Unilateral macrostomia repair using modified muscle reconstruction: Long-term anthropometric evaluation of static and smile symmetry.

BACKGROUND: Macrostomia, or transverse facial cleft, substantially impairs facial esthetics and oral function. Although various repair techniques have been reported, achieving satisfactory bilateral symmetry, particularly during dynamic expressions such as smiling, continues to be a challenge. Here, we introduce our modified muscle reconstruction technique for unilateral macrostomia repair and evaluate long-term outcomes in static and smile symmetry using anthropometric measurements. METHODS: A retrospective analysis was conducted on all patients who underwent unilateral macrostomia repair at

Hydrocephalus: Treatment, Current Research, and Challenges.

Hydrocephalus is a complex neurological condition characterized by derangements in cerebrospinal fluid (CSF) dynamics including disrupted CSF flow/absorption and/or excess CSF production that leads to ventricular enlargement and presents both diagnostic and therapeutic challenges. Its origins span from congenital anomalies to acquired cases following hemorrhage, infection, or tumors. Hydrocephalus is suspected in the setting of appropriate clinical signs/symptoms, and the diagnosis is confirmed with brain imaging and, in rare cases, invasive intracranial pressure monitoring. Surgical intervent

Facial Nerve Schwannoma Mimicking Post-Traumatic Middle-Ear Disease: A Diagnostic Pitfall.

Facial nerve schwannoma may present with otological symptoms despite preserved facial nerve function, creating a diagnostic challenge when the lesion involves the middle ear. A 75-year-old man developed aural fullness and hearing loss following craniofacial trauma. Otoscopy revealed a dark reddish retrotympanic mass. Computed tomography demonstrated soft-tissue opacification of the middle ear and mastoid with a tegmen tympani defect, raising suspicion of post-traumatic middle-ear disease with cerebrospinal fluid leakage. Facial function remained normal. Surgical exploration confirmed a dural d

Navigating the Pancreatic Duct: Up-to-Date Radiologic Expedition through Intraluminal Pathologic Entities.

Several congenital, traumatic, infectious, inflammatory, and neoplastic processes, as well as iatrogenic interventions, can alter intraluminal contents of the pancreatic duct, leading to adverse outcomes. Variant ductal anatomy may result in clinically significant complications such as duodenal obstruction, acute and chronic pancreatitis, and increased risk of developing gallbladder or biliary malignancies. The presence of calculi, blood, pus, and other unintended iatrogenic material in the pancreatic duct can be associated with a range of complications. Neoplastic involvement of the pancreati

Anomalous right coronary artery and aborted sudden cardiac death.

Anomalous aortic origin of the right coronary artery poses serious risks when involving high-risk features such as a long intramural course or a slit-like orifice, with demand ischaemia as the likely pathophysiology of sudden cardiac arrest. This case links sudden cardiac arrest to demand ischaemia, confirmed by cardiac MRI showing a transmural infarct in the right coronary artery territory. This is a rare case demonstrating transmural infarction in a patient with an anomalous aortic origin of the right coronary artery, as demonstrated by cardiac MRI findings.

Factors predicting blood transfusion in endoscopic metopic strip craniectomy-analysis of 102 consecutive cases.

PURPOSE: Endoscopic strip craniectomy (ESC) provides a safe surgical approach for the management of metopic craniosynostosis. However, it has a higher reported blood transfusion requirement than ESC for other sutures. We sought identify factors that may be related to greater blood transfusion use. METHOD: Retrospective consecutive case series. All metopic ESC cases were identified from the institutional database. Case notes reviewed following pre-drafted proforma, with regression analysis to identify risk factors for transfusion. RESULTS: One hundred two consecutive cases of ESC for metopic

Donor-derived unlicensed NK cells promote the hematopoietic engraftment and survival after allogeneic hematopoietic stem cell transplantation.

Successful hematopoietic engraftment is critical for the prognosis of allogeneic hematopoietic stem cell transplantation (HSCT), as delayed or failed engraftment remains a major cause of transplant failure. Natural killer (NK) cells, particularly recipient-derived unlicensed NK (U-NK) cells, have been reported to promote hematopoietic recovery under non-myeloablative conditioning (non-MAC). However, following myeloablative conditioning (MAC), recipient-derived NK cells are largely depleted, highlighting the need to investigate the role of donor-derived NK cells. In this study, we analyzed a cl

Paediatric patient with FLNC and CTNNA3 variants presenting with frequent premature ventricular contractions and systolic dysfunction: a case report.

We present the case of an 8-year-old male with frequent premature ventricular contractions and mild left ventricular dysfunction, who was found to carry heterozygous variants in both FLNC and CTNNA3. Medical therapy resulted in improved systolic function and reduced arrhythmia burden. This rare combination suggests a potential genotype-phenotype correlation in paediatric arrhythmogenic cardiomyopathy.

Birt-Hogg-Dubé syndrome.

Birt-Hogg-Dubé (BHD) syndrome is an autosomal dominant tumour predisposition disorder caused by pathogenic variants in the folliculin (FLCN) gene. It is characterized by benign skin tumours, renal neoplasia and cystic lung disease, with a risk of spontaneous pneumothorax. Although historically considered rare, population genomic data indicate that FLCN pathogenic variants are substantially more common than expected from clinically ascertained cases, suggesting that many variant carriers are not currently recognized in clinical practice. Advances in the molecular biology of BHD syndrome have es

Establishment and in vitro validation of a sheep-specific, low-cost TLR7/8 agonist R848-mediated X/Y sperm separation system.

Sex control is crucial for maximizing the economic benefits of sheep husbandry: male lambs increase farm income by 30-40% due to superior growth and feed efficiency, while female lambs are essential for fine-wool production and population expansion. However, the only commercialized method (flow cytometry) has critical sheep-specific limitations, including 25-30% flow channel blockage, sperm DNA oxidative damage, and prohibitive cost for large-scale breeding. TLR7/8 agonist R848 has been validated for sperm separation in mice, cattle and goats, but no sheep-specific optimized system has been es

The Influence of Pre-operative Mitral Regurgitation in Post Operative Recovery of Ventricular Function in Anomalous Origin of the Left Coronary Artery from the Pulmonary Artery (ALCAPA).

We attempted to study the influence of pre-operative mitral regurgitation on the recovery of ventricular function in paediatric patients with repaired anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA). Eighty-eight patients under 18 years who underwent ALCAPA repair from 2003 to 2022 were included. Baseline clinical and echocardiographic data, including MR and left ventricular ejection fraction (LVEF), were recorded pre-operatively and at multiple post-operative follow-ups (discharge, 1 month, 6 months, 1 year, and annually thereafter). Friedman's Test and correla

Development and Validation of the 9-item Parkinson's Disease Screening Questionnaire.

OBJECTIVE: We aimed to develop and validate a brief, self-administered questionnaire integrating motor and prodromal non-motor symptoms for screening Parkinson's disease (PD). METHODS: We conducted a three-phase study. Phase 1 used a prospectively registered systematic review of 107 studies to generate a 52-item draft. In Phase 2, the draft was refined in 205 participants (122 with early PD and 83 healthy controls) using item-level areas under the receiver operating characteristic curves (AUCs) and age- and sex-adjusted multivariable logistic regression. Phase 3 validated the 9-item Korean PD

Altered IL-23/IL-17A immune axis in Parkinson's disease: An exploratory analysis.

OBJECTIVE: Parkinson's disease (PD) is a progressive neurodegenerative disorder with a multifactorial basis. Immune dysregulation, both in the periphery and the brain, has been suggested to contribute to neurodegeneration in PD. Recent research has suggested a crucial role of T lymphocytes, especially the Th17 lineage, in α-synuclein-mediated neurodegenerative disorders. Whether Th17 cells exhibit pathogenic potential in PD is not adequately known. In this study, the role of cytokines that regulate the commitment, development, pathogenic transformation, and effector function of Th17 cells was

Transcriptomic profiling suggests restoration of epidermal barrier-associated lipid metabolism in Egr1-deficient skin following house dust mite exposure.

BACKGROUND: House dust mite (HDM) is a major environmental allergen that promotes cutaneous inflammation and disrupts epidermal barrier-associated processes. Early growth response 1 (EGR1) is an inducible transcription factor implicated in inflammatory skin responses; however, its relationship with epidermal barrier-associated lipid metabolism during allergen exposure remains poorly understood. OBJECTIVE: This study investigated whether Egr1 deficiency is associated with preservation of epidermal barrier-related features and restoration of lipid metabolic transcriptional programs following HD

Perioperative Management of a Toddler With Tuberous Sclerosis and Dilated Cardiomyopathy Undergoing Open Abdominal Aortic Aneurysm Repair: A Case Report.

Toddler abdominal aortic aneurysms (AAA) are rare and lack defined perioperative guidelines. Aneurysms may be associated with tuberous sclerosis complex (TSC) but rarely occur with concurrent dilated cardiomyopathy. This case report describes the anesthetic and surgical considerations for a two-year-old male with TSC and dilated cardiomyopathy with low ejection fraction undergoing open infrarenal AAA repair. Our most important perioperative considerations included advanced hemodynamic monitoring, management of intraoperative blood loss, and the physiological changes associated with aortic clam

Sudden cardiac arrest during diagnostic workup: a rare case of high-risk interarterial left anterior descending artery anomaly.

BACKGROUND: Coronary artery anomalies are anatomical abnormalities involving abnormal origin, course, or termination of any of the three main epicardial arteries. These abnormalities may go unnoticed during a person's lifetime; however, they can occasionally present acutely or chronically as angina, myocardial infarction, cardiac arrhythmias, or even sudden cardiac death. CASE PRESENTATION: A 65-year-old woman with angina and stress-induced ST-segment depression in a treadmill test. Coronary angiography showed the absence of the left main coronary artery with separate origins of the left ante

[Uptade on myometrial vascular lesions].

Enhanced myometrial vascularity is an acquired condition, secondary to trophoblastic invasion, often mistakenly confused with congenital arteriovenous malformation, from which it differs in its pathophysiology and evolution. Diagnosis is based on transvaginal ultrasound, combining B-mode and color Doppler imaging. Most often asymptomatic, it resolves spontaneously after complete expulsion of the trophoblast. In cases of significant bleeding, active management may be necessary, guided by the severity of symptoms rather than imaging criteria alone.
