Symptomatic intraocular pressure dysregulation during clopidogrel exposure after patent foramen ovale closure in a patient with ocular hypertension and angle crowding: a Case Report.

Clopidogrel is widely used after structural heart interventions, but symptomatic intraocular pressure (IOP) abnormalities without clinically evident hemorrhage are rarely reported. We describe a 52-year-old woman who developed recurrent ocular discomfort, eye pain, and blurred vision after percutaneous patent foramen ovale closure followed by clopidogrel-containing antiplatelet therapy. Baseline pre-exposure assessment showed ocular hypertension with angle crowding features but no definite glaucomatous structural or functional damage. Symptoms improved after clopidogrel withdrawal and recurred

Congenital Hypothyroidism and Atherosclerosis: An Endocrine Model of Early-Life Cardiovascular Risk.

Congenital hypothyroidism (CH) is a common endocrine disorder characterized by thyroid hormone deficiency and has dramatic consequences if diagnosis and treatment are delayed. Although neonatal screening (NNS) and early levothyroxine (L-T4) treatment have markedly improved neurodevelopmental outcomes, evidence suggests that cardiovascular alterations may persist into later life. Thyroid hormones regulate lipid metabolism, endothelial nitric oxide (NO) availability, myocardial relaxation, vascular tone and arterial wall homeostasis. Early clinical studies in patients with CH reported variable l

Factors associated with in-hospital mortality of neonates with anorectal malformations managed at a tertiary centre in uganda: a retrospective cohort study.

BACKGROUND: Anorectal malformations (ARM) are common congenital anomalies among neonates presenting to paediatric surgical units in Uganda. Optimal care is often hindered by significant health system gaps. However, factors influencing mortality among neonates with ARM in Uganda have not been formally evaluated. OBJECTIVE: To determine factors associated with in-hospital mortality among neonates admitted with anorectal malformations at a tertiary hospital in Uganda. METHODS: This retrospective cohort study included consecutive neonates with ARM managed at a single teaching hospital in Uganda

Situs Inversus Totalis and Severe Early-Onset Developmental Epileptic Encephalopathy in a Child With a Homozygous CFAP52 Mutation: A Case Report.

We report a 2-year-old boy with a homozygous CFAP52 mutation presenting with situs inversus totalis, severe epileptic developmental encephalopathy, and global developmental delay, expanding the phenotypic spectrum of CFAP52-related ciliopathies. Clinical findings included multifocal epileptiform discharges, thin corpus callosum, and a small subdural hematoma. Trio whole-exome sequencing also revealed a homozygous NCAPG2 variant of uncertain significance, however, its clinical significance remains unproven and no causal relationship can be established. This case highlights the diagnostic value

Identification of Neonatal Prevalence Trends in Gastroschisis: 2016-2024.

BACKGROUND: Gastroschisis prevalence in the United States has varied over the last several decades, with recent reports indicating a downward trend through 2022. OBJECTIVE/METHODS: To determine if this trend continued beyond the COVID-19 Public Health Emergency, we examined data from the Children's Hospital Association's Pediatric Health Information System (PHIS) for critically ill neonates admitted from 2016 to 2024. RESULTS: The national prevalence for critically ill neonates with gastroschisis significantly decreased (p ≤ 0.0001) between 2016 and 2024, with consistent declines across all

Feminizing Therapy in People With Klinefelter Syndrome and Gender Incongruence: Considerations for Treatment Without Testosterone Blockers.

BACKGROUND: Klinefelter syndrome (KS; 47, XXY) usually involves male sex development and gender identity. Small studies suggest gender incongruence may be more common in KS, but feminizing gender‑affirming hormone therapy in this group has been scarcely reported. OBJECTIVES: To evaluate current feminizing hormone therapy in individuals with KS and the number of individuals with KS attending our gender clinic. MATERIALS AND METHODS: We conducted a care evaluation of individuals with KS referred to the Center of Expertise on Gender Dysphoria (Amsterdam UMC) between February 1972 and May 2025,

Comparison of the predictive ability for PDA surgery between early serum NT-proBNP level and echocardiographic PLASE score.

BACKGROUND: Although serum NT-proBNP is a useful marker of patent ductus arteriosus (PDA) severity in the early postnatal period, its ability to predict the future need for PDA surgery remains unclear. This study aimed to compare the predictive abilities of early NT-proBNP and the PLASE score using an established echocardiographic model. METHODS: Infants born at 23-29 weeks of gestation between 2019 and 2024 and admitted to a tertiary neonatal intensive care unit were included. Serum NT-proBNP levels and echocardiographic indices at 3 days of age were retrospectively collected. Three predicti

Anesthetic Management in the Elective Separation of Omphalopagus Conjoined Twins With Shared Hepatic Parenchyma: A Case Report.

Separation of conjoined twins is a rare and complex surgical endeavor with significant anesthetic implications, particularly in omphalopagus twins, given the risk of massive hemorrhage during hepatic transection and hemodynamic instability from cross-circulation. Blood management strategies include the use of tranexamic acid, viscoelastic monitoring, and preparation for rapid transfusion. We report the anesthetic management of the elective surgical separation of three-year-old female omphalopagus conjoined twins with shared hepatic parenchyma. Extensive multidisciplinary planning, simulation-b

Perioperative Anesthetic Management for Bariatric Surgery in a Patient With Achondroplasia and Prior Limb-Lengthening: A Case Report.

Achondroplasia presents significant anesthetic challenges that may be amplified by morbid obesity. A 29-year-old woman with achondroplasia and morbid obesity (body mass index 69.0 kg/m²) underwent bariatric surgery after prior lower-limb lengthening of 17 cm. Despite her increased height, she retained achondroplasia-related torso proportions and airway features. Standard ramped positioning was ineffective because of her disproportionately short torso. High-flow nasal oxygen was used to maintain oxygenation during prolonged airway management. After failed intubation with a standard videolaryngo

Altered development in auditory white matter and cortical response latency in youth with neurofibromatosis type I.

OBJECTIVES: Neurofibromatosis type 1 (NF1) is a genetic disorder that affects brain development and increases the risk for neurodevelopmental conditions, including autism spectrum disorder. Given similar behavioral phenotypes and potential shared neurobiological processes in NF1 and autism spectrum disorder, this study evaluates two markers of brain maturation in youth with NF1 and typically developing youth, including auditory white matter and cortical response latency. BASIC METHODS: Participants, aged 8-12 years, completed a multimodal neuroimaging protocol that included MRI with diffusion

Valosin-Containing Protein Contributes to Plexiform Neurofibroma Formation and Represents a Novel Therapeutic Target.

Neurofibromatosis type 1 (NF1) patients are predisposed to develop plexiform neurofibromas (PNFs). By cross-comparison of RNA sequencing and RUNX1-CHIP sequencing data on mouse PNFs, we found that transcripts encoding the NF1-interacting p97/valosin-containing protein (VCP) gene are overexpressed in PNFs. Co-immunoprecipitation confirmed that VCP bounded to neurofibromin. Western blot and immunostaining confirmed VCP overexpression in both mouse and human PNFs. Treatment of primary mouse PNF Schwann cells with CB-5083, a p97/VCP inhibitor, led to accumulation of poly-ubiquitinated proteins and

Surgical Management of Pancreatic Head Arteriovenous Malformation: A Case Report.

Pancreatic arteriovenous malformations (PAVMs) are uncommon abnormalities of pancreatic blood vessels that result in direct, high-flow connections between arteries and veins. There are currently fewer than 200 reported cases in the literature of this particular anomaly. They are often clinically asymptomatic but may present with abdominal pain, gastrointestinal bleeding, or features of portal hypertension, and can resemble cystic or neoplastic lesions on imaging. We report a case of a 48-year-old female with a pancreatic head mass presenting with non-specific abdominal pain and jaundice. Owing

Modelling kidney cystogenesis using human kidney tubuloid cultures.

BACKGROUND: Renal cyst formation, as observed in autosomal dominant polycystic kidney disease (ADPKD), is a life-threatening condition with no effective cure yet. The molecular mechanisms underlying primary cilia dysfunction, which causes cyst formation and disease development, are not well understood. Human kidney tubuloids offer a promising model system to investigate the disease mechanisms of PKD in physiologically relevant 3D structures. However, their inherent cystic morphology poses a challenge in effectively modelling kidney cystogenesis. Therefore, our study aims to refine the culture
