Optimal timing of ventriculoperitoneal shunt insertion relative to cranioplasty post-decompressive craniectomy: a frequentist network meta-analysis.

OBJECTIVE: Optimal timing of ventriculoperitoneal shunting (VPS) relative to cranioplasty (CP) after decompressive craniectomy remains controversial. VPS may be performed before, during, or after CP, but no consensus exists on which sequence minimizes complications. We performed a systematic review and network meta-analysis to compare the safety profiles of pre-, simultaneous, and post-CP shunting. METHODS: We searched PubMed/MEDLINE, Embase, and CENTRAL from inception to October 2025. Eligible studies directly compared at least two timing strategies in patients undergoing both CP and VPS. Pr

Hemispheric surgery for hemimegalencephaly and hemispheric cortical dysplasia in infants below 12 months of age-A single-center experience.

OBJECTIVE: Infants with early drug-resistant epilepsy caused by hemimegalencephaly (HME) and hemispheric cortical dysplasia (HCD) pose substantial surgical and critical care challenges and have poor seizure outcome prognosis. This study evaluated the safety, complications, and seizure outcomes of hemispheric surgeries in infants younger than 12 months with HME and HCD. METHODS: A retrospective analysis included patients younger than 12 months who underwent functional hemispherectomy or hemispherotomy, between 2005 and 2024, at a national epilepsy surgery referral center. Data included demogra

Ventriculosubgaleal shunt placement as a temporary option for the management of postinfectious hydrocephalus: a single-arm meta-analysis.

OBJECTIVE: Postinfectious hydrocephalus (PIH) is a severe complication following CNS infections and often requires urgent CSF diversion. However, definitive shunt placement procedures are often unfeasible in this setting due to active infection or residual inflammatory debris. In such scenarios, ventriculosubgaleal shunt (VSGS) insertion has emerged as a widely used temporary strategy. METHODS: The authors conducted a systematic search of publications in the PubMed, Embase, and Cochrane Library databases, covering studies published up to June 2025. Risk of bias was assessed using Cochrane's R

Stereotactic radiosurgery for pineal gland metastases: results from the international radiosurgery research foundation.

PURPOSE: Metastases to the pineal gland are rare. Surgical excision can be associated with high rates of morbidity. Alternatively, stereotactic radiosurgery (SRS) for brain metastases in the pineal region has not been vigorously studied. METHODS: We performed a multi-institutional retrospective study for patients treated with SRS for pineal region metastases at treatment centers that comprise the International Radiosurgery Research Foundation (IRRF). Demographics, tumor characteristics, treatment parameters, and clinical outcomes were collected. The primary endpoint was local tumor control (LC

Incidental Diagnosis of Complete Agenesis of the Corpus Callosum in a 62-Year-Old Man Presenting With Dizziness.

Agenesis of the corpus callosum (ACC) is a congenital malformation most commonly identified in prenatal or early childhood imaging, with adult diagnoses being relatively rare. We present the case of a 62-year-old man with a history of intellectual disability who was incidentally found to have complete ACC during evaluation for dizziness and chest discomfort. Initial noncontrast computed tomography (CT) of the brain demonstrated ventricular enlargement concerning for hydrocephalus, prompting further evaluation. Subsequent magnetic resonance imaging (MRI) clarified the diagnosis, revealing class

Bilateral decompressive craniectomy in pediatric patients: A systematic review.

Traumatic brain injury (TBI) and other acute neurological insults remain leading causes of pediatric morbidity and mortality. In cases where elevated intracranial pressure (ICP) is refractory to maximal medical therapy, decompressive craniectomy (DC) may be considered as a salvage intervention. Although bilateral, bifrontal, bitemporal, and cruciate decompressive approaches are used in selected patients with diffuse cerebral edema or bihemispheric swelling, their role, timing, complications, and outcomes remain poorly defined in pediatric populations. Following PRISMA guidelines, PubMed, Scopu

Spontaneous Uterine Artery Rupture in Pregnancy, With Subsequent Severe Foetal Intracranial Haemorrhage.

Spontaneous rupture of the uterine artery during pregnancy is an exceptionally rare and life-threatening obstetric emergency. It often presents with sudden hemodynamic instability in the absence of external bleeding, posing diagnostic and management challenges. While maternal outcomes depend on timely intervention, foetal outcomes may be significantly affected by acute uteroplacental insufficiency. We report a case of a 23-year-old gravida 2 para 1+0 woman at 26 weeks of gestation who presented with sudden hypovolemic shock without external haemorrhage. A prior anomaly scan at 23 weeks was unr

Schinzel-Giedion Syndrome in One of Dizygotic Twins: Confirmation of a De Novo SET Binding Protein 1 (SETBP1) Variant and Classic Multisystem Phenotype.

Schinzel-Giedion syndrome (SGS) is a very rare congenital disorder linked to de novo pathogenic variants in the SET Binding Protein 1 (SETBP1) gene. The condition is usually suspected from a recognisable craniofacial pattern, severe developmental delay, early neurological problems, and congenital abnormalities involving several organ systems. As the disorder is uncommon and presents with wide systemic involvement, diagnosis can be missed or delayed unless the clinical features are carefully recognised and followed by appropriate genetic testing. We report the case of a two-year-old girl who wa

Quantitative CT Imaging Features Associated With High Risk of Elevated Intracranial Pressure in Children Under 2 Years of Age With Craniosynostosis: A Patient-Control Study.

OBJECTIVE: Elevated intracranial pressure (ICP) is an important complication of craniosynostosis, and early identification may help optimize surgical planning and treatment. Although invasive ICP monitoring remains the reference standard, its routine use in infants and young children is limited by procedural risks. This study aimed to evaluate whether quantitative computed tomography (CT) parameters could serve as noninvasive imaging markers associated with a high risk of elevated ICP in children under 2 years of age with craniosynostosis, compared with age- and sex-matched controls. METHODS:

ARMC2 loss impairs cilia structure and leads to primary ciliary dyskinesia symptoms in mouse organs.

In humans and mice, deficiency in ARMC2 causes Multiple Morphological Abnormalities of the Flagellum (MMAF), a condition defined by absent or aberrant sperm flagella with a disorganized axoneme. Affected men are infertile but with no other obvious signs characteristics/typical for primary cilia dyskinesia (PCD). Given the similarity between cilia and flagella axonemes we investigated a possible role of ARMC2 in cilia functioning. In Armc2-deficient mice, the length of cilia was reduced in trachea and oviduct. Ciliary beating was also affected, leading to tracheal mucus accumulation and female

Neurophysiological insights into iNPH: ABR and CAEP responses to lumbar puncture and shunt surgery.

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is a neurological syndrome resulting from increased cerebrospinal fluid volume without significant increase in intracranial pressure. The diagnosis is difficult and investigating other tools that can assist in this process is essential. Auditory Evoked Potentials (AEPs) can be used to assess the auditory pathway integrity and monitor cognitive auditory functions. The objective of this study was to investigate the applicability of the AEPs as a potential monitoring tool for detecting clinical improvements associated with cerebrospinal

Associations of DTI Metrics with Structural and Cerebrovascular Disease Imaging Metrics in Idiopathic Normal Pressure Hydrocephalus.

BACKGROUND AND PURPOSE: Idiopathic normal pressure hydrocephalus (iNPH) symptoms can be alleviated through shunt placement. However, the identification of suitable patients is often challenging despite several existing biomarkers. DTI is commonly used to assess white matter integrity, with DTI analysis along the perivascular space being a recently described though less extensively evaluated metric. It has been proposed to monitor glial-lymphatic (glymphatic) activity in the brain, the impairment of which may play a role in several neurologic diseases. The goal of this study is to evaluate the

The Impact of Programmable Valves on the Risk and Severity of Subdural Collections in Patients With Normal Pressure Hydrocephalus.

BACKGROUND AND OBJECTIVES: Normal pressure hydrocephalus (NPH) is characterized by the classic triad of cognitive decline, gait instability, and urinary incontinence in the setting of ventriculomegaly with normal intracranial pressure. Cerebrospinal fluid diversion is the current standard treatment, yet it carries a risk of overdrainage, resulting in subdural hematoma or hygroma. Different valves have been developed to mitigate this risk, yet consensus remains unclear regarding optimal valve for NPH. METHODS: We performed a retrospective cohort study on all patients with NPH who underwent cer
