INTRODUCTION: Idiopathic normal pressure hydrocephalus (iNPH) is a progressive neurological disorder characterized by Hakim's triad: cognitive decline, gait disturbance, and urinary urgency or incontinence, accompanied by ventriculomegaly and normal cerebrospinal fluid (CSF) pressure. Ventriculoperitoneal shunting (VPS) is the standard treatment, with outcomes varying depending on the type of valve utilized. In this study, we compared the impact of programmable versus fixed-pressure valves on treatment outcomes.
METHODS: A retrospective analysis was conducted on patients with iNPH treated at

BACKGROUND: Despite decades of technological development, cerebrospinal fluid (CSF) shunts remain associated with high failure rates and limited structural evolution. While bibliometric analyses have characterized innovation trends across several neurosurgical subspecialties, no dedicated patent analysis of CSF shunt technology exists. This study characterized the evolution of CSF shunt-related intellectual property (IP) and identified features associated with influential innovation.
METHODS: A retrospective patent bibliometric analysis was performed using the Lens database. Granted patents r

Apathy is a highly prevalent neuropsychiatric feature of normal pressure hydrocephalus (NPH), but its clinical correlates and relevance to cerebrospinal fluid (CSF) removal trial response are poorly understood. We evaluated 161 adults with possible or probable NPH whose informants completed the Frontal Systems Behavior Scale (FrSBe), which provides normative ratings for premorbid (i.e. before symptom onset) and current apathy. Apathy change was defined as current minus premorbid ratings of apathy. Among the sample, 69.6% showed clinically elevated apathy (T-score ≥ 65) compared to 18.0% premor

Intracranial pressure (ICP) is a key physiological parameter for the diagnosis and management of traumatic brain injury, hydrocephalus, stroke, and other neurological disorders. ICP sensors are typically required to provide accurate pressure measurement at low pressure while maintaining long-term stability in biofluid environments. Although conventional liquid-based ICP systems remain the clinical standard, their bulky and tethered configurations increase the risk of infection and tissue injury and restrict patient mobility. Advances in miniaturized implantable sensors have created new opportu

OBJECTIVE: To evaluate the association between untreated admission serum potassium levels and quantitative responsiveness to external lumbar drainage (ELD) in patients with hydrocephalus.
METHODS: This single-center retrospective study included 349 unique patients with hydrocephalus after reconciliation and deduplication of clinical records collected between 2020 and 2026. Serum potassium was defined as the first measurement obtained at hospital admission before treatment. ELD responsiveness was defined as an improvement of ≥10% in Timed Up and Go or 10-m walking time, ≥3 points in the Mini-M

In recent years, the role of mechanosensation in lymphatic system development, functional homeostasis, and disease progression has received increasing attention. As key mechanosensitive receptors in lymphatic endothelial cells, Piezo1/2 ion channels respond to biomechanical stimuli such as fluid shear stress and interstitial pressure, thereby regulating calcium influx and downstream signaling pathways. These channels play pivotal roles in physiological processes, including lymphatic network formation, valve development, and the maintenance of meningeal lymphatic function. This review systemati

INTRODUCTION: Endoscopic third ventriculostomy (ETV) has been proposed for selected patients with idiopathic normal pressure hydrocephalus (iNPH), but predictors of sustained benefit are uncertain. Phase-contrast MRI (PC-MRI) may quantify cerebrospinal fluid (CSF) pulsatility and support patient selection.
RESEARCH QUESTION: Is sustained clinical improvement after primary ETV associated with preoperative aqueductal CSF flow parameters measured by PC-MRI?
MATERIAL AND METHODS: In this prospective single-centre observational study, patients with iNPH underwent primary ETV after standardized di

STUDY OBJECTIVE: Hydrocephalus is a rare, potentially reversible cause of gonadotropic dysfunction, amenorrhoea, and delayed puberty; fewer than 60 cases have been reported. This scoping review aimed to characterize hydrocephalus-associated amenorrhoea, assess time to postoperative menstrual (re)occurrence, and identify factors influencing recovery.
METHODS: This scoping review followed Joanna Briggs Institute methodology, with a systematic search of PubMed, Embase, Google Scholar, and Web of Science for cases of amenorrhoea associated with hydrocephalus. Two reviewers independently screened

BACKGROUND: Spinal cord stimulation (SCS) is a treatment approved for the management of chronic pain refractory to conservative medical treatment. Ventriculoperitoneal (VP) shunts can pose unique challenges during spinal procedures. This case highlights the careful considerations when evaluating patients with VP shunts for SCS and contributes to the growing body of evidence supporting the safe use of neuromodulation therapies in this medically complex population.
CASE REPORT: A 58-year-old woman with communicating hydrocephalus status post VP shunt placement presented with refractory lower ba

Acquired hydrocephalus in preterm infants follows intraventricular hemorrhage (IVH) or central nervous system infection and is associated with inflammatory cerebrospinal fluid (CSF) profiles. We integrated clinical magnetic resonance imaging (MRI), CSF cytokine profiling, primary human choroid plexus (ChP) epithelial and endothelial cell assays, and independent transcriptomic and proteomic datasets to investigate ChP remodeling. Infants requiring CSF diversion exhibited greater ChP thickness independent of gestational age, ventricular size, and IVH severity, supporting its potential as a bioma

Tuberculous meningitis (TBM) is the most severe form of extrapulmonary tuberculosis, carrying the highest mortality among all forms. This multicentric prospective cohort study conducted in North India aimed to determine the incidence of complications, functional outcome (modified Rankin scale [mRS]), and mortality in TBM. Patients 12 years old and older with TBM were followed up until completion of 9 months of antitubercular treatment. Patients with concomitant HIV infection, other infections causing meningitis, or multidrug-resistant tuberculosis were excluded. The primary outcome was mRS at

Transcranial Doppler (TCD) ultrasonography provides noninvasive measurements of cerebral hemodynamics, but its clinical role in normal pressure hydrocephalus (NPH) remains uncertain. We reviewed its diagnostic, prognostic, and monitoring applications. Five databases were searched through June 10, 2025 for quantitative TCD studies in adults with suspected or confirmed NPH. Study-level eligibility and denominators were reconciled against full reports. Design-specific critical appraisal distinguished physiological association, treatment response, and diagnostic discrimination. Exploratory meta-an

Background The prevalence of incomplete disproportionately enlarged subarachnoid space hydrocephalus (DESH) and its cognitive relevance remain unclear. Purpose To determine the prevalence of complete and incomplete DESH and their associations with baseline cognitive impairment and follow-up cognitive decline. Materials and Methods This prospective community-based cohort (age ≥ 65 years) included participants who underwent baseline brain MRI and were followed for 5-7 years. Two radiologists assessed baseline MRI scans for core DESH features: Evans index greater than 0.3, enlargement of the Sylv

BACKGROUND AND OBJECTIVES: Conventional CSF drainage strategies-using external ventricular drainage (EVD) for acute hydrocephalus and permanent shunts (ventriculoperitoneal [VP], ventriculoatrial [VA], lumboperitoneal [LP]) for chronic hydrocephalus-are often inadequate in complex clinical scenarios (e.g., patients with intracranial hemorrhage, active infection, or unstable conditions who cannot undergo permanent shunt surgery). This creates an urgent need to comprehensively review drainage modalities to address the core challenge of reducing surgical frequency while effectively controlling in

A 44-year-old man developed otogenic sigmoid sinus thrombosis with cerebellar abscess, hydrocephalus, and recurrent posterior fossa infection. Staged neurosurgical drainage and canal-wall-down mastoidectomy achieved source control. Cultures were negative. Anticoagulant exposure was perioperative/early postoperative. Despite persistent venous non-opacification, he was discharged improved, afebrile, without neurological deficit, and with remitted otoliquorrhea.
