Impact of Valve Selection on Clinical Outcomes in Idiopathic Normal Pressure Hydrocephalus: A Retrospective Cohort Study.

INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) is a progressive neurological disorder characterized by Hakim's triad: cognitive decline, gait disturbance, and urinary urgency or incontinence, accompanied by ventriculomegaly and normal cerebrospinal fluid (CSF) pressure. Ventriculoperitoneal shunting (VPS) is the standard treatment, with outcomes varying depending on the type of valve utilized. In this study, we compared the impact of programmable versus fixed-pressure valves on treatment outcomes. METHODS: A retrospective analysis was conducted on patients with iNPH treated at

The Evolution of Technological Innovation in CSF Shunt Technology: A Patent Bibliometric Analysis Encompassing the Past 60 Years.

BACKGROUND: Despite decades of technological development, cerebrospinal fluid (CSF) shunts remain associated with high failure rates and limited structural evolution. While bibliometric analyses have characterized innovation trends across several neurosurgical subspecialties, no dedicated patent analysis of CSF shunt technology exists. This study characterized the evolution of CSF shunt-related intellectual property (IP) and identified features associated with influential innovation. METHODS: A retrospective patent bibliometric analysis was performed using the Lens database. Granted patents r

Apathy in normal pressure hydrocephalus: frequency, clinical correlates, and relationship to cerebrospinal fluid removal response.

Apathy is a highly prevalent neuropsychiatric feature of normal pressure hydrocephalus (NPH), but its clinical correlates and relevance to cerebrospinal fluid (CSF) removal trial response are poorly understood. We evaluated 161 adults with possible or probable NPH whose informants completed the Frontal Systems Behavior Scale (FrSBe), which provides normative ratings for premorbid (i.e. before symptom onset) and current apathy. Apathy change was defined as current minus premorbid ratings of apathy. Among the sample, 69.6% showed clinically elevated apathy (T-score ≥ 65) compared to 18.0% premor

Implanted intracranial pressure sensors: mechanisms, fabrication, and system integration.

Intracranial pressure (ICP) is a key physiological parameter for the diagnosis and management of traumatic brain injury, hydrocephalus, stroke, and other neurological disorders. ICP sensors are typically required to provide accurate pressure measurement at low pressure while maintaining long-term stability in biofluid environments. Although conventional liquid-based ICP systems remain the clinical standard, their bulky and tethered configurations increase the risk of infection and tissue injury and restrict patient mobility. Advances in miniaturized implantable sensors have created new opportu

Predictive value of intraoperative bulbocavernosus reflex monitoring for postoperative voiding dysfunction after lumbosacral spine surgery: a systematic review and meta-analysis.

PURPOSE: Postoperative voiding dysfunction (POVD) is a significant complication of spinal dysraphism and lumbosacral spinal tumor resection. Conventional intraoperative neurophysiological monitoring modalities primarily assess the descending motor pathway and may not fully evaluate the integrity of the sacral reflex pathway. Intraoperative bulbocavernosus reflex (BCR) monitoring provides a real-time assessment of the sacral reflex arc's afferent and efferent pathways and may predict postoperative urinary function. However, its diagnostic performance and clinical utility remain incompletely def

Evaluation of Perinatal Prevalence, Geographic Disparities, and Survival of Neural Tube Defects - China, 2006-2024.

INTRODUCTION: Neural tube defects (NTDs) are a major cause of perinatal mortality and lifelong disability. However, long-term trends in NTD prevalence, regional disparities, and survival patterns in China remain poorly characterized. METHODS: Data from the Chinese Birth Defects Monitoring Network (CBDMN) were analyzed for perinatal NTDs occurring between 2006 and 2024. Bayesian Poisson regression models incorporating subtype, geographic strata, maternal age, calendar year, and interaction effects were applied to estimate long-term trends in perinatal prevalence and survival. Targeting Piezo1/2 ion channels for modulation of the lymphatic system: mechanisms, applications, and therapeutic prospects.

In recent years, the role of mechanosensation in lymphatic system development, functional homeostasis, and disease progression has received increasing attention. As key mechanosensitive receptors in lymphatic endothelial cells, Piezo1/2 ion channels respond to biomechanical stimuli such as fluid shear stress and interstitial pressure, thereby regulating calcium influx and downstream signaling pathways. These channels play pivotal roles in physiological processes, including lymphatic network formation, valve development, and the maintenance of meningeal lymphatic function. This review systemati

In ovo exposure to triphenyltin hydroxide induces genotoxicity, cytotoxicity and developmental toxicity in chick embryos, Gallus gallus domesticus.

Triphenyltin hydroxide (TPTH) is a widely used agricultural fungicide and environmental pollutant well known for its potent toxicological potential. Despite this, its embryotoxic effects remain unexplored in avian models. This study presents a novel evaluation of TPTH's acute toxicity, genotoxicity, cytotoxicity, teratogenic potential, and enzymatic and tissue architectural potential using the in ovo chick embryo model. Fertilized chicken eggs were exposed to varying TPTH concentrations that further revealed a time-dependent median lethal dose (LD50) decline from 547.37 µg/egg at 24 h to 395.7

Endoscopic third ventriculostomy for idiopathic normal pressure hydrocephalus: association of cerebrospinal fluid flow dynamics with outcome.

INTRODUCTION: Endoscopic third ventriculostomy (ETV) has been proposed for selected patients with idiopathic normal pressure hydrocephalus (iNPH), but predictors of sustained benefit are uncertain. Phase-contrast MRI (PC-MRI) may quantify cerebrospinal fluid (CSF) pulsatility and support patient selection. RESEARCH QUESTION: Is sustained clinical improvement after primary ETV associated with preoperative aqueductal CSF flow parameters measured by PC-MRI? MATERIAL AND METHODS: In this prospective single-centre observational study, patients with iNPH underwent primary ETV after standardized di

The role of rapamycin in neural tube defects: insights from bioinformatics analysis and experimental validation.

Neural tube defects (NTDs) are the most severe congenital malformations of the central nervous system. Fetal developmental homeostasis disruptions heighten NTDs risk through aberrant maternal-fetal signaling and periconceptional medication exposure. Rapamycin, an inhibitor of the mTOR signaling pathway, is a widely used clinical drug whose effects on embryonic neural development remain unclear. This study aimed to identify the potential drug targets for rapamycin in embryonic neural development. 18 rapamycin-NTDs targets were obtained by the network pharmacology approach. Functional enrichment

Biallelic variants in PIKFYVE cause a hypomyelinating leukodystrophy with endolysosomal dysfunction.

The PIKfyve-FIG4-VAC14 lipid kinase complex generates phosphoinositide PI(3,5)P2. Pathogenic variants in VAC14 and FIG4 cause neurodevelopmental disorders and PIKFYVE has been linked to autosomal dominant fleck corneal dystrophy. We searched for the underlyging, genetic cause of a neurodevelopmental disorder in five affected individuals from three families using exome- or genome sequencing. Cranial MRI images and clinical phenotypes were analyzed. Size, number and acidity of cellular vacuoles were investigated in patient-derived fibroblasts from family 1 using LysoSensor acidophilic probes. Le

CNPY3 variants associated with a recessive developmental and epileptic encephalopathy affect inflammasome and TLR function.

BACKGROUND: Canopy FGF signalling regulator 3 (CNPY3) is an endoplasmic reticulum (ER)-resident chaperone essential for toll-like receptor (TLR) folding, trafficking and inflammasome activation. In rare paediatric cases, biallelic CNPY3 variants have been associated with developmental and epileptic encephalopathy (DEE), but available data remain sparse. METHODS: 50 individuals from 38 unrelated families with biallelic CNPY3 variants and DEE were evaluated for exome and genome findings, standardised clinical phenotype descriptions, EEG results, and neuroimaging data. A selection of variants un

Changes in birth certificate-reported prevalence of congenital anomalies at live birth during the COVID-19 pandemic: a nationwide study of 26 million United States live births.

BACKGROUND: Although the majority of studies have indicated that maternal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection and coronavirus disease 2019 (COVID-19) vaccination are not associated with neonatal birth defects, it is relevant to examine whether the disease burden changed during the COVID-19 pandemic, as other influencing factors may have also changed. We aimed to compare the birth certificate-reported prevalence of congenital anomalies at live birth before and during the pandemic. METHODS: Using public birth data, all live births from 2016 to 2022 in the Unit

Imaging of Congenital Anomalies of the Spinal Cord: Part 1.

Spinal dysraphism (SD) encompasses a spectrum of congenital malformations that develop during spinal embryogenesis (between the second and sixth gestational weeks). Broadly, SDs are classified into "open" or "closed" based on the presence (or lack of) of an overlying cutaneous defect. In general, open SDs are associated with significant neurologic impairments warranting prompt diagnosis and surgical correction-including in utero interventions. Radiology plays a key role in identification and distinguishing SDs thus providing information for surgical triage, prognostication, and outcome assessm

Choroid plexus remodeling associated with inflammatory CSF exposure in preterm acquired hydrocephalus.

Acquired hydrocephalus in preterm infants follows intraventricular hemorrhage (IVH) or central nervous system infection and is associated with inflammatory cerebrospinal fluid (CSF) profiles. We integrated clinical magnetic resonance imaging (MRI), CSF cytokine profiling, primary human choroid plexus (ChP) epithelial and endothelial cell assays, and independent transcriptomic and proteomic datasets to investigate ChP remodeling. Infants requiring CSF diversion exhibited greater ChP thickness independent of gestational age, ventricular size, and IVH severity, supporting its potential as a bioma
