Nocardia vulneris: a rare pathogen in actinomycetoma - a case report and review of the literature.

Actinomycetoma is a chronic, progressive bacterial infection characterized by granuloma formation, with Nocardia vulneris being a rare causative agent. A 32-year-old healthy man from El Salvador presented with painless nodules, scars and discharging sinus tracts on his back. Examination of the exudate revealed small white grains, and Nocardia species were isolated through culture. Molecular identification of N. vulneris was confirmed via 16S rDNA gene amplification and sequencing. The patient was treated with trimethoprim-sulfamethoxazole and dapsone, resulting in significant clinical improvem

A Rare Case of Actinomycetoma In Pregnancy: Effective Treatment with the Combination of Amoxicillin-Clavulanic Acid.

INTRODUCTION: Actinomycetoma is a chronic granulomatous of the skin and subcutaneous tissue caused by filamentous bacterial infection. In pregnancy, this disease is associated with poor clinical outcomes and often unresponsive to standard therapy. Although actinomycetoma is primarily treated with antibiotics, nevertheless, many recommended antibiotics cannot be used during pregnancy due to the potential for fetal harm. Report of actinomycetoma in pregnancy are infrequent. The management of mycetoma in pregnancy requires special attention in various aspects, particularly regarding the safety of

Right Thumb Mycetoma: A Case Report and Review of the Literature.

Mycetoma, or maduromycosis, is a chronic granulomatous infection involving the skin, subcutaneous tissues, and bones, most commonly affecting the lower extremities. Hand involvement is rare and often leads to diagnostic delays due to its atypical presentation. Mycetoma is classified as actinomycetoma (bacterial) or eumycetoma (fungal), each requiring distinct management approaches. We report a rare case of actinomycetoma affecting the right thumb in an 81-year-old man, initially misdiagnosed as a giant cell tumor. Magnetic resonance imaging revealed a well-defined subcutaneous mass without bon

Mycetoma (Madura Foot): A Case Report of Misdiagnosis and Recovery in a Young Patient.

Mycetoma is a chronic infection with a higher incidence in tropical and subtropical nations. We are presenting a rare case of mycetoma in a resident of Sudan, which is a temperate region, along with a short discussion. This case presentation discusses a 13-year-old male with a chronic foot discharge misdiagnosed as an abscess for two years. The condition, characterized by the development of a painless nodule leading to an open wound with purulent discharge, exemplifies the typical progression of actinomycetoma or eumycetoma. Following deep surgical debridement and antifungal therapy with itrac

Posaconazole effectiveness in rare invasive fungal infections: A systematic literature review.

INTRODUCTION: Mucormycosis, hyalohyphomycosis, chromoblastomycosis, and fungal mycetoma are rare invasive fungal infections (IFIs) that cause significant morbidity and mortality in immunocompromised patients. Few effective treatment options are available for these IFIs. METHODS: We performed a systematic literature review of MEDLINE and Embase to identify studies published from 2005 (year of posaconazole approval) to 22 October 2022, reporting the efficacy/effectiveness of posaconazole monotherapy or combination therapy for treating mucormycosis, hyalohyphomycosis, chromoblastomycosis, and my

South-East Asia regional neglected tropical disease framework: improving control of mycetoma, chromoblastomycosis, and sporotrichosis.

Mycetoma, chromoblastomycosis, and sporotrichosis are fungal neglected tropical diseases (NTDs) recognized by the World Health Organization. These implantation diseases cause substantial morbidity, disability, decreased quality of life, and can lead to long-term complications including tissue fibrosis, skin cancer, and amputation. The 2024-2030 South-East Asia Regional NTD Strategic Framework includes mycetoma but neglects the full extent of mycetoma endemicity in the region. Furthermore, the framework excludes chromoblastomycosis and sporotrichosis. We describe the data demonstrating fungal N

Spectrum of clinically significant melanized fungi in NIH hospitalized patients and their antifungal susceptibility profiles.

Melanized fungi have occasionally been identified as causative agents of severe phaeohyphomycoses, chromoblastomycosis, and mycetoma. In a retrospective study conducted from January 2012 to December 2022, a total of 133 melanized fungi were isolated from hospitalized patients at the NIH Clinical Center, both with and without known underlying predisposing factors. Isolate identification was based on phenotypic characteristics, matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-ToF MS), and PCR sequencing of the rDNA internal transcribed spacer (ITS) region. Bridging gaps in the elimination of deep mycoses: a comparative analysis of mycetoma and chromoblastomycosis control strategies in Rwanda and China.

Deep mycoses, largely caused by mycetoma and chromoblastomycosis, is a chronic fungal infection prevalent in tropics and subtropics regions. Deprived populations in these regions are disproportionately affected. However, control strategies for deep mycoses on a global scale remain limited even though these diseases are included in WHO's roadmap for 2021-2030 NTDs. China and Rwanda, with distinct climates, economic conditions and health systems and policies, are both burdened with this debilitating fungal infection. This review, therefore, focused on providing a cross-case evaluation of the con

Elephantiasis and verrucous lesions in an Ethiopian man living in Italy.

Elephantiasis refers to chronic, severe lymphedema caused by the abnormal accumulation of interstitial fluid due to reduced lymph transport and lymph stasis. This condition is observed secondary to injury, infection, cancer or cancer-related treatment, or congenital abnormalities of the lymphatic system. One of the several forms of elephantiasis is Elephantiasis Nostras Verrucosas (ENV), where "nostras" refers to a non-filarial form of lymphedema that occurs primarily in non-tropical areas. We describe a case of ENV in an Ethiopian man living in Italy, presenting with fever, hypotension, woody

When to suspect and how properly early detect and treat patients with endemic mycoses.

Endemic mycoses are caused by dimorphic fungi and eventually molds, as the case of implantation mycoses. In general, these diseases are acquired through trauma or inhalation of fungal elements in the environment, and less frequently by zoonotic acquisition or transmitted during organ transplantation. The target population for endemic mycoses is usually represented by normal hosts with low-income and intensive outdoor activities. Awareness of these diseases remains limited, even in regions with high prevalence, resulting in delayed diagnosis, and affecting the quality of life and outcomes of pa

Leishmaniasis.

Leishmaniasis is an endemic disease in Asia, Africa, the Americas and Southern Europe caused by Leishmania parasites and transmitted through the bite of female sandflies, resulting in Leishmania replication in macrophages. The condition manifests either as visceral leishmaniasis, a potentially fatal systemic disease that causes persistent fever, enlargement of the liver and spleen (hepatosplenomegaly), and a reduction in almost all blood cells (pancytopenia), or as cutaneous leishmaniasis, characterized by ulcerative or non-ulcerative skin lesions. Disease manifestation and severity are determ

The utility of artificial intelligence in the management of dengue fever: a perspective on future directions.

Dengue fever remains a significant public health challenge, particularly in tropical and subtropical regions. With millions affected each year and the increasing prevalence of the disease due to urbanisation and climate change, effective management strategies are crucial. The incorporation of artificial intelligence (AI) in healthcare presents transformative possibilities for the management of dengue fever. AI-driven systems can improve disease surveillance by analyzing extensive volumes of epidemiological, environmental, and socio-behavioral data to identify early warning indicators and forec

The power morcellator is an instrument with a fast rotating cylindrical knife which aims to divide the tissue into smaller pieces or fragments. The Food and Drug Administration (FDA) issued a press release in April 2014 that discouraged the use of these power morcellators. This article has the objective to review the literature related to complications by power morcellation of uterine fibroids in laparoscopy

Carcinoma Cuniculatum Masquerading as Eumycetoma: An Unacquainted Entity Posing a Diagnostic Dilemma.

Carcinoma cuniculatum (CC) is a rare and distinct clinicopathological variant of well-differentiated squamous cell carcinoma. It is a rare and slow-growing tumor with a peculiar infiltrative growth pattern resembling rabbit burrows (cuniculi). It usually occurs over the plantar aspect of the foot but can also occur at other sites like the oral cavity and genitals. The pathogenesis is unknown, with various hypotheses of trauma as proposed by different authors. It is essential to be aware of this entity as it commonly mimics benign and other low-grade squamous cell carcinoma (SCC). Diagnosis of

Painful mycetoma: a study to understand the risk factors in patients visiting the Mycetoma Research Centre (MRC) in Khartoum, Sudan.

BACKGROUND: Mycetoma, a neglected tropical disease endemic to Sudan, is traditionally characterized as a painless condition. However, recent studies reporting painful mycetoma lesions have challenged this assumption. With limited reports in the medical literature regarding pain in mycetoma, this study was conducted to investigate the risk factors for painful mycetoma lesions in patients visiting the Mycetoma Research Centre (MRC) in Khartoum, Sudan. METHODS: This cross-sectional study aimed to identify the risk factors for painful mycetoma lesions. The study analyzed electronic records of 481
