INTRODUCTION: Mycetoma is a chronic granulomatous infection, common throughout tropical regions, and is considered a neglected disease that mostly affects impoverished populations. Mycetoma is divided into eumycetoma, caused by fungi, and actinomycetoma, caused by filamentous bacteria. Clinical presentation is distinctive, and making the diagnosis is usually not difficult; however, access to safe and effective treatments is a major challenge. There is not a single best treatment, with the choice of treatment depending on etiology, severity and extent of disease, and patient comorbidities.

PURPOSE: Cutaneous botryomycosis is a chronic, granulomatous infection of the skin caused by different bacteria that form granular aggregates. We aim to provide a complete, comprehensive, and updated review of cutaneous botryomycosis, including a review of cases published in the last decade.
RESULTS: Patients with botryomycosis typically present multiple nodules, sinuses, abscesses, fistulae, and ulcers, which develop during weeks or months. The current gold standard for diagnosing botryomycosis involves a combination of histopathological examination and microbial culture. A definitive diagno

Actinomycosis is a rare gram-positive bacterial infection, and disseminated actinomycosis (DA) is even more rare in the modern antibiotic era. Histopathologic and microbiological confirmation are the gold standard, and often organisms are difficult to isolate from the culture specimen. We present a 42-year-old immunocompetent patient with DA, who presented initially with right foot swelling and sinus tract with discharge and subsequently went on to present with multifocal involvement, particularly cardiac involvement, which was unresponsive to penicillin, on 68 Ga-FAPI-46 PET/CT.

Skin-related neglected tropical diseases (NTDs) represent an emerging public health and economic challenge in India. In the present prospective observational study, out-of-pocket expenditure and catastrophic health expenditure among patients diagnosed with mycetoma, subcutaneous mycoses, leishmaniasis, and cutaneous tuberculosis and nontuberculous mycobacterial (TB-NTM) infections were assessed. Using structured questionnaires, direct and indirect costs were recorded over a 6-month period. Of the 21 patients enrolled (median age 42 years), 42.9% were household heads and 61.9% lived below the i

There has been a substantial increase in the number of cases of invasive fungal infections worldwide, which is associated with a growing number of immunosuppressed patients and a rise in antifungal resistance. Some fungi that were previously considered harmless to humans have become emerging pathogens. One of them is Purpureocillium lilacinum, a ubiquitous filamentous fungus commonly found in the environment, especially in the air and soil. P. lilacinum belongs to a bigger group of hyaline fungi that cause hyalohyphomycosis, a fungal infection caused by fungi with colorless hyphae. Although th

Scedosporium, a widespread filamentous fungus found in diverse environments, has experienced a rise in cases due to escalating malignancies and chronic immunosuppression. Clinical manifestations span mycetoma, airway involvement, and various infections, with osteomyelitis being a notable complication. We present a case of a 77-year-old female initially displaying cutaneous Scedosporium signs, which progressed to osteomyelitis. The patient, with a history of trauma, chronic low dose steroid use, and underlying conditions, presented with a foot injury caused by her dog. Despite initial managemen

BACKGROUND: To date, surgery is the best approach to maximize a cure for symptomatic aspergilloma, but this is not without risk of both morbidity and mortality. The aim of this study is to present the characteristics and outcomes of 77 patients diagnosed with aspergilloma who underwent surgery at Chiang Mai University Hospital (CMUH), and to identify risk factors for composite major postoperative complications (CMPC).
METHODS: This is an observational retrospective cohort study carried out at CMUH over a period of 11 years from January 1, 2010, to February 28, 2021. Patient characteristics an

In light of a collapsing healthcare system in Sudan, attacks on healthcare institutions and staff have markedly increased since the eruption of war on the 15th of April, costing many precious lives and valuable hospitals. Around 60 attacks on health facilities have been reported so far, many occupied by one or the other sides of the conflict, and the rest exhibit medication shortages and safety issues; hence, two-thirds of the medical centers are nonfunctioning. More than 200 violations against medical staff were recorded all over the country, which led to the death of 38 healthcare workers. K

Pulmonary tuberculosis is a notorious disease as it can cause severe morbidity and mortality. In this case, we discuss a 75-year-old male tuberculosis patient from a rural area with no underlying comorbidities who failed to continue anti-tubercular medication after two months. The case discusses the diagnostic modalities confirming the diagnosis, sputum culture for Mycobacterium tuberculosis, imaging studies, including X-ray and CT of the chest, and laboratory parameters for identifying pulmonary mycetoma. The patient is now on anti-tubercular therapy (isoniazid, rifampicin, pyrazinamide, and

Intracavitary pulmonary aspergilloma is a persistent and life-threatening infection that carries a mortality rate of up to 15%. It occurs when Aspergillus species gain entry to an existing lung cavity. In the absence of definitive treatment, patients may succumb to severe complications such as massive hemoptysis, cachexia, or secondary infections. Aspergillomas often show limited response to antifungal medications, mainly due to insufficient drug concentrations within the cavities. Surgery is frequently the preferred treatment option, but it poses significant risks, and many individuals are in

Aspergilloma also known as fungal ball or mycetoma, is a saprophytic mycotic infection caused by Aspergillus species which usually colonizes pre-existing cavitary or cystic lesions in the lung. Here, we have a rare case of idiopathic pulmonary fibrosis (IPF) with bilateral bronchiectasis complicated by aspergilloma. Although the existence of aspergilloma is common in pre-existing lung cavities, its coexistence in patients with IPF is a rarity, and the incidence of such cases in the literature remains sparse. Here is an interesting case report of aspergilloma co-existing with IPF. This article

Using two databases, this bibliometric analysis was done for the papers published by the Faculty of Medicine, University of Khartoum (FMUK), from 2019 to 2023. Data were extracted from SCImago for all Sudan, and from PubMed for the publications by FMUK and its associated research centres, the Institute of Endemic Diseases, and the Mycetoma Research Center. The analysis of publications included the count and type of publications, the journals, and national and international collaboration assessment. The publications from FMUK show improvement over time in number and quality, a growth that is si

OBJECTIVES: The aim was to assess the developmental attributes of sphenoid sinuses affected by fungal balls and describe a surgical approach which reestablishes gravity-dependent drainage to compensate for any mucociliary dysfunction.
METHODS: A within-patient case-control analysis was performed on sphenoid sinus dimensions from patients with sphenoid sinus fungal ball (SSFB). Radiological dimensions were assessed to determine the dominant or larger sinus by volume and width. Pneumatization in the sagittal and lateral extent was assessed. The influence of sinus size and pneumatization variant

BACKGROUND: Nakaseomyces glabratus (N. glabratus) formerly known as Candida glabrata (C. glabrata), is an endogenous opportunistic pathogen, which is generally located in the gastrointestinal tract but can spread in immunocompromised patients. N. glabratus is the second most common pathogen that causes candidemia in several countries. N. glabratus virulence factors may increase antifungal resistance and reduce the number of available treatment options. High resistance to azoles and increasing resistance to echinocandins have been previously reported in N. glabratus.

BACKGROUND: Massive hemoptysis (MH) is a serious condition that carries with it a high rate of morbidity and mortality.
OBJECTIVE: This review highlights the pearls and pitfalls of massive hemoptysis, including presentation, diagnosis, and management in the emergency department (ED) based on current evidence.
DISCUSSION: MH is a rare but deadly condition. It is defined clinically as any bleeding from the tracheobronchial tree that compromises respiratory or circulatory function. The bronchial artery system is the primary source in the majority of cases of MH. The most common cause is tubercu

A 46-year-old farmer living in a rural area, who sustained an injury to his left foot 10 years ago, consulted due to progressive swelling of the same foot. An MRI revealed the "dot-in-circle" sign, suggestive of mycetoma of the foot, also known as Madura foot. This condition primarily affects soft tissues and bones and can occasionally lead to potentially severe visceral complications. Foot involvement is predominant, observed in 80% of cases.
