Mycetoma: A critical appraisal of educational content on YouTube.

BACKGROUND: Mycetoma is a fungal neglected tropical disease. Accurate dissemination of information is critical in endemic areas. YouTube, a popular platform for health information, hosts numerous videos on mycetoma, but the quality and reliability of these videos remain largely unassessed. METHODS: We used modified DISCERN and Global Quality Score (GQS) for reliability and quality respectively. Video duration, views, likes, and comments were recorded. Spearman's rank correlation and Mann-Whitney U tests were used to identify correlations between metrices and quality scores. RESULTS: We inclu

Mycetoma Leading to Effusive Constrictive Pericarditis in an Immunocompetent Individual: A Rare Encounter.

Nocardial infections are rare but serious, often leading to systemic and cardiopulmonary complications. This is the first reported case of Nocardia beijingensis causing constrictive pericarditis in an immunocompetent individual. We present a 37-year-old Caucasian female patient with no significant medical history who developed pericarditis symptoms after handling crates from China. Despite initial treatment for presumed viral pericarditis, her condition worsened. Further investigation identified Nocardia beijingensis as the cause of effusive constrictive pericarditis. The patient underwent a p

Eumycetoma Caused by Aspergillus terreus: A Case Report and Literature Review.

Mycetoma is a granulomatous disease of subcutaneous tissue that is caused by different species of aerobic filamentous bacteria (actinomycetoma) or fungi (eumycetoma). Mycetoma treatment depends on the causative pathogen, and it mainly consists of antimicrobial interventions or surgery. Aspergillus terreus is an etiologic agent of invasive aspergillosis and a rare cause of eumycetoma that is common in central and southern China. The present case report describes a 45-year-old female patient with subcutaneous swelling and multiple active and healed sinuses over the dorsal aspect of the left foot

Dot-in-circle sign in cervical actinomycotic mycetoma: An extremely rare case report.

Actinomycosis is an unusual, chronic granulomatous infection caused by Actinomycetes spp. The organism also causes mycetoma, a neglected tropical disease in endemic regions. We present a very uncommon case of extensive actinomycosis of the soft tissues in the neck with perivertebral extension that showed the dot-in-circle sign on magnetic resonance imaging. A 29-year-old male patient presented with progressively enlarging hard posterior neck swelling of 4 years duration. Subsequently, magnetic resonance imaging showed the dot-in-circle sign in an avidly enhancing infiltrative lesion with exten

Early presentation of mycetoma (Madura Foot): A case report.

Mycetoma, commonly known as Madura foot, is a chronic and progressively destructive granulomatous disease caused by a fungus or anaerobic filamentous bacteria that affects the skin, subcutaneous tissue, and bones primarily in tropical and subtropical regions, with males between the ages of 20-40 having occupational exposure to outdoor environments, such as farming, predominantly affected. It is one of the World Health Organization's 17 "neglected tropical diseases," characterized by a clinical trial of localized mass-like soft tissue injury with draining sinuses that discharge grains of infect

Primary actinomycotic osteomyelitis of metacarpal bones managed successfully with surgical debridement and Welsh regimen.

Mycetoma of the hand is a chronic granulomatous disease seen in the tropics and subtropical regions, mainly affecting the skin and subcutaneous tissue of the foot. Primary actinomycotic osteomyelitis involving the metacarpals is rarely reported in the literature. The conventional treatment for actinomycosis has been high-dose penicillin. We report a case of chronic osteomyelitis in the metacarpals of the hand caused by actinomycetes in a woman in her 40s. She underwent surgical intervention in the form of wound debridement and curettage, followed by a 6-month course of Welsh regimen after a bi

Fungal ball due to a species of the Scedosporium apiospermum complex in a post-tuberculosis patient.

BACKGROUND: Scedosporium species are considered emerging pathogens causing illness in immunocompetent and immunocompromised hosts. CASE REPORT: A case of non-invasive pulmonary (fungal ball) infection by Scedosporium apiospermum complex in a 49-year-old female with bronchiectasis and cavities secondary to tuberculosis is described. The patient had a history of three years of cough and hemoptysis. A computed tomography scan of the thorax revealed the presence of a cavity in the lower lobe of the right lung, associated with bronchiectasis. A combination of surgical debridement and antifungal th

Evaluation of secondary cytoreduction surgery in platinum-resistant ovarian cancer patients within three-line recurrent: a multicenter, randomized controlled study.

BACKGROUND: Central dark-signal intensity with high-signal, hypertrophic mucosal wall of paranasal sinuses on T2-weighted images (T2WI) is a characteristic magnetic resonance imaging (MRI) feature of sinonasal fungus ball. However, this finding is usually interpreted as non-fungal chronic sinusitis with central normal sinus air. In addition, T1-weighted images (T1WI) and T2WI are basic sequences of all magnetic resonance (MR) examinations. Therefore, we evaluated the usefulness of T1WI for detecting fungus balls comparing with computed tomography (CT) findings and T2-weighted MRI findings. ME

Histopathology in the Diagnosis of Tinea Capitis: When to Do, How to Interpret?

The diagnosis of tinea capitis is usually made by clinical signs and direct microscopic examination. Early diagnosis of this dermatophytic infection, which may cause permanent hair loss if not treated appropriately, is very crucial. In recent years, the use of dermoscopy has helped with early diagnosis. However, when tinea capitis has an atypical course and develops in adulthood, it can be confused with several diseases, such as psoriasis, seborrheic dermatitis, folliculitis decalvans, acne keloidalis, and dissecting cellulitis. Due to the different treatment approaches and prognoses, it is im

Hydatid Disease: A Radiological Pictorial Review of a Great Neoplasms Mimicker.

Hydatid cyst is a common name for the larval stage of a tapeworm species of Echinococcus granulosus, which is transmitted from animals to humans via the fecal-oral route. Hydatid cysts predominantly affect the liver (75%), followed by the lung (15%), and they can affect many organs in the human body. Medical imaging modalities are the keystone for the diagnosis of hydatid cysts with high sensitivity and specificity. Ultrasound imaging with high resolution is the first choice for diagnosis, differential diagnosis, staging, establishing a role in interventional management, and follow-up, and it

Pulmonary Mycetoma With a Concomitant Reactivation of Pulmonary Tuberculosis Infection: A Case Report and Clinical Pathological Review.

Tuberculosis is a bacterial infection caused by Mycobacterium tuberculosis. It primarily affects the lungs but can also spread to other body parts. One of the possible symptoms of pulmonary tuberculosis (TB) is hemoptysis. In the case of TB, aspergillomas can develop in the cavitary lesions of TB and result in a deteriorating clinical situation. The current case report describes a 63-year-old female previously treated for TB who presented with hemoptysis, fever, and a 4 cm focal density in the right upper lobe on chest X-ray. The patient was found to have concomitant TB and aspergillosis, mani

Evaluation of the Antiparasitic and Antifungal Activities of Synthetic Piperlongumine-Type Cinnamide Derivatives: Booster Effect by Halogen Substituents.

A series of synthetic N-acylpyrrolidone and -piperidone derivatives of the natural alkaloid piperlongumine were prepared and tested for their activities against Leishmania major and Toxoplasma gondii parasites. Replacement of one of the aryl meta-methoxy groups by halogens such as chlorine, bromine and iodine led to distinctly increased antiparasitic activities. For instance, the new bromo- and iodo-substituted compounds 3 b/c and 4 b/c showed strong activity against L. major promastigotes (IC50 =4.5-5.8 μM). Their activities against L. major amastigotes were moderate. In addition, the new com

A Detailed Review of Surgical Management of Uncommon Cutaneous Disorders.

A variety of cutaneous disorders can present to the general surgeon either directly or by referral for surgical intervention. Some conditions are commonly seen and operated on by general surgeons which include lipoma, epidermoid cyst, etc. On the other hand, some are uncommon conditions like dermatofibrosarcoma protuberans and chondroid syringoma which require a high index of suspicion to diagnose. Most general surgeons are not familiar with the latest guidelines to treat such uncommon conditions. In this article, we provide a review of uncommon cutaneous disorders requiring surgical intervent

Burden of fungal infections in Kenya.

The burden of fungal infections has been on the rise globally and remains a significant public health concern in Kenya. We estimated the incidence and prevalence of fungal infections using all mycology publications in Kenya up to January 2023, and from neighbouring countries where data lacked. We used deterministic modelling using populations at risk to calculate the disease burden. The total burden of serious fungal infections is estimated to affect 6,328,294 persons which translates to 11.57% of the Kenyan population. Those suffering from chronic infections such as chronic pulmonary aspergil
