Electrolyte imbalance in Asphyxiated term neonates: Incidence, predictors, and outcomes from a prospective cohort study in Northern Uganda.

BACKGROUND: Birth asphyxia is a major cause of neonatal morbidity and mortality, particularly in low-resource settings. Hypoxia and metabolic derangements that occur during asphyxia predispose neonates to electrolyte abnormalities, which may worsen the clinical course and contribute to poor outcomes. Early recognition and management of such imbalances can improve survival and prevent long-term neurological damage. This study aimed to determine the incidence, predictors, and early outcomes of electrolyte imbalance among term neonates admitted with birth asphyxia in Lira Regional Referral Hospit

Classic nonketotic hyperglycinemia and symmetrical intrauterine growth retardation, diagnostic challenges, and fatal outcome: a case report and review of the literature.

BACKGROUND: Nonketotic hyperglycinemia is a rare metabolic disorder caused by glycine accumulation due to defects in the glycine cleavage system. While severe metabolic disorders can theoretically affect fetal growth, nonketotic hyperglycinemia is not recognized as a common or typical cause of symmetric intrauterine growth retardation. CASE PRESENTATION: This case report describes a Iranian 36-week male neonate with symmetrical intrauterine growth restriction, born to consanguineous parents with a history of preeclampsia and prior fetal demise. Despite initial stabilization, the infant develo

Neonatal Hypoxic-Ischemic Encephalopathy: From the Limitations of Therapeutic Hypothermia to Precision Intervention.

Therapeutic hypothermia (TH) is the current standard of care for moderate-to-severe neonatal hypoxic-ischemic encephalopathy (HIE). However, its clinical application faces significant limitations, including a 44-55% rate of death or disability, a narrow 6‑h therapeutic window, inequitable global access, and inconsistent patient selection. These challenges highlight the need for emerging strategies that may complement or integrated with TH in the future. This review systematically synthesizes emerging therapeutic strategies beyond TH, encompassing four cutting-edge directions: first, targeting

Hemodynamic effects of umbilical cord milking versus early cord clamping in nonvigorous intrauterine growth restricted neonates: a randomized controlled trial.

UNLABELLED: The effect of placental transfusion on the nonvigorous intrauterine growth restricted (IUGR) neonates with pre-existing subclinical myocardial dysfunction and altered cerebral hemodynamics is largely unknown. The present study was planned to evaluate the effect of umbilical cord milking (UCM) on left ventricular output (LVO) of nonvigorous IUGR neonates of > 28-weeks' gestation. In the present randomized control trial, 74 nonvigorous IUGR neonates were randomly allocated to UCM (4-times squeezing of 20-cm intact cord, n = 37) and ECC (cord clamping within 30 s, n = 37). The primary

Screening for large-for-gestational-age neonates at term: evidence of a labeling effect and increased intervention without neonatal benefit.

BACKGROUND: Routine third-trimester ultrasound screening for suspected large-for-gestational-age (LGA) and macrosomic fetuses is increasingly used to guide obstetric management and reduce delivery-related morbidity. However, ultrasound estimation of fetal weight at term has limited diagnostic accuracy, and evidence supporting interventions to reduce morbidity in large fetuses remains limited. Together, these uncertainties raise questions regarding the benefit of routine ultrasound screening for fetal largeness at term. OBJECTIVE: To evaluate the diagnostic accuracy of routine third-trimester

Alterations of telomere-associated genes and underlying mechanisms in hypoxic-ischemic encephalopathy.

BACKGROUND: Hypoxic-ischemic encephalopathy (HIE) is a severe neurological disorder with complex pathogenesis. The role of telomere-associated genes in HIE remains unclear. This study aims to investigate their alterations and mechanisms to provide new therapeutic insights. METHODS: Telomere-associated differentially expressed genes (Telomere-DEGs) were identified from neonatal mouse HIE transcriptome datasets (GSE23317, GSE144456) in GEO. GO and KEGG enrichment analyses were performed. A protein-protein interaction network was constructed using STRING, and the best Telomere-DEGs (BTDGs) were

Neonatal outcomes related to parental holding during therapeutic hypothermia for neonates with hypoxic ischemic encephalopathy: a systematic review protocol.

OBJECTIVE: This review will evaluate neonatal outcomes related to parental holding during therapeutic hypothermia for treatment of hypoxic ischemic encephalopathy in neonates. INTRODUCTION: Neonates diagnosed with or at risk for hypoxic ischemic encephalopathy, an acquired brain injury at birth, receive therapeutic hypothermia as a neuroprotective intervention. However, despite a lack of evidence on associated harm, parents are often restricted from holding the newborn until the therapeutic hypothermia treatment is completed. ELIGIBILITY CRITERIA: The review will include all experimental and

Necrotizing Enterocolitis in a Term Newborn With Hypoxic-Ischemic Encephalopathy.

Necrotizing enterocolitis (NEC) predominantly affects preterm infants and remains uncommon in term neonates. When it occurs in full-term newborns, it usually reflects underlying vulnerability or cumulative perinatal risk factors. We report the case of a term male newborn (40 + 1 weeks' gestation) who developed NEC in the context of multiple risk factors, including perinatal asphyxia with hypoxic-ischemic encephalopathy managed by therapeutic hypothermia, suspected early-onset neonatal infection requiring empirical antibiotics, and formula feeding. On day 4 of life, the infant presented with si

EXPRESS: NEUROPROTECTIVE EFFICACY OF AN OMEGA-3 DIGLYCERIDE EMULSION IN A PIGLET MODEL OF NEWBORN HYPOXIC-ISCHEMIC BRAIN DAMAGE.

BACKGROUND: Acute injection of omega-3 fatty acids (n-3) formulated in a novel diglyceride (DG) lipid emulsion is neuroprotective after hypoxic-ischemic encephalopathy (HIE) in neonatal rodents. METHODS: the efficacy of acute n-3 DG emulsion treatment was tested in 2-3-day-old piglets subjected to bilateral carotid artery clamping and 8% O₂ exposure for 30 min. Animals were randomly assigned to normothermia or hypothermia (HT, 34 ± 0.5 °C), then to n-3 DG or vehicle administration 30 min post-HI. Brain activity was assessed by amplitude-integrated EEG over 6 h. Brain injury was analyzed by TU

Intracranial hemorrhage identified after initiating therapeutic hypothermia: two case reports of neonatal hypoxic-ischemic encephalopathy due to hypovolemic shock with anemia: a case report.

BACKGROUND: Therapeutic hypothermia for moderate-to-severe hypoxic-ischemic encephalopathy in neonates effectively improves neurological outcomes when initiated within 6 hours of birth. However, coagulopathy is a potential side effect of therapeutic hypothermia and requires careful monitoring for signs of hemorrhage. Uncontrolled hemorrhage is the primary exclusion criterion for therapeutic hypothermia. Herein, we report two cases of intracranial hemorrhage that, despite being massive enough to cause hypovolemic shock with anemia, could not be detected by cranial ultrasonography before the ini

Timing-Dependent Effects of Dexamethasone in a Mouse Model of Neonatal Hypoxic-Ischemic Brain Injury: A Transcriptomic Analysis of Synaptic Signaling and Calcium Homeostasis.

The optimal timing and therapeutic role of dexamethasone for neuroprotection in neonatal hypoxic-ischemic (HI) brain injury remain unclear. We investigated whether dexamethasone-mediated neuroprotection is time-dependent and explored its underlying molecular mechanisms in a neonatal HI mouse model. The Rice-Vannucci model (unilateral carotid artery ligation followed by 8% O2 for 90 min) was constructed utilizing postnatal day 7 mice who received vehicle (n = 5), dexamethasone pre-treatment (0.5 mg/kg, 6 h before HI; n = 6), or dexamethasone post-treatment (0.5 mg/kg, 6 h after HI; n = 6). The Involvement of HIF-1α and BDNF in Neonatal Hypoxic-Ischemic Insult to the Cerebral Germinal Matrix.

Perinatal asphyxia is a major contributor to neonatal morbidity and mortality, particularly among preterm infants, whose brains are highly vulnerable to hypoxic-ischemic injury. The germinal matrix (GM), owing to its vascular fragility and high metabolic demand, is especially susceptible in this context. This study analyzed 118 germinal matrix samples from neonates, stratified into three groups according to gestational age-Extremely Preterm (EP), Late Preterm (LP), and Term (T)-to investigate the immunopositivity of hypoxia-inducible factor 1-alpha (HIF-1α) and brain-derived neurotrophic facto
