Clinical outcomes during pregnancy and the postpartum period in women with myasthenia gravis and their newborns.

BACKGROUND: Myasthenia gravis (MG) is an autoimmune disease of the neuromuscular junction associated with increased maternal and fetal risks during pregnancy and the puerperal period. OBJECTIVE: To describe the longitudinal course of MG, maternal and neonatal outcomes, and the occurrence of transient neonatal myasthenia gravis (TNMG) from the pregestational period until 12 months postpartum in pregnancies complicated by MG. METHODS: A retrospective, longitudinal case-series study of pregnancies in women with MG who were followed at a single tertiary center for neuromuscular disorders. Diseas

A Case of HNRNPU-Related Neurodevelopmental Disorder Presenting With Acute Encephalopathy and Basal Ganglia Lesions.

We report a case in which a girl diagnosed with HNRNPU-associated neurodevelopmental disorder presented with acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) and generalized hypotonia accompanied by chorea-like involuntary movements. On the first day of hospitalization, she presented with fever and generalized clonic seizures. Twenty four hours later, she continued to experience impaired consciousness and generalized hypotonia. Her consciousness improved gradually, but chorea-like involuntary movements of the limbs were observed. From the seventh day of hospitaliza

Etiological Diagnosis and Disease Course of Birk-Barel Syndrome in an Adult Woman with a KCNK9 Variant.

INTRODUCTION: Birk-Barel syndrome (BIBARS), also known as KCNK9 imprinting syndrome, is an extremely rare genetic condition caused by pathogenic variants in the KCNK9 gene located on the long arm of chromosome 8 (8q24). This gene is paternally imprinted and therefore only expressed from the maternal allele. KCNK9 encodes for a member of the two-pore domain potassium channel (K2P) subfamily. BIBARS syndrome is characterized by congenital hypotonia and weakness of proximal muscles, intellectual disability, behavioural problems, dysmorphic features, feeding problems and epilepsy. The syndrome is

Fatal Infantile Cardiomyopathy Associated with a Homozygous MYL2 c.413T>A (p.Met138Lys) Variant: A Case Expanding the Recessive MYL2 Phenotypic Spectrum.

BACKGROUND/OBJECTIVES: Infantile cardiomyopathy is a rare but often life-threatening condition in which monogenic causes are particularly relevant, especially when cardiac disease is preceded by hypotonia or multisystem involvement. Among sarcomeric genes, MYL2, encoding the ventricular regulatory myosin light chain, plays a critical role in myocardial contractility. However, biallelic MYL2-associated disease remains exceptionally rare, and its clinical spectrum is not fully defined. This study aims to describe a novel case and further delineate the phenotype of recessive MYL2-related cardiomy

Gillespie Syndrome: Greek Patient with Novel Mutation and Atypical Presentation - Expansion of the Phenotype and Literature Review.

INTRODUCTION: Gillespie syndrome is characterized by bilateral aniridia, cerebellar hypoplasia leading to ataxia, congenital hypotonia, and variable degrees of intellectual disability. The condition is attributed to pathogenic variants in ITPR1, either in a biallelic manner or as heterozygous dominant-negative mutations. CASE PRESENTATION: Herein, we report the case of a male neonate with generalized hypotonia, and bilateral aniridia, identified during routine newborn eye screening. His abdominal ultrasound revealed a left dysplastic pleiocystic kidney, and his brain magnetic resonance imagin

Endocrine-informed monitoring of scoliosis in Prader-Willi syndrome: integrating neuroendocrine pathophysiology, growth hormone therapy, and pubertal transition.

Prader-Willi syndrome (PWS) is a neuroendocrine disorder characterized by hypothalamic dysfunction, congenital hypotonia, abnormal growth trajectories, and impaired pubertal development, all of which contribute to a markedly increased risk of scoliosis, with a cumulative prevalence reaching up to 70-80% by skeletal maturity, significantly exceeding that of idiopathic scoliosis. Unlike idiopathic scoliosis, spinal deformity in PWS follows a distinct bimodal pattern, with critical vulnerability during infancy and a second acceleration during pubertal transition. Growth hormone (GH) therapy, a co

Myasthenia Gravis in Pregnancy: Prenatal and Postnatal Diagnostic Challenges-A Narrative Review.

Myasthenia gravis (MG) is a prevalent autoimmune disorder affecting neuromuscular junctions, typically characterized by muscle weakness due to autoantibodies targeting acetylcholine receptors (AChR) or muscle-specific kinase (MuSK). Generalized MG is a more severe form of the condition than ocular MG. Although MG can strike at any age, young adult women are typically affected, especially in their reproductive years. MG is rare during pregnancy, with the first trimester and the postpartum period being the most common times for exacerbations. The influence of MG on pregnancy outcomes remains amb

Severe neonatal-onset PYROXD1-related myopathy with a novel homozygous missense variant: expanding the phenotypic spectrum.

PYROXD1-related myopathy is a rare genetic disorder with variable age of onset, ranging from the neonatal period to late adulthood. We report a male infant presenting with congenital hypotonia, progressive severe weakness, respiratory insufficiency, stridor, and ophthalmoplegia. Muscle biopsy revealed fibre size variation, internalized nuclei, and myofibrillary changes with sarcoplasmic disorganization. Genetic testing identified a novel homozygous missense variant in the PYROXD1 gene (NM_024854.5:c.1498G>A, p.Asp500Asn), located at the C-terminal end, affecting the final amino acid before the

Transient Neonatal Myasthenia Gravis in a Newborn Infant Presenting to an Emergency Department.

A four-day-old female born full term via uncomplicated vaginal delivery presented to the emergency department with poor oral intake. There was a significant maternal history of myasthenia gravis requiring pyridostigmine and intravenous immunoglobulin infusions five days per month. It was suspected that the patient had transient maternal acetylcholine antibodies causing the weakness and poor oral intake. The patient was admitted to the neonatal intensive care unit for further evaluation and management. Laboratory evaluation revealed elevated acetylcholine receptor antibodies at 2.69 nmol/L, whi

A Homozygous Missense COL1A1 Variant (p.Glu684Lys) Associated with an Arthrochalasia-like Ehlers-Danlos Syndrome Phenotype: A Case Report.

BACKGROUND/OBJECTIVES: Arthrochalasia Ehlers-Danlos syndrome (aEDS) is a rare connective tissue disorder characterized by severe joint hypermobility, congenital hip dislocation, skin hyperextensibility, and muscle hypotonia. It is typically caused by heterozygous splice-site variants in COL1A1 or COL1A2, leading to exon 6 skipping. Autosomal recessive forms are extremely rare and have been reported predominantly in families from Saudi Arabia carrying the homozygous COL1A1 missense variant c.2050G>A, p.(Glu684Lys), with clinical presentations ranging from severe to mild. Case Report: A novel HNRNPK gene variant causing Au-Kline syndrome; the genotype and phenotype of the oldest individual ever described.

This report describes the diagnostic and treatment course of an adult woman with the ultra-rare Au-Kline syndrome (AUKS). Since the age of 32 years, when the syndrome was yet undiagnosed, the patient suffered from behavioral decline coinciding with changes in living conditions. At the age of 38, following a bladder infection, she presented with a delirium for which antipsychotic medication was started. The delirium resolved but, while the medication was continued, the pre-existing behavioral symptoms persisted. Clinicpal genetic evaluation revealed a history of congenital hypotonia, developmen

A novel ASCC1 splice-site variant broadens the phenotypic spectrum of spinal muscular atrophy with congenital bone fractures type 2.

Spinal muscular atrophy with congenital bone fractures type 2 (SMABF2) is an ultra-rare neuromuscular disorder caused by pathogenic variants affecting the ASC-1 complex, most commonly ASCC1. The disorder is typically characterized by severe congenital hypotonia, prenatal or congenital fractures, and early respiratory failure. We report a girl with a novel homozygous intronic donor-site variant, c.95+5G>C, in ASCC1. In contrast to the classic SMABF2 phenotype, she had no congenital fractures and survived until 7 years of age. Her clinical presentation included generalized hypotonia, areflexia,

De novo and inherited variants in DDX39B cause a novel neurodevelopmental syndrome.

DDX39B is a conserved member of the DEAD-box family of ATP-dependent RNA helicases, critical in mRNA metabolism across eukaryotes. DDX39B is also a core component of the TRanscription-EXport (TREX) super protein complex, and recent studies have highlighted the important role of its subunits in neurodevelopmental disorders. Here, we describe six individuals from five families, four harbouring de novo missense variants in DDX39B and one with an inherited splicing variant, presenting with variable developmental delay, congenital hypotonia, epilepsy, short stature, skeletal abnormalities, dysmorph

Managing the Uncertainty of "Precision" While Navigating Goals of Care: A Framework for Collaborative Interpretation of Complex Genomic Testing Results in Critically-Ill Neonates.

Each year, many neonates are born with genetic diagnoses that carry a range of prognoses. As the types and availability of genetic testing have expanded, neonatal intensive care units (NICUs) have served as "launching points" for their clinical application. Broad genetic testing has both improved diagnostic precision and expanded uncertainty. Genetic information may be explicitly uncertain, as in the case of a variant of unknown significance (VUS). But it is also frequently uncertain whether/how the information relates to a patient's phenotype or what it may mean for a child's future. Even wit

Application of the neonatal 'floppy module' to older infants: can it be used for differential diagnosis?

UNLABELLED: In this study we applied the 'floppy module', originally developed for newborns with hypotonia, to older infants in order to establish the range of findings in both low-risk infants and in those with hypotonia due to different etiologies. Data were collected from 413 assessments obtained in 159 low-risk infants assessed at different ages between the age of 3 and 24 months. The distribution of findings in the low-risk infants was similar to the data previously obtained in low-risk newborns, with all assessments having optimal findings (column 3) on all items. The only item with a sm

Whole Blood Multi-OMIC Analysis Is Effective in Clinical Interpretation of Splicing Aberrations in PLOD1 -Related Kyphoscoliotic Ehlers-Danlos Syndrome.

Kyphoscoliotic Ehlers-Danlos syndrome (kEDS) Type 1 is a rare hereditary connective tissue disorder due to biallelic deleterious variants in PLOD1 and is mainly characterized by hypotonia, congenital kyphoscoliosis, eye fragility, hyperextensible skin, Marfanoid habitus, and joint hypermobility. In PLOD1-kEDS, deleterious variants are typically loss-of-function alleles. Therefore, the identification of private non-canonical splicing variants might deserve functional studies to refine their clinical interpretation. In a 7-year-old boy with congenital hypotonia, kyphoscoliosis, joint hypermobili
