Pannexin-1 channel activity regulates neurogenesis and cell survival in the developing cortex.

Mutations in genes encoding a range of ion-conducting proteins disrupt development of the cerebral cortex in humans, often causing polymicrogyria (PMG), yet how ion conduction guides the development of cortical architecture is not clear. Here, we describe three individuals with brain malformations including PMG and microcephaly in whom de novo , missense mutations were identified in PANX1 - encoding an ATP and ion conducting channel. We show that these PMG-associated PANX1 mutations (p.D14H, p.M37R, and p.N338T) disrupt normal glycosylation and confer gain-of-function with respect to ATP relea

Multilocus Genetic Variants in a Child With Neuro-Ichthyosis: A Case of Pharmacoresistant Epilepsy and Developmental Delay Associated With CC2D2A, ABCA12, DOCK6 Variants, and a 14q31.3-q32.11 Deletion.

Neuro-ichthyosis is a rare group of disorders characterized by the coexistence of neurological dysfunction and ichthyotic skin changes. We report a 5-year-old girl born to consanguineous parents who presented with pharmacoresistant epilepsy, severe developmental delay, microcephaly, and ichthyosis. Family history revealed similarly affected siblings, suggesting a hereditary basis. Despite multiple antiepileptic drugs, seizures remained uncontrolled. Whole exome sequencing identified a homozygous CC2D2A variant consistent with Joubert syndrome type 9, along with heterozygous variants in ABCA12

Expanding the mutational spectrum of congenital microcephaly in Pakistani families.

Autosomal recessive primary microcephaly (MCPH) is a genetically heterogeneous neurodevelopmental disorder characterized by a markedly reduced head circumference (-3 to -5 standard deviations) at birth, with relatively preserved brain architecture. Affected individuals often present with mild to moderate intellectual disability, and the condition is more prevalent in populations with high rates of consanguinity, such as Pakistan. To date, pathogenic variants in at least 32 genes have been associated with MCPH, with ASPM and WDR62 accounting for the majority of cases (68% and 14%, respectively)

A Fully-Integrated Bayesian Approach for the Imputation and Analysis of Derived Outcome Variables With Missingness.

Derived variables are variables that are constructed from one or more source variables through established mathematical operations or algorithms. For example, body mass index (BMI) is a derived variable constructed from two source variables: weight and height. When using a derived variable as the outcome in a statistical model, complications arise when some of the source variables have missing values. In this paper, we propose how one can define a single fully integrated Bayesian model to simultaneously impute missing values and sample from the posterior. We compare our proposed method with al

First case of Fahr's disease with homozygous mutations of SLC20A2, among the Libyan children.

BACKGROUND: Fahr’s disease is a rare neurodegenerative condition characterized by a combination of neurological and psychiatric manifestations, along with diffuse brain calcifications. It primarily affects adults and older individuals with heterozygous mutations of the Solute Carrier Family 20 Member 2 Gene (SLC20A2). Few cases have been reported in the pediatric population, typically associated with biallelic mutations. This study presents the first national report of Fahr’s disease in two Libyan children, aged five and three years, respectively. Both siblings exhibited similar symptoms, incl

The First 1000 Days of PKU: A Narrative Review of Maternal PKU and Early Life Management After Positive Newborn Screening.

Background/Objectives: The first 1000 days of life represent a critical window for growth and neurodevelopment, during which nutrition strongly influences brain development and metabolic programming. In phenylketonuria (PKU), dietary management is essential to prevent neurological impairment and later-life risk of non-communicable diseases (NCDs). This review examines current evidence on PKU from pregnancy through complementary feeding, highlighting the impact of nutritional strategies on neurodevelopmental and metabolic outcomes. Methods: This narrative review, following PRISMA guidelines, us

The role of ATP9A (c.1091G > C; p.(Arg364Thr)) variant in cognitive impairment: diagnostic insight from whole exome sequencing.

BACKGROUND: The ATP9A gene encodes a P4-type ATPase involved in phospholipid translocation, essential for vesicular trafficking and neuronal development. Pathogenic ATP9A variants cause autosomal recessive neurodevelopmental disorders characterized by intellectual disability and microcephaly, yet the impact of missense variants remains poorly understood. METHODS: A 7-year-old female patient with cognitive impairment, microcephaly, and developmental delay was admitted to Başakşehir Çam and Sakura City Hospital. Whole exome sequencing (WES) using Illumina technology identified a novel homozygou

Prenatal small head circumference: Which biomarkers best predict microcephaly?

OBJECTIVES: Prenatal ultrasound detection of a small head circumference (US-SHC) is a key indicator of microcephaly. This retrospective study (2015-2024) aimed to determine the incidence of SHC at birth (b-SHC) and microcephaly at 6 months, and to identify prenatal biomarkers. METHODS: We included 55 fetuses with us-SHC and median postnatal follow-up of 28 months (range: 7-112). RESULTS: Among the 47 fetuses with available outcomes, b-SHC was confirmed in 31 (66%), while persistent microcephaly at 6 months was observed in 16 of 34 (47%) liveborns. Fetal brain MRI, particularly cerebral bipar

Nucleolar Protein Nop2 Promotes Neural Differentiation by Regulating Ribosome Biogenesis-Related Processes.

The NOP2 nucleolar protein (NOP2) serves pivotal functions in ribosome biogenesis, cell cycle regulation and embryonic development, yet its spatiotemporal dynamics and mechanistic contributions to neural development remain elusive. Through CRISPR/Cas9-mediated knockout modeling in zebrafish, we demonstrated that NOP2 nucleolar protein homolog (yeast) (nop2) deficiency induced embryonic lethality within 3-5 days post-fertilization (dpf), accompanied by pathognomonic microcephaly and cerebral edema. Mechanistic analysis suggested that nop2 ablation triggered differentiation impairment in neural

Genetic architecture and prognostic significance of suspected fetal microcephaly: evidence from prenatal exome sequencing in a large prospective cohort.

BACKGROUND: Fetal microcephaly (FMIC) is a neurodevelopmental disorder with heterogeneous etiologies and uncertain prenatal prognosis. Discrepancies between prenatal and postnatal head circumference (HC) measurements may confound ultrasound-based diagnosis, underscoring the need for genetic stratification to improve risk assessment. METHODS: This prospective cohort study analyzed data from 301 fetuses with suspected FMIC collected between 2014 and 2022. Trio-prenatal exome sequencing (pES) was performed in 301 fetuses with suspected FMIC and normal results on karyotyping and chromosomal micro

Two siblings with CCDC32-related cardiofacioneurodevelopmental syndrome diagnosed by clinical RNA-sequencing and review of literature.

Cardiofacioneurodevelopmental syndrome (CFNDS, MIM:619123) is a rare genetic disorder caused by bi-allelic pathogenic variants in CCDC32. So far, CFNDS has only been described in four living individuals and one terminated fetus from four families, and the clinical phenotype can include microcephaly, facial malformations, developmental delay, cerebellar hypoplasia, and cardiac anomalies. We present a family with two affected individuals who were diagnosed through clinical RNA sequencing (RNA-seq) after conventional DNA diagnostics did not yield a molecular cause. Skipping of two exons in CCDC32

Bi-allelic loss-of-function variants in JKAMP cause a neurodevelopmental syndrome associated with dysregulation of GPR37 trafficking.

The endoplasmic reticulum (ER) serves as a key hub for protein homeostasis, maintaining a strict quality-control system that ensures only properly folded proteins reach their destinations, while misfolded proteins are degraded via ER-associated degradation (ERAD) or selective ER-phagy. JKAMP, which encodes an ER-resident transmembrane protein involved in ERAD, has not previously been associated with human disease. Here, we report bi-allelic loss-of-function variants in JKAMP in 14 affected individuals from 10 unrelated families presenting with a neurodevelopmental syndrome characterized by int

Systematic analysis of homozygous autosomal copy number losses in exomes improves diagnostic yield and uncovers ultra-rare recessive disorders.

Systematic analysis of copy number variants (CNVs) in large datasets is challenging and there are limited studies of homozygous copy number losses in rare disease exome datasets. Here we leveraged the genomic uniqueness and relative under-representation of the Indian population in the current public genomic databases and identified 42,386 possible homozygous losses (median count 20 per individual, range 0 - 55; median size 2.95 kb, range 99 bp - 4.76 Mb) in a heterogeneous cohort of 2,021 individuals with suspected Mendelian disorders, who had undergone exome sequencing using 12 different capt

Clinical Exome Sequencing as a Key Diagnostic Tool: A Rare de novo TRIO Variant in Dizygotic Twins.

INTRODUCTION: TRIO-associated neurodevelopmental disorders are rare genetic conditions caused by pathogenic variants in the TRIO gene, which plays a crucial role in neuronal development. Mutations in the TRIO gene are increasingly associated with autosomal dominant inheritance. However, de novo occurrences in dizygotic twins remain rare and poorly understood. CASE PRESENTATION: In this work, clinical exome sequencing (CES) was performed on dizygotic twin sisters who presented with neurodevelopmental delay, intellectual disability, behavioral issues, and microcephaly.
