Operative hysteroscopy vs vacuum aspiration for treatment of retained products of conception: A systematic review and meta-analysis.

OBJECTIVE: To generate comparative evidence on outcomes of hysteroscopic (HS) vs vacuum aspiration (VA) for retained products of conception (RPOC). METHODOLOGY: We included all study designs comparing HS with VA for RPOC and available on PubMed, CENTRAL, Scopus, Embase, and Web of Science up to 15th January 2025. Surgical and pregnancy outcomes were analyzed. RESULTS: Four studies were included. Meta-analysis showed that procedure duration was significantly longer with HS as compared to VA. The risk of intra-operative and post-operative complications did not differ between the two groups. Th

Healthcare experiences and needs of patients with pregnancy of unknown location: a qualitative study.

BACKGROUND: Patients presenting with bleeding or pelvic pain in early pregnancy are commonly found to have pregnancies of unknown location (PUL), with no sonographic findings of intrauterine nor ectopic pregnancy. We aimed to explore healthcare experiences and needs of patients presenting with PUL. METHODS: This was a qualitative study using semi-structured interviews to explore healthcare experiences among patients with PUL. Patients were eligible if they presented with bleeding or abdominopelvic pain to the OB/GYN office or emergency department and were found to have a PUL followed in an el

Predictive value of OGTT parameters and clinical markers in gestational diabetes mellitus: a prospective randomized controlled trial from a tertiary center in Türkiye.

BACKGROUND: Gestational diabetes mellitus (GDM) remains a major obstetric concern, yet the optimal screening strategy and the prognostic value of oral glucose tolerance test (OGTT) parameters remain debated. We aimed to compare the diagnostic yield and clinical outcomes of a two-step OGTT strategy (50 g glucose challenge followed by 100 g OGTT) versus a one-step 75 g OGTT approach, and to evaluate the predictive performance of individual OGTT time points for pregnancy complications and treatment requirement. METHODS: In this prospective randomized controlled trial, 1,439 pregnant women underg

[Pain management during pregnancy : A balancing act between benefits and risks].

Pain management during pregnancy poses a considerable challenge in everyday clinical practice. The relief of maternal discomfort must be weighed against the potential risks of using medication (nonsteroidal anti-inflammatory drugs, paracetamol, steroids, opioids, gabapentinoids). The current data on congenital anomalies, complications during pregnancy, neonatal adaptation or withdrawal symptoms, and long-term psychomotor developmental disorders in children exposed in utero must be taken into account. However, refraining from treating chronic maternal pain during pregnancy can also cause proble

Possibilities and Limitations of Prenatal Diagnosis of Rare Imprinting Syndromes: Prader-Willi Syndrome.

Background: Prader-Willi syndrome (PWS) is a multisystemic complex imprinting disorder. Prenatal diagnosis of PWS is still a challenge with non-specific ultrasound markers and limitations for diagnosis with non-invasive screening methods. Prenatal suspicion and early postnatal diagnosis are essential for promoting healthy growth and development, preventing complications, and providing healthcare professionals and families with the necessary support and resources for effective management. Presentation: We report two PWS cases caused by maternal uniparental disomy, who presented with IUGR, chara

Outcomes Associated With Illicit Fentanyl Exposure During Pregnancy: A Retrospective Cohort Study.

BACKGROUND: Illicitly manufactured fentanyl (IMF) is the predominant opioid in the US drug supply and is a major contributor to morbidity and mortality during pregnancy. While prenatal opioid exposure is an established risk factor for neonatal opioid withdrawal syndrome (NOWS), the differential effects of IMF vs other opioids on outcomes are largely unknown. Therefore, we aimed to evaluate the associations between prenatal IMF exposure and fetal, perinatal, and neonatal health outcomes. METHODS: We conducted a retrospective cohort study of pregnant women with opioid use disorder (OUD) and the

Beyond Standard Algorithms: Diagnostic, Procedural, and Analgesic Challenges in Refractory Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) remains a leading cause of preventable maternal morbidity and mortality, with management often complicated by overlapping etiologies, delayed manifestations of hypovolemia, and procedural challenges related to analgesia availability. We report a case of recurrent PPH in a 24-year-old gravida 2, para 2 woman following spontaneous vaginal delivery without neuraxial anesthesia, in whom persistent bleeding refractory to uterotonic therapy was associated with discordant findings of a firm uterine fundus and a boggy lower uterine segment. Hemorrhage control was delayed by

Pregnancy and perinatal complications in untreated subfertile couples following singleton spontaneous conception: a narrative review.

BACKGROUND: Literature shows that ovulation induction, intrauterine insemination, and assisted reproductive technology are independently associated with increased risks for pregnancy, perinatal, and childhood health complications. However, the independent association of couples’ subfertility/infertility with these risks has not been yet isolated precisely. This narrative review aims to ascertain the independent association of female and male reproductive disorders displayed by untreated subfertile couples with pregnancy, perinatal, and childhood complications following singleton spontaneous co

Monocyte Monolayer Assay as a Tool in the Early Assessment of the Severity of the Alloantibodies in Patients with Hemolytic Disease of the Fetus and Newborn.

INTRODUCTION: Hemolytic disease of the fetus and newborn (HDFN), caused by the transplacental passage of maternal alloantibodies, can lead to progressive fetal organ failure and death from hydrops fetalis. Because serological tests often have limitations in predicting the clinical severity of these antibodies, in vitro assays have become promising tools, allowing the evaluation of the IgG-mediated extravascular hemolysis mechanism via macrophage Fc receptors. We aim to evaluate the monocyte monolayer assay (MMA), considered the most clinically validated tool for predicting in vivo survival in

From weeks to hours: rapid whole-genome sequencing reduces diagnostic odyssey in Menke-Hennekam syndrome-a case report.

BACKGROUND: Menke-Hennekam syndrome (MKHK) is a rare autosomal dominant disorder caused by mutations in the CREBBP and EP300 genes. The absence of established diagnostic criteria and non-specific clinical manifestations complicate timely diagnosis and management. This report presents a case of MKHK in which early diagnosis and intervention were achieved through the application of rapid whole-genome sequencing (rWGS), a tool that offers superior speed and genomic coverage compared to whole-exome sequencing (WES). CASE PRESENTATION: This case report describes a male Han Chinese neonate who pres

The Effect of Polycythemia Vera on Pregnancy Complications. The Analysis of an American Population Database With Over 9 Million Deliveries.

OBJECTIVES: Polycythemia Vera (PV) is characterized by overproduction of erythrocytes, leading to a hypercoagulable state. Evidence regarding associations between PV and pregnancy outcomes is limited. The aim of our study was to provide robust evidence regarding the prevalence of pregnancy complications in women with PV. METHODS: We conducted a retrospective cohort study using the HCUP-NIS database, a large American database, between 2004 and 2014. Pregnancies in women with PV were identified via ICD-9 codes. All other pregnancies formed the control group. RESULTS: Our cohort included 104 pr

A Swift Solution: Early Removal of the JADA Device in the Management of Postpartum Hemorrhage.

Postpartum hemorrhage (PPH) continues to be a major threat to maternal health worldwide. Current management strategies include uterotonic agents, intrauterine tamponade devices, and surgical interventions. These methods have variable efficacy, carry potential complications, and contribute to significant healthcare costs. The JADA device is an emerging and promising tool for managing PPH. We report the case of a 19-year-old G2P1011 at 39w3d who experienced uterine atony following a spontaneous vaginal delivery. Despite administration of Pitocin and tranexamic acid, the patient continued to have

Efficacy of OB balloon tamponade in cesarean delivery for placenta previa and low-lying placenta: a retrospective comparative study.

BACKGROUND: Placenta previa and low-lying placenta are associated with substantial peripartum hemorrhagic complications, particularly during cesarean delivery. This retrospective study compared the efficacy and safety of a newly developed OB balloon—featuring a double-balloon structure designed to simultaneously compress the uterine and vaginal sides—with the conventional Bakri balloon for intrauterine tamponade. METHODS: We retrospectively analyzed cases of placenta previa or low-lying placenta managed with either the OB or Bakri balloon during cesarean delivery. The primary outcome was ballo

Evaluation of the Efficacy of Systemic Inflammatory Markers in Predicting Medical Treatment Failure in Patients with Tubo-Ovarian Abscess.

BACKGROUND/OBJECTIVES: Tubo-ovarian abscess (TOA), a serious complication of pelvic inflammatory disease, poses a serious risk to sexually active women, characterized by high rates of morbidity and mortality. Despite antibiotic therapy being the primary treatment for non-ruptured, stable TOA cases, failure rates of medical management remain as high as 30%. This study investigates the potential of systemic inflammatory markers, particularly the systemic immune-inflammation index (SII), in predicting the necessity for surgical intervention following the failure of medical treatment in TOA patien
