Antiepileptic drugs taken by pregnant epileptic women are known human teratogens. They may also cause pharmacological side effects in the newborn, i.e. sedation and or withdrawal symptoms. We examined the relationship between the maternal antiepileptic therapy, neonatal behaviour and later neurological functions in infancy. The study comprised 40 children exposed in utero to a single antiepileptic drug (phenobarbitone, phenytoin, valproic acid). Valproic-acid-exposed children were the highest compromised, except for apathy, which was most profound in phenobarbitone-exposed neonates. Valproic a

Use of illicit and licit drugs during pregnancy is a major public health concern, as it can have adverse effects on the developing fetus. Infants born of women addicted to narcotics, cocaine, alcohol, or polydrugs often undergo a characteristic withdrawal syndrome and may have physical, behavioral, and/or neurological abnormalities. As it is not feasible to ascertain whether these functional changes in human infants are produced by abused substance(s) per se or by a combination of complex socioeconomic factors and polydrug use, researchers in recent years have developed and utilized various in

In a retrospective study we analysed clinical findings and hematological data of 16 neonates born from mothers using methadone and other drugs during pregnancy. Most infants presented with symptoms of drug withdrawal and needed medication. Hemoglobin concentration and leucocyte counts were within normal limits, whereas all infants showed thrombocytosis after the first week of life with thrombocyte counts (median) of 566,750 (week 2), 628,500 (week 3), 629,500 (week 4), 617,000 (week 5) and 639,150/mm3 (week 6). No thrombocytosis was found in 18 normal control infants which showed significantly

Large numbers of human infants are exposed in utero to illicit opiate drugs such as heroin and to prescribed opiates such as methadone. It is therefore important to understand the effects of acute and chronic exposure to drugs such as these on the immature organism. A necessary step in understanding the long-term effects of early drug exposure is to describe fully how the drug acts on neurobehavioral systems in the neonate. Because infants typically experience withdrawal from opiates, either in utero or shortly after parturition, a fuller understanding of the consequences of opiate withdrawal

Although there are a number of studies and individual case reports concerning the use of benzodiazepines in human pregnancy, the data concerning teratogenicity and effects on postnatal development and behaviour are inconsistent. There is evidence from studies in the 1970s that first trimester exposure to benzodiazepines in utero has resulted in the birth of some infants with facial clefts, cardiac malformations, and other multiple malformations, but no syndrome of defects. Diazepam and chlordiazepoxide are amongst the drugs most frequently implicated in the earlier studies. However, data from

It has long been known that maternal addiction to opiates during pregnancy produces passive addiction in the newborn. When the synthetic opiate, methadone, became widely used for the treatment of heroin addiction, attention was focused on its possible reproductive and developmental toxicity. The clinical data clearly indicate that prenatal exposure to methadone produces a neonatal abstinence syndrome and that the symptoms, characterized by generalized CNS arousal, persist for as long as 4-6 month after birth. Long-term neurobehavioral follow-up studies to pre-school age have not found any obvi

BACKGROUND: The number of newborn infants with symptoms suggesting drug withdrawal is increasing. As only part of prenatally exposed infants show typical drug withdrawal, and drug-use reported by addicted mothers is often unreliable the prevalence of neonates that were exposed to illicit drugs before birth is unknown. The purpose of this study was to evaluate prospectively the prevalence of drugs in meconium and to define risk factors for intrauterine drug exposure.
METHODS: During a period of 4 months meconium was collected twice in 420 nonselected newborn infants. Meconium was analysed with

Drug-abusing (n = 25) and nonusing (n = 55) pregnant women from a publicly supported prenatal clinic were tested for level of social support and of pregnancy anxiety during the last half of pregnancy. Differences found between the groups were fewer than expected. Drug abusers did not differ from nonusers in overall level of social support or in Appraisal, Belonging, or Tangible subscales. Abusers were found to report lower levels of self esteem; lower self esteem was predicted by drug abuse, having more children and lower socioeconomic status. Drug abusers did not differ from nonusers in their

Use of addicting drugs among women during pregnancy exposes newborns to potentially serious disorders. A group of symptoms referred to as neonatal withdrawal syndrome (NWS) may occur in infants born to mothers addicted to certain drugs because, at birth, the infants suddenly are cut off from the drug supply. Classes of drugs that cause NWS are those that produce addiction in adults, including the opioids (heroin, methadone, morphine), barbiturates, alcohol, and benzodiazepines. Many of the manifestations of NWS occur regardless of the class of drug, including irritability, hyperactivity, abnor

A total of 505 newborns who were exposed to illicit drugs during intrauterine life were studied to investigate the prevalence, growth parameters, newborn manifestations, and other effects of intrauterine exposure to cocaine and phencyclidine (PCP). The results support the hypotheses that both drugs have serious effects on growth parameters, but this effect was much more pronounced in the cocaine group than in the PCP group. Furthermore, the incidence of borderline microcephalic infants (head circumferences less than the 10th percentile) was much higher in the cocaine group. It also appears tha

OBJECTIVE: To define the relationships between neonatal opiate withdrawal and drug-related factors such as maternal methadone dosage, maternal and neonatal plasma levels, and rate of decline of methadone in neonatal plasma.
METHODS: Twenty-one methadone-dependent women and their newborn infants were studied. Fourteen of the women used other illicit drugs. The severity of neonatal withdrawal was assessed with a standardized scoring system. Venous blood samples for methadone levels were collected from the mothers within 24 hours of delivery and from their newborns within 24 hours of birth and o

The major clinical uses for opioids are to control pain, suppress cough, and to treat diarrhea. These drugs, however, have the potential for abuse. It is postulated that the significant mood-altering effects of opioids combined with their pharmacology, in which tolerance and physical and psychological dependence occur, account for their abuse liability. A classification of the groups of patients that commonly present with complications of opioid use is reviewed briefly before discussing the clinical pharmacology of the opioids and their acute and toxic effects.

Alcohol-related seizures have been recognized since the time of Hippocrates. Most such seizures are related to acute abstinence from chronic, high doses of alcohol use. Increasing use of illicit drugs, especially cocaine, has dramatically increased the incidence of acute drug-toxicity-related seizures. In cases of alcohol- or drug-related seizures, occult structural and infectious causes must be ruled out. Treatment usually focuses on management of the alcohol or drug abuse. Rarely are anticonvulsants indicated.

In the few past years the Netherlands Centre for Monitoring of Adverse Reactions to Drugs and the Teratology Information Service of the National Institute of Public Health and Environmental Protection received several reports of neurological symptoms in neonates, which were the result of maternal treatment with psychotropic drugs during late pregnancy. The results are summarised in this article. The neonatal symptoms (withdrawal symptoms, 'floppy infant syndrome' and extrapyramidal syndrome) are described together with the psychotropic drugs which may cause them.

The effect of the maternal use of benzodiazepines (BZD) on the fetus and the newborn infant has been studied in a representative series of 17 newborn infants (BZD group). The pregnancy and the perinatal period were characterized by 20 items. On the 2nd day of life, a neurologic investigation was performed and comprised a total of 38 items, subgrouped into items of reflexes/reactions, tonus, and other symptoms and signs. An optimum finding for each item was selected. The results were compared with a group of 21 newborns fetally exposed to psychotropic drugs other than BZD (drug group) and a ref

The increasing incidence of drug abuse in central Europe leads to a growing number of children with intrauterine drug exposure. During pregnancy various risks affect these children. Postnatally severe symptoms of withdrawal are observed frequently. The psychosocial development is endangered. Between 1986 and 1990 30 drug dependent women attended the obstetric department of the University Hospital Basel during pregnancy. The data of 21 children born to mothers drug dependent at birth were evaluated. 11 children were born in 1990. 12 mothers received Methadon, 10 were polydrug abusers. Positive

BACKGROUND: The number of newborn infants exposed to drugs in utero is on the increase in many European countries. As drug use reported by addicted pregnant women is unreliable there is a need for an accurate test to determine the drugs to which an infant has been exposed in utero. The purpose of this study was to evaluate the reliability of toxicology testing in meconium compared with traditional urine testing.
METHODS: From twenty newborn infants born to drug-dependent mothers, meconium and urine were collected as soon as possible after birth and tested for drugs with the same radioimmunoas

A sample of 29 psychiatrically ill women and a non-ill comparison group were recruited during pregnancy. Twelve of the ill mothers were receiving antipsychotic and anti-anxiety medication during the final trimester of pregnancy. The behavior of their infants was assessed at 3 and 14 days of age using the Brazelton Neonatal Behavioral Assessment Scale. Compared to infants of non-ill mothers and infants of ill nonmedicated mothers, infants whose mothers received antipsychotic drugs--particularly those in the phenothiazine family--showed a stable pattern of poor neonatal motor functioning that in

The management of severe depression during pregnancy and lactation is a serious concern to both physicians and parents, since the use of tricyclic antidepressant drugs (TCAs) has not been proven to be safe. This is a preliminary report of our ongoing studies of four groups of women treated with tricyclic antidepressants before and during pregnancy and/or lactation. The four groups are women who: became pregnant while on TCAs (n = 9); were prescribed TCAs during pregnancy (n = 9); were prescribed TCAs during lactation (n = 20); or were clinically depressed during lactation, but who refused TCAs
