[Epilepsy and pregnancy (review of the literature)].

The relationship between pregnancy and epilepsy has often been object of particular attention and care by many epileptologists. The aim of this survey of the literature is to give an up-to-date and comprehensive review of this relation and to focalize the many related problems which still are unsolved. The following principal chapters are discussed: 1) the influence of pregnancy on epilepsy; 2) the influence of epilepsy on pregnancy and delivery; 3) the influence of epilepsy on newborns; 4) the teratogenic effects of antiepileptic medication. Many conflicting data make it very difficult to sum

[The newborn infant of mothers under treatment with anticonvulsants. Description of 5 cases].

The effects of fetal exposure to the anticonvulsant drugs and their clinical, diagnostic and therapeutic problems have been considered. The authors have reported five cases of newborns from addicted mothers. Particularly discussed are some clinical and pathologic correlations involved in the exposure to the Hydantoin and to the Sodium Valproate.

Neonatal drug withdrawal.

The Committee on Drugs of the American Academy of Pediatrics recommends that thoughtful consideration be given to the need for administration of pharmacologic agents to newborn infants who have symptoms of drug withdrawal. Supportive care should be the first line of therapy, and objective methods such as an abstinence scoring sheet should be used to determine the need for instituting and then discontinuing pharmacologic treatment. When appropriate, specific drug therapy should be used for treatment of withdrawal symptoms (ie, phenobarbital for phenobarbital withdrawal and opiate for opiate wit

Fetal outcome in narcotic-dependent women: the importance of the type of maternal narcotic used.

The records of 239 infants born to 228 women dependent on narcotic drugs were reviewed to determine if type of drug abused and adequacy of prenatal care would affect pregnancy and fetal outcome. Seventy-nine (33%) pregnancies occurred in women in supervised methadone maintenance, 78 (32%) in women on unsupervised methadone maintenance, 49 (21%) in women on street heroin, and 33 (14%) in women who were multiple drug users. Although the presence of withdrawal symptoms did not differ with respect to type of drug abused, the outcome was significantly better in those infants born to women on superv

[Drug withdrawal syndrome in the neonate].

The relationship between maternal abuse of methadone and neonatal signs of withdrawal has been studied. The main symptoms are frequent vomiting or diarrhea, hyperpyrexia, moderate or severe irritability and tremors, inability to sleep between feedings, and clinically apparent convulsions. Physicians caring for such high-risk infants should recognize the potential seriousness of this problem.

[Fetal defects caused by the passive consumption of drugs].

The neuroactive drugs taken by pregnant women have two principals side effects: a teratogenic effects and a postnatal withdrawal effect. Three are the characteristic syndromes: the hydantoin-barbiturate syndrome, the trimethadione-paramethadione syndrome, the fetal alcohol syndrome. The prenatal exposure to heroine and to morphine interfere mainly with the cerebral development. Drug addiction, like postnatale withdrawal syndrome, are mainly due to passive exposure to drug narcotic (heroine, methadone), to sedative-hypnotic agents (barbiturates Diazepam Chlordiazepoxide) and to alcohol. The cig

Anticonvulsants during pregnancy and lactation. Transplacental, maternal and neonatal pharmacokinetics.

Few data are available on placental transfer of anticonvulsants during early pregnancy. Nevertheless, it has been demonstrated that at this early stage of gestation, considerable amounts of phenytoin, primidone/phenobarbitone and carbamazepine as well as some of their metabolites are already present in fetal tissues. Potentially reactive metabolites of anticonvulsants can be formed by the fetal liver and accumulate in some organs. At term, most anticonvulsants are present in neonatal plasma in concentrations similar to those in maternal plasma. Valproic acid, on the other hand, can accumulate

Drug withdrawal syndromes-- a literature review.

Drug withdrawal syndromes reportedly have been caused by numerous pharmacological agents, but only a few drugs have been adequately studied in this regard. Criteria for evaluating drug withdrawal syndromes have been proposed. Sedative-hypnotic agents, opiates, corticosteroids, clonidine, tricyclic antidepressant medications and beta-adrenergic blocking agents meet the criteria for such syndromes. Gradual tapering of the dose of these drugs is recommended when therapy must be discontinued. Whether or not other drugs cause rebound reactions is questionable, but caution should be used when discon

Abuse of alcohol, drugs and tobacco during pregnancy--consequences for the child.

There is an increasing awareness that alcohol and drug abuse and smoking are not only harmful for the consumer but will also, when taken by a pregnant woman, adversely affect her unborn child. The consequence of the abuse will result from a combination of specific toxic effects of abusing substances and a nonspecific effect from the often very unstable environment of an alcohol- or drug-dependent mother. Poor prenatal care is a common finding in alcohol and drug abuse leading to a high incidence of complications during pregnancy and delivery including premature labor and small-for-gestational-

Side effects on fetus and infant of psychotropic drug use during pregnancy.

Psychotropic drugs are used frequently for the treatment of emotional as well as other disorders. With usage so widespread, many pregnant women receive psychotropic drugs. Maternal ingestion of these drugs may produce, in the fetus, side effects including withdrawal symptoms. Withdrawal signs in the fetus as a result of maternal intake of opiates, hypnotics, analgesics, and tricyclic antidepressant drugs have been reported. Fetal side effects can occur by maternal ingestion of nuroleptic medications, lithium, antidepressants, anxiolytic sedatives, anticonvulsants, and bromides. The author feel

Narcotic dependency in pregnancy. Methadone maintenance compared to use of street drugs.

The course of pregnancy and delivery in 28 women under closely supervised methadone maintenance (group 1) was compared with that of 57 women using heroin or methadone under less controlled circumstances (group 2) and with that of 30 women free of mood-altering medications (group 3). Women in group 1 had the lowest incidence of coexisting medical problems (p=.025), with an incidence of fetal distress not statistically different from that of women in group 3. Infants born to women group 2 had the highest fetal distress (p less than.05), with four congenital defects, one stillbirth, and one neona

Drug addiction in pregnancy and the neonate.

Drug addiction during pregnancy has been managed in two ways: the detoxification program and the methadone-maintenance treatment program. In the detoxification program, there have been few complications for the mother and child except for low birth weight, meconium-stained amniotic fluid, and an increase in breech presentation. The methadone-maintenance treatment program at this time is the most satisfactory approach to the treatment of heroin addiction. The complications of pregnancy in this program are similar to those of the average obstetric population. Low birth weight at term is still fr

Drug addiction in pregnancy.

32 pregnancies in 29 drug-addicted women were studied ouver eight years in one obstetric department working in close liaison with a drug dependency centre. Of the 24 pregnancies which came to term, only 10 were seen at the antenatal clinic before 22 weeks' gestation. 19 of the women were addicted to heroin or methadone or a mixture of both, 4 were taking soft drugs, and 1 was taking pethidine only. Labour commenced spontaneously in 17 pregnancies and was induced in 7. All labours lasted less than twenty hours. 23 infants survived. Those born to the mother taking pethidine and to 4 mothers who

Naloxone.

Narcotic analgesics and related drugs act as agonists on several receptors that are responsible for their effects on pain perception, mood and feeling state, and respiration, as well as other pharmacologic actions. Naloxone is the first discovered antagonist that is devoid of agonistic activity and appears to be a competitive antagonist at several receptors. The ability of naloxone to displace or prevent the binding of agonistic narcotics is partly responsible for its antagonistic effects. The ability of naloxone to rectify narcotic-depressed homeostats and precipitate abstinence is also relat

Maternal anticonvulsants and perinatal risk.

According to data found in the literature, children born to epileptic mothers on anticonvulsant therapy have an increased perinatal mortality rate, namely 2-3 times the average. The congenital malformations attributed to anticonvulsant drugs cannot fully account for this high mortality rate. A case is described in which a severe bleeding disorder manifested itself in successive offspring. A discussion follows in which this defect in blood coagulation in the newborn and the role played by vitamin K is considered as representing an important and preventable cause of neonatal death and morbidity.
