Beyond hypoxic-ischemic encephalopathy: genetic insights and precision diagnosis in neonatal encephalopathies.

Neonatal encephalopathies are a heterogeneous group of early-onset neurological disorders. While hypoxic-ischemic encephalopathy (HIE) has long been considered the predominant cause, a growing number of genetic, metabolic, immune-mediated, and toxic etiologies may clinically mimic HIE, posing diagnostic challenges. Timely recognition of these alternative causes is essential to initiate targeted therapies and provide appropriate family counseling. This article reviews the main clinical presentations and pathogenetic mechanisms of neonatal encephalopathies and introduces a structured diagnostic

Developmental outcomes in infants treated with therapeutic hypothermia before and during the COVID-19 pandemic.

BACKGROUND: Infants with hypoxic ischemic encephalopathy (HIE) are at high-risk for adverse neurodevelopmental outcomes, which may be further impacted by disruptions to social and healthcare systems such as during the COVID-19 pandemic. This study aimed to compare outcomes of infants with HIE born before versus during the pandemic. METHODOLOGY: We retrospectively compared neurodevelopmental outcomes in infants with HIE born pre-pandemic (10/2017-12/2019) versus during the pandemic (03/2020-09/2021), using the Bayley Scales of Infant Development-4 at 12 and 18-24 months. RESULTS: Among 73 inf

Development and validation of a machine learning-based early prediction model for fetal acidosis in vaginal deliveries.

UNLABELLED: Fetal acidosis (FA) is a prevalent clinical symptom in newborns during the perinatal period, which can lead to higher risks of hypoxic ischemic encephalopathy, seizures, intraventricular hemorrhage and delayed developmental milestones. However, there is no effective tool for the risk assessment and prediction of FA in neonates. Therefore, our objective is to develop a FA prediction model in newborns by using machine-learning algorithms. Parturients and their neonates who delivered at the Fuzhou University Affiliated Provincial Hospital between January 2018 and December 2023 were in

Expert Consensus Approach to Developing Inpatient Common Data Elements for Neonatal Encephalopathy Research.

BACKGROUND AND OBJECTIVES: Neonatal encephalopathy (NE)-including hypoxic-ischemic encephalopathy (HIE)-is associated with significant morbidity and mortality worldwide. While data registries provide hypothesis-generating data, aid in quality improvement, and track management/outcomes over time, we recently demonstrated that only 4 of 1,281 (0.3%) variables were collected by all 22 international NE/HIE registries: birth weight, gestational age, and 1- and 5-minute Apgar scores. In response, our team set out to develop a set of common inpatient data elements to enable future harmonization of NE

Mild neonatal hypoxic-ischemic encephalopathy: current evidence and ongoing controversies.

Neonatal hypoxic-ischemic encephalopathy (HIE) remains a leading cause of neonatal morbidity and mortality worldwide. Therapeutic hypothermia (TH) is an established neuroprotective therapy for infants with moderate to severe HIE; however, its role in the management of mild HIE remains uncertain. This review summarizes current evidence regarding neurodevelopmental outcomes in infants with mild HIE and critically evaluates available data on the use of TH in this population. Although mild HIE was excluded from the original hypothermia trials, emerging observational studies suggest that a proporti

Hypoxic-Ischemic Encephalopathy in Newborns: Pathophysiology, Early Identification, and Management.

Hypoxic-ischemic encephalopathy (HIE) is a severe neurological condition resulting from impaired oxygen delivery and reduced cerebral blood flow to the neonatal brain during the perinatal period. It remains one of the leading causes of neonatal mortality and long-term neurodevelopmental disability worldwide, despite significant advances in obstetric and neonatal care. The pathophysiology of HIE is complex and evolves through a cascade of events beginning with primary energy failure, followed by secondary energy failure characterized by mitochondrial dysfunction, oxidative stress, inflammation,

Neonatal Outcomes in Hypoxic Ischemic Encephalopathy Exposed to Chorioamnionitis, a Retrospective Cohort Study.

BACKGROUND: Hypoxic ischemic encephalopathy (HIE) leads to neonatal mortality and long-term morbidities. Clinical and histological chorioamnionitis is an additional risk factor for neonates with HIE, which could increase morbidity and brain injury. We aimed to assess the short-term outcomes in neonates with HIE exposed to chorioamnionitis. METHODS: We conducted a retrospective cohort study using the Canadian Neonatal Network database. We included neonates with ≥35 weeks gestational age with HIE treated with therapeutic hypothermia (TH) born between January 2011 and December 2021. Chorioamnion

Inflammatory indices and HALP score as accessible biomarkers of severity in neonatal hypoxic-ischemic encephalopathy.

BACKGROUND: Hypoxic-ischemic encephalopathy (HIE) involves systemic inflammatory responses that may be reflected by routine blood-derived indices. OBJECTIVE: To evaluate the associations of systemic inflammatory indices and the hemoglobin-albumin-lymphocyte-platelet (HALP) score with HIE severity and short-term outcomes, and to characterize their temporal changes from the pre-hypothermia measurement to the post-treatment measurement at 72-96 postnatal hours. METHODS: This retrospective study included 555 neonates: 185 with mild HIE, 185 with moderate-to-severe HIE treated with therapeutic hy

Melatonin treatment for neonatal encephalopathy.

RATIONALE: Neonatal encephalopathy (NE) affects 1.15 million newborns annually and is associated with a high incidence of mortality and long-term neurodevelopmental disability (NDD). Therapeutic hypothermia (TH) is the only proven therapeutic intervention available. However, it is only partially effective, and evidence suggests it is not safe in low-income countries. Melatonin is a promising neuroprotective treatment in NE as it provides anti-inflammatory properties. Pre-clinical models have demonstrated improved outcomes with melatonin. However, its effect on human newborns is unclear. OBJEC

Neonatal PBPK model of midazolam and its metabolites for perinatal asphyxia and therapeutic hypothermia.

BACKGROUND AND PURPOSE: Therapeutic hypothermia (TH) decreases morbidity and mortality in neonates with perinatal asphyxia (PA) and moderate-to-severe hypoxic-ischaemic encephalopathy. Midazolam is an antiepileptic and substrate of CYP3A4. Drug metabolism during PA/TH remains partially characterized, and disentangling effects of PA and TH is difficult, making precision dosing challenging. The I-PREDICT project aims to develop a framework for drug disposition in PA/TH neonates, using physiologically based pharmacokinetic (PBPK) modelling. The predictive performance of the model was explored for
