Choroid plexus remodeling associated with inflammatory CSF exposure in preterm acquired hydrocephalus.

Acquired hydrocephalus in preterm infants follows intraventricular hemorrhage (IVH) or central nervous system infection and is associated with inflammatory cerebrospinal fluid (CSF) profiles. We integrated clinical magnetic resonance imaging (MRI), CSF cytokine profiling, primary human choroid plexus (ChP) epithelial and endothelial cell assays, and independent transcriptomic and proteomic datasets to investigate ChP remodeling. Infants requiring CSF diversion exhibited greater ChP thickness independent of gestational age, ventricular size, and IVH severity, supporting its potential as a bioma

Outcomes in Patients with Tuberculous Meningitis: A Prospective Cohort Study in North India.

Tuberculous meningitis (TBM) is the most severe form of extrapulmonary tuberculosis, carrying the highest mortality among all forms. This multicentric prospective cohort study conducted in North India aimed to determine the incidence of complications, functional outcome (modified Rankin scale [mRS]), and mortality in TBM. Patients 12 years old and older with TBM were followed up until completion of 9 months of antitubercular treatment. Patients with concomitant HIV infection, other infections causing meningitis, or multidrug-resistant tuberculosis were excluded. The primary outcome was mRS at

Transcranial doppler ultrasonography in normal pressure hydrocephalus: a systematic review and exploratory meta-analysis of diagnostic, prognostic, and monitoring roles.

Transcranial Doppler (TCD) ultrasonography provides noninvasive measurements of cerebral hemodynamics, but its clinical role in normal pressure hydrocephalus (NPH) remains uncertain. We reviewed its diagnostic, prognostic, and monitoring applications. Five databases were searched through June 10, 2025 for quantitative TCD studies in adults with suspected or confirmed NPH. Study-level eligibility and denominators were reconciled against full reports. Design-specific critical appraisal distinguished physiological association, treatment response, and diagnostic discrimination. Exploratory meta-an

Association of Disproportionately Enlarged Subarachnoid Space Hydrocephalus Features at MRI with Cognitive Decline.

Background The prevalence of incomplete disproportionately enlarged subarachnoid space hydrocephalus (DESH) and its cognitive relevance remain unclear. Purpose To determine the prevalence of complete and incomplete DESH and their associations with baseline cognitive impairment and follow-up cognitive decline. Materials and Methods This prospective community-based cohort (age ≥ 65 years) included participants who underwent baseline brain MRI and were followed for 5-7 years. Two radiologists assessed baseline MRI scans for core DESH features: Evans index greater than 0.3, enlargement of the Sylv

CSF drainage strategy and complication management of hydrocephalus.

BACKGROUND AND OBJECTIVES: Conventional CSF drainage strategies-using external ventricular drainage (EVD) for acute hydrocephalus and permanent shunts (ventriculoperitoneal [VP], ventriculoatrial [VA], lumboperitoneal [LP]) for chronic hydrocephalus-are often inadequate in complex clinical scenarios (e.g., patients with intracranial hemorrhage, active infection, or unstable conditions who cannot undergo permanent shunt surgery). This creates an urgent need to comprehensively review drainage modalities to address the core challenge of reducing surgical frequency while effectively controlling in

Novel Use of an Intravenous Flow Regulator for Controlled Cerebrospinal Fluid Drainage in External Ventricular Drains: A Prospective Single-Center Pilot Study of 300 Patients.

BACKGROUND: Accurate control of cerebrospinal fluid (CSF) drainage through external ventricular drains (EVDs) is essential in neurosurgical intensive care. Conventional systems rely on hydrostatic level adjustment, which is operator-dependent and imprecise. OBJECTIVE: This article evaluates the safety, feasibility, and performance of a simple intravenous flow regulator, commonly used in infusion sets, as a cost-effective means of controlling CSF drainage in EVDs across all clinical indications. MATERIALS AND METHODS: A prospective observational study was conducted in 300 patients requiring E

Ventriculoperitoneal Shunt Overdrainage Has Transient and Steady-State Components Strongly Dependent on Cerebroventricular Compliance: A Benchtop Experimental Study.

BACKGROUND AND OBJECTIVES: Using benchtop testing, we previously demonstrated that ventriculoperitoneal shunts are susceptible to chronic overdrainage, not only because of a siphoning effect but also because of intracranial pressure pulsations. These pulsations may be rhythmic (cardiac-related) or sporadic (Valsalva maneuvers) and can drive excess cerebrospinal fluid (CSF) drainage at both horizontal and vertical positions, an effect that is strongly dependent on system compliance. The objective was to characterize the temporal dynamics of CSF flow through a differential pressure valve across

Ventriculolumbar Irrigation Effects on Delayed Cerebral Infarction and Outcome After Aneurysmal Subarachnoid Hemorrhage: A Prospective Registry Study.

BACKGROUND AND OBJECTIVES: Aneurysmal subarachnoid hemorrhage (aSAH) remains associated with high morbidity and mortality, largely determined by delayed cerebral ischemia (DCI). Active blood clearance and vasodilation (ABCD) therapy targets key pathophysiological mechanisms of DCI by enhancing the clearance of blood from cerebrospinal fluid and counteracts vasospasm through continuous intrathecal fibrinolytic and spasmolytic irrigation. Although ABCD therapy has previously been implemented using technically demanding treatment approaches, this study introduces ventriculolumbar irrigation (VLI)

Successful treatment of a patient with ventriculo-peritoneal shunt-associated meningitis caused by Streptococcus pyogenes.

Ventriculoperitoneal shunt (VPS)-associated bacterial meningitis is a significant neurosurgical complication that can occasionally present atypical symptoms. We report a rare case of Streptococcus pyogenes meningitis in a 40-day-old infant with a history of hydrocephalus managed by VPS placement. The patient presented with vomiting, diarrhea, fever, and -abdominal distension. Microbial culture of the removed VPS valve confirmed the diagnosis, revealing Streptococcus pyogenes. Despite the strain's sensitivity to penicillin G, the patient was treated with cefotaxime and vancomycin due to negativ

Multiple Neurological Complications and Multidrug-Resistant Bacterial Infections During Prolonged Intensive Care for Undocumented Meningoencephalitis in Côte d'Ivoire: A Case Report.

Acute meningoencephalitis requiring intensive care may be complicated by severe neurological injury and nosocomial infections. Hydrocephalus is well recognized, whereas spinal cord ischemia, central diabetes insipidus, and flaccid tetraparesis are rarely reported. Their coexistence, together with successive infections involving multidrug-resistant organisms, presents substantial diagnostic and therapeutic challenges. A previously healthy 32-year-old man presented with neck and back pain following a road traffic accident, subsequently developing cerebellar signs, acute confusion, and meningeal

Prospective Randomized Controlled Study Comparing Decompressive Craniectomy with Watertight Duraplasty and without Watertight Duraplasty in Traumatic Brain Injury.

BACKGROUND: Decompressive craniectomy (DC) is a life-saving procedure for refractory intracranial hypertension following traumatic brain injury (TBI). Despite its widespread use, there is no consensus regarding optimal dural management. Whether watertight duraplasty is necessary during DC remains controversial. OBJECTIVE: The aim of the study is to compare clinical outcomes, postoperative complications, and cranioplasty-related outcomes following DC performed with and without watertight duraplasty in patients with TBI. MATERIALS AND METHODS: This prospective randomized controlled study inclu

Distinct predictors of patient quality of life and care partner burden in idiopathic normal pressure hydrocephalus: the role of neuropsychiatric symptoms.

Idiopathic normal pressure hydrocephalus (iNPH) is characterized by ventriculomegaly with gait, urinary, and cognitive dysfunction. It can also manifest with neuropsychiatric symptoms. We investigated predictors of patient quality of life (QoL) and care partner burden in patients with suspected iNPH. Patients (N = 186, Mage=74.73 ± 8.36 years, Meducation=14.50 ± 3.44, 38.7% female, 96.2% non-Hispanic white) self-reported their mobility, urinary and neuropsychiatric symptoms (anxiety, depression, apathy), and QoL. Care partners reported on emotional burden. Global cognition was measured with Mi

Radiological DESH score cannot predict outcome after shunt surgery: A Swedish prospective follow-up study.

BackgroundDisproportionately enlarged subarachnoid-space hydrocephalus (DESH) has been proposed as a radiological predictor of shunt responsiveness in idiopathic normal pressure hydrocephalus (iNPH), but previous studies have reported inconsistent results. We investigated the association between preoperative DESH score and postoperative outcome after shunt surgery in a prospective iNPH cohort.MethodsThis prospective study included 101 consecutive patients with iNPH from the Gothenburg Prediction of Outcome in iNPH (POiNT) study. Preoperative DESH scores were assessed on MRI using previously pu

Clinical and biomarker predictors of α-synuclein seeding in cognitive impairment and motor parkinsonism.

Dopamine transporter (DAT) imaging and α-synuclein seed amplification assay (SAA) are biomarkers for Lewy body disease (LBD). We aimed to identify clinical and biomarker factors associated with SAA positivity in patients with cognitive impairment and frequent motor parkinsonism. Dual-phase DAT positron emission tomography (PET) scans, cerebrospinal fluid (CSF) phosphorylated-tau/β-amyloid ratio (PTAR) measurement, and SAA were performed in 108 patients, including 99 with motor parkinsonism. Diagnoses, which were not mutually exclusive, included normal pressure hydrocephalus (NPH, N = 60), Alzh

Papillary Tumor of the Pineal Region Presenting With Obstructive Hydrocephalus: A Diagnostic and Management Challenge.

We report a 45-year-old woman with a papillary tumor of the pineal region (PTPR), a neuroepithelial neoplasm of the pineal gland, who presented with a one-year history of headaches, visual disturbance, and gait impairment and was found to have a pineal mass with obstructive hydrocephalus. She underwent endoscopic third ventriculostomy (ETV) with biopsy; the initial radiologic impression favored pineocytoma. Immunohistochemistry was nondiagnostic on the limited sample, and the diagnosis of PTPR was established only after review at two academic neuropathology centers. DNA methylation profiling w
