Diagnosis and Management of a Long-Standing Eumycetoma of the Wrist in Somalia: A Case Report in a Resource-Limited Setting.

INTRODUCTION: Mycetoma is a chronic, progressive, granulomatous inflammatory disease affecting subcutaneous tissues, most commonly caused by traumatic inoculation of certain fungi (eumycetoma) or bacteria (actinomycetoma). Common etiological agents for eumycetoma include Madurella mycetomatis and Trematosphaeria grisea, while actinomycetoma is frequently caused by Nocardia or Streptomyces species. It is endemic in tropical and subtropical regions, including Somalia. The clinical triad of a painless subcutaneous mass, multiple draining sinuses, and the presence of grains in the discharge is cha

Actinomycetoma is a rare, chronic, and progressive bacterial and fungal infection that affects subcutaneous tissues, fascia, and bones. This a case of a 27-year-old Sudanese male, shepherd, with a mycetoma lesion in the right hand caused by Streptomyces fradiae. He presented with swelling and deformity of the hand, and subsequent imaging showed soft tissue swelling, deep infiltration of the hand and sinus formation. A skin biopsy of the lesion confirmed the diagnosis of actinomycetoma, and the patient was treated with ceftriaxone IV for 14 days followed by oral amoxicillin clavulanic acid for

Left Gluteal and Perianal Eumycetoma Lesion Extending Into the Left Greater Sciatic Foramen, Pelvic Cavity to the External Anal Sphincter.

Mycetoma is a chronic subcutaneous infection that primarily affects the lower extremities, although atypical anatomical presentations are possible. We describe a Sudanese farmer, age 31, who developed a left gluteal eumycetoma due to Madurella mycetomatis. Magnetic resonance imaging (MRI) showed widespread deep tissue involvement that extended into the pelvic cavity, sciatic foramen, and external anal sphincter despite a minor external lesion. Over the course of a year, the patient showed clinical improvement with lesion encapsulation while receiving oral itraconazole. In order to prevent adva

The 'Madura Hand'.

Mycetoma or maduramycosis is a chronic deforming granulomatous musculoskeletal infection caused by bacteria (Actinomycetes sp) or fungi, most notably, Madurella mycetomatis. Approximately 70% of infections involve the foot, followed by hands. The diagnosis is suspected on the basis of a clinical triad of painless subcutaneous mass, multiple sinus tracts, and characteristic grain discharge. Histological examination of the discharge or biopsy is helpful for differentiating in between the causative organism. Imaging plays a crucial role in the management and prognostication of mycetoma by delinea

Eumycetoma Caused by Acremonium Species in a Farmer From Western Guatemala: A Case Report.

Eumycetoma is a chronic subcutaneous infection caused by filamentous fungi and is clinically characterized by swelling, nodules, sinus tracts, and grain discharge. Acremonium species are rare opportunistic fungal pathogens implicated in eumycetoma, with few reports in the dermatologic literature. This report describes a 64-year-old male farmer from rural western Guatemala with a 14-year history of chronic dermatosis localized to the right foot. Clinical examination identified nodular lesions, crusting, and edema, consistent with mycetoma. Diagnosis was confirmed through mycological studies and

Dots, Circles, and Deception: Radiological Diagnosis of an Unusual Mycetoma.

Mycetoma is a chronic granulomatous fungal or bacterial infection of the subcutaneous tissues, often with a characteristic clinical pattern, but in some patients, the presentation may be subtle or atypical, which can complicate diagnosis. Here, we report an unusual and rare presentation of mycetoma in a 26-year-old male who developed a chronic, progressively enlarging swelling over the leg. The lesion clinically resembled a soft tissue tumor, with no discharging sinuses present throughout the course of evaluation. Initial clinical impression favored a neoplastic or inflammatory etiology rather

ReVAR: a data-driven algorithm for generating aero-optic phase screens.

The propagation of light through a turbulent flow field around an aircraft results in optical distortions commonly known as aero-optic effects. The development of methods to mitigate these effects requires large amounts of realistic aero-optic data. However, methods for obtaining these data, including experiment, computational fluid dynamics, and simple phase screen generation algorithms (e.g., boiling flow), each have significant drawbacks such as high cost, high computation, limited quantity, and/or inaccurate statistics. More recently, data-driven algorithms have been proposed that are comp

Actinomycetoma Due to Nocardia brasiliensis: Recognition in Primary Care.

Mycetoma is a chronic, progressively destructive subcutaneous infection caused by aerobic filamentous bacteria (actinomycetoma) or true fungi (eumycetoma). It represents one of the most neglected tropical diseases, disproportionately affecting vulnerable populations in resource-limited rural settings. In Latin America, actinomycetoma predominates, most commonly caused by Nocardia brasiliensis. Delayed diagnosis is frequent due to its indolent course and limited awareness at the primary care level, often resulting in severe deformity, disability, or amputation. We report the case of a 43-year-o

Bedside Monitoring of Mycetoma: A Neglected Undertaking in a Neglected Tropical Disease.

Mycetoma is a prevalent yet neglected tropical disease, particularly in eastern India, where its diagnosis remains challenging due to the low sensitivity of histopathology and culture tests. This manuscript highlights bedside monitoring techniques that aid in assessing therapeutic response, including lesion mapping, girth measurement, sinus count, grain assessment, range of motion evaluation, modified visual analogue scale, and patient-reported outcomes. These methods provide valuable insights for clinicians managing mycetoma, facilitating early detection of treatment response and improving pa

Unusual Presentation of Eumycetoma (Madura Foot): A Case Report.

Mycetoma, commonly referred to as Madura foot, is a chronic granulomatous infection of the skin and subcutaneous tissues caused by fungi (eumycetoma) or filamentous bacteria (actinomycetoma). It typically presents in the foot following traumatic inoculation in endemic regions. Extra-pedal presentations are uncommon and may pose diagnostic and therapeutic challenges. We present the case of a 21-year-old previously healthy male patient with an unusual eumycetoma involving the posterior chest wall (back), an atypical anatomical site. The diagnosis was initially suspected clinically due to the dis

Histopathological Diagnosis of Infectious Diseases: Experience From a Tertiary Care Center in a Sub-Saharan African Country.

IntroductionInfectious diseases are very common worldwide, especially in developing countries. In some instances, patients may present with atypical clinical symptoms requiring surgical resections for therapeutic or diagnostic purposes. In such circumstances, pathology (histopathology) remains a valuable diagnostic tool. Our aim is to report histopathological features of infectious diseases diagnosed at our pathology laboratory.MethodsThis is a retrospective study including infectious diseases histologically diagnosed at our pathology laboratory over a period of about 4 years. The histopatholo

A qPCR identification scheme to differentiate actinomycetoma from eumycetoma.

BACKGROUND: Mycetoma is a Neglected Tropical Disease of the skin. It is characterized by large tumorous lesions in the subcutaneous tissue which hinder the patients in their daily activities. The disease can be caused by either bacteria (actinomycetoma) or fungi (eumycetoma), and treatment depends on the entity. Therefore, in 2022, a Target Product Profile was published by the World Health Organisation in which it was stated that there is an urgent need to develop a diagnostic tool that can differentiate the two reliably within a day. OBJECTIVES: To develop a qPCR that can not only differenti

Antifungal activity of the antimicrobial peptide RP557 against priority fungal pathogens.

Background. Natural host defence molecules, part of innate immunity and the first line of defence, are evolutionarily conserved. Some pharmaceutical properties undesirable for clinical use led to the rational design of synthetic molecules with constructed peptide arrangements, giving a novel therapeutic avenue. A prior publication showed synthetic peptide RP557 inhibition and killing of fluconazole-sensitive and resistant Candida species isolates, biofilm inhibition, no resistance induction, direct membrane action, negligible mammalian cell toxicity and topical efficacy in a rodent vaginal can

Modelling approaches for predicting the distribution of skin NTDs: A systematic review.

BACKGROUND: Skin neglected tropical diseases (NTDs) such as cutaneous leishmaniasis, lymphatic filariasis, mycetoma, and podoconiosis affect millions in endemic regions, but are under-recorded despite causing significant burdens. Predictive modelling has been used to estimate the distribution and prevalence of some of these diseases, and predictions may be useful for identifying at-risk populations and guiding interventions. This review synthesises the literature on modelling approaches to predict skin NTD distributions, aiming to identify prevalent methodologies, evaluate their strengths and
