated term newborn requires attention to the derangements in multiple organ sy

Columnar alterations of NADH fluorescence during hypoxia-ischemia in immature rat brain.

Cerebral hypoxia-ischemia was produced in 7-day postnatal rats by unilateral carotid artery ligation combined with systemic hypoxia (8% O2). Levels of high energy phosphates, which were only slightly altered in the contralateral hemisphere, were nearly depleted in the ipsilateral hemisphere during the 3-h hypoxic insult. With hypoxia of between 1 and 3 hours' duration, columnar alterations of cortical NADH fluorescence occurred in the same location and regional pattern as did histologic damage demonstrated previously (Rice et al., 1981). In regions exhibiting columns of NADH fluorescence, ther

Hypoxic-ischemic encephalopathy in term neonates: perinatal factors and outcome.

Ninety-five infants of 37 weeks' gestation or greater with evidence of hypoxic-ischemic encephalopathy following perinatal asphyxia were prospectively identified in the neonatal period. The degree of encephalopathy was graded the staging system of Sarnat and Sarnat. Six infants died, 78 infants were sequentially followed in the Neonatal Follow-up Clinic, and in five additional infants, follow-up information was available. The mean duration of follow-up was 19.3 months. Fifty-eight (65%) of the 89 infants followed were normal or mildly handicapped, six (7%) died, and the remainder had significa

Seizures, hypoxic-ischemic brain injury, and intraventricular hemorrhage in the newborn.

The review deals with neonatal seizures, perinatal hypoxic-ischemic brain injury, and neonatal intraventricular hemorrhage. neonatal seizures are the most prominent signals of the largest number of neonatal neurological disorders. The convulsive phenomena may be subtle. The predominant etiological process is hypoxic-ischemic encephalopathy. Prognosis is related primarily to the neurological disease responsible for the seizures. Treatment may be specific for the underlying disorder (e.g., glucose or calcium) or less specific (i.e., therapy with anticonvulsant drugs). Prompt control of the seizu

Estimated cerebral blood flow in term infants with hypoxic-ischemic encephalopathy.

Estimated cerebral blood flow (eCBF) was measured sequentially in seven term infants with evidence of hypoxic-ischemic encephalopathy (HIE) and compared with that of normal-term infants. The eCBF was determined by a noninvasive method involving brief bilateral jugular venous occlusion with simultaneous measurement of occipitofrontal circumference. There was a significant decrease in eCBF on day 2 (30 +/- 4 ml/min/100 g brain weight) and on day 4 (36 +/- 5 ml/min/100 g brain weight) compared with control values (56 +/- 4 ml/min/100 g brain weight; 54 +/- 4 ml/min/100 g brain weight) (P less tha

Preliminary results of a computed tomography study of neonatal brain hypoxia--ischemia.

Sixty-five neonates who suffered hypoxia or ischemia during the perinatal period underwent computed tomography (CT) of the brain. The findings were divided into six categories: local ischemia, general ischemia, intracerebral hemorrhage, subarachnoid hemorrhage, intraventricular hemorrhage, and normal. Follow-up clinical evaluation (16 patients) and CT brain scans (5 patients) were available in some patients. The implications of CT in the prognosis of these patients is discussed.
