BACKGROUND: Although Therapeutic Hypothermia in the last two decades promises hope in reducing the problems of asphyxiated babies in developed countries, it is still not available in most parts of the world, and the results available in developing countries are not promising. OBJECTIVE: In this study, we aimed to compare Hypoxic Ischemic Encephalopathy (HIE) infants receiving total hypothermia (TH) and erythropoietin (EPO). METHODS: In a prospective cohort study, we compared the developmental prognosis of infants with HIE receiving TH (control group, N = 26) with those receiving EPO (case grou

OBJECTIVE: Hypoxic-Ischemic Encephalopathy (HIE) is a leading cause of neonatal death and long-term disabilities, leading researchers to explore novel candidate biomarkers to improve early diagnosis and guide treatment strategies. miR223, miR30a and miR30e have attracted attention as potential biomarkers for HIE due to its involvement in key cellular processes like inflammation, cell death, and neuroprotection. The rationale of this study was to examine longitudinal maternal and infant circulating miRNA blood levels from T0–T3 to 48 h of life in relation to perinatal metabolic acidosis and neo

BACKGROUND: Prenatal exposures influence childhood neurodevelopment. Placental histopathology has been associated with abnormal early childhood neurodevelopment, albeit often confounded by prematurity and/or fetal growth restriction. Most pregnant people, however, have term births, and some of these children have abnormal neurodevelopment despite the absence of adverse birth outcomes. Leveraging placental histopathology may help distinguish infants at a higher risk of subsequent neurodevelopmental impairment following a term birth.
OBJECTIVE: To investigate the association of placental histop

INTRODUCTION: Birth asphyxia remains a major contributor to neonatal morbidity and mortality. Identifying clinical and biochemical characteristics linked to mortality may help improve care in resource-limited settings. This study examined the relationship between admission clinical and biochemical features and short-term mortality among asphyxiated neonates.
METHODS: An institutional-based cross-sectional study was conducted among 288 asphyxiated neonates admitted to public NICUs in Addis Ababa between January and December 2022. Data were extracted using a pre-tested tool, and associations we

OBJECTIVE: Therapeutic hypothermia (TH) reduces mortality in neonatal hypoxic-ischemic encephalopathy (HIE), yet nearly half of treated infants suffer adverse outcomes. This study aimed to define the "Hypothermia Resistance" phenotype by characterizing the 4-day trajectories of multi-organ dysfunction and identifying the physiological signals associated with severe MRI injury.
STUDY DESIGN: This retrospective cohort study included 24 term neonates with moderate-to-severe HIE treated with whole-body hypothermia. Patients were stratified into "Responder" (n = 18) and "Resistant" (n = 6) phenoty

BACKGROUND: Despite major advances in neonatal intensive care, mortality in neonatal intensive care units (NICUs) remains a persistent reality. Most deaths now occur after withholding or withdrawing life-sustaining therapies (WWLST), yet some remain sudden or unanticipated. Understanding how and under which circumstances infants die is essential to improving anticipatory communication, ethical consistency, and family-centered support in neonatal end-of-life (EOL) care.
METHODS: We conducted a retrospective study including all infants who died in the tertiary NICU of Montpellier University Hos

BACKGROUND: To investigate the therapeutic mechanisms of miR-9-5p-overexpressing human umbilical cord mesenchymal stromal cells (hUC-MSCs) in neonatal rat models of hypoxic-ischemic brain damage (HIBD).
METHODS: Fresh neonatal umbilical cords were collected to isolate and culture human umbilical cord mesenchymal stromal cells (hUC-MSCs). Recombinant adenovirus was used to amplify miR-9-5p and transduce hUC-MSCs, generating miR-9-5p-overexpressing cells. Functional assessments included: ELISA to evaluate secretory function (e.g., neurotrophic and anti-inflammatory factors), real-time cell anal

Hydroxycarboxylic acid receptor 1 (HCAR1) is a G-protein-coupled lactate receptor expressed in the brain and plays a role in neuronal excitability and repair after injury. Hypoxic-ischemic encephalopathy (HIE) is the most common cause of brain injury and seizures in term neonates. The goal of this study was to describe HCAR1 expression and function in the neonatal brain and understand its role in HIE-associated seizures. HCAR1 expression was measured using quantitative reverse transcriptase polymerase chain reaction in postnatal day (p)10-50 mice. Neuronal properties and spontaneous excitatory

Hypoxia-ischemia (HI)-related brain injury impacts millions of neonates worldwide. Male neonates are two times more susceptible to developing HI. We have previously reported that the administration of the neurotrophin receptor tyrosine kinase B (TrkB) agonist 7,8-dihydroxyflavone (DHF) following neonatal HI increases hippocampal TrkB phosphorylation and improves hippocampal-dependent learning and memory in early adult life only in females. We hypothesize that sex differences in HI outcomes are due to alterations in neonatal hippocampal steroid content, mainly the neural testosterone. At postna

BACKGROUND: Appropriate terminology and definitions of neonatal encephalopathy (NE), hypoxic-ischemic encephalopathy (HIE), and perinatal asphyxia (PA) remain controversial. Participant criteria used in therapeutic hypothermia (TH) trials are frequently used as case definitions for NE/HIE/PA but studies are inconsistent. This review aims to assess variations in terminology and case participant criteria between trials for NE/HIE/PA.
METHODS: Search strategy retrieved articles from databases (Embase, MEDLINE, CENTRAL, CDSR and WHO) for randomized controlled trials (RCTs) of interventions for NE

BACKGROUND: Fetal growth restriction (FGR) is a major contributor to perinatal morbidity and mortality. While guidelines address timing of delivery, the optimal mode-induction of labor (IOL) versus planned cesarean delivery (CD)-remains uncertain.
OBJECTIVE: To evaluate the association between planned mode of delivery and neonatal outcomes in pregnancies complicated by late onset FGR (LOFGR).
STUDY DESIGN: We conducted a retrospective cohort study at a tertiary Canadian center (2017-2022). Singleton pregnancies with LOFGR (> 34 weeks' gestation), defined by Society for Maternal-Fetal Medicin

BACKGROUND: Neonatal hypoxic-ischemic encephalopathy (HIE) is a leading cause of neurological disability and mortality in newborns, with limited therapeutic options beyond hypothermia. Bromodomain and extra-terminal domain (BET) proteins function as epigenetic readers that regulate gene expression by recognizing acetylated lysine residues on histones. Among BET inhibitors, (+)-JQ1 (JQ1) has recently garnered attention due to its potent anti-inflammatory and antioxidant properties. This study aims to investigate the neuroprotective effects of JQ1 and elucidate the underlying mechanisms in the c

INTRODUCTION: Cortical activity is crucial for neural organisation during early life, especially in vulnerable infants who have suffered neurological compromise and/or require intensive care. However, its sensory determinants are poorly understood. In hospital, acutely unwell infants sometimes wear eye shields. We used this natural experiment to test the hypothesis that reduction of external visual inputs will attenuate occipital cortical activity.
METHODS: We analysed 9-channels video-EEG from 18 sleeping neonates (median gestational age 39 weeks (IQR 36-40); 14/18 had mild-moderate hypoxic-

OBJECTIVE: The objective of this study is to determine the predictive value of magnetic resonance imaging (MRI), amplitude-integrated electroencephalography (aEEG), the Hammersmith Neonatal Neurological Examination (HNNE), and the General Movements Assessment (GMA) for cerebral palsy (CP) in neonates with hypoxic-ischemic encephalopathy (HIE), and to evaluate whether combining these modalities improves diagnostic accuracy.
STUDY DESIGN: In this prospective two-center cohort study, 53 term or late-preterm infants with HIE treated with standardized therapeutic hypothermia (33.5°C for 72 hours)

Neuropathological examination plays a critical role in medico-legal cause-of-death investigation, especially in determining the cause and manner of death in pediatric autopsies. Although a comprehensive neuropathological examination is recommended, limited data exists of the diagnostic yield of neuropathology consultations in such cases. This retrospective register-based study reviewed Finnish pediatric medico-legal autopsies from 2016 to 2022 involving full neuropathological examination performed by a neuropathologist, with a focus on age-related differences in consultation rates, themes, and

BACKGROUND: Therapeutic hypothermia improves survival and neurodevelopmental outcomes in neonates with hypoxic-ischemic encephalopathy when initiated within 6 hours of birth. However, in low- and middle-income countries, delays in referral and access to tertiary care often preclude early initiation and the benefits of therapeutic hypothermia beyond the recommended window remain uncertain. We aimed to assess whether initiating therapeutic hypothermia between 6 and 12 hours after birth is associated with a higher risk of mortality and/or brain injury than initiation within 6 hours in neonates wi

Neonatal hypoxic-ischemic encephalopathy (HIE) is frequently complicated by seizures that persist despite therapeutic hypothermia (HT), suggesting injury mechanisms insensitive to HT. Here, we tested the hypothesis that astrocytic glutamate-aspartate transporter (GLAST) abnormalities in the neocortex contribute to cortical hyperexcitability and seizure burden after HIE, and that HT mitigates this astrocyte-mediated mechanism. We examined the vulnerability of GLAST in the neocortex of human neonatal hypoxic-ischemic encephalopathy (HIE) and in a piglet model of hypoxia-ischemia (HI). We determi

INTRODUCTION: Neonatal encephalopathy, particularly situations involving neonatal hypoxic-ischaemic encephalopathy (NHIE) and neonatal stroke (NS), covers complex neonatal conditions associated with a risk of neurodevelopmental difficulties. Early prevention programmes have shown benefits for neurodevelopment, but few address both NHIE and NS populations. Following an initial joint development phase with families and professionals, we identified two areas of intervention: (1) Making infant care and referral more streamlined and (2) Increasing parent empowerment. The PRevention and support for

Neonatal encephalopathy (NE) following perinatal hypoxia-ischemia (HI) can lead to a wide range of neurodevelopmental consequences, ranging from mild motor problems to severe cerebral palsy, epilepsy, cognitive impairments, and behavioral issues. There is growing recognition of the long-term effects of NE during childhood, but little is known about the outcomes in adulthood, particularly from the perspective of the individuals affected and their families. Existing research primarily relies on outcome measures from neurodevelopmental assessments, which often fail to capture the lived experience
