We report a case in which a girl diagnosed with HNRNPU-associated neurodevelopmental disorder presented with acute encephalopathy with biphasic seizures and late reduced diffusion (AESD) and generalized hypotonia accompanied by chorea-like involuntary movements. On the first day of hospitalization, she presented with fever and generalized clonic seizures. Twenty four hours later, she continued to experience impaired consciousness and generalized hypotonia. Her consciousness improved gradually, but chorea-like involuntary movements of the limbs were observed. From the seventh day of hospitaliza

INTRODUCTION: Birk-Barel syndrome (BIBARS), also known as KCNK9 imprinting syndrome, is an extremely rare genetic condition caused by pathogenic variants in the KCNK9 gene located on the long arm of chromosome 8 (8q24). This gene is paternally imprinted and therefore only expressed from the maternal allele. KCNK9 encodes for a member of the two-pore domain potassium channel (K2P) subfamily. BIBARS syndrome is characterized by congenital hypotonia and weakness of proximal muscles, intellectual disability, behavioural problems, dysmorphic features, feeding problems and epilepsy. The syndrome is

BACKGROUND/OBJECTIVES: Infantile cardiomyopathy is a rare but often life-threatening condition in which monogenic causes are particularly relevant, especially when cardiac disease is preceded by hypotonia or multisystem involvement. Among sarcomeric genes, MYL2, encoding the ventricular regulatory myosin light chain, plays a critical role in myocardial contractility. However, biallelic MYL2-associated disease remains exceptionally rare, and its clinical spectrum is not fully defined. This study aims to describe a novel case and further delineate the phenotype of recessive MYL2-related cardiomy

INTRODUCTION: Gillespie syndrome is characterized by bilateral aniridia, cerebellar hypoplasia leading to ataxia, congenital hypotonia, and variable degrees of intellectual disability. The condition is attributed to pathogenic variants in ITPR1, either in a biallelic manner or as heterozygous dominant-negative mutations.
CASE PRESENTATION: Herein, we report the case of a male neonate with generalized hypotonia, and bilateral aniridia, identified during routine newborn eye screening. His abdominal ultrasound revealed a left dysplastic pleiocystic kidney, and his brain magnetic resonance imagin

Prader-Willi syndrome (PWS) is a neuroendocrine disorder characterized by hypothalamic dysfunction, congenital hypotonia, abnormal growth trajectories, and impaired pubertal development, all of which contribute to a markedly increased risk of scoliosis, with a cumulative prevalence reaching up to 70-80% by skeletal maturity, significantly exceeding that of idiopathic scoliosis. Unlike idiopathic scoliosis, spinal deformity in PWS follows a distinct bimodal pattern, with critical vulnerability during infancy and a second acceleration during pubertal transition. Growth hormone (GH) therapy, a co

PYROXD1-related myopathy is a rare genetic disorder with variable age of onset, ranging from the neonatal period to late adulthood. We report a male infant presenting with congenital hypotonia, progressive severe weakness, respiratory insufficiency, stridor, and ophthalmoplegia. Muscle biopsy revealed fibre size variation, internalized nuclei, and myofibrillary changes with sarcoplasmic disorganization. Genetic testing identified a novel homozygous missense variant in the PYROXD1 gene (NM_024854.5:c.1498G>A, p.Asp500Asn), located at the C-terminal end, affecting the final amino acid before the

BACKGROUND/OBJECTIVES: Arthrochalasia Ehlers-Danlos syndrome (aEDS) is a rare connective tissue disorder characterized by severe joint hypermobility, congenital hip dislocation, skin hyperextensibility, and muscle hypotonia. It is typically caused by heterozygous splice-site variants in COL1A1 or COL1A2, leading to exon 6 skipping. Autosomal recessive forms are extremely rare and have been reported predominantly in families from Saudi Arabia carrying the homozygous COL1A1 missense variant c.2050G>A, p.(Glu684Lys), with clinical presentations ranging from severe to mild.

We report clinical and genetic features in four patients from 3 independent families with an ultra-rare autosomal recessive myopathy associated with biallelic pathogenic or likely pathogenic variants in MSTO1. Exome or genome sequencing was used to identify genetic variants in patients with suspected hereditary myopathy who had negative results on targeted genetic panels. Age at diagnosis ranged from 13 to 30 years. All patients exhibited myopathy of variable severity. Two had congenital hypotonia and global developmental delay, while the remaining two developed muscle weakness at ages 2 and 5

This report describes the diagnostic and treatment course of an adult woman with the ultra-rare Au-Kline syndrome (AUKS). Since the age of 32 years, when the syndrome was yet undiagnosed, the patient suffered from behavioral decline coinciding with changes in living conditions. At the age of 38, following a bladder infection, she presented with a delirium for which antipsychotic medication was started. The delirium resolved but, while the medication was continued, the pre-existing behavioral symptoms persisted. Clinicpal genetic evaluation revealed a history of congenital hypotonia, developmen

Spinal muscular atrophy with congenital bone fractures type 2 (SMABF2) is an ultra-rare neuromuscular disorder caused by pathogenic variants affecting the ASC-1 complex, most commonly ASCC1. The disorder is typically characterized by severe congenital hypotonia, prenatal or congenital fractures, and early respiratory failure. We report a girl with a novel homozygous intronic donor-site variant, c.95+5G>C, in ASCC1. In contrast to the classic SMABF2 phenotype, she had no congenital fractures and survived until 7 years of age. Her clinical presentation included generalized hypotonia, areflexia,

DDX39B is a conserved member of the DEAD-box family of ATP-dependent RNA helicases, critical in mRNA metabolism across eukaryotes. DDX39B is also a core component of the TRanscription-EXport (TREX) super protein complex, and recent studies have highlighted the important role of its subunits in neurodevelopmental disorders. Here, we describe six individuals from five families, four harbouring de novo missense variants in DDX39B and one with an inherited splicing variant, presenting with variable developmental delay, congenital hypotonia, epilepsy, short stature, skeletal abnormalities, dysmorph

Each year, many neonates are born with genetic diagnoses that carry a range of prognoses. As the types and availability of genetic testing have expanded, neonatal intensive care units (NICUs) have served as "launching points" for their clinical application. Broad genetic testing has both improved diagnostic precision and expanded uncertainty. Genetic information may be explicitly uncertain, as in the case of a variant of unknown significance (VUS). But it is also frequently uncertain whether/how the information relates to a patient's phenotype or what it may mean for a child's future. Even wit

Kyphoscoliotic Ehlers-Danlos syndrome (kEDS) Type 1 is a rare hereditary connective tissue disorder due to biallelic deleterious variants in PLOD1 and is mainly characterized by hypotonia, congenital kyphoscoliosis, eye fragility, hyperextensible skin, Marfanoid habitus, and joint hypermobility. In PLOD1-kEDS, deleterious variants are typically loss-of-function alleles. Therefore, the identification of private non-canonical splicing variants might deserve functional studies to refine their clinical interpretation. In a 7-year-old boy with congenital hypotonia, kyphoscoliosis, joint hypermobili

PIEZO2 encodes a mechanosensitive ion channel essential for touch perception, proprioception, and interoception, with biallelic loss-of-function variants known to cause a recessive mechanosensory neuropathy characterized by hypotonia and skeletal abnormalities; however, intragenic duplications have rarely been reported. We present a nine-year-old male with global motor delay, congenital hypotonia, distal muscle weakness, sensory neuropathy, thoracolumbar neuromuscular scoliosis, and multiple orthopedic abnormalities, along with microcephaly, cryptorchidism, chronic urinary incontinence, and ea

UNLABELLED: Wiedemann-Steiner syndrome (WSS) is a rare autosomal dominant neurogenetic disorder caused by monallelic variants in KMT2A gene, characterized by neuromotor developmental delay, intellectual disability, microcephaly, seizures, behavioral disorders, dysmorphic facial features, hirsutism, and systemic anomalies. The KMT2A gene encodes a histone lysine methyltransferase crucial for the regulation of gene expression during early developmental stages. In this study, the clinical and molecular findings of 15 Turkish patients with WSS confirmed by whole exome sequencing are reported. Vari

King-Denborough Syndrome (KDS) is a congenital myopathy (CM) characterised by myopathy, dysmorphic features and susceptibility to malignant hyperthermia. The objective of this study was to investigate the genotype-phenotype correlation in Black African patients presenting with CM, specifically those with KDS-like phenotypes, who remained undiagnosed for over 25 years. A cohort of 67 Black African patients with CM was studied, of whom 44 were clinically evaluated and diagnosed with KDS. Whole-exome sequencing (WES) was performed as part of an international genomics study (ICGNMD) to identify po

Congenital myopathies and congenital muscular dystrophies encompass heterogeneous clinical and genetic groups of disorders characterised by muscle weakness with antenatal or early postnatal onset. These conditions are categorised according to distinctive myopathological features and causative genes. Despite advances in diagnosis through massive parallel sequencing and progress in understanding the underlying pathogenesis, many aspects of these disorders remain poorly understood. MYL1-related congenital myopathy is an ultra-rare and severe condition, associated with a deficiency of essential/al

Salih myopathy is autosomal recessive hereditary early-onset myopathy with fatal cardiomyopathy. It is a rare and heterogeneous form of congenital titinopathies (TTN). Affected children have delayed motor development, normal mental development, and in further course dilated cardiomyopathy. Motor functions have a tendency to improve, but death occurs most often before 20 years of age due to arrhythmias. Our patient is a 2-year-old girl, born in severe perinatal asphyxia, with global hypotonia and poor spontaneous movements. She required immediate endotracheal intubation and mechanical ventilati
