Hypercapnia enhances airway smooth muscle contractility via STIM1-dependent Ca2+ signaling.

Hypercapnia, elevated carbon dioxide (CO2), is common in advanced chronic obstructive pulmonary disease (COPD) and predicts poor clinical outcomes. Traditionally considered a consequence of disease severity, hypercapnia may drive disease progression by promoting airway dysfunction. Here, we show that hypercapnia acts as an active stressor, driving airway smooth muscle (ASM) constriction through a stromal interaction molecule 1-dependent (STIM1-dependent) pathway. Hypercapnia rapidly activates ERK, triggering sarcoplasmic reticulum calcium (Ca2+) release via phosphorylation of the inositol 1,4,

Cerebello-Pontine Angle Tumors in Children: An Update on Challenging Neoplasms.

Introduction: Cerebellopontine angle (CPA) tumors are rare in children. As a result, knowledge on them is still limited, often concerning old series. The goal of this study is to provide an update on these challenging neoplasms by presenting a large series compared with those available in the literature and focusing on tumor characteristics, molecular pattern, extent of tumor removal, surgical complications, and outcome. Methods: All children with CPA tumors consecutively operated on between 2010 and 2020 (minimum follow-up: 5 years) and with complete follow-up data were considered. Retro-sigm

Beyond β-amyloid: transthyretin cerebral amyloid angiopathy.

Cerebral amyloid angiopathy (CAA) is most commonly associated with β-amyloid deposition, but other amyloid proteins may accumulate in leptomeningeal and superficial cortical vessels. We report a man first evaluated neurologically at 44 years of age after approximately one year of recurrent upper-limb and perioral sensory symptoms with speech disturbance, retrospectively considered transient focal neurological episodes (TFNEs). During follow-up, he developed bilateral vitritis and progressive behavioral, psychiatric, and cognitive impairment. Brain computed tomography showed diffuse linear and

Assessment of VP Shunt Valve Settings Using Reconstructions from Non-Contrast Head CT: A Comparative Study with Conventional Radiographs.

PURPOSE: The aim of the study was to investigate whether the ventriculoperitoneal (VP) shunt valve setting can be reliably assessed using maximum intensity projection (MIP) reconstructions from non-contrast, full-dose head CT scans, and how this method performs in comparison to conventional lateral skull radiographs. METHODS: This retrospective study included 41 adult patients (mean age 59 ± 25 years) with Codman Certas programmable VP shunt valves who underwent lateral skull X‑ray and a same-day, non-contrast head CT scan between January and July 2024. From the CT data, MIP reconstructions o

Intracranial hemorrhage identified after initiating therapeutic hypothermia: two case reports of neonatal hypoxic-ischemic encephalopathy due to hypovolemic shock with anemia: a case report.

BACKGROUND: Therapeutic hypothermia for moderate-to-severe hypoxic-ischemic encephalopathy in neonates effectively improves neurological outcomes when initiated within 6 hours of birth. However, coagulopathy is a potential side effect of therapeutic hypothermia and requires careful monitoring for signs of hemorrhage. Uncontrolled hemorrhage is the primary exclusion criterion for therapeutic hypothermia. Herein, we report two cases of intracranial hemorrhage that, despite being massive enough to cause hypovolemic shock with anemia, could not be detected by cranial ultrasonography before the ini

Morphometric analysis of the inferior petrosal sinus and cerebellopontine angle cistern to assess feasibility of endovascular shunting in pediatric patients.

BACKGROUND: Hydrocephalus is commonly treated using invasive surgical shunt placement with associated morbidity and frequent revision in children. The eShunt System, an endovascular miniature transdural shunt implanted via the inferior petrosal sinus (IPS), has shown promise in adults, but its pediatric application remains undefined because of limited data on IPS and cerebellopontine angle cistern (CPAC) morphometry. MATERIALS AND METHODS: Consecutive patients from two pediatric hospitals with high-resolution brain MRI were retrospectively analyzed. IPS and CPAC measurements were evaluated ag

Can common household devices alter the settings of programmable shunt valves?

OBJECTIVE: Various household devices contain magnets or emit electromagnetic fields and it is hypothesised that the strength of these magnets may be enough to affect the settings on programmable cerebrospinal fluid shunt valves. The aim of this study is to identify changes to the valve settings of seven commonly used programmable valves, when they are near common household devices generating a magnetic field. METHODS: Several commonly encountered household devices were used, with and without rotational movement, with reference to seven commonly used programmable valves (the Codman Certas and

Modern concepts of CSF disorders in monosutural craniosynostosis.

INTRODUCTION: Although hydrocephalus is randomly encountered in monosutural craniosynostosis (CS), CSF disorders may present more subtly and accompany these pictures. Indeed, dilation of subarachnoid spaces is frequently reported. The pathogenesis of these disorders is still under debate, as well as their prognostic significance. METHODS: A thorough review of the literature has been performed. Accordingly, pathogenic theories, eventual impact of surgical strategy, and prognostic significance are discussed. RESULTS: Most data come from scaphocephaly and trigonocephaly. Focal dilation of subar

Myocardial Injury in Aneurysmal Subarachnoid Haemorrhage: Clinical Correlates and Impact on Mortality in a Single-Centre Australian Intensive Care Unit Cohort.

Background Myocardial injury is a recognised complication of aneurysmal subarachnoid haemorrhage (aSAH), arising from catecholamine-mediated neuro-cardiac mechanisms. While international data describe variable incidence and clinical significance, contemporary Australian evidence remains limited. This study aimed to determine the incidence, predictors, and clinical impact of myocardial injury following aSAH in a tertiary Australian intensive care unit (ICU). Methods We performed a retrospective cohort study of adults admitted with aSAH to the Royal Melbourne Hospital ICU between 1 June 2020 and

Chiari Malformation Type I: A Review of Pathophysiology, Cerebrospinal Fluid Flow Dynamics, Diagnosis, Surgical Management, and Its Relationship to Syringomyelia.

Chiari malformation type I (CM-I) is a neurological disorder characterized by the herniation of the cerebellar tonsils through the foramen magnum, resulting in the obstruction of cerebrospinal fluid (CSF) flow and the compression of the brainstem and upper cervical spinal cord. These disturbances contribute to symptoms such as headaches exacerbated by Valsalva maneuvers, dizziness, neck pain, muscle weakness, sensory disturbances, and coordination difficulties. This review provides an overview of the pathophysiology of CM-I, the role of disrupted CSF dynamics, and diagnostic approaches includi

Clinical Impact of Ultrafast Cranial MRI Implementation in Children Under Six Years of Age.

Background: Young children requiring neurosurgical care frequently undergo repeated neuroimaging. Whereas CT involves exposure to ionizing radiation, conventional MRI is time-consuming and often necessitates sedation in non-cooperative children. To address these limitations, ultrafast cranial MRI (UF-MRI) based on T2-HASTE sequences was implemented at our institution in 2019 for selected indications. The aim of this study was to evaluate the real-world implementation of UF-MRI in children younger than six years of age. Methods: We retrospectively analyzed cranial MRI examinations consisting ex

Plasma levels of thrombin-cleaved osteopontin as a surrogate biomarker of subsequent chronic shunt-dependent hydrocephalus development following aneurysmal subarachnoid hemorrhage.

Chronic shunt-dependent hydrocephalus (SDHC) is a major contributor to poor outcome after aneurysmal subarachnoid hemorrhage (aSAH), reportedly with arachnoid fibrosis implicated in its pathophysiology. In the pathogenesis of fibrosis, N-terminal half of osteopontin (OPN N-half), generated via thrombin cleavage of full-length osteopontin (FL-OPN), is linked to a potent fibrinogenic factor. In this study, clinical data and plasma samples were prospectively collected from 141 consecutive aSAH patients treated with surgical clipping, and plasma FL-OPN and OPN N-half levels were measured. Plasma F

Ruptured Posterior Inferior Cerebellar Artery Aneurysm Presenting With Cerebellar Intraparenchymal and Intraventricular Hemorrhage: A Case Report.

Aneurysmal subarachnoid hemorrhage (aSAH) may present with intraparenchymal and intraventricular extension, particularly in posterior circulation aneurysms, which warrants careful evaluation for secondary causes, including an underlying macrovascular lesion. We report the case of a 39-year-old woman who presented with a sudden-onset severe headache, nuchal pain, and transient loss of consciousness. Initial non-contrast cranial computed tomography demonstrated a dominant left cerebellar intraparenchymal hemorrhage with subarachnoid and intraventricular extension, accompanied by acute hydrocepha

Distinct Clinical Phenotypes of Infection and Mechanical Dysfunction in Ventriculoperitoneal Shunt Complications.

Introduction Ventriculoperitoneal shunt complications, particularly infection and mechanical dysfunction, represent major causes of morbidity and revision surgery in patients with hydrocephalus. Objective This study aimed to evaluate our single-center experience with ventriculoperitoneal shunt revisions and to compare shunt infection and mechanical shunt dysfunction in order to identify institution-specific clinical, laboratory, and cerebrospinal fluid (CSF) characteristics that may facilitate differential diagnosis. Results Among the patients, 140 (35.9%) were diagnosed with ventriculoperiton

Ventriculo gallbladder shunt as an alternative cerebrospinal fluid diversionary procedure.

BACKGROUND: In the complex management of hydrocephalus, ventriculoperitoneal (VP) shunt insertion is the mainstay for cerebrospinal fluid (CSF) diversion. However, when traditional routes fail or are contraindicated - due to congenital abnormalities, infections, extensive adhesions, or previous abdominal surgeries - surgeons must consider less conventional alternatives. The second line is the ventriculo-atrial shunt then the ventriculo-pleural. The ventriculo-gallbladder (VG) shunt, although rarely used, presents a fascinating option with unique physiological advantages. Here, we present an in
