Unilateral hydrocephalus is a rare form of ventricular enlargement, typically resulting from obstruction of the cerebrospinal fluid (CSF) flow at the foramen of Monro. Unilateral hydrocephalus is rarely reported as a complication of recurrent ischemic stroke. While tumours, cysts, and infections are common etiologies, vascular causes are rarely reported. We present a 58-year-old female patient who presented to the emergency room in an irritable state. She also had acute right-sided weakness of upper and lower limbs along with aphasia. Neuroimaging revealed Gliosis with encephalomalacic changes

INTRODUCTION: We conducted a systematic review and meta-analysis to compare the effectiveness of shunts with and without anti-siphon devices (ASDs) or other flow-regulating systems in preventing cerebrospinal fluid (CSF) overdrainage and its associated complications.
METHODS: Following established guidelines, we searched PubMed, Embase, Scopus, Cochrane, and Web of Science for clinical studies evaluating adult patients diagnosed with NPH who underwent CSF shunting with ASDs or flow-regulating valves, and included a control group of patients with standard CSF shunts lacking such mechanisms. No

BACKGROUND: Long-term treatment for hydrocephalus typically involves permanent cerebrospinal fluid (CSF) diversion through shunt placement. Ventriculoperitoneal (VP) shunts are often the first-line treatment; however, they may be deferred for alternative modalities such as ventriculopleural (VPL) shunts in low-pressure hydrocephalus or cases of a hostile abdomen. Our emphasis on minimally invasive techniques led to the adoption of a single incision endoscopic approach for distal shunt placement within the pleural space.
METHODS: 11 patients who underwent VPL or syringopleural (SPL) shunt plac

Neurocognitive impairments are common in paediatric central nervous system (CNS) tumours and can relate to clinical and demographic factors. The impact of social or environmental factors, such as family psychosocial risk, is less well understood. We examined neurocognitive functioning in children with CNS tumours shortly after diagnosis and associations with psychosocial risk. This cohort study included children who underwent neuropsychological assessments within three months of diagnosis. Proportion, chi-square and t-tests were used to compare neurocognitive outcomes to norms and evaluate pre

Idiopathic normal pressure hydrocephalus (iNPH) is a common and potentially reversible cause of gait disturbance, cognitive decline, and urinary incontinence in the elderly, with current treatments relying primarily on cerebrospinal fluid (CSF) diversion procedures that are associated with notable complications and failure rates. The choroid plexus, as the principal site of CSF production, represents a biologically plausible therapeutic target. Stereotactic radiosurgery (SRS) offers a non-invasive approach for precise ablation of the central nervous system pathologies; however, its role in iNP

INTRODUCTION: Normal pressure hydrocephalus (NPH) is a cause of neurological impairment, with programmable shunt valves serving as the standard treatment. However, magnetic interference can result in altered pressure settings, affecting valve functionality.
RESEARCH QUESTION: This systematic review investigates the effects of magnetic resonance imaging (MRI) on programmable shunt valves in NPH patients.
MATERIAL AND METHODS: Conducted in adherence to PRISMA guidelines, an extensive literature search across PubMed and Embase databases from January 2000 to January 2025, was performed. Studies

Hydrocephalus includes a diverse group of disorders with different causes, symptoms, and treatment options. Despite progress in surgery and diagnostics, the overall understanding of clinical research in hydrocephalus remains limited. We performed a scoping review to systematically examine registered clinical trials, aiming to identify trends in treatment approaches, research areas, and gaps in the current evidence. A comprehensive search of ClinicalTrials.gov was conducted to identify all registered clinical trials related to hydrocephalus. Trials were included regardless of study design, phas

Chiari type I malformation (CMI) is a congenital hindbrain anomaly that may disrupt cerebrospinal fluid (CSF) dynamics and lead to intracranial hypertension. Although papilledema is a well-recognized manifestation of elevated intracranial pressure, it is typically bilateral and symmetric. Asymmetrical papilledema is uncommon, particularly in pediatric patients, and may pose a diagnostic challenge. We report a 10-year-old boy with no neurological or visual complaints who was referred after asymmetrical optic disc edema was detected during a routine ophthalmic examination. Visual acuity was pres

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a potentially reversible neurological disorder whose diagnosis is complicated by clinical overlap with neurodegenerative diseases. Fluid biomarkers may provide insight into disease mechanisms and support differential diagnosis.
METHODS: We performed a small exploratory study at the Neurology and Neurosurgery Sections of the University Hospital of Perugia, between 2024 and 2025, comparing cerebrospinal fluid (CSF) and plasma biomarkers among patients with iNPH, Alzheimer's disease (AD), Parkinson's disease (PD), and non-neurodegene

BACKGROUND: Although MRI and diffusion tensor imaging (DTI) based studies have evaluated the Splenial Angle (SA) in distinguishing normal pressure hydrocephalus (NPH), its reproducibility and diagnostic utility on CT remain untested.
OBJECTIVE: To assess the diagnostic performance and intra/inter-observer repeatability of CT-derived SA measurements for differentiating NPH from Alzheimer's and Parkinson's diseases.
MATERIALS AND METHODS: This retrospective study included 325 individuals: 87 with NPH, 71 with Alzheimer's disease, 66 with Parkinson's disease, and 101 healthy controls. Evans Ind

Primary angiitis of the central nervous system (PACNS) is a rare inflammatory vasculopathy that frequently mimics more common cerebrovascular disorders. We report a 44-year-old woman who presented with acute posterior circulation stroke symptoms consistent with lateral medullary syndrome. Initial neuroimaging revealed a right cerebellar infarct with minimal non-aneurysmal subarachnoid hemorrhage and multifocal intracranial arterial narrowing. Concomitant atrial fibrillation and a patent foramen ovale initially suggested a cardioembolic source; however, subsequent high-resolution vessel wall ma

Adenylosuccinate lyase deficiency (ADSLD) is an ultra-rare autosomal recessive disorder of purine metabolism. The most severe neonatal phenotype presents with profound encephalopathy from birth, hypotonia, respiratory insufficiency, and early seizures. A preterm infant was delivered at 34+0 weeks with antenatal progressive microcephaly and mild polyhydramnios. At birth, the neonate exhibited absent spontaneous respiration and marked hypotonia, necessitating ventilation and intubation. Respiratory distress syndrome (RDS) was managed with surfactant, but two extubation attempts with non-invasive

Vertebral artery dissection (VAD) can cause posterior-circulation ischemia after cervical manipulation; postpartum physiological changes may increase vascular vulnerability. A 30-year-old woman, nine months postpartum, developed an abrupt occipital headache, vertigo, nausea, and gait imbalance during a commercial neck massage. On examination, she had truncal ataxia and gait instability without focal limb weakness. We obtained 1.5-T MRI (diffusion-weighted imaging (DWI)/apparent diffusion coefficient (ADC), T1-weighted imaging (T1), T2-weighted imaging (T2), fluid-attenuated inversion recovery

Pneumocephalus is defined as the presence of intracranial air and is a rare but potentially significant complication following ventriculoperitoneal (VP) shunt placement. Delayed presentations are uncommon and remain poorly described in the literature. The report demonstrates the case of a 74-year-old male patient who developed delayed spontaneous intraventricular pneumocephalus 10 months after VP shunt insertion in September 2022 using a high-pressure fixed valve for obstructive hydrocephalus secondary to a tectal plate lesion. The patient presented in July 2023 with posture-dependent frontal

Sporadic vestibular schwannomas are typically slow-growing, unilateral cerebellopontine angle benign tumors that occur in middle-aged and older adults and rarely present in individuals younger than 30 years. We describe the case of a 21-year-old man who presented with progressive unilateral hearing loss and tinnitus and was found to have a large vestibular schwannoma causing brainstem compression and obstructive hydrocephalus. Tumor analysis demonstrated a somatic mutation in neurofibromatosis type 2 (NF2), confined to the tumor, supporting a diagnosis of sporadic vestibular schwannoma in the

Headache is a common reason for consulting inpatient neurology and the most common outpatient reason for referral to neurology clinics. Idiopathic intracranial hypertension is a common cause of secondary chronic headaches. Presence of typical patient traits and comorbid risk factors such as obesity could lead to bias and misdiagnosis without investigation into other etiologies. Primary leptomeningeal medulloblastoma in adults is an extremely rare cause of headache that initially mimicked idiopathic intracranial hypertension in this case. A 25-year-old male with months of intractable headaches

Endoscopic endonasal surgery (EES) offers a direct ventral corridor to the skull base, potentially enabling maximal resection of extensive cholesteatomas (epidermoid cysts) in the posterior cranial fossa and parasellar region. However, outcomes and complications specific to this approach require detailed analysis. A retrospective review was conducted on 34 patients who underwent EES for cholesteatomas of the posterior fossa and parasellar region at a single center from 2011 to 2025 year. Extent of resection (near-total > 95%, subtotal 90-95%), complications, and neurological outcomes were anal

BACKGROUND: The TNFRSF1A gene encodes TNFR1, which regulates inflammation and apoptosis. Variants in this gene present with an autoinflammatory phenotype and are linked to tumor necrosis factor receptor associated periodic syndrome (TRAPS), an orphan monogenic autoinflammatory disease (AID) that can mimic systemic juvenile idiopathic arthritis (sJIA) and other rheumatic diseases. Despite known mechanisms and classification criteria, diagnosing and choosing therapy for patients with TNFRSF1A variants remains challenging. Assessing phenotypic characteristics and considering alternative therapies
