The First Autochthonous Case of Actinomycetoma in a Bedouin Patient in the Israeli Negev Region.

We report the first autochthonous case of actinomycetoma in a Bedouin patient from Israel's Negev Region. The patient presented with plantar and interdigital (web) nodular lesions. The time to diagnosis from the first medical contact was 4 years, and several treatment modifications were made until a satisfactory clinical response was achieved. This case underscores the potential underdiagnosis of mycetoma in nonendemic areas, the limited data regarding optimal treatment regimens, and the need for heightened clinical awareness.

Mycetoma managment: Therapeutic challenges and the role of pharmacovigilance.

Mycetoma presents numerous therapeutic challenges, particularly due to delays in diagnosis, the toxic nature of existing antifungals and antibiotics treatments, and the lack of robust clinical evidence to guide care. This neglected tropical disease, which primarily affects low-resource regions, is further complicated by socio-economic barriers that limit access to healthcare and treatment. These challenges underscore the urgent need for better treatment options, more comprehensive research, and strengthened pharmacovigilance systems to monitor treatment safety and efficacy. Pharmacovigilance p

Development of the Mycetoma Activity and Severity clinical Scale (MASS): an international Delphi study.

Mycetoma is a chronic, subcutaneous infection that causes local tissue destruction, leading to deformity, disability, and social stigma. Safe and effective treatments for mycetoma are urgently needed; however, there is no systematic measure that allows objective clinical assessment of disease severity and activity, which makes it difficult to accurately assess the effectiveness of treatments and draw comparisons between studies. A Delphi study was conducted, involving a panel of 23 mycetoma experts from five continents, to establish consensus on the structure, wording, and content of a clinica

Burden of mycetoma in Burkina Faso: case series and systematic review.

Mycetoma is a chronic inflammatory disease characterized by progressive and devastating granulomatous subcutaneous tissue involvement. It primarily occurs in regions classified within the 'mycetoma belt', encompassing various countries. Notably, West African nations, including Burkina Faso (BF), are situated within this belt, yet there is a significant lack of data regarding mycetoma in these regions. With this background, this study set out to determine the mycetoma prevalence in BF. A comprehensive review was conducted to document the presence of mycetoma in BF. Searches were performed in Pu

Actinomadura welshii sp. nov., a New Mycetoma Agent in Mexico.

Actinomadura isolates obtained from seven human mycetoma cases in Mexico were characterized using nucleotide sequence analysis of a portion of the small subunit ribosomal RNA gene. Most isolates were identified as Actinomadura madurae. However, one isolate, LIID-AQ337, showed inconclusive results. To determine its identity, genomic DNA from LIID-AQ337 was subjected to whole-genome sequencing using both short- and long-read sequencing, resulting in a genome of 8,163,638-bp spread in 22 contigs. Comparative analysis against currently available genomes of other Actinomadura species suggested that

Successful Management of a Refractory Scedosporium (Pseudallescheria) boydii Keratitis With Sclerokeratoplasty.

Here, we present the successful sclerokeratoplasty surgery in resistant Scedosporium (Pseudallescheria) boydii keratitis. A 47-year-old male patient presented with complaints of 4-mm corneal infiltration and 1-mm hypopyon after an eye injury with organic material. Topical treatments with vancomycin (50 mg/mL), ceftazidime (50 mg/mL), voriconazole (10 mg/mL), and amphotericin B (0.5 mg/mL) were started hourly. Because of the significant progression despite the treatment, emergency therapeutic penetrating keratoplasty was performed. In this patient, in whom Scedosporium boydii growth was detecte

Immune response in BALB/c mice to an attenuated strain of Nocardiabrasiliensis.

INTRODUCTION: We recently developed an attenuated strain of Nocardia brasiliensis HUJEG-1, named NBP200, through 200 continuous passaging. When administered to naïve mice, this attenuated strain confers resistance to infection with the wild-type strain. Although the immune response to the parental strain has been previously studied, the analysis of the immune response to the attenuated strain will help to understand the immune mechanisms involved in resistance against this microorganism. METHODS: Initially, experimental mice were divided into two groups and infected with either the attenuated

Orbital actinomadura madurae actinomycetoma: Case report and literature review.

The reported patient is a middle-aged farmer from West Sudan who presented with a painless mass in the right upper eyelid that progressively enlarged over time. His medical treatment journey was prolonged and difficult. The mass was initially misdiagnosed as lacrimal gland pleomorphic adenoma. The patient was later referred to a specialised center, where wide local excision confirmed the diagnosis of Actinomadura madurae actinomycetoma. Although orbital mycetoma is uncommon, it is a serious condition that should be considered in the differential diagnosis in endemic areas.

Ibrexafungerp prolongs survival and reduces the eumycetoma grain size in Galleria mellonella infection models of two different causative agents of eumycetoma.

OBJECTIVES: Eumycetoma is a neglected tropical fungal disease of the subcutaneous tissue that is currently not treatable with medication only. Standard itraconazole therapy is combined with surgical excision of the lesion. Recently, ibrexafungerp, a novel oral antifungal agent inhibiting 1,3-β-D-glucan synthesis, was approved by the United States Food and Drug Administration (FDA) for the treatment of vulvovaginal candidiasis. In this study we determined the in vitro activity and in vivo efficacy of ibrexafungerp in eumycetoma causative agents. METHODS: In vitro activity of ibrexafungerp and

The development of a multiplex recombinase polymerase amplification reaction to detect the most common causative agents of eumycetoma.

PURPOSE: Mycetoma is a neglected tropical disease that affects the subcutaneous tissue. The disease can be caused by over 90 different pathogens, including both bacteria (actinomycetoma) and fungi (eumycetoma). While diagnostic tools for eumycetoma causative agents are available, these are generally not well suited for use in endemic regions. This study aims to develop an isothermal based multiplex recombinase polymerase amplification reaction (RPA), that can be integrated in the diagnostic workflow of endemic regions. METHODS: The RPA was designed targeting the Internal Transcribed Spacer (I

Increased expression of the PD-1/PD-L1 regulatory axis in tissue leukocytes from experimental actinomycetoma by Nocardia brasiliensis in BALB/c mice.

The development of experimental actinomycetoma in mice pioneered the study of infective mechanisms in nocardiosis. However, the understanding of lymphocyte activation in actinomycetoma remains incomplete. In this study, we used flow cytometry to evaluate the cellularity and expression of regulatory receptors on leukocytes from spleen and infected tissue in a mice model of experimental actinomycetoma induced by Nocardia brasiliensis. Our results indicate that neutrophils dominate cellularity in infected tissue, representing >90 % of infiltrated leukocytes. Among lymphocytes, the percentage of T

Aspergillus nidulans as an agent of subcutaneous mycetoma in a horse.

Mycetoma is a fungal chronic skin proliferative lesion rarely encountered in horses and often associated with fungi such as Scedosporium spp., Madurella spp., Phialophora spp., Curvularia spp., and less frequently with Aspergillus spp. A case of subcutaneous mycetoma in the cervical region in a 7-year-old male Quarter Horse, diagnosed through cytological, histological, cultural and molecular methods, is reported. Aspergillus nidulans, usually implicated in guttural pouch mycosis, was identified. This unusual case represents the second case report of mycetoma caused by Aspergillus nidulans in t

Pharmacokinetics and Pharmacodynamics of Fosravuconazole, Itraconazole, and Hydroxyitraconazole in Sudanese Patients With Eumycetoma.

BACKGROUND: The first clinical trial on eumycetoma was recently conducted in Sudan, comparing oral fosravuconazole, prodrug of active ravuconazole, with the standard-of-care oral itraconazole. Building on this trial, the present study aimed to characterize the pharmacokinetics-pharmacodynamics (PK-PD) of ravuconazole, itraconazole, and hydroxyitraconazole in patients with eumycetoma and guide selection of either a 200-mg or 300-mg dose of fosravuconazole. METHODS: Nonlinear mixed-effects modeling was used to develop population PK models in 52 patients receiving 3 daily loading doses followed

Iron regulatory pathways differentially expressed during Madurella mycetomatis grain development in Galleria mellonella.

Mycetoma is a chronic granulomatous infection of the subcutaneous tissue, most often caused by the fungal pathogen Madurella mycetomatis. Characteristic of the infection is the formation of grains. However, knowledge of the function and formation of the grain is limited. Here, we use a Galleria mellonella larvae infection model and transcriptomic profiling to identify processes associated with M. mycetomatis grain formation. Larvae were infected with M. mycetomatis and, after 4, 24, 72 and 168 h post-inoculation, RNA was extracted from larval content and sequenced. We found that 3498 G. mellon

Subcutaneous mycoses - The hidden mould, a case series.

Subcutaneous mycoses are a heterogenous group of infections common in subtropical and tropical regions. These infections have an insidious onset and slowly evolve as the pathogen survives and adapts to the host tissue environment. Common subcutaneous fungal infections include Sporotrichosis, Chromoblastomycosis, Mycetoma, Subcutaneous Zygomycosis, Subcutaneous Phaeohyphomycosis, Lobomycosis and Rhinosporidiosis. Even with rapid urbanization, the incidence of subcutaneous mycoses is on the rise. Confirmation by histopathological examination is necessary.

Streptomyces species: A friend or foe? the diagnostic dilemma of cutaneous and mucocutaneous infections.

Mycetoma and various invasive forms of the disease are caused by etiological agents like Nocardia, Actinomycetes, and Streptomyces spp. Streptomyces spp. is notable for its production of antibiotics. However, it is also most commonly associated with actinomycetoma, especially Streptomyces somaliensis (S. somaliensis), Streptomyces griseus. (S. griseus), S. sudanensis, and S. viridis. We present a series of cases with varied clinical presentations ranging from cutaneous nodules to plunging ranula in which Streptomyces spp have been isolated in culture with further species-level identification u

A Draft Genome Assembly for the Neglected Tropical Fungal Disease Pathogen Emarellia grisea and Comparison to Other Pleosporalean Agents Causing Eumycetoma in Humans.

Mycetoma is a neglected tropical disease, characterized by slowly evolving tumorous subcutaneous masses containing either bacteria (e.g., Actinomycetoma) or fungi (e.g., Eumycetoma; Madura foot). The fungal agents of mycetoma, including several genera belonging to the Pleosporales order of Ascomycetes, were designated high-priority pathogens in the WHO Fungal Priority Pathogens List of 2023 to signal the need for additional research and development and public health action. Despite this, many of the fungal causative agents are poorly described, especially in terms of the genomic basis for dise

Amphotericin B intercalated in layered clays as effective antifungal systems against eumycetoma causative agents.

Eumycetoma and other subcutaneous mycosis are severe chronic infections with a high incidence in tropical and subtropical regions, whose current treatment has a low curation rate. In the search for more effective and affordable therapies, hybrid materials based on amphotericin B (AmB), an antifungal drug with a broad spectrum of action, appear promising for localized treatment at the site of infection. In this context, the immobilization of AmB in suitable nanocarriers would be helpful to minimize its adverse side effects and ensure a controlled release over time. For this purpose, montmorillo

Detecting and Classifying Mycetoma in Histopathological Images Using DenseNet and U-Net.

Mycetoma, recognised by the WHO as a Neglected Tropical Disease, has significant diagnostic hurdles which lead to severe health consequences. Mycetoma can be caused by certain types of bacteria (actinomycetoma) or fungi (eumycetoma). Identifying whether mycetoma is bacterial or fungal plays a significant role in treatment planning, and an incorrect diagnosis can seriously affect patient prognosis and outcome. This paper explored the feasibility of using state-of-the-art artificial intelligence (AI) computer vision methods to effectively detect and diagnose mycetoma grains from histopathologica
