Prenatal diagnosis of cardiac rhabdomyoma: implications for predicting tuberous sclerosis complex and guiding perinatal management.

OBJECTIVE: To identify prenatal predictors of tuberous sclerosis complex (TSC) in fetuses with one or more cardiac rhabdomyomas (CR), evaluate an integrated multimodal diagnostic workflow using fetal magnetic resonance imaging (MRI) and trio whole-exome sequencing (trio-WES) and characterize perinatal outcomes. METHODS: This was a retrospective cohort study of 80 fetuses that were diagnosed prenatally with one or more CR between February 2016 and December 2024 at a single tertiary center and that received a definitive TSC diagnosis either prenatally (via genomic or clinical criteria) or by po

Omphalocele-exstrophy-imperforate anus-spinal defect syndrome in a monozygotic twin pregnancy: a case report and literature review.

BACKGROUND: The Omphalocele, Exstrophy, Imperforate Anus, and Spinal Defects Complex (OEIS complex) is a rare birth defect with a prevalence of 1 in 200,000-400000 pregnancies and 1 in 82,000-200000 live births. While cases in female monozygotic twins have been reported, the perinatal outcomes of the unaffected co-twin remain insufficiently addressed. CASE PRESENTATION: We report a spontaneously conceived monochorionic monoamniotic twin pregnancy where Twin B was diagnosed with OEIS. Prenatal ultrasound at 12 + 4 weeks identified an abdominal cystic mass in Twin B, which enlarged progressivel

Spectrum of potentially lethal cardiac conditions presenting with fetal sinus bradycardia: a report on 34 cases.

OBJECTIVE: Fetal sinus bradycardia (FSB) is usually a benign antenatal finding but it can be a marker of an underlying serious condition that manifests in later life. The aim of this study was to report the antenatal course and postnatal outcome of FSB. METHOD: This study was a retrospective review of all cases of persistent FSB, defined as a fetal heart rate (FHR) < 3rd percentile for gestational age (GA) with 1:1 atrioventricular conduction on at least two consecutive fetal echocardiograms, diagnosed between January 2014 and December 2024 at the Brompton Centre for Fetal Cardiology, London,

Virtual Post Mortem in Fetuses and Newborns for the Verification of Congenital Heart Malformations.

Post-mortem examinations of fetuses and newborns with congenital anomalies pose significant challenges, especially in cases of induced termination of pregnancy. To address this, we explored the use of virtual post-mortem examination (VPM) via computed tomography angiography (CTA) as a potential alternative. The study was conducted at Sheba Medical Center from January 2013 to June 2019. It focused on 20 cases involving severe congenital heart defects or lung malformations affecting the heart. VPM imaging was performed using CTA with contrast medium injected into the umbilical artery and vein. P

Intrauterine-diagnosed fetal intracranial adamantinomatous craniopharyngioma: A case report with histopathological and molecular characterization.

Craniopharyngiomas are the most common non-neuroepithelial intracerebral neoplasms in children but rarely occur before birth, and are typically associated with a poor prognosis. We present the case of a 39-year-old woman with a spontaneously conceived singleton pregnancy. At 20 weeks of gestation, ultrasound detected a 44×25 mm heterogeneously hyperechogenic intracranial mass located at the base of the sphenoid bone, extending bilaterally. Due to the poor prognosis, legal pregnancy termination was performed, followed by fetal autopsy. The 20-week-old male fetus weighed 528g. External examinati

Outcome of monochorionic diamniotic twin pregnancy with selective fetal growth restriction and continuous or intermittent absent or reversed end-diastolic umbilical artery flow: international multicenter cohort study.

OBJECTIVES: Monochorionic diamniotic (MCDA) twins with selective fetal growth restriction (sFGR) and either continuous (cAREDF) or intermittent (iAREDF) absent or reversed end-diastolic flow in the umbilical artery face significant fetal and neonatal risks. This study evaluated fetal and neonatal outcomes in these cases and compared outcomes for the larger twin following selective reduction (SR) vs expectant management (EM). METHODS: This was an international retrospective cohort study of MCDA twin pregnancies with sFGR and cAREDF or iAREDF from five fetal medicine centers over 7 years (2016-

Clinical course and perinatal management of fetuses and newborns affected by trisomy 13 and 18: a retrospective single-centre cohort study.

Objectives Trisomy 13 (T13) and 18 (T18) are lethal chromosomal abnormalities. While the prognosis is generally poor, recent evidence suggests that some infants with T13 and T18 are able to survive longer than previously supposed. These findings underline the importance of an individualised approach to perinatal/neonatal care. Methods This retrospective study included 193 pregnancies with T13 and T18 in a single tertiary center between 2005 and 2023. Fetal and maternal characteristics, overall survival, and perinatal and postnatal management were analysed. Results Of 193 affected fetuses, 142

Sex-selective abortion bans and the birth outcomes of Asian immigrants.

The pro-life movement in the United States has achieved significant legislative victories in restricting abortion access. Among these, sex-selective abortion bans (SSABs)-prohibitions on abortions sought based on fetal sex-have garnered little scholarly attention. Advocates of SSABs often invoke xenophobic stereotypes, claiming that Asian immigrants hold a cultural preference for sons and engage in sex-selective abortions of female fetuses. This study examines the impact of SSABs on Asian immigrant mothers' birth outcomes, hypothesizing that these laws foster a stigmatizing social climate that

Gestational alloimmune liver disease treated with intravenous immunoglobulin resulting in maternal drug-induced liver injury.

Gestational alloimmune liver disease (GALD) is a rare pregnancy complication involving maternal antibody-mediated fetal liver injury that recurs in 90% of affected pregnancies. The fetal and neonatal mortality rate is high, up to 82%, and affected fetuses can develop fetal hydrops, growth restriction, and oligohydramnios. Intravenous immunoglobulin (IVIG) administration can prevent GALD recurrence. We present a patient whose three live births were impacted by GALD. Her first pregnancy was an elective termination. Her second pregnancy resulted in neonatal demise due to autopsy-proven GALD. Her t

Mini-laparotomy to avoid vascular injury during fetoscopic laser ablation in a case of twin-twin transfusion syndrome with anterior wall placenta: Report of a case complicated with renal dysplasia and cord-around-trunk in donor cotwin and review of the literature.

OBJECTIVE: Twin-twin transfusion syndrome (TTTS) is a special complication of monochorionic (MC) twins, with unbalanced flows through vascular anastomoses within the placenta, causing hemodynamic derangement that can adversely affect perinatal outcome. The initial clinical manifestations of TTTS may be hypovolemic and oliguric of the donor cotwin (the "struck" twin), and hypervolemia and polyureic of the recipient cotwin under prenatal ultrasound. However, the mortality rate can reach 90 % if left untreated. Fetoscopic laser ablation (FLA) is considered the golden treatment standard of TTTS. H

Diagnostic accuracy of ultrasound screening for fetal structural abnormalities during the first and second trimester of pregnancy in low-risk and unselected populations.

BACKGROUND: Prenatal ultrasound is widely used to screen for structural anomalies before birth. While this is traditionally done in the second trimester, there is an increasing use of first-trimester ultrasound for early detection of lethal and certain severe structural anomalies. OBJECTIVES: To evaluate the diagnostic accuracy of ultrasound in detecting fetal structural anomalies before 14 and 24 weeks' gestation in low-risk and unselected pregnant women and to compare the current two main prenatal screening approaches: a single second-trimester scan (single-stage screening) and a first- and

Maternal and fetal outcomes of antiepileptic treatments during pregnancy: A retrospective study.

INTRODUCTION: Epilepsy is a disease that affects a significant proportion of the female population worldwide. The management of anti-seizure medications during pregnancy and the potential adverse outcomes to both the mother and fetus represent a significant challenge. This retrospective study aimed to evaluate the impact of anti-seizure medications during pregnancy by comparing maternal and fetal outcomes between pregnant women with and without epilepsy. METHODS: A total of 242 participants were analysed, including 112 with epilepsy and 130 healthy pregnant controls. Maternal age, medical his

Maternal and fetal/neonatal outcomes in pregnancy, delivery and postpartum following bariatric surgery and comparison with pregnant women with obesity: a study protocol for a prospective cohort.

BACKGROUND: Being obese can lead to various complications during pregnancy, such as Gestational Diabetes Mellitus (GDM), pregnancy induced hypertension (PIH), Pre-Eclampsia (PE), and Large Gestational Age (LGA). Although bariatric surgery is an effective way to treat obesity, it can also result in complications and may be linked to having small for gestational age (SGA) babies. This cohort study protocol aims to compare the maternal and fetal/neonatal outcomes of two groups of Iranian pregnant women: those who have undergone bariatric surgery and those who are obese but have not had bariatric
