Abdominal defect reconstruction due to advanced lobomycosis with anterolateral thigh (alt) flap: a case report.

Lobomycosis, also known as lacaziosis, is a chronic subcutaneous mycosis caused by Lacazia loboi and endemic to tropical regions of South America. Medical therapy alone has demonstrated limited efficacy, and wide surgical excision remains the mainstay of treatment, particularly in extensive or long-standing disease. However, radical resection may result in complex soft tissue defects requiring advanced reconstructive strategies. We report the case of a 42-year-old male with advanced lobomycosis involving the left lower hemiabdomen. Wide local excision with 2 cm macroscopic margins was performe

Collagen depletion, not keloid formation, defines lobomycosis lesions: a paired skin analysis.

Lobomycosis is a chronic cutaneous infection caused by the uncultivable fungus Paracoccidioides lobogeorgii. Lesions are often described as keloid-like, but the organization of the dermal extracellular matrix remains poorly characterized. We analyzed paired lesional and non-lesional skin from five patients in the Brazilian Amazon using histology and immunohistochemistry. Picrosirius red staining revealed depletion and disorganization of type I collagen in lesional dermis. Immunohistochemistry showed modest differences in biglycan and decorin expression, with spatial redistribution of biglycan

Forest-river interfaces shape lobomycosis risk in the Amazon Basin.

BACKGROUND: Lobomycosis is a chronic implantation mycosis caused by the uncultivable fungus Paracoccidioides lobogeorgii. The disease occurs predominantly in the Amazon Basin and has traditionally been associated with traumatic inoculation during exposure to forest environments. However, many patients live, work, or travel in riverine landscapes, suggesting that environmental determinants beyond forest exposure may influence the spatial occurrence of disease. METHODOLOGY/PRINCIPAL FINDINGS: We conducted a spatial case-background study including 192 confirmed lobomycosis cases and two environm

Revisiting the natural history of Jorge Lobo's disease: a mycological enigma spanning unresolved taxonomy, uncertain transmission, and inadequate therapy.

SUMMARYJorge Lobo's disease (JLD) is a chronic cutaneous and subcutaneous mycosis endemic to the Amazon Basin and tropical rainforests of Latin America. Despite more than a century of study, the disease remains underrecognized, poses significant diagnostic challenges, and is often therapeutically refractory. Here, we synthesize historical, clinical, and ecoepidemiological evidence; provide a practical diagnostic approach; and highlight unresolved questions. Clinically, JLD presents as slowly progressive keloid-like nodules and plaques on exposed areas. Diagnosis relies on direct microscopy and

Adipophilin-stained lipid droplets and M2 macrophages in Jorge Lobo's disease.

OBJECTIVES: Jorge Lobo's disease (JLd), a cutaneous mycosis caused by the fungus Paracoccidioides lobogeorgii, follows a chronic course and is refractory to any treatment modality. On histopathological view, the dermis presents crowded macrophages that have M2 polarization, which is associated with an anti-inflammatory profile and maintenance of the infection. Lipid droplets (LDs) are organelles responsible for neutral lipid storage for energetic needs. Intracellular pathogens may utilize LDs both to evade the immune response and as a source of nutrients. LDs in JLd have not yet been evaluated

Minimally invasive punch cytology with maceration for diagnosing Jorge Lobo disease in the Brazilian Amazon.

Jorge Lobo disease (JLD) is a neglected cutaneous mycosis prevalent in the Brazilian Amazon, where limited access to histopathology delays diagnosis and care. We assessed minimally invasive punch cytology with maceration (MIPC-M), a simple procedure that uses a 1-mm punch followed by potassium hydroxide maceration and light microscopy, as a field-ready alternative. In a prospective, cross-sectional study (2014-2019) across 11 municipalities in the Brazilian Amazon, individuals ≥12 years with clinically suggestive lesions underwent three cytological approaches in parallel-adhesive tape cytology

Partial alpha-tubulin gene amplification by nested PCR as a tool for Paracoccidioides ceti identification.

Paracoccidioidomycosis ceti (PCMC), also called lobomycosis, is a cutaneous disease affecting cetaceans worldwide. It is caused by Paracoccidioides ceti, an uncultivable fungal species recently classified within the Paracoccidioides genus. Although several molecular markers have been used to investigate the PCMC pathogen, the alpha-tubulin gene (TUB1), commonly utilized in genetic studies of cultivable Paracoccidioides, has remained unexplored in this taxon. In this study, we applied a nested polymerase chain reaction targeting TUB1 to amplify fungal sequences from two new cases of PCMC in Bra

Jorge Lobo's Disease in Child with Tick Exposure, Brazil.

Jorge Lobo's disease (JLD), caused by Paracoccidioides lobogeorgii, primarily affects inhabitants of the Amazon Forest. We report a 9-year-old boy in Brazil who had JLD diagnosed after a tick bite. The rarity of pediatric cases likely reflects surveillance gaps. Increased clinical awareness is crucial for early JLD detection and intervention, especially in endemic regions.

Subcutaneous mycoses - The hidden mould, a case series.

Subcutaneous mycoses are a heterogenous group of infections common in subtropical and tropical regions. These infections have an insidious onset and slowly evolve as the pathogen survives and adapts to the host tissue environment. Common subcutaneous fungal infections include Sporotrichosis, Chromoblastomycosis, Mycetoma, Subcutaneous Zygomycosis, Subcutaneous Phaeohyphomycosis, Lobomycosis and Rhinosporidiosis. Even with rapid urbanization, the incidence of subcutaneous mycoses is on the rise. Confirmation by histopathological examination is necessary.

Chromoblastomycosis in French Guiana: Epidemiology and Practices, 1955-2023.

Chromoblastomycosis (CBM) is a chronic neglected fungal disease, usually met in tropical areas. French Guiana is a South American territory with limited epidemiological data. This retrospective study concerned all patients with CBM proven by at least one paraclinical examination and diagnosed in French Guiana between 1950 and 2023. In total, 23 patients were included, mostly males (87%) of Creole origin, living in the coastal region (87%) and involved in outdoor occupations (74%). Lesions were mostly observed on the lower limbs (78.3%), with a median time to diagnosis of four years. Laboratory

A review of lobomycosis and lobomycosis-like skin disease in cetaceans worldwide, with new data from the Gulf of Guayaquil, Ecuador.

Lobomycosis, also called paracoccidioidomycosis ceti, is a chronic mycotic cutaneous disease affecting odontocetes. Lobomycosis-like disease (LLD) has a clinical presentation consistent with lobomycosis but lacks a histological and molecular diagnosis. We review the literature on lobomycosis aetiology, clinical signs and pathogenesis, species affected and geographic distribution and examine the factors influencing the presence, transmission and prevalence of the disease, to better understand its ecology. In addition, we provide unpublished information on LLD in two common bottlenose dolphin (T

Macrophage immunophenotypes in Jorge Lobo's disease and lepromatous leprosy- A comparative study.

Jorge Lobo's disease (JLD) and lepromatous leprosy (LL) share several clinical, histological and immunological features, especially a deficiency in the cellular immune response. Macrophages participate in innate and adaptive inflammatory immune responses, as well as in tissue regeneration and repair. Macrophage function deficiency results in maintenance of diseases. M1 macrophages produce pro-inflammatory mediators and M2 produce anti-inflammatory cytokines. To better understand JLD and LL pathogenesis, we studied the immunophenotype profile of macrophage subtypes in 52 JLD skin lesions, in co
