[Intracranial hemorrhages in the full-term newborn infant. Psychomotor development and neurologic sequelae].

Recently increased utilization of high definition T. C. scan and U. S. has led, in recent years, to diagnose and to localize the intracranial haemorrhage of the term newborns. It has been possible to study the outlook of this disease. The neuro-psychological development of the children, affected by intracranial haemorrhage at the birth, is more impaired when the haemorrhage is intraparenchimal: intracerebellar or intracerebral. Our cases show that a haemorrhage causes only discord on the neuro-psycholological development. When, on the other hand, haemorrhage is associated with anoxicbrain dama

Predictive validity of the "Movement Assessment of Infants".

Early identification of neuromotor deficits, cerebral palsy or other neurological handicaps, is a focus of concern for neurologists, pediatricians, and developmental therapists. Among infants at risk for developing these handicaps are those with low birthweight, idiopathic respiratory distress syndrome, and early central nervous system insults. The Movement Assessment of infants (MAI), a neuromotor assessment tool, was developed for the purpose of evaluating high-risk infants participating in the University of Washington's Neonatal Intensive Care Unit Followup Clinic. The predictive validity o

[Prognostic value of the neurodevelopmental status in the first year of life in children with increased perinatal risk].

Among 503 high-risk newborns followed-up systematically in a prospective study up to 5 years of age, abnormalities of the development of posture, of maturation of tone and of psychomotor development were diagnosed at the age of 6 months in 70 children (13.9%) who form the study group. Among the other 433 high-risk newborns who presented a normal psychomotor development at the age of 6 months, 73 were selected at random and form the control group. In the study group, 59% of the children had a development which became normal during the second year of life (transitory disorders), 21% had persiste

The neurofaciodigitorenal (NFDR) syndrome.

We describe in two brothers a previously apparently unreported multiple congenital anomalies/mental retardation (MCA/MR) syndrome of high, prominent forehead, vertical groove on tip of nose, "cowlick," ear anomalies, acrorenal field defect (incipient unilateral triphalangism, broad halluces, with unilateral renal agenesis in one of the boys), megalencephaly associated with congenital hypotonia, severe mental retardation and highly abnormal EEG without seizures, intrauterine growth retardation and primordial shortness of stature in one brother. This is a Group III ("provisionally private") MCA/

Exchange transfusion in neonatal myasthenia gravis.

An infant with transient neonatal myasthenia gravis had a double-blood-volume exchange transfusion because of maternal-fetal blood group incompatibility. This seemed to accelerate both decline in antiacetylcholine antibody titer and clinical improvement.

Myopathy with multiple minicore--report of two siblings.

Two cases of non-progressive congenital hypotonia are described in siblings, male and female, aged 5 and 9 years, respectively, which morphologically correspond to myopathy with multicore or minicore. The study of these 2 cases is compared with those described in the literature, with special emphasis on the analysis of the histochemical picture. The disease in all the cases is defined by the presence of multiple small foci of loss of cross striation with loss of activity of myofibrillar ATPase and oxidative enzymes. Furthermore, a predominance and hypotrophy of type I fibers and in some cases

The behavior of jaundiced infants undergoing phototherapy.

Ten full-term jaundiced infants were examined with the Brazelton Scale before, during and following phototherapy. They were compared with 10 non-jaundiced control infants examined at the same post-partum ages. All infants were appropriate for gestational age and free of perinatal complications other than hyperbilirubinemia. Differences on the orienting items of the Brazelton examinations (primarily visual orienting) were found prior to the onset of phototherapy or separation, and persisted for the length of the study. The greatest over-all differences were noted during phototherapy and separat

[Congenital muscular dystrophy].

Four patients with typical signs of congenital muscular dystrophy (C.M.D.), as described in the literature, are reported. In two young sisters born from consanguineous parents the presenting signs were severe congenital hypotonia in one and hypotonia with arthrogryposis in the other. The two other cases were adult patients with a long standing disease, the onset haring been marked by a transient neonatal hypotonia in one and by a congenital torticollis in the other. All 4 patients had progressively increasing muscle retractions, with absent reflexes in three. C.P.K. was moderately increased in

[Ullrich syndrome: a hypotonic disorder of early infancy, difficult to define as an entity].

Three cases, a female and two males, with the diagnosis of Ullrich syndrome are presented. Major clinical findings included congenital hypotonia, joint contractures, high-arched palate, prominent calcaneus, scoliosis, hyperhidrosis and normal intelligence. The course was not progressive or even underwent slow improvement. Our cases followed the autosomal recessive pattern of inheritance. Muscle enzymes were all within the normal ranges. EMG showed findings that were consistent with a chronic myogenic pattern. Muscle biopsy revealed variation in the size of muscle fiber diameters and large incr

Congenital hypotonia revisited.

The definition of neuromuscular diseases affecting infants has depended on factors as various as the rate of progression of the illness, the clinical picture, and, recently, the morphologic peculiarities in the muscle biopsy. A review of the literature suggests that there are discrepancies in the classification of such illnesses, no matter what system is used. In some instances, a single diagnosis seems to include patients with quite separate illnesses, whereas other patients with seemingly identical diseases have been given different diagnoses.

Recurrent common duct stones, with special reference to primary common duct stones.

A series of 1,358 cholelithic patients operated on at 32 affiliated hospitals during the last year and 492 patients operated on at authors' hospital during the last 21 years were reviewed with special reference to the cases of recurrent common duct stones. Of the former series 15 patients and of the latter series 12 patients respectively were reoperated on for recurrent, but not for residual, common duct stones, and all of them had in the markedly dilated common duct bilirubin stones showing characteristics of stasis stone-primary in common duct. Two representative cases are presented briefly.

Inherited human collagen lysyl hydroxylase deficiency: ascorbic acid response.

A patient is described with congenital hypotonia, lax joints, friable skin, hemorrhagic scars, high-arched palate, and borderline microcornea. Acid hydrolyzed whole skin collagen had a reduced hydroxylysine content of 0.5 residues per 1,000 as compared to 5.1 +/- 0.7 in control skin. Collagen lysyl hydroxylase in dialyzed subcellular fractions of cultured skin fibroblasts required L-ascorbate as a principal cofactor. Activity of this enzyme in cultured skin fibroblasts derived from this patient, his father, and mother were 17%, 66%, and 39% of control values, respectively. Collagen prolyl hydr

Multicore disease. Report of a case with lack of fibre type differentiation.

A six year old boy had congenital hypotonia and nonprogressive proximal muscular weakness, with mild abnormalities in the E. M. G. and normal serum enzyme levels. There was lack of fibre type differentiation in the quadriceps muscle biopsy. The fibres had high oxidative enzyme activity and low ATPase 9.4 activity. In almost every fibre there were multiple areas of focal decrease of oxidative enzyme activity, resembling in few of them the lesion described in Central Core Disease. There was abscence of mitochondria and disorganization of the sarcomere with streaming of the Z line within the lesi

Delay in the maturation of muscle fibers in infants with congenital hypotonia.

Muscle biopsies of hypotonic children have shown delayed maturation of a fetal type of muscle fibers: subsarcolemmal halo devoid of activity for mitochondrial dehydrogenases, type II predominance and in some cases abnormal dispersion of fiber diameter. Fiber subtypes within group II were also abnormal. One case has definite embryonic characteristics with presence of myoblasts. Not a single clinical pattern was present in these patients and a variety of associated disorders were recognized. Some patients had a clinical picture corresponding to congenital benign hypotonia as described by Walton.

Understanding the floppy baby.

Muscle tone develops in a orderly sequence through gestation and continues to change after birth. Hypotonia is frequently found in infants, and pathological degrees must be differentiated from normal variations. This distinction is possible if the clinician understands how muscle tone is regulated and modifies his examination to include some special clinical signs. A variety of pathological conditions can influence muscle tone, and hypotonia can be an early and valuable clue to recognizing neuromuscular, CNS, metabolic, and other disease states in this age group.

Studies of malformation syndromes of man XIB: the cerebro-hepato-renal syndrome of Zellweger: comparative pathology.

A study of 11 autopsied cases of the cerebro-hepato-renal syndrome of Zellweger (ZS) is reported. All cases had severe, persistent congenital hypotonia, hepatic lobular disarray, renal cortical cysts and pulmonary hypoplasia. Many had cardiovascular malformations, hepatomegaly, cerebral cortical gyral maldevelopment and pancreatic islet hyperplasia. Additional, less frequent findings are delineated. Results of iron content studies of hepatic and renal tissues are related to age of survival and possible development of fibrosis.

[Further remarks on histochemistry applied to myodiagnosis: findings of "type predominance" (author's transl)].

Data concerning muscular biopsies (histochemically examined) of three patients affected by Charcot-Marie-Tooth disease, neurogenic atrophy of spondilosic origin and benign congenital hypotonia, are described. The common finding was a histochemical appearence of "type predominance". This point and the possible "neurogenic" origin of benign congenital hypotonia, are discussed.
