Perinatal outcomes of twin pregnancies complicated by early twin-to-twin transfusion syndrome treated with fetoscopic laser surgery.

OBJECTIVE: The aim of this study was to evaluate the perinatal outcomes of monochorionic diamniotic (MCDA) twin pregnancies complicated by early twin-to-twin transfusion syndrome (TTTS) treated with fetoscopic laser surgery (FLS), and to compare rates of fetal survival across Quintero stages. METHODS: This was a multicenter retrospective cohort study of MCDA pregnancies complicated by TTTS diagnosed ≤ 18 weeks' gestation (early TTTS) that underwent FLS from January 2007 to August 2023. Monoamniotic twins, triplets or higher-order multiple gestations, and pregnancies complicated by genetic or

Hysteroscopic fundal fixation of LNG-IUS in adenomyosis: a prospective feasibility and safety study.

BACKGROUND: Expulsion of the levonorgestrel-releasing intrauterine system (LNG-IUS) limits its effectiveness in adenomyosis management, particularly in patients with anatomical risk factors. This study aimed to assess the feasibility and short-term safety of a novel hysteroscopic fixation technique designed to improve device retention. METHODS: In this single-center prospective feasibility study, 52 patients with adenomyosis underwent placement of a modified LNG-IUS (integrated with a GyneFix anchor) using a customized delivery cannula under hysteroscopic guidance. Feasibility outcomes include

Intrahepatic cholestasis of pregnancy and offspring neurodevelopment: bile acid-mediated mechanisms and long-term neurodevelopmental outcomes.

PURPOSE: Intrahepatic cholestasis of pregnancy (ICP) is a pregnancy-specific liver disorder characterized by pruritus and elevated maternal serum total bile acids. While clinical management has focused on bile acid-based risk stratification and delivery planning to reduce perinatal risks, emerging evidence suggests possible longer-term effects on offspring neurodevelopment. This review synthesizes current epidemiologic and mechanistic evidence linking ICP with offspring neurodevelopmental outcomes. METHODS: PubMed, Embase, and Web of Science were searched using controlled vocabulary and free-

Identifying biomarkers of the serum proteins and metabolites associated with severe preeclampsia.

BACKGROUND: Preeclampsia is a severe pregnancy-related disorder characterized by hypertension and proteinuria, and affects approximately 2–8% of pregnancies worldwide. It poses substantial risks to both maternal and fetal health, including preterm birth, intrauterine growth restriction, and life-threatening complications, such as eclampsia and organ failure. However, the mechanisms underlying preeclampsia, especially those involving blood-associated proteins, are poorly understood. This study aimed to identify critical blood proteins linked to severe preeclampsia and explore their potential me

Evaluation of sST2 Levels in Infants of Mothers with Gestational Diabetes.

Objectives: Gestational diabetes is linked to increased inflammatory and metabolic stress during the neonatal period. Among the biomarkers elucidating the relationship between diabetes and inflammation, the interleukin-33 (IL-33)/ST2 signaling pathway is of particular interest. Research on the IL-33/sST2 axis in pregnancies complicated by diabetes indicates that these biomarkers are associated with maternal metabolic disorders and inflammation. Therefore, evaluating sST2 levels in infants of diabetic mothers is essential for identifying a biological marker of systemic inflammation resulting fr

VCP variants cause Adams-Oliver syndrome with or without pulmonary hypertension.

PURPOSE: Adams-Oliver syndrome (AOS) is a genetically heterogeneous disorder with cardinal features of aplasia cutis congenita and terminal limb reduction defects. A minority of individuals with AOS develop potentially lethal pulmonary hypertension (PH) in infancy, a subgroup that has been refractory to genetic explanation. METHODS: We studied a cohort of individuals with AOS and no genetic diagnosis by genome and exome sequencing. We characterized rare, identified substitution variants in valosin-containing protein (VCP) in vitro using ATP hydrolysis, cryogenic-electron microscopy, thermal s

Long-Term Gastrointestinal Outcomes in Children Exposed in Utero to Maternal Chronic Hypertension.

BACKGROUND: Maternal hypertension has been linked to adverse pregnancy and neonatal outcomes, but long-term effects on offspring, particularly gastrointestinal (GI) health, are less well studied. Chronic hypertension may impair uteroplacental perfusion and promote a pro-inflammatory intrauterine environment, potentially disrupting foetal gut development; however, its independent effect on offspring GI health has not been examined. OBJECTIVES: To evaluate whether in utero exposure to maternal chronic hypertension, independent of preeclampsia, is associated with long-term GI morbidity in offspr

Duplicated hypoglossal canal prevalence in a fetal and infantile population: neurosurgical considerations.

PURPOSE: The hypoglossal canal can develop bony ingrowths that create the appearance of hypoglossal canal duplication, encroach upon neurovasculature, and complicate condylectomy and transcondylar neurosurgical approaches. Numerous reports have identified the prevalence of the duplicated hypoglossal canal in adults; yet, data in the pediatric population remains limited. Therefore, this study assesses hypoglossal canal duplication among a fetal and infantile population. METHODS: The study assessed hypoglossal canals in dry exoccipital bones from a modern population of 112 fetal and infantile c

Cell migration and adhesion in autophagy-induced damage of human trophoblast cells.

INTRODUCTION: Intrahepatic cholestasis of pregnancy (ICP) is a common pregnancy complication. It has relatively minor impacts on the mother, but elevated bile acid levels pose severe risks to the fetus, potentially causing adverse pregnancy outcomes such as preterm birth, intrauterine distress, and fetal death. Autophagy plays a pivotal role in embryonic development, implantation, and pregnancy maintenance. Emerging evidence indicates autophagy is involved in ICP, yet its underlying mechanism remains unclear. METHODS: An ICP-associated autophagy-impaired cell model was established. This was a

Decreased Maternal Weight Gain in Pregnancy Is Seen with Buprenorphine Medication-Assisted Treatment.

OBJECTIVES: Medication-assisted treatment (MAT) is associated with low maternal weight gain, and low maternal weight gain is associated with infant mortality. Many MAT patients also are smokers. This study investigates buprenorphine administration during pregnancy as a possible independent risk factor for low maternal weight gain. To our knowledge, this is the first study that compares pregnant buprenorphine patients with both smoking and nonsmoking pregnant control groups. METHODS: Our institution offers a combined MAT and prenatal care program for pregnant patients with opioid use disorder.

A Challenging Case Report of Severe Asherman's Syndrome: Managing Uterine Perforation During Surgery.

INTRODUCTION: Asherman's syndrome (AS), characterized by intrauterine adhesions (IUAs), can significantly distort uterine anatomy and complicate hysteroscopic adhesiolysis. Although hysteroscopic adhesiolysis is the standard treatment, severe adhesions increase the risk of complications such as uterine perforation. CASE ILLUSTRATION: We report a 31-year-old woman with severe AS who developed uterine perforation during hysteroscopic adhesiolysis. During dissection of dense adhesions, a small fundal perforation (0.5 cm × 0.5 cm) was suspected after sudden loss of resistance and bleeding. The pr

Metagenomic analysis of tongue samples from healthy subjects identifies distinct microbiome orotypes.

IMPORTANCE: Two-thirds of women in labor receive supplemental oxygen to reverse perceived fetal hypoxemia and prevent acidemia. Oxygen is routinely administered for category II fetal heart tracings, a class of fetal tracing used to designate intermediate risk for acidemia. This liberal use of oxygen may not be beneficial, particularly because neonatal hyperoxygenation is harmful. OBJECTIVE: To test the hypothesis that room air is noninferior to oxygen in improving fetal metabolic status among patients with category II fetal heart tracings. DESIGN, SETTING, AND PARTICIPANTS: This was a random

Mental health in expectant mothers after medically assisted reproduction and spontaneous conception: the nuMoM2b study.

BACKGROUND: Mental health disorders are common during pregnancy and affect up to 20% of individuals. Women who conceive via medically assisted reproduction (MAR) may face increased psychological stress due to treatment burden and uncertainty, potentially elevating their risk of mental health conditions. However, evidence comparing mental health in women conceiving via MAR versus spontaneous conception remains inconsistent. METHODS: We analyzed data from 9289 nulliparous women in the prospective US-based nuMoM2b cohort, including 196 who conceived via assisted reproductive technology (ART), 17
