Low vitamin B6 status associated with slow growth in healthy breast-fed infants.

To evaluate the effect of vitamin B6 status on infant growth, we studied longitudinally anthropometry and the erythrocyte parameters that reflect long-term vitamin B6 status [erythrocyte pyridoxal 5'-phosphate concentration (EPLP), erythrocyte aspartate transaminase basal activity (EAST0), and its activation co-efficient (alpha EAST)] in 44 infants. The infants were exclusively breast-fed for 6 mo, given additional solids according a uniform schedule from 6-9 mo, and formula after 9 mo, if needed. In seven of these infants, a low vitamin B6 status (EPLP < 10th, and EAST0 > 10th or alpha EAST >

Prospects and programs for fertility reduction, 1990-2015.

What is the likelihood that each of the 37 developing countries with populations of 15 million or more in 1990 will reach replacement fertility by the year 2015? These countries have a combined population of 3.9 billion, 91 percent of the population of all developing countries. For this article, a composite index was used as the basis for predicting future levels of total fertility. The index was constructed from socioeconomic variables (life expectancy at birth, infant mortality rates, percent adult literacy, ratio of children enrolled in primary or secondary school, percent of the labor forc

Indicators for monitoring the growth of peruvian infants: weight and length gain vs attained weight and length.

OBJECTIVES: To determine appropriate indicators and age intervals for growth monitoring in this population, the relationship between monthly weight and length indicators and nutritional status at 12 months was evaluated among 102 low-income Peruvian infants. METHODS: Cutoffs for defining adequate vs slow growth were obtained from published reference data, and sensitivities and specificities of each indicator were calculated. RESULTS: Low weight gain in early infancy, especially from 1 to 2 months, is useful for predicting low weight at 1 year (sensitivity = 81%, specificity = 65%, 25th perce

[Supplementation of energy intake in premature infants by parenteral administration of lipids].

Premature infants are at risk for malnutrition and essential fatty acid deficiency. The authors administered 0.5 g of lipids by parenteral route as an adjuvant to enteral feeding, in 30 healthy premature infants aged 14 days to 6 weeks, with a body weight at birth of 1,879 +/- 325 g, presenting a slow rate of weight gain. This led to a long term improvement of the body weight gain preventing malnutrition. The serum level of total triglycerides, cholesterol, phospholipids and glycemia remained within normal limits.

A semiautomatic device for feeding liquid milk-replacer diets to infant pigs.

We describe the design and use of a device that enables the semiautomatic feeding of various quantities of liquid, milk-replacer diet. The apparatus consists of a programmable fraction collector (with attachments for preparative work) driven by a high-flow peristaltic pump. To reduce bacterial growth and lipid oxidation of the liquid diet, we kept the milk-replacer in an ice bath and bubbled a slow stream of nitrogen through it at all times. The liquid diet was kept in suspension by continuous recirculation and agitation. The device can be used to dispense prespecified amounts of either the sa

Management strategies for promoting successful breastfeeding.

Clinicians can promote a successful breastfeeding experience by providing support, anticipatory guidance and practical information. This article presents the components of early follow-up and guidelines for assessment. Management strategies for common problems are discussed, such as nipple soreness, cracked nipples, plugged ducts and mastitis, insufficient infant weight gain, perceived inadequacy of milk supply, breast-milk jaundice, sexual adjustment and failure at breastfeeding. Breastfeeding guidelines for employed mothers and adoptive mothers are indicated.

[Slow virus infection in the field of obstetrics and gynecology--with special reference to HBV, HTLV-1 and HCV].

The slow virus infection (SVI) established by Gajdusek DC in 1964 has been known to involve not only Kuru or Creutzfeldt-Jakob disease but also hepatitis B virus (HBV) infection and very recently human T-cell lymphotropic virus type 1 (HTLV-1) or hepatitis C virus (HCV) infection. These all viruses potentially develop serious, irreversible disease, ie, hepatoma or adult T-cell leukemia, after long latent periods. HBV, HTLV-1 and HCV can be transmitted vertically from carrier mothers to their offspring, and therefore, are serious SVIs in the field of obstetrics. HB immunoglobulin (HBIG) and HB

The slow-gaining breastfeeding infant.

Several options are outlined for caring for the slow-gaining breastfed infant when the goal is to continue breastfeeding. A review of pertinent causative factors and simple management solutions are included. Infants with organic causes of slow weight gain can safely be breastfed by using a feeding tube device at the breast. Other feeding options can be explored with the parents, but safety, comfort of the infant, energy conservation, the parents' emotional and time capabilities, and long-term ability to maintain lactation must be considered. A careful follow-up plan is outlined, mostly for enc

[Rapid intravenous rehydration in acute diarrhea].

With the use of oral rehydration, the need for the use of endovenous rehydration has decreased considerably. Albeit, the use is still necessary in severely dehydrated patients or when oral rehydration fails. Textbooks produced in developed countries recommend slow administration of fluids to correct dehydration in 12 to 24 hours. In developing countries, due to the great number of severely dehydrated patients, this approach is not useful. We developed, during the last 20 years, an approach to intravenous rehydration that permits rehydration of the severely ill patient in a much shorter time (2

Current therapy of bronchopulmonary dysplasia.

In summary, little progress has been made in the past several years with respect to the treatment of the baby with BPD. The conduct of convincing clinical research seems to be a casualty of budget cuts and a rush to learn the tools of molecular biology. To date, there are no clinical trials that have convincingly demonstrated that long-term diuretic, bronchodilator, vasodilator, or antioxidant therapy is effective in the treatment of chronic BPD. Short-term corticosteroid therapy hastens extubation, but long-term outcome is unaffected and serious questions remain about its safety. Multicenter

Sucking technique and its effect on success of breastfeeding.

We investigated the prognostic value of sucking technique (faulty vs correct) during the first week after birth in relation to the long-term success of breastfeeding. At discharge from the maternity ward, 82 healthy mother-infant pairs were observed for assessment of breastfeeding technique and followed for four months by regular telephone checkups. Correct sucking technique was defined as the infant having a wide-open mouth, with the tongue under the areola, and expressing milk from the breast by slow, deep sucks; faulty technique was defined as superficial nipple sucking. The study populatio

Breast-feeding and infant growth in the first six months.

Weight and length of 67 breast-fed infants were followed every two weeks from birth to 6 months. All infants were healthy and developed normally. Weight curves were found to be parallel to the NCHS and Bangkok curves up to 4 months then bending slightly but not statistically significant. This supports "the breast-fed infants having a different growth pattern from the mixed (breast-formula) fed" finding observed in studies from developed countries. In comparison to the NCHS and Bangkok length data, rural male infants were significantly (p less than 0.01 and 0.05 respectively) shorter from birth

Studies on the interaction of a thiol-dependent hydrogen peroxide scavenging enzyme and phenylalanine hydroxylase.

Rat liver phenylalanine hydroxylase is irreversibly inactivated by a H2O2-dependent process. Since H2O2 can be produced by autooxidation of the tetrahydropterin cofactor required for the hydroxylation reaction, in vitro assays are usually carried out in the presence of added catalase. On the basis of a dithiothreitol-dependent protecting assay of phenylalanine hydroxylase, carried out in the absence of catalase, we have isolated an enzyme fraction from neonatal rat livers which has similar properties to the known enzyme, glutathione peroxidase. The developmental time course for phenylalanine h

Effect of formula feeding on oral absorption of caffeine in premature infants.

The systemic absorption of orally administered caffeine was evaluated in 16 premature infants receiving therapy for recurrent apnea. An oral bolus dose of caffeine (10 mg/kg) was given during a fasted state (n = 8) or immediately preceding formula (n = 8). The first-order absorption rate constant (ka) was estimated from the rise in the plasma concentration-time curve and extent of absorption was estimated by the area under the concentration-time curve. Therapeutic concentrations of caffeine were achieved rapidly followed by very slow elimination of drug. No difference between the fasted and fe

Epidemiological patterns of scurvy among Ethiopian refugees.

In the Horn of Africa, scurvy is a serious public health problem for refugees who are dependent on standard relief food (cereals, legumes, and oil). To assess the risk factors and to quantify the potential magnitude of scurvy among these displaced communities, we reviewed data collected from 1985 to 1987 by relief programmes in five refugee camps in Somalia and one in the Sudan. Outbreaks of clinical scurvy occurred among refugees in all the camps from 3 to 4 months after their arrival. The incidence of scurvy in two camps was, respectively, 14% over a period of 4 months and 19.8% over a perio

[Neonatal necrotizing enterocolitis].

Necrotizing enterocolitis (NEC) is an extremely severe clinical entity in whose pathogenesis at least two of the following factors are involved: intestinal ischaemia, pathogenic bacterial colonization and excessive substrate in the intestinal contents. Common risk factors are prematurity, rapid increase of feeding volume, polycythemia and perinatal asphyxia. Clinical features are abdominal distention, gastric retention, bloody stools and intestinal wall pneumatosis; while its complications are bowel perforation, peritonitis and shock. Early diagnosis allows prompt medical treatment based upon

Preneoplastic rat liver cells in vitro: slow progression without promoters, hormones, or growth factors.

In order to determine early changes in liver cells during carcinogenesis and to compare them with normal or neoplastic hepatocytes, an experimental model was established which allowed enrichment of this population at early stages of carcinogenesis and provided sufficient viable material for biochemical and cytogenetic analysis. This paper describes a method that allows in vitro selection and propagation of hepatocytes after in vivo initiation by alkylating agents, without the use of hormones, growth factors, or promoters which might affect their progression. From 6 different rat livers (5 init
