Multiple Neurological Complications and Multidrug-Resistant Bacterial Infections During Prolonged Intensive Care for Undocumented Meningoencephalitis in Côte d'Ivoire: A Case Report.

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

Papillary Tumor of the Pineal Region Presenting With Obstructive Hydrocephalus: A Diagnostic and Management Challenge.

We report a 45-year-old woman with a papillary tumor of the pineal region (PTPR), a neuroepithelial neoplasm of the pineal gland, who presented with a one-year history of headaches, visual disturbance, and gait impairment and was found to have a pineal mass with obstructive hydrocephalus. She underwent endoscopic third ventriculostomy (ETV) with biopsy; the initial radiologic impression favored pineocytoma. Immunohistochemistry was nondiagnostic on the limited sample, and the diagnosis of PTPR was established only after review at two academic neuropathology centers. DNA methylation profiling w

Bobble-Head Doll Syndrome Caused by Aqueductal Web-Associated Obstructive Hydrocephalus in an Adult: A Case Report with Video.

BACKGROUND: Bobble-head doll syndrome (BHDS) is a rare movement disorder usually observed in children and associated with third ventricular lesions or hydrocephalus. CASE REPORT: A 24-year-old man presented with progressive memory impairment, gait instability, urinary incontinence, and irregular no-no head tremor with a sensory trick. MRI revealed severe obstructive hydrocephalus due to an aqueductal web. Ventriculoperitoneal shunting was performed, and the tremor resolved within one week. Neurological symptoms improved, and he remained independent 18 months after surgery. DISCUSSION: Adult

Macroprolactinoma with an atypical course.

Prolactinomas are among the most common pituitary adenomas. The typical symptoms of hyperprolactinemia associated with a prolactinomaprimarily include symptoms of hypogonadism resulting from the suppression of the hypothalamic-pituitary-gonadal axis.In men and postmenopausal women, symptoms related to the mass effect of the tumor usually predominate. In some cases, however, prolactinomas may initially present with atypical symptoms, which can make it difficult to establish the correct diagnosis. An extensive literature search covering the past 30 years was conducted using the PubMed database. T

Masquerade Syndrome Presenting as Optic Neuritis and Retinitis Due to Pituitary Germinoma.

Masquerade syndrome is a collective term for various ocular diseases presenting with clinical features resembling intraocular inflammation. Delays in definitive diagnosis and treatment can not only lead to irreversible vision loss but also impact the patient's prognosis regarding survival. We report the case of a 26-year-old male patient who presented with impaired vision in the right eye, which had persisted for six months; neuroretinitis was diagnosed. Although infectious neuroretinitis was suspected, tests for pathogens such as Mycobacterium tuberculosis, Toxoplasma, Toxocara, and Bartonell

Morphological subtypes of Chiari malformation type I and clinical outcomes after posterior fossa decompression.

OBJECTIVE: Chiari malformation type I (CM-I) is the most common structural hindbrain defect in humans and causes debilitating headaches and other neurological symptoms frequently requiring surgery. Although patients' brains have heterogeneous shapes, diagnosis is based on a single radiographic measure with only 1 available pathology-directed treatment. The authors sought to determine whether there are distinct morphological subtypes of CM-I and to determine whether patients with different subtypes differ in clinical response to neurosurgical decompression. METHODS: For this cohort study, 186

Cytomegalovirus (CMV) Central Nervous System Infection and Recurrent Posterior Circulation Ischemic Strokes in an Immunocompetent Patient.

Cytomegalovirus (CMV) infection in an immunocompetent adult is rarely symptomatic. Neurological manifestations are uncommon and are predominantly characterized by meningitis, myelitis, and acute polyradiculoneuritis. We report a rare case of an immunocompetent patient who presented with prolonged headaches complicated by recurrent posterior circulation ischemic strokes associated with intracranial stenoses of the basilar trunk and posterior cerebral arteries. Analysis of his cerebrospinal fluid (CSF) revealed lymphocytic meningitis with moderate elevation of the CSF protein level, normal CSF g

Isolated Aphasia as an Epileptic Manifestation of Neurosarcoidosis: A Case Report.

Acute aphasia requires urgent evaluation for stroke, but recurrent and stereotyped language disturbances may also reflect focal seizures. We report the case of a 23-year-old man brought to the emergency department (ED) with aphasia that had been present for approximately six hours. Over the preceding week, he had experienced several similar but briefer episodes that had resolved spontaneously. He was concurrently undergoing investigation at another institution for persistent hiccups and near-daily vomiting of more than one month's duration. Delayed posterior tilting of a giant basilar tip aneurysm following stent-assisted coiling with therapeutic P1 occlusion: A case report.

BACKGROUND: Giant basilar tip aneurysms remain difficult to treat due to their location, bifurcation anatomy, adjacent perforators, and potential for mass effect. Endovascular treatment, including stent-assisted coiling, is a valuable option. Despite angiographic occlusion, mass effect may still be clinically significant. CASE DESCRIPTION: A 67-year-old man presented with headache and was diagnosed with a 27 × 17-mm giant unruptured basilar tip aneurysm. Preoperative angiography demonstrated perforators arising predominantly from the left P1 segment and a well-developed right posterior commun

Recurrent Left Abducens Nerve Palsy and Idiopathic Intracranial Hypertension During Pregnancy: A Diagnostic Challenge.

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without an identifiable mass lesion, hydrocephalus, cerebral venous sinus thrombosis, or abnormal cerebrospinal fluid composition. Diagnosis during pregnancy can be challenging because headache and visual symptoms overlap with several neurologic and obstetric disorders. We report a previously healthy 20-year-old primigravida with a body mass index of approximately 33 kg/m² who presented at 17 weeks' gestation with intermittent headache, photophobia, blurred vision, horizontal diplopia, nasal deviation

MRI Evaluation and Reporting in Suspected and Established Idiopathic Normal Pressure Hydrocephalus, AJR Expert Panel Review.

This AJR Expert Panel Narrative Review summarizes MRI features of idiopathic normal pressure hydrocephalus (iNPH) and their utility in various clinical scenarios, describes imaging features suggestive of alternate or co-pathology, and provides a framework for reporting such findings. iNPH is characterized by gait impairment, cognitive decline, and urinary incontinence that may be improved with CSF shunting. Imaging is an important part of the diagnostic assessment for iNPH by identifying findings supportive of a diagnosis of iNPH or of alternate diagnoses or co-pathology. Although MRI features

Regeneration can take place across Drosophila compartments or segments with different Hox gene expression.

OBJECTIVES: Optic pathway/hypothalamic gliomas (OPHGs) tend to occur in young children. Treatment options consist of surgical resection, radiation therapy (RT) and chemotherapy. Due to complications induced by surgery and RT, chemotherapy has gained significant recognition for the treatment of OPHG in young children. Chemosensitivity of OPHG in very young children under 3 years of age has not been well documented. We analyzed 14 patients who were treated with chemotherapy with or without surgery. MATERIALS AND METHODS: Fourteen children younger than 3 years (median age of 10 months) with OPHG

Hypercapnia enhances airway smooth muscle contractility via STIM1-dependent Ca2+ signaling.

Hypercapnia, elevated carbon dioxide (CO2), is common in advanced chronic obstructive pulmonary disease (COPD) and predicts poor clinical outcomes. Traditionally considered a consequence of disease severity, hypercapnia may drive disease progression by promoting airway dysfunction. Here, we show that hypercapnia acts as an active stressor, driving airway smooth muscle (ASM) constriction through a stromal interaction molecule 1-dependent (STIM1-dependent) pathway. Hypercapnia rapidly activates ERK, triggering sarcoplasmic reticulum calcium (Ca2+) release via phosphorylation of the inositol 1,4,

Cerebello-Pontine Angle Tumors in Children: An Update on Challenging Neoplasms.

Introduction: Cerebellopontine angle (CPA) tumors are rare in children. As a result, knowledge on them is still limited, often concerning old series. The goal of this study is to provide an update on these challenging neoplasms by presenting a large series compared with those available in the literature and focusing on tumor characteristics, molecular pattern, extent of tumor removal, surgical complications, and outcome. Methods: All children with CPA tumors consecutively operated on between 2010 and 2020 (minimum follow-up: 5 years) and with complete follow-up data were considered. Retro-sigm

Beyond β-amyloid: transthyretin cerebral amyloid angiopathy.

Cerebral amyloid angiopathy (CAA) is most commonly associated with β-amyloid deposition, but other amyloid proteins may accumulate in leptomeningeal and superficial cortical vessels. We report a man first evaluated neurologically at 44 years of age after approximately one year of recurrent upper-limb and perioral sensory symptoms with speech disturbance, retrospectively considered transient focal neurological episodes (TFNEs). During follow-up, he developed bilateral vitritis and progressive behavioral, psychiatric, and cognitive impairment. Brain computed tomography showed diffuse linear and

Assessment of VP Shunt Valve Settings Using Reconstructions from Non-Contrast Head CT: A Comparative Study with Conventional Radiographs.

PURPOSE: The aim of the study was to investigate whether the ventriculoperitoneal (VP) shunt valve setting can be reliably assessed using maximum intensity projection (MIP) reconstructions from non-contrast, full-dose head CT scans, and how this method performs in comparison to conventional lateral skull radiographs. METHODS: This retrospective study included 41 adult patients (mean age 59 ± 25 years) with Codman Certas programmable VP shunt valves who underwent lateral skull X‑ray and a same-day, non-contrast head CT scan between January and July 2024. From the CT data, MIP reconstructions o

Intracranial hemorrhage identified after initiating therapeutic hypothermia: two case reports of neonatal hypoxic-ischemic encephalopathy due to hypovolemic shock with anemia: a case report.

BACKGROUND: Therapeutic hypothermia for moderate-to-severe hypoxic-ischemic encephalopathy in neonates effectively improves neurological outcomes when initiated within 6 hours of birth. However, coagulopathy is a potential side effect of therapeutic hypothermia and requires careful monitoring for signs of hemorrhage. Uncontrolled hemorrhage is the primary exclusion criterion for therapeutic hypothermia. Herein, we report two cases of intracranial hemorrhage that, despite being massive enough to cause hypovolemic shock with anemia, could not be detected by cranial ultrasonography before the ini
