Ventriculoperitoneal shunts (VPSs) are the standard treatment for hydrocephalus but are associated with significant long-term complications, including distal catheter migration. Umbilical extrusion is an exceptionally rare manifestation, particularly in adults, and has seldom been described following minimally invasive gynecologic surgery. We report the case of a 35-year-old female patient with a VPS placed in early childhood who presented with persistent serous drainage from a prior umbilical port site four months after a robotic-assisted laparoscopic hysterectomy. Initial imaging demonstrate

OBJECTIVE: This study aims to develop and validate a more efficient diagnostic system for suspected ventriculoperitoneal shunt dysfunction by integrating traditional radionuclide shuntgraphy with SPECT imaging.
METHODS: This retrospective study examined patients with suspected shunt malfunction admitted to our neurosurgery department from 2021 to 2024, who underwent SPECT/CT imaging of the ventricular shunt. SPECT/CT results were categorized as three groups based on tracer behavior: normal shunt function, partial obstruction, and shunt obstruction. Baseline data, imaging characteristics and c

BACKGROUND AND OBJECTIVES: Current practice for normal pressure hydrocephalus (NPH) relies on clinical presentation, imaging, and various clinical procedures to determine the need for neurosurgical intervention, specifically the installation of a ventriculoperitoneal shunt (VPS). This study aimed to evaluate the utility of a TMS-evoked potential (TEP)-based assessment (Delphi-MD) for predicting sustained response (over one year) to VPS, serving as an ancillary test to the cerebrospinal fluid tap test (CTT).
METHODS: Forty-one patients with possible iNPH underwent Delphi-MD evaluation of wave

BACKGROUND: Gorlin syndrome is a multisystem developmental and cancer-predisposing disorder commonly caused by germline PTCH1 variants. The Kimonis clinical criteria capture the canonical phenotype; however, central nervous system (CNS), cranial, and cerebrovascular conditions documented beyond these criteria remain poorly defined.
METHODS: We conducted a retrospective descriptive cohort study of 32 patients who fulfilled the Kimonis clinical criteria and received care at either or both hospitals in Japan. Clinical information through March 31, 2026, including sufficiently documented care els

Quadrigeminal cistern arachnoid cysts are rare intracranial lesions. While most intracranial arachnoid cysts in adults are incidentally detected and asymptomatic, quadrigeminal cistern arachnoid cysts, located adjacent to the dorsal midbrain and cerebral aqueduct, may become symptomatic, most often with obstructive hydrocephalus. Trochlear nerve palsy secondary to quadrigeminal cistern arachnoid cysts has been reported only rarely, and the pathophysiology and optimal management remain unclear. We report a 60-yr-old man who presented with progressive diplopia due to left trochlear nerve palsy.

Intracranial pressure (ICP) is a key physiological parameter for the diagnosis and management of traumatic brain injury, hydrocephalus, stroke, and other neurological disorders. ICP sensors are typically required to provide accurate pressure measurement at low pressure while maintaining long-term stability in biofluid environments. Although conventional liquid-based ICP systems remain the clinical standard, their bulky and tethered configurations increase the risk of infection and tissue injury and restrict patient mobility. Advances in miniaturized implantable sensors have created new opportu

Tuberculous meningitis (TBM) is the most severe form of extrapulmonary tuberculosis, carrying the highest mortality among all forms. This multicentric prospective cohort study conducted in North India aimed to determine the incidence of complications, functional outcome (modified Rankin scale [mRS]), and mortality in TBM. Patients 12 years old and older with TBM were followed up until completion of 9 months of antitubercular treatment. Patients with concomitant HIV infection, other infections causing meningitis, or multidrug-resistant tuberculosis were excluded. The primary outcome was mRS at

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

We report a 45-year-old woman with a papillary tumor of the pineal region (PTPR), a neuroepithelial neoplasm of the pineal gland, who presented with a one-year history of headaches, visual disturbance, and gait impairment and was found to have a pineal mass with obstructive hydrocephalus. She underwent endoscopic third ventriculostomy (ETV) with biopsy; the initial radiologic impression favored pineocytoma. Immunohistochemistry was nondiagnostic on the limited sample, and the diagnosis of PTPR was established only after review at two academic neuropathology centers. DNA methylation profiling w

BACKGROUND: Bobble-head doll syndrome (BHDS) is a rare movement disorder usually observed in children and associated with third ventricular lesions or hydrocephalus.
CASE REPORT: A 24-year-old man presented with progressive memory impairment, gait instability, urinary incontinence, and irregular no-no head tremor with a sensory trick. MRI revealed severe obstructive hydrocephalus due to an aqueductal web. Ventriculoperitoneal shunting was performed, and the tremor resolved within one week. Neurological symptoms improved, and he remained independent 18 months after surgery.
DISCUSSION: Adult

Prolactinomas are among the most common pituitary adenomas. The typical symptoms of hyperprolactinemia associated with a prolactinomaprimarily include symptoms of hypogonadism resulting from the suppression of the hypothalamic-pituitary-gonadal axis.In men and postmenopausal women, symptoms related to the mass effect of the tumor usually predominate. In some cases, however, prolactinomas may initially present with atypical symptoms, which can make it difficult to establish the correct diagnosis. An extensive literature search covering the past 30 years was conducted using the PubMed database. T

Masquerade syndrome is a collective term for various ocular diseases presenting with clinical features resembling intraocular inflammation. Delays in definitive diagnosis and treatment can not only lead to irreversible vision loss but also impact the patient's prognosis regarding survival. We report the case of a 26-year-old male patient who presented with impaired vision in the right eye, which had persisted for six months; neuroretinitis was diagnosed. Although infectious neuroretinitis was suspected, tests for pathogens such as Mycobacterium tuberculosis, Toxoplasma, Toxocara, and Bartonell

OBJECTIVE: Chiari malformation type I (CM-I) is the most common structural hindbrain defect in humans and causes debilitating headaches and other neurological symptoms frequently requiring surgery. Although patients' brains have heterogeneous shapes, diagnosis is based on a single radiographic measure with only 1 available pathology-directed treatment. The authors sought to determine whether there are distinct morphological subtypes of CM-I and to determine whether patients with different subtypes differ in clinical response to neurosurgical decompression.
METHODS: For this cohort study, 186

Cytomegalovirus (CMV) infection in an immunocompetent adult is rarely symptomatic. Neurological manifestations are uncommon and are predominantly characterized by meningitis, myelitis, and acute polyradiculoneuritis. We report a rare case of an immunocompetent patient who presented with prolonged headaches complicated by recurrent posterior circulation ischemic strokes associated with intracranial stenoses of the basilar trunk and posterior cerebral arteries. Analysis of his cerebrospinal fluid (CSF) revealed lymphocytic meningitis with moderate elevation of the CSF protein level, normal CSF g

Acute aphasia requires urgent evaluation for stroke, but recurrent and stereotyped language disturbances may also reflect focal seizures. We report the case of a 23-year-old man brought to the emergency department (ED) with aphasia that had been present for approximately six hours. Over the preceding week, he had experienced several similar but briefer episodes that had resolved spontaneously. He was concurrently undergoing investigation at another institution for persistent hiccups and near-daily vomiting of more than one month's duration. BACKGROUND: Giant basilar tip aneurysms remain difficult to treat due to their location, bifurcation anatomy, adjacent perforators, and potential for mass effect. Endovascular treatment, including stent-assisted coiling, is a valuable option. Despite angiographic occlusion, mass effect may still be clinically significant.
CASE DESCRIPTION: A 67-year-old man presented with headache and was diagnosed with a 27 × 17-mm giant unruptured basilar tip aneurysm. Preoperative angiography demonstrated perforators arising predominantly from the left P1 segment and a well-developed right posterior commun

Idiopathic intracranial hypertension (IIH) is characterized by elevated intracranial pressure without an identifiable mass lesion, hydrocephalus, cerebral venous sinus thrombosis, or abnormal cerebrospinal fluid composition. Diagnosis during pregnancy can be challenging because headache and visual symptoms overlap with several neurologic and obstetric disorders. We report a previously healthy 20-year-old primigravida with a body mass index of approximately 33 kg/m² who presented at 17 weeks' gestation with intermittent headache, photophobia, blurred vision, horizontal diplopia, nasal deviation

This AJR Expert Panel Narrative Review summarizes MRI features of idiopathic normal pressure hydrocephalus (iNPH) and their utility in various clinical scenarios, describes imaging features suggestive of alternate or co-pathology, and provides a framework for reporting such findings. iNPH is characterized by gait impairment, cognitive decline, and urinary incontinence that may be improved with CSF shunting. Imaging is an important part of the diagnostic assessment for iNPH by identifying findings supportive of a diagnosis of iNPH or of alternate diagnoses or co-pathology. Although MRI features

OBJECTIVES: Optic pathway/hypothalamic gliomas (OPHGs) tend to occur in young children. Treatment options consist of surgical resection, radiation therapy (RT) and chemotherapy. Due to complications induced by surgery and RT, chemotherapy has gained significant recognition for the treatment of OPHG in young children. Chemosensitivity of OPHG in very young children under 3 years of age has not been well documented. We analyzed 14 patients who were treated with chemotherapy with or without surgery.
MATERIALS AND METHODS: Fourteen children younger than 3 years (median age of 10 months) with OPHG
