Neonatal abstinence syndrome: strategies for care of the drug-exposed infant.

Neonatal abstinence syndrome (NAS), a clinical condition that has been recognized for over 30 years, remains a significant clinical issue, although our knowledge of abstinence, its treatment, and outcome continues to grow. The condition is now complicated by polydrug use (which is becoming more prevalent) as well as concomitant use of tobacco, and psychoactive substances that are frequently prescribed to pregnant women. This article reviews the neonatal effects of a variety of substances, discusses the state of the art for clinical care of drug-exposed infants, including NAS patients, and upda

[New finding: transitory horizontal pendular nystagmus secondary to neonatal abstinence syndrome].

UNLABELLED: New finding: transient horizontal pendular nystagmus as a sequella to post-natal withdrawal syndrome. BACKGROUND: Children born of mothers who abused illegal substances during pregnancy can present with signs and symptoms of a withdrawal syndrome, including irritability, jitteriness, diarrhoea. Usually these symptoms appear from a few hours up to a few days after birth. It is frequently followed by a post-withdrawal period of several weeks duration with crying, excitability, sleep irregularities and feeding difficulties. The purpose is to report the presence of nystagmus in infant

OxyContin and neonatal abstinence syndrome.

Opioids are a commonly abused class of drugs. OxyContin abuse, a long-acting oxycodone derivative, has been increasingly identified as a potent narcotic resulting in drug dependence, overdose and even death. Use during pregnancy may result in withdrawal symptoms in the neonate. However, detection of the drug and its metabolites needs special methods in order to initiate appropriate therapy.

Opiate treatment for opiate withdrawal in newborn infants.

BACKGROUND: Neonatal abstinence syndrome (NAS) due to opiate withdrawal may result in disruption of the mother-infant relationship, sleep-wake abnormalities, feeding difficulties, weight loss and seizures. Treatments used to ameliorate symptoms and reduce morbidity include opiates, sedatives and non-pharmacological treatments. OBJECTIVES: To assess the effectiveness and safety of using an opiate, compared to a sedative or non-pharmacological treatment, for treatment of NAS due to withdrawal from opiates. The evidence for use of different opiates was assessed in subgroup analyses. SEARCH STRA

Prevention of the serotonin reuptake inhibitor discontinuation syndrome.

Prevention of the serotonin reuptake inhibitor discontinuation syndrome (SRIDS) is an important issue. The author suggests: (1) serotonin reuptake inhibitors (SRIs) should be used only when they are necessary. Sometimes tablets should be replaced with other treatment modalities; (2) patients should be given the lowest dosage of SRIs possible; (3) patients who have a history of medication noncompliance, who have experienced the discontinuation symptoms in the past, or who have treatment-emergent anxiety are at highest risk for experiencing the SRIDS and need closer monitoring; (4) SRIs should b

Lactic acidosis and anti-retroviral drugs.

(1) Lactic acidosis is a serious, often fatal adverse reaction to nucleoside HIV reverse transcriptase inhibitors. (2) The presenting symptoms are non specific, and include gastrointestinal upset, fatigue, fever, and breathlessness. They appear gradually after several weeks or months of antiretroviral treatment. (3) Moderate hyperlactataemia, with few if any symptoms, is common in patients taking nucleoside inhibitors, and is not predictive of severe lactic acidosis. (4) Cases of lactic acidosis were reported in newborns whose mothers were given nucleoside inhibitors to reduce the risk of moth

Perinatal outcome following third trimester exposure to paroxetine.

BACKGROUND: Paroxetine hydrochloride is commonly used for maternal depression, panic disorder, and obsessive-compulsive disorder. The drug readily crosses the human placenta. Although it does not appear to increase teratogenic risk, there have been case reports of neonatal withdrawal. Symptoms were described soon after birth and lasted up to 1 month. OBJECTIVE: To investigate whether there is a clinically important discontinuation syndrome in neonates exposed to paroxetine in utero. METHODS: Prospective, controlled cohort study. PATIENTS: Fifty-five pregnant women counseled prospectively by

Neonatal withdrawal syndrome after in utero exposure to selective serotonin reuptake inhibitors.

UNLABELLED: Selective serotonin reuptake inhibitors (SSRIs) are a new group of antidepressants used in mild to moderate cases of depression. In studies evaluating the safety of SSRIs during pregnancy, no increase in major anomalies has been reported. This might have led to increasing off-label prescription of SSRIs to pregnant women. Neonatal withdrawal syndrome commonly occurs in infants exposed during the third trimester to drugs known to cause addiction. We report five cases of neonatal withdrawal syndrome after third trimester in utero SSRI exposure. In three cases the mother used paroxeti

Effect of perinatal buprenorphine exposure on development in the rat.

The developmental effects of exposure to various doses of buprenorphine, methadone, or water during the perinatal period were studied in the rat. Rats were exposed to buprenorphine (0.3, 1.0, or 3.0 mg/kg/day), methadone (9 mg/kg/day), and/or water prenatally, postnatally, or both pre- and postnatally, via maternally implanted osmotic minipumps. Fetal and maternal mortality and morbidity were assessed, as well as the acquisition of several developmental milestones, pup weight gain, precipitated withdrawal, and the antinociceptive effect of morphine. Although perinatal exposure to buprenorphine

Prominence of withdrawal symptoms in polydrug-exposed infants.

The problem of licit and illicit drug use during pregnancy continues to be a major perinatal health issue in this country. It is estimated that 200,000 infants are born each year to women who used illegal drugs while they are pregnant. Much information is available regarding the physiologic and neurobehavioral signs and symptoms of withdrawal expressed in infants exposed in utero to individual substances such as heroin, marijuana, methadone, alcohol, barbiturates, and PCP. However, little information is available related to the signs and symptoms of withdrawal observed in infants exposed to ma

Can we use anxiolytics during pregnancy without anxiety?

QUESTIONOne of my patients suffers from anxiety and was using lorazepam to treat it. When she became pregnant, she stopped the medication immediately, but now she is worried about the potential effect on the baby because she was using the drug just after conception. Is this class of drugs safe during pregnancy? What should she do if she needs antianxiety treatment during the rest of her pregnancy?ANSWEREvidence to date from cohort studies did not identify a notable association between use of benzodiazepines and increased risk of major malformations, including oral cleft. Comparison of a rocking bed and standard bed for decreasing withdrawal symptoms in drug-exposed infants.

PURPOSE: To determine if the use of a mechanical rocking bed with maternal intrauterine sounds would decrease symptoms of withdrawal and promote neurobehavioral adaptation in drug-affected infants. METHODS: This was a repeated measures experimental design, with infants randomized to a standard bed or a rocking bed. The sample consisted of 14 full-term infants who were prenatally exposed to methadone plus other illicit drugs. RESULTS: Infants receiving the rocking bed therapy experienced a significant increase in withdrawal symptoms and sleep deprivation and displayed suboptimal neurobehavior

Pyridoxine-dependent seizures in an older child.

The case of a 12-year-old girl with pyridoxine-dependent seizures is reported. She developed status epilepticus just after birth and ordinary antiepileptic drugs were administered without effect. Her seizures ceased only on the administration of pyridoxine. Status epilepticus associated with the withdrawal of pyridoxine occurred three times, and ceased only after the renewed administration of pyridoxine. She now has mental retardation and mild spastic diplegia. The reported cases of pyridoxine-dependent seizures usually have been neonates and infants. Older patients were not fully investigated

Inhalant abuse in pregnancy.

Information from a variety of sources suggests the possibility of adverse effects of maternal inhalant abuse, although a well-controlled, prospective study in this area has not been conducted. One source of this concern is the data from occupational exposure to some of the abused solvents, specifically toluene and TCE, with numerous reports suggesting increased spontaneous abortion and fetal malformations. There are also data suggesting decreased fertility and an increased risk for spontaneous abortion in health care workers exposed to nitrous oxide. The relevance of these studies to problems

Neonatal drug addiction.

In the twenty years since the first case of neonatal drug addiction (resulting from the mother's use during pregnancy of morphine, heroin, methadone, cocaine) was referred to our attention, there has been a steady increase in the number of cases per year. Heroin alone or in association with methadone now represents the drug used by approximately 80% of addicted mothers. Over the past few years the mean age of mothers has increased; the number of drug users who do not appear to be addicts has also increased and a number of cases have lately been discovered only on the basis of neonatal symptoms

Barbiturates.

Barbiturate use by adolescents has increased gradually in the past several years, but few adolescents use this class of drugs regularly. Instead, these sedatives are used most often to treat unpleasant effects of illicit stimulants, to reduce anxiety, and to get "high". Short-acting barbiturates such as pentobarbital and secobarbital are the preferred drugs of abuse. Barbiturates are dangerous drugs, with a narrow therapeutic index between the dose required for sedation and the dose that will cause coma and death. They are physiologically addicting if taken in high doses over 1 month or more,

Precipitated morphine withdrawal induces a conditioned aversion in the preweaning rat.

Opiate abstinence in the adult of many species, including humans, alters autonomic function and motor behavior, and induces a negative affective state. The neurobehavioral bases of each consequence of opiate withdrawal differs. Little attention has been paid to the issue of drug withdrawal in infants, although it is a common consequence of the maternal use of illegal and legal drugs. Infant rats as young as 7 days of age that experience opiate withdrawal show an abstinence syndrome consisting of developmentally appropriate behaviors that differ from those of the adult rat, including fewer auto

Sedation with intravenous midazolam in the pediatric intensive care unit.

Physical and emotional distress can have important effects on patients in the pediatric intensive care unit (ICU). Intravenous (IV) infusion of benzodiazepines is an important adjunct to assisted ventilation and other potentially distressing ICU procedures. Combined with intermittent or continuous infusion of opioids, the benzodiazepines provide smooth control of anxiety, pain, and agitation. Intravenous midazolam (Versed Roche Laboratories) is distinguished from diazepam (Valium, Roche Products) by its water solubility, short elimination half-life, and generally short duration of action. Thes

A risk-benefit assessment of flumazenil in the management of benzodiazepine overdose.

The worldwide expansion in the use of benzodiazepines has led to their frequent, and often inappropriate, use and to increase in their involvement in self-induced poisoning and iatrogenic overdosing. Flumazenil is a specific and competitive antagonist at the central benzodiazepine receptor, reversing all effects of benzodiazepine agonists without tranquillising or anticonvulsant actions. Incremental intravenous bolus injections of flumazenil 0.1 to 0.3 mg are the most effective and well tolerated in the diagnosis and treatment of pure benzodiazepine overdose; additional boluses or an infusion

Characterization of neonatal rat morphine tolerance and dependence.

The administration of morphine and fentanyl by continuous intravenous infusion has been shown to produce analgesic tolerance and physical dependence in human neonates. In animals, daily repeated morphine bolus injections is a common method of inducing neonatal rat tolerance and dependence. Yet this method differs from the intravenous route reported to affect human neonates. Alzet osmotic minipumps were implanted in postnatal day 14 rats to provide a continuous morphine infusion more closely mimicking the clinical picture. Rats remained naive or were infused with saline or morphine (0.7 mg/kg/h
