Methadone levels and neonatal withdrawal.

The purpose of this study was to observe the effects of methadone exposure in utero, with special reference to maternal and neonatal methadone concentrations and neonatal withdrawal. Two groups of mother-infant pairs were studied. In the first group, serum methadone concentrations were determined in infants at 1, 6 and 24 h after delivery. In the second group, blood was obtained at 24, 48, 72 and 96 h after birth. There was no correlation between neonatal serum levels and the intensity of withdrawal symptoms. There was no relationship between maternal methadone dose at delivery or maternal ser

Diazepam abuse in pregnant women on methadone maintenance. Implications for the neonate.

The authors found that the course and treatment of narcotic withdrawal in two neonates was complicated by prenatal exposure to high doses of diazepam, or Valium (Roche Laboratories, Nutley, NJ). Both of the mothers were on methadone maintenance for narcotic dependency prior to the diagnosis of pregnancy. The authors documented maternal intake of diazepam in the range of 40-60 mg/day for a duration of 4-27 weeks prior to delivery. Both infants initially responded well to medical therapy for narcotic withdrawal, but at 7-14 days of age, withdrawal symptoms intensified, requiring an increase in t

Issues of risk assessment: lessons from the use and abuse of drugs during pregnancy.

Therapeutic uses and risks associated with the opioids, cannabis, cocaine, and phencyclidine are discussed as well as limitations of the developmental animal and clinical pediatric literature, especially with respect to problems of quantitative risk assessment. Human and animal developmental findings for methadone and cannabis are compared with respect to long-term behavioral effects with special emphasis on pharmacological and interpretive issues. It is suggested that neonatal withdrawal is the most serious neurobehavioral sequela associated with maternal use of opioid and other sedative-hypn

Substance withdrawal.

As long as drug use and abuse persist in our society, a significant number of patients with various types of drug withdrawal will present to Emergency Departments. It is imperative that we recognize the signs and symptoms of drug withdrawal and render the appropriate treatments. Although it may be easy to recognize "skid row" alcoholics, a drug abuser with track marks on his (or her) arms or with a perforated nasal septum, the executive alcoholic, the elderly patient on chronic diazepam therapy, or the "blue collar" worker using cocaine may be more common and more elusive. Because most drug ab

Perinatal outcome of infants exposed to cocaine and/or heroin in utero.

While cocaine is now used much more frequently than heroin by women of childbearing age, we have found that a significant number of mothers have abused both drugs during their pregnancy. We studied 86 infants who were born to women with a history of cocaine and/or heroin use during pregnancy. The newborns were observed over a 5-day hospital period using a standardized abstinence scoring system and urine drug screening of both mother and infant. Of these, 35 had maternal and/or newborn urine test results that were positive for cocaine only (cocaine group), 14 that were positive for heroin only

Occurrence of strabismus in infants born to drug-dependent women.

Maternal drug abuse during pregnancy exposes the fetus to the possibility of toxic or addicting effects. At a methadone hydrochloride maintenance program providing medical care to drug-dependent women and their infants, ophthalmologic examinations of all infants born during a 36-month period were performed. Twenty-nine infants prenatally exposed to psychoactive drugs were seen at birth and on follow-up. The 24% prevalence of strabismus found in the drug-exposed infants was significantly greater than that of 2.8% to 5.3% reported in the general population. These data suggest that maternal drug

Prediction of relapse in schizophrenia.

Despite the proven efficacy of neuroleptic drugs in the acute and maintenance pharmacotherapy of schizophrenia, practical methods for identifying patients who require neuroleptic treatment to prevent relapse are lacking. This study evaluated the use of a methylphenidate challenge test to predict the outcome in 34 stable outpatients with schizophrenia receiving neuroleptic treatment. Patients received two infusions, one of methylphenidate and one of placebo, in randomized order one week apart while receiving neuroleptic treatment and again three weeks after drug withdrawal. Perinatal cocaine and methamphetamine exposure: maternal and neonatal correlates.

Maternal and neonatal growth, behavior, and physiologic organization were evaluated in 104 mother-infant pairs with positive results of urine toxicology screens. ANOVA comparison of cocaine, methamphetamine, and cocaine plus methamphetamine groups revealed no significant differences in perinatal variables. The Finnegan withdrawal scoring scheme demonstrated that all three groups of infants had altered neonatal behavioral patterns, characterized by abnormal sleep patterns, poor feeding, tremors, and hypertonia. Infants exposed to cocaine or methamphetamine or both were combined and compared wit

Neonatal neurological status in a low-risk population after prenatal exposure to cigarettes, marijuana, and alcohol.

The neurological status of 9- and 30-day-old infants, as assessed by the Prechtl neurological examination, was significantly and differentially related to prenatal exposure of cigarettes, marijuana, and alcohol. Data on approximately 250 babies, born to healthy, white, predominantly middle-class women, were analyzed using discriminant function analyses controlling for potentially confounding variables. Prenatal cigarette exposure was associated with hypertonicity and increased nervous system excitation, particularly at 30 days, prenatal marijuana exposure was associated with symptoms similar t

[Cerebrovascular complications in obstetrics and beta mimetic drugs].

The authors report a new case of a cerebro-vascular accident following the stopping of an infusion of a beta-mimetic drug to inhibit uterine contractions. The clinical symptomatology and the spontaneous recovery (over more than 6 months) makes it correct to call this pathological condition "an acute cerebral vascular disorder of a benign nature". It is not known how the cerebral vessels were affected by the beta-mimetic drugs.

The rational use of psychotropic drugs in pregnancy and postpartum.

When a pregnant woman or nursing mother requires psychotropic medication, one must consider the effect of such medications on the fetus or baby. The authors review the evidence for teratogenic effects of such drugs given during pregnancy, toxic or withdrawal effects in the newborn and dangers to the breast-fed baby. Suggestions are made for judicious use of psychotropic drugs in pregnant or nursing women.

Neonatal abstinence syndrome, pharmacotherapy and developmental outcome.

The majority of infants born to drug-dependent women undergo neonatal abstinence syndrome (NAS) and often require pharmacotherapy for the treatment of withdrawal symptoms. Phenobarbital, paregoric, and diazepam have been recommended for the treatment of the syndrome. While some investigators have examined the efficacy of these agents in treating NAS, there are no data regarding the use of specific pharmacologic agents and developmental outcome. This study evaluated 85 infants born to drug-dependent women who were maintained on methadone during pregnancy. Severity of infant withdrawal was asses

Risks incurred by children of drug-addicted women: some medical and legal aspects.

A review of recent literature shows a dramatic increase in the number of drug-addicted pregnant women, which increases the number of infants susceptible to the adverse effects of such addiction. The majority of new-born infants born to mothers addicted to opiates show a narcotic withdrawal syndrome. The risk of prematurity and other health disorders occurring in a new-born infant is substantially increased by the mother's abuse of narcotics during pregnancy. The microbial infection of children by a contaminated drug abused in the family, and accidental poisoning of children by drugs, have also

The neonatal narcotic abstinence syndrome: a brief review.

Because substance abuse experts are not available in many institutions, the consulting psychiatrist is required to diagnose and manage conditions such as the neonatal narcotic abstinence syndrome, which is readily recognized and treated. The authors discuss morbidity due to this syndrome in neonates born to narcotic addicted mothers. The clinical usefulness of neonatal narcotic abstinence scales is reviewed, with special reference to their application in treatment. The dosing of various drugs currently in use is also discussed.

Legal applications and implications for neurotoxin research of the developing organism.

Research on perinatal/prenatal exposure of the immature organism to neurotoxins, including drugs of abuse, therapeutic drugs, and environmental toxins is of interest not only to the scientific community but to the legal community as well. Research findings on damage to the developing organism are being assimilated into public policy through criminal and civil court decisions as well as the enactment of statutes and administrative regulations. This article describes cases concerning injuries to infants resulting from prenatal or perinatal neurotoxin exposure, relates the interpretation of relev

Long-lasting effects of early barbiturates on central nervous system and behavior.

Forty years of prescribing barbiturates to pregnant women and infants, and thirty years of animal research have shown that barbiturates affect the developing central nervous system (CNS) and behavior. This paper compiles and reviews animal and selected human literature in this research area. Early barbiturate exposure in animals reduces brain weight with related changes in brain biochemistry and neuromorphology. Significant changes may be found in surviving adult offspring. Evidence of CNS and behavioral damage in human beings due to early barbiturate exposure is not clearcut, however, confoun

Severe abuse of psychotropic drugs during pregnancy with good perinatal outcome.

A case of severe abuse of phenothiazines, benzodiazepines and barbiturates and smoking 40 cigarettes a day throughout pregnancy is described. Intoxication requiring hospitalization occurred 3 times during pregnancy. The mother was hospitalized for the last 10 days of pregnancy and only lorazepam and oxazepam were given in order to improve the co-operation of the mother and to alleviate fetal withdrawal symptoms. At 39 weeks a healthy baby boy of 3150 g was delivered. Obvious withdrawal symptoms in the neonate appeared at the age of 20 hours. Due to repeated drug intoxications of the mother aft
