Randomized trial comparing the efficacy of a novel manual breast pump with a standard electric breast pump in mothers who delivered preterm infants.

OBJECTIVE: The benefits of human milk for preterm infants are widely recognized, yet technological advances in milk expression have been slow. We compared the efficacy of a standard electric pump (EP; Egnell) used in 94% of United Kingdom neonatal units with a novel manual pump (MP; Avent ISIS) designed to operate more physiologically by simulating the infant's compressive action on the areola during breastfeeding. METHODS: We randomized 145 women who delivered infants of <35 weeks' gestation to use the MP or the EP and measured total milk volume expressed while using the randomized pump duri

Leptin levels in preterm human breast milk and infant formula.

OBJECTIVE: Leptin, a hormone present in breast milk, is involved in energy regulation and metabolism. The purpose of this investigation was to determine whether leptin is present in either preterm breast milk (PBM) or preterm formula (PF). The effects of delivery methods and pasteurization on leptin levels also were evaluated. METHODS: PBM samples were obtained from 29 mothers who delivered infants at between 23 and 34 weeks' gestation. Leptin levels were measured in PBM and PF with the use of a radioimmunoassay specific for human and bovine leptin, respectively. Milk samples were pasteurized

From gavage to oral feedings: just a matter of time.

The ability to safely and efficiently feed by mouth is based on oral-motor competence, neurobehavioral organization, and gastrointestinal maturity. Although most infants successfully make the transition to oral feedings as they approach term gestation, infants who were very immature at birth have historically had the most difficulty achieving this milestone. Postnatal complications (chronic lung disease, intraventricular hemorrhage, seizures, and so on) are also associated with delays in the onset of the first feeding, as well as with delays in achieving full enteral intake. Weaning the infant

Rapid versus slow rate of advancement of feedings for promoting growth and preventing necrotizing enterocolitis in parenterally fed low-birth-weight infants.

BACKGROUND: Very premature infants fed by gavage are unable to regulate their own enteral intake. Therefore the rate at which feedings are advanced must be determined by caregivers. While advancing feedings too rapidly may increase the risk of necrotizing enterocolitis, advancing feedings too slowly might result in undernutrition or prolonged hospital stay. OBJECTIVES: For low-birth-weight premature infants receiving parenteral fluids, to assess the effect of different rates of advancement of enteral feedings beginning at the same age on measures of feeding tolerance and neonatal outcome. Lamotrigine in pregnancy: pharmacokinetics during delivery, in the neonate, and during lactation.

PURPOSE: To investigate the pharmacokinetics of lamotrigine (LTG) during delivery, during the neonatal period, and lactation. METHODS: High-performance liquid chromatography was used to determine plasma and milk levels of LTG in nine pregnant women with epilepsy treated with LTG, and plasma levels in their 10 infants. Samples were obtained at delivery, the first 3 days postpartum, and at breast-feeding 2-3 weeks after delivery. RESULTS: At delivery, maternal plasma LTG concentrations were similar to those from the umbilical cord, indicating extensive placental transfer of LTG. There was a sl

Gastric electrical activity and gastric emptying in term and preterm newborns.

The aims of this study were to evaluate the gastric electrical activity and gastric emptying in preterm and term newborns and to assess the development of gastric motility by comparing newborns of different gestational ages. The cutaneous electrogastrography and the ultrasonographic study of the gastric emptying were performed before and after milk formula in three groups of infants: 12 preterm newborns with a gestational age of 28-32 weeks, 11 preterm newborns with a gestational age of 32-36 weeks, and 10 full-term newborns with a gestational age of 36-40 weeks. All recording sessions were pe

The mother of all HIV challenges.

AIDS researchers are finding cheaper and simpler ways to slow the spread of HIV from mother to child, and more pregnant women, even in the poorest countries, have access to anti-HIV drugs and formula--thanks to the largesse of donors, discounts from industry, new trade laws, and the tenacity of individual clinicians. But just as researchers offer ways to clear one enormous hurdle--drug availability--they run smack into other ones, ranging from social stigmas that discourage testing to disinterest on the part of cash-strapped health authorities to a deeply ingrained culture of breast-feeding--o

Dysphagia in children with a 22q11.2 deletion: unusual pattern found on modified barium swallow.

OBJECTIVES: To delineate feeding dysfunction in a population of children with a 22q11.2 deletion and report the associated findings noted during the modified barium swallow (MBS). STUDY DESIGN: Seventy-five children with a chromosome 22q11.2 deletion and history of persistent feeding difficulty received a feeding evaluation, including an MBS for those children for whom there was concern about airway penetration. RESULTS: A consistent pattern of feeding difficulty, independent of palatal or cardiac involvement, emerged from the evaluations. This group typically has trouble coordinating the su

Anticonvulsants and breast feeding: a critical review.

Progress in the diagnosis and management of seizure disorders and the availability of effective anticonvulsive medications has enabled increasing numbers of epileptic women of child-bearing age to raise families. Breast feeding, which these women may wish to choose, provides health, nutritional, immunological, developmental, social, economic and environmental benefits. The traditional anticonvulsants, such as phenytoin, carbamazepine and valproic acid (valproate sodium), are generally considered safe for use during breast feeding; however, observation for adverse effects is recommended. The us

Mydriatics slow gastric emptying in preterm infants.

OBJECTIVE: Preterm infants are more likely to have episodes of abdominal distention, emesis, and gastric residuals 24 hours after their first screening examination for retinopathy of prematurity (ROP) than on the day preceding the examination. Because these feeding problems reflect abnormalities in motor function, the purpose of this study was to compare antral and duodenal motor activity and gastric emptying in preterm infants before and after the instillation of mydriatics. STUDY DESIGN: Using a low compliance continuous perfusion manometric system, we recorded antral and duodenal fasting m

A randomised controlled trial of the effects of a cross-cut feeding teat on infant feeding, crying, waking and sleeping behaviour.

UNLABELLED: Feedback from mothers using a "cross-cut" teat indicated its use for problem feeders, particularly "windy" or "colicky" babies. The manufacturer commissioned a trial to see whether this observation was clinically valid. A randomised controlled trial comparing the cross-cut teat with a standard single-hole teat was designed. Both teats were identical in construction apart from the feed hole at the tip of the teat. A standard teat pierced with one hole is usually sold as slow, medium or fast flow rate, depending on the size of the hole. The cross-cut has two slits in the form of a cr

Postnatal development of northern fulmar chicks, Fulmarus glacialis.

The slow growth and large fat stores characteristic of many pelagic seabird chicks were generally assumed to reflect infrequent and unpredictable food provisioning by parents. Much less attention has been focused on the importance of intrinsic physiological processes in shaping patterns of development. In this study, we examined postnatal growth and changes in water content of different organs in fulmar chicks, Fulmarus glacialis, from Fair Isle, United Kingdom. After correcting for body size, mass growth rate was as high as in inshore-feeding species, which did not support the notion of an ex

"What can I do to enhance the development of a premature infant with chronic lung disease?".

CASE. Timmy was born at 32 weeks of gestation after an uncomplicated pregnancy until there was a spontaneous rupture of the membranes and preterm labor associated with chorioamnionitis. A 2-month hospitalization in the neonatal intensive care unit (NICU) was associated with pneumonia, a Grade II intraventricular hemorrhage, chronic lung disease, and a slow weight gain in the nursery. He was discharged to home with plans for ongoing care by his pediatrician. The primary care pediatrician attended a multidisciplinary conference with the NICU staff and Timmy's parents. At the time of discharge fr

Zinc absorption, distribution, excretion, and retention by healthy preterm infants.

Zinc (Zn) is an essential nutrient for growth, but little is known about Zn absorption, distribution, excretion, and retention in preterm infants. Nine infants with gestational age 32+/-1 wk (mean+/-SE), birth weight 1.44+/-0.08 kg, postnatal age 14+/-3 d, on Zn intake of 23+/-3 micromol/kg per d via enteral feeding of preterm formula were studied. A stable Zn isotope (70Zn) was administered orally or i.v., and plasma, red blood cells, urine, and feces were sampled for up to 30 d. Samples were analyzed for Zn by inductively coupled plasma atomic emission spectrometry and for isotope enrichment

Randomized trial of "slow" versus "fast" feed advancements on the incidence of necrotizing enterocolitis in very low birth weight infants.

OBJECTIVE: To determine whether the rate of feed advancement affects the incidence of necrotizing enterocolitis (NEC). STUDY DESIGN: Prospective randomized controlled trial involving 185 formula-fed infants with birth weight 501 to 1500 g and gestational age </=34 weeks. Infants were randomized into 2 groups: "slow" (n = 98), who received 15 cc/kg/d increments (a 10-day schedule to full feeds) and "fast" (n = 87), who received 35 cc/kg/d increments (a 5-day schedule to full feeds) of Similac Special Care 20 cal/oz. Feeds were increased only if well tolerated as defined by a protocol. RESULTS

Slow weight gain and low milk supply in the breastfeeding dyad.

There are a large number of women who perceive a reduction in milk supply. With appropriate, knowledgeable advice, most are able to continue breastfeeding successfully. If an infant is not gaining weight normally, the mother's milk production must be assumed to be low (usually a secondary phenomenon); meanwhile, consider the possibility of an organic problem in mother or infant. The complex interactional nature of the problem requires attention to history, physical examination, differential diagnosis, and thoughtful problem solving. There are situations that require infant supplementation for

Spontaneous thrombosis of vein of Galen aneurysmal malformation.

Two cases of aneurysmal malformations of the vein of Galen (AVG) with spontaneous thrombosis are reported. Angiogram and MRI before thrombosis demonstrated AVGs with slow arteriovenous shunts and associated stagnation of contrast in the venous sac secondary to severe outflow restriction. Based on these findings, one patient was managed conservatively, and the other underwent placement of a ventriculoperitoneal shunt. Surveillance of the lesions with subsequent MRIs revealed spontaneous thrombosis of the AVGs with excellent clinical outcomes. Proposed mechanisms of spontaneous thrombosis includ

Development of gastric slow waves in preterm infants measured by electrogastrography.

The aim of this study was to investigate the developmental process of gastric myoelectrical activity (GMA) in preterm infants. Nineteen healthy preterm infants were studied. GMA was recorded using surface electrogastrography, and six follow-up studies were performed in each subject. Spectral analysis methods were applied to compute the parameters of the electrogastrogram (EGG). The results showed that there was a developmental process of GMA with age during the first 6 mo of life. 1) The percentage of normal slow waves showed a progressive increase after birth (36.7 +/- 6.1, 37.8 +/- 6.2, 47.0
