Complex Type Split Cord Malformation: A Two-Center Study.

INTRODUCTION: Complex spinal dysraphism is the condition where multiple dysraphic pathologies are observed in the same patient. Complex type split cord malformation (SCM) is a compound dysraphic phenotype involving disruptions at multiple developmental stages, characterized by the co-occurrence of SCM and spina bifida aperta. This study aimed to understand the differences between complex SCMs from classical SCM cases in terms of embryological aspects, perioperative management, and follow-up. METHODS: Patients with SCM associated with spina bifida aperta who were treated at Dicle University an

De novo mutations and environmental modifiers: lessons from neural tube defects.

Spina bifida is a clinically and etiologically heterogeneous group of neural tube defects (NTDs) that includes meningomyelocele. While folic acid (FA) supplementation has reduced the incidence by 30-50%, genetic contributors remain only partially understood. New trio sequencing technology has identified de novo mutations (DNMs) in 20-25% of patients. Two recent large-scale genomic studies identified DNMs in 187 candidate genes and a recurrent 22q11.2 deletion as risk factors. Partial penetrance and variable expressivity are frequent, suggesting that risk is dependent upon FA and other modifier

Cranioplasty Timing After Decompressive Craniectomy: A Meta-Analysis of 4703 Patients.

BACKGROUND AND OBJECTIVES: Cranioplasty (CP) is a critical neurosurgical intervention typically performed after decompressive craniectomy (DC). However, the optimal timing remains uncertain due to conflicting evidence and heterogeneous study designs. This pooled comparative meta-analysis aims to address this gap. METHODS: We conducted a systematic search for studies reporting outcomes of patients who underwent primary DC followed by CP on September 2024. Ultra-early CP was defined as ≤35 days, early CP as ≤86 days, and late CP as >86 days after DC. Outcomes included functional recovery measur

Clinicoradiological Dissociation in a Giant Lytic Posterior Fossa Epidermoid Cyst: A Case Report.

Intracranial epidermoid cysts (ECs) are rare benign congenital lesions characterized by slow growth and late clinical presentation. Giant ECs of the posterior fossa are exceptionally uncommon and typically associated with early neurological deterioration due to the confined anatomy in this region. We report the case of a 74-year-old woman presenting with chronic headache and mild vertigo, with no focal motor or cranial nerve deficits. MRI revealed a giant extra-axial EC of the left posterior fossa with occipital bone lysis on CT scan. Despite the radiological findings, no acute hydrocephalus o

Pilocytic Astrocytoma Causing Brainstem Compression in Pregnancy: Case Report with Literature Review.

Background: Primary central nervous system tumours in pregnancy are exceptionally rare, with posterior fossa lesions presenting particular diagnostic and management challenges due to their confined anatomical location and proximity to critical neurovascular structures. Pilocytic astrocytoma (PA), typically a paediatric tumour, is uncommon in adults and exceedingly rare in pregnant patients. The physiological changes in pregnancy can obscure tumour-related symptoms, contributing to diagnostic delay and increased maternal-fetal risk. Methods: We report the case of a 24-year-old pregnant woman at

Is the sitting position safe in posterior fossa surgery? An analysis of air embolism risk, complications, and surgical outcomes.

In posterior fossa surgery, patient positioning (sitting, prone, or park-bench) requires a balance between surgical exposure, blood loss, and hemodynamic effects. Due to inter-center variability and case heterogeneity in the literature, no definitive superiority has been demonstrated. This study compared the effects of different positions on surgical outcomes and complications based on a single-center experience and investigated independent predictors of postoperative complications. A retrospective observational analysis was performed on 336 consecutive patients undergoing posterior fossa surg

Long-term brain pressure monitoring via a discrete microimplant; a first-in-human safety and initial efficacy trial in adults and children with hydrocephalus.

Emerging neurotechnologies such as brain-computer interfaces and implantable sensors offer considerable promise in the treatment of a broad range of neurological conditions. The key challenges are reducing the implant size, powering it, and confirming long-term accuracy and safety. Here we report the development of a novel type of implantable medical device that measures intracranial pressure long term and which weighs only 0.28 g. Currently the management of hydrocephalus patients relies heavily on non-specific symptoms e.g. headache and there is a lack of actionable data to drive decisions t

Genome admixture analysis of 1,030 Ugandan infants with neonatal sepsis and hydrocephalus demonstrates geographical stratification of population disease risk.

BACKGROUND: Neonatal disorders such as post-infectious hydrocephalus exhibit a higher incidence in Africa, where the intricate relationships between genetic ancestry, environmental exposures, and other risk factors likely contribute to the increased incidence. METHODS: To start to characterize the common genetic architecture of Ugandan infants, we analyzed genome sequencing data from 1,030 Ugandan infants recruited from studies targeting neonatal sepsis and hydrocephalus. We employed genetic admixture analysis and integrated geospatial data to examine the relationships between genetic backgro

Laser insterstitial thermal therapy followed by craniotomy for tumor resection for pediatric diffuse midline glioma: illustrative case.

BACKGROUND: Resection of thalamic and other subcortical tumors has historically been associated with high morbidity and mortality. Laser interstitial thermal therapy (LITT) followed by craniotomy for resection of ablated tumor is a promising new combined technique. Here the authors describe the first application of this technique in a pediatric patient. OBSERVATIONS: A 6-year-old boy presented with 6 weeks of progressive right hemibody weakness and aphasia. Imaging demonstrated a 4.5-cm left basal ganglia/thalamic mass with moderate hydrocephalus. He underwent endoscopic third ventriculostomy

Cortical folding and thickness characteristics of gray matter in fetuses with non-severe ventriculomegaly.

BACKGROUND: Ventriculomegaly (VM) is a common fetal brain anomaly. While non-severe VM is often not associated with other anatomical abnormalities during the fetal period, some affected fetuses remain at risk of neurodevelopmental impairment. Characterizing whole-brain cortical morphological changes in fetal gray matter associated with VM may help identify biomarkers predictive of adverse neurodevelopmental outcomes. We aimed to determine whether and how VM is associated with cortical morphological characteristics of fetal gray matter regions in fetuses with mild or moderate VM, compared with

Antisense oligonucleotide-mediated knockdown therapy in two infants with severe KCNT1 epileptic encephalopathy.

KCNT1-related epileptic encephalopathy, including epilepsy of infancy with migrating focal seizures, is a severe neurodevelopmental disorder associated with refractory seizures, profound neurologic impairment and premature death. It is caused by de novo genetic variants in KCNT1 that alter the function of Slack, an evolutionarily conserved sodium-gated potassium channel that modulates neuronal firing patterns and excitability. Pathogenic KCNT1 variants lead to overactive Slack channels, boosting total neuronal potassium currents by up to 40%, driving cortical hyperexcitability and causing seiz

The Heart-Brain Connection: Acute Intracranial Pressure Elevation as a Cause of Isolated Bradycardia.

Bradyarrhythmias are commonly attributed to intrinsic conduction disease, ischemia, or medication effects; however, not all etiologies are cardiac in origin. Acute elevation in intracranial pressure (ICP) can provoke clinically significant bradycardia, sometimes without the complete Cushing triad. We describe a 50-year-old nonverbal man with marginal zone lymphoma on rituximab, prior cerebrovascular accident, seizure disorder, and chronic hydrocephalus managed with a ventriculoperitoneal (VP) shunt who presented with lethargy and hypotension during rituximab infusion and was found to have pneu

Malposition of a Peripherally Inserted Central Catheter Into the Internal Vertebral Venous Plexus in a Neonate With Central Nervous System Malformations.

Peripherally inserted central catheters (PICCs) are widely used in neonatal intensive care units (NICUs); however, malposition into the paravertebral venous system is a rare but potentially life-threatening complication. Migration of a PICC into the internal vertebral venous plexus is particularly uncommon, and direct imaging confirmation of this pathway has rarely been reported. We report a term neonate with myelomeningocele, Chiari II malformation, and hydrocephalus in whom a PICC inserted via the left great saphenous vein migrated from the ascending lumbar vein into the internal vertebral v

Evaluation of the endoscopic third ventriculostomy success score for stereotactic prepontine stenting in patients with aqueductal stenosis.

INTRODUCTION: Stereotactic prepontine stenting (STS) has been shown to be a safe and effective alternative treatment to standard endoscopic third ventriculostomy (ETV) for the treatment of triventricular hydrocephalus (TVH). For ETV a success score (ETVSS) allows risk stratification for treatment failure, which was validated in young patients. For patients with TVH treated with STS a comparable success score was not performed to date. This was the reason why we wanted to test whether the ETVSS is also applicable to this patient cohort. RESEARCH QUESTION: Is the ETVSS also eligible for patient
