Adaptation of the Shokunbi et al. modified Golgi-Cox for assessing dendritic injury in the hypothalamus: A protocol for hydrocephalus research.

BACKGROUND: Dendritic injury in the hypothalamic paraventricular nucleus (PVN) contributes to neuroendocrine dysfunction in hydrocephalus, but reliable protocols for visualizing dendritic architecture in this region are limited. Standard Golgi-Cox methods often yield inconsistent impregnation in the small, periventricular hypothalamus of juvenile rats. NEW METHOD: We adapted the modified Golgi-Cox protocol developed at the Anatomy Laboratory, University of Ibadan, by Shokunbi et al., for the hypothalamus of juvenile rats with kaolin-induced hydrocephalus. Key modifications include reduction o

The Impact of Dural Venous Sinus Compromise on the Development of Hydrocephalus in Children and Young Adults: A Retrospective Study.

BACKGROUND: The dural venous sinus system (DVSS) plays a central role in cerebral venous drainage and cerebrospinal fluid (CSF) reabsorption. Compromise of the DVSS is traditionally thought to predispose patients to elevated intracranial pressure and hydrocephalus. However, research evaluating this relationship is often scarce. OBJECTIVE: To determine whether compromise of the DVSS system, especially the superior sagittal sinus, is associated with an increased risk of hydrocephalus in pediatric and young adult patients. METHODS: We performed a retrospective cohort study (n=62) of pediatric a

Refractory cavernous sinus thrombophlebitis complicated by brain abscess and infectious hydrocephalus: a case report and literature review.

BACKGROUND: Cavernous sinus thrombophlebitis (CST) is a rare, life-threatening infection. Its management becomes exceptionally challenging when complicated by intracranial abscesses, hydrocephalus, and multidrug-resistant pathogens such as methicillin-resistant Staphylococcus aureus (MRSA). CASE PRESENTATION: A 52-year-old woman with poorly controlled diabetes presented with headache, eye pain, diplopia, fever, and progressive ocular symptoms. Outpatient ceftriaxone and subsequent ertapenem were ineffective. Neuroimaging confirmed CST with early brain abscess formation. Despite systemic antimi

Decompressive Craniectomy: Surgical Techniques and Complication Avoidance.

Decompressive craniectomy (DC) is a procedure commonly employed to relieve medically-refractory intracranial hypertension by removing a portion of the skull, thereby allowing for cerebral expansion and reducing the risk of internal herniation and secondary brain injury. This intervention is indicated in patients with severe traumatic brain injury, malignant middle cerebral artery infarction, or other pathologies resulting in elevated intracranial pressure. Although the procedure can be life-saving, it carries risks including hemorrhage, infection, and postoperative hydrocephalus. This review p

European Stroke Organisation (ESO), European Association of Neurosurgical Societies (EANS) and European Society for Minimally Invasive Neurological Therapy (ESMINT) guideline on aneurysmal subarachnoid haemorrhage.

Aneurysmal subarachnoid haemorrhage (aSAH) results from the rupture of an intracranial aneurysm. The case-fatality after aSAH is approximately 40% and those who survive often have functional, cognitive or emotional sequelae. We prepared guidelines according to Grading of Recommendations, Assessment, Development and Evaluations (GRADE) methodology, using data from meta-analyses, randomised trials, prospective observational and case-control studies, prospective registries with external validation and single-arm cohort studies with > 50 patients with aSAH. Based on high levels of evidence, we rec

The impact of human brain geometry on the transport of an intrathecal tracer.

BACKGROUND: Intrathecal contrast-enhanced magnetic resonance imaging (MRI), utilizing the contrast agent gadobutrol as cerebrospinal fluid (CSF) tracer is emerging as a useful method to study glymphatic function in the human brain. A consistent finding with this technique is large inter-individual variability regarding tracer propagation. In this study, we outline an approach which predicts the distribution of tracer in the parenchyma based only on geometric information from brain tissue as captured by MRI, addressing one possible explanation for this variability. METHODS: Registrations are co

Optimising Neurosonography in the NICU: An Audit of 130 Cases and Proposal of a Referral and Reporting Protocol.

Introduction Neurosonography is a first-line, safe bedside modality for evaluating neonates at risk of intracranial pathology. International guidelines recommend routine screening in every preterm infant and targeted scans in neonates with systemic risk factors. However, in practice, it is frequently both underutilised (missed scans in at-risk neonates) and overutilised (repeated scans in low-risk infants without clinical indication). This results in either delayed detection of critical conditions or unnecessary imaging with minimal clinical yield. We conducted an audit of 130 neonatal neuroso

When Gastroenteritis Struck the Brain: Post-infectious Parkinsonism Suggestive of Neuropsychiatric Systemic Lupus Erythematosus in a Patient With Aqueductal Stenosis.

Parkinsonism is a rare neuropsychiatric manifestation of systemic lupus erythematosus (SLE), the pathogenesis of which has been proposed to involve immune-mediated vasculopathy. Gastrointestinal infections may exacerbate autoimmune responses, triggering disease flares. We present a 26-year-old woman who developed acute parkinsonism with mutism, oromandibular dystonia, and horizontal square wave jerks following a diarrheal illness. Neurological examination revealed cogwheel rigidity and dysphagia. Brain MRI showed periventricular hyperintensities, ventricular dilation, and aqueductal stenosis w

Radiologic Characteristics and Clinical Correlates of Quadrigeminal Cistern Arachnoid Cysts: A Retrospective Study.

Introduction Quadrigeminal cistern arachnoid cysts (QACs) represent a rare subgroup of arachnoid cysts. Patients may present with headache, visual disturbances, gait imbalance, or nausea, typically secondary to obstructive hydrocephalus. Hydrocephalus is frequently assessed using the Evans Index (EI). Vertebral artery hypoplasia (VAH) is generally defined as a decrease in the diameter of the vertebral artery. It has been associated with posterior circulation ischemia, vertebrobasilar insufficiency (VBI), vertigo, and headache. However, the relationship between QAC-related mass effect and vascu

Novel endoscopic posterior fossa cyst fenestration via an occipital meningocele defect in neonatal Dandy-Walker malformation: illustrative case.

BACKGROUND: Dandy-Walker malformation (DWM) is characterized by cerebellar vermis hypoplasia, absence of the lateral apertures (of Luschka) and the median aperture (of Magendie), cystic dilation of the fourth ventricle, and posterior fossa enlargement. Occipital meningoceles (OMCs) involve an occipital skull defect with herniation of the meninges and CSF. Fewer than 40 cases of concurrent OMC and DWM (DWM-OMC) have been reported in the literature, with none managed with transmeningocele endoscopic fenestration. OBSERVATIONS: A preterm female neonate, delivered via cesarean section, presented

Pediatric craniopharyngioma: when hypoglycemia reveals a brain tumor. Illustrative case.

BACKGROUND: Craniopharyngiomas (CPs) usually manifest with symptoms of intracranial hypertension secondary to obstructive hydrocephalus, visual disturbances, or growth failure. Hypoglycemia as the sentinel presentation is exceptionally rare and reflects hypothalamic-pituitary dysfunction. OBSERVATIONS: A previously healthy 4-year-old girl presented with progressive headache and vomiting for 4 days, followed by coma (Glasgow Coma Scale score 6). Emergency testing revealed profound hypoglycemia (capillary glucose 40 mg/dL [2.2 mmol/L]); consciousness returned rapidly after an intravenous dextro

Transvenous Repositioning of a Distal Ventriculoatrial Shunt Catheter Terminating in the Coronary Sinus in a Pediatric Patient With a Duplicated Superior Vena Cava.

Ventriculoatrial shunts (VAS) are an alternative to ventriculoperitoneal shunts (VPS) for cerebrospinal fluid (CSF) diversion in patients with peritoneal contraindications. Although rare, complications such as catheter migration or malposition can occur and may be exacerbated by underlying congenital venous anomalies. Endovascular techniques have emerged as a minimally invasive option for VAS revision, yet reports remain limited and primarily focus on acquired malpositions. To our knowledge, no prior case describes endovascular VAS repositioning in the context of a duplicated superior vena cav

Evaluation of an artificial intelligence model for the identification of obstructive hydrocephalus on computed tomography of the head.

OBJECTIVE: Obstructive hydrocephalus is a critical radiographic finding requiring emergent treatment. Its identification on head CT by an AI model could facilitate sooner life-saving interventions, although there are common co-occurring findings, including intracranial hemorrhage, that can confound this interpretation. This external validation assessed the accuracy of an AI model at identifying obstructive hydrocephalus, including in the presence or absence of other findings. MATERIALS AND METHODS: This retrospective cohort included 177 thin (≤ 1.5 mm) series and 194 thick (> 1.5 and ≤ 5 mm)

Ventriculo-gallbladder shunt reflux: A case study and review of the literature.

BACKGROUND: Ventriculo-gallbladder (VGB) shunt represents an uncommon yet well-documented surgical intervention for the treatment of hydrocephalus through cerebrospinal fluid (CSF) diversion into the gastrointestinal tract through the gallbladder. When other ventricular shunt types have failed or are contraindicated, VGB shunts are effective. In exceptionally rare circumstances, biliary reflux can occur and cause accumulation of bile in the ventricular system. The authors report a case of bile reflux through a ventricular gallbladder shunt with eventual patient recovery after surgical interven

Bilateral Blindness in a Patient With Vestibular Schwannoma and Empty Sella Syndrome: A Case Report.

We report a rare clinical presentation of a T4b vestibular schwannoma causing severe compression of the brainstem and fourth ventricle, associated with obstructive hydrocephalus and empty sella syndrome, in a patient presenting with profound neurological and ophthalmological deficits, most notably complete bilateral blindness. Radical microsurgical resection was achieved via a lateral suboccipital (retromastoid) approach, resulting in gross-total tumor removal with preservation of most cranial nerves. Postoperatively, the patient developed House-Brackmann grade III facial palsy. Hydrocephalus

Topographical Patterns of Intraventricular Hemorrhage as a Novel Prognostic Imaging Biomarker in Intracerebral Hemorrhage.

AIMS: Current intraventricular hemorrhage (IVH) scoring systems quantify individual hemorrhage site features but do not account for differences in overall IVH distribution. We aimed to develop a scoring system based on overall IVH topographical patterns, and evaluate its value for predicting outcomes in intracerebral hemorrhage (ICH). METHODS: We conducted a retrospective analysis of a database including 385 patients with spontaneous ICH, IVH, and hematoma volumes < 40 mL between June 2019 and October 2024. We identified independent predictors of 30-day mortality, evaluated whether IVH spatia

Total Mastectomy Preserving a Ventriculoperitoneal Shunt on the Affected Side: A Case Report.

Ventriculoperitoneal (VP) shunts are the standard treatment for hydrocephalus, and the distal catheter is often routed subcutaneously across the anterior chest wall. The development of ipsilateral breast cancer is rare, with fewer than 15 cases reported in the English literature, and there is no established consensus regarding the management of the shunt catheter during mastectomy. We report a 58-year-old shunt-dependent woman with a right-sided VP shunt placed for post-subarachnoid hemorrhage hydrocephalus, who was diagnosed with right breast cancer (cT2N1M0, stage IIB). Preoperative imaging

Complication profile of two surgical techniques, neurosurgical experience, and operative setting for pediatric external ventricular drainage implantation.

PURPOSE: External ventricular drain (EVD) insertion, a standard procedure for treating acute hydrocephalus, is also associated with significant risks. This retrospective study compares two surgical techniques routinely employed at our clinic and analyzes the influence of surgeon experience and the operative setting on complication rates in a pediatric population. METHODS: The study population was divided into two cohorts based on the surgical procedures used to access the ventricular system: the BoltKit system (BS) or drain tunnelling (DT). Statistical analysis balanced age, sex, site and sid

Disproportionately elevated sulcal index (DESI): An automatically driven index representing disproportionate subarachnoid space enlargement in brain MRI scans.

INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) is frequently underdiagnosed due to non-specific symptoms and the risks of invasive testing. While disproportionately enlarged subarachnoid space hydrocephalus (DESH) is a hallmark imaging feature, manual assessment is subjective and qualitative. We developed the Disproportionately Elevated Sulcal Index (DESI), a fully automated deep learning-based volumetric biomarker, to objectively quantify these morphological changes. METHODS: We trained a U-Net model with an EfficientNet-B0 encoder on T1-weighted MRI scans from the Baltimore L
