BACKGROUND: The optimal management of vestibular schwannoma (VS) associated with hydrocephalus in octogenarians remains controversial. We report two patients aged 80 years or older with Koos grade IV VS who underwent surgical treatment.
CASE DESCRIPTION: Case 1 involved an 86-year-old woman who presented with progressive gait disturbance caused by a 3.5-cm cystic right VS associated with hydrocephalus. Case 2 involved an 81-year-old woman who presented with cognitive impairment and gait disturbance due to a 3.0-cm cystic left VS with hydrocephalus. In both patients, magnetic resonance imaging

OBJECTIVE: Chiari malformation type I (CM-I) is the most common structural hindbrain defect in humans and causes debilitating headaches and other neurological symptoms frequently requiring surgery. Although patients' brains have heterogeneous shapes, diagnosis is based on a single radiographic measure with only 1 available pathology-directed treatment. The authors sought to determine whether there are distinct morphological subtypes of CM-I and to determine whether patients with different subtypes differ in clinical response to neurosurgical decompression.
METHODS: For this cohort study, 186

BACKGROUND: Despite successful CSF diversion for pediatric obstructive hydrocephalus, up to 40% of children in Sub-Saharan Africa develop persistent neurodevelopmental delay, growth arrest, and executive dysfunction. Models based on axonal injury or ischemia alone do not fully explain this burden.
HYPOTHESIS: We propose the Hypothalamic Compression-Endocrine-Glymphatic Dysfunction Theory. Third ventricular expansion compresses and shears periventricular hypothalamic neurons, particularly TRH-secreting cells of the paraventricular nucleus. This disrupts pulsatile TRH release and impairs TSH gl

Congenital hydrocephalus is a heterogeneous neurodevelopmental disorder in which ventricular enlargement can arise from impaired cerebrospinal fluid (CSF) flow, aqueductal obstruction, or altered ventricular wall integrity. Although human CRB2 mutations are associated with hydrocephalus, aqueductal abnormalities, and periventricular heterotopia, the relevant cell types and pathogenic sequence remain poorly defined. Here, using lineage-specific Crb1/2 conditional mouse models, we show that Crb2-dependent polarity and adhesion in neural progenitors are required to preserve ventricular system int

Hydrocephalus following intraventricular hemorrhage (IVH) remains a serious neurological condition with limited treatment options beyond invasive surgical interventions, underscoring the urgent need to elucidate its underlying mechanisms. Emerging evidence implicates impaired glymphatic system function in the pathogenesis of hydrocephalus, particularly through dysregulation of aquaporin-4 (AQP4) expression in astrocytes, which is essential for cerebral waste clearance and fluid homeostasis. Since mast cells are key mediators of neuroinflammation and can influence cerebrospinal fluid (CSF) dyna

Cytomegalovirus (CMV) infection in an immunocompetent adult is rarely symptomatic. Neurological manifestations are uncommon and are predominantly characterized by meningitis, myelitis, and acute polyradiculoneuritis. We report a rare case of an immunocompetent patient who presented with prolonged headaches complicated by recurrent posterior circulation ischemic strokes associated with intracranial stenoses of the basilar trunk and posterior cerebral arteries. Analysis of his cerebrospinal fluid (CSF) revealed lymphocytic meningitis with moderate elevation of the CSF protein level, normal CSF g

Normal pressure hydrocephalus (NPH) is a potentially treatable neurological condition that may be underdiagnosed. We conducted a qualitative study with aims to (1) explore the breadth and impact of symptoms from onset and (2) identify barriers and facilitators to obtaining care. We conducted semi-structured interviews with people who had been diagnosed and treated for NPH and their family members and analysed these using reflexive thematic analysis, with input from people with lived experience. From 14 interviews, we identified four symptomatic themes: (1) loss and return of self with treatmen

Acute aphasia requires urgent evaluation for stroke, but recurrent and stereotyped language disturbances may also reflect focal seizures. We report the case of a 23-year-old man brought to the emergency department (ED) with aphasia that had been present for approximately six hours. Over the preceding week, he had experienced several similar but briefer episodes that had resolved spontaneously. He was concurrently undergoing investigation at another institution for persistent hiccups and near-daily vomiting of more than one month's duration. Chest computed tomography (CT) and positron emission

PURPOSE: This study's objective was to investigate barometric pressure changes that are most strongly associated with symptom exacerbations in shunted hydrocephalus patients. To achieve this, we conducted in-depth analysis of hospital encounter data and examined their correlation with variations in sea level pressure (SLP). Ultimately, our findings may inform hydrocephalus management strategies by identifying potential environmental triggers, reducing unnecessary hospital visits, and guiding the future design of CSF diversion technologies that are more responsive to external conditions.

Suprasellar arachnoid cysts are uncommon pediatric lesions that may cause obstructive hydrocephalus and optic pathway compromise, often with insidious clinical progression. We report an 11-year-old boy who presented with a 7-year history of progressive gait disturbance and imbalance. Neurological examination was largely normal, and visual acuity was preserved; however, fundoscopic examination revealed bilateral optic disc pallor. Brain magnetic resonance imaging demonstrated a giant cerebrospinal fluid-intensity cyst centered in the third ventricle with inferior extension into the suprasellar

BACKGROUND: Data on ceftaroline cerebrospinal fluid (CSF) levels in children are limited. This study examined ceftaroline diffusion into CSF among children with ventriculoperitoneal shunt (VPS)-related ventriculitis and external ventricular drains.
METHODS: Patients received a single dose of 15 mg/kg of iv ceftaroline (maximum 600 mg) over 2 hours. Simultaneous plasma and CSF samples were obtained at 3 time points to measure ceftaroline concentration: end of infusion (0 hours), and 2 and 6 hours postinfusion. The areas under the curve (AUCs) for plasma and CSF were calculated using linear int

AIM: To present cases of spontaneous cerebrospinal fluid (CSF) rhinorrhea and otorrhea, and to conduct a systematic literature review examining the relationship between benign tumor location and the leak side. MATERIAL and METHODS: A PRISMA-based systematic review was conducted to evaluate studies on spontaneous CSF leaks caused by benign intracranial tumors, alongside three newly reported cases. RESULTS: All three cases exhibited CSF leaks ipsilateral to the tumor: Case 1 involved a pituitary ACTH-secreting macroadenoma with rhinorrhea; Case 2, a pineocytoma with hydrocephalus and nasal encep

Patients with primary or metastatic brain tumours can deteriorate abruptly, and the acute physician is often the first to respond. This review covers the recognition and initial management of the neurological emergencies most likely to need critical care: raised intracranial pressure from peritumoural oedema and herniation; seizures and status epilepticus, including non-convulsive status epilepticus (NCSE); intracranial haemorrhage; acute hydrocephalus; and venous thromboembolism. It also covers two treatment-related emergencies that the general physician must now recognise: immune effector ce

BACKGROUND AND OBJECTIVES: Most patients with aneurysmal subarachnoid hemorrhage (aSAH) initially present to local/low-volume facilities and require interhospital transfer (IHT) for neurovascular care. We aimed to evaluate the transfer interval as a vulnerable phase in aSAH care, focusing on in-transit neurological and radiological changes, hydrocephalus development, and process delays.
METHODS: This retrospective cohort study included adults with aSAH treated at our tertiary care center between 2010 and 2023. Patients were categorized as direct admissions or IHT. Among IHT patients, neurolog

BACKGROUND: Giant basilar tip aneurysms remain difficult to treat due to their location, bifurcation anatomy, adjacent perforators, and potential for mass effect. Endovascular treatment, including stent-assisted coiling, is a valuable option. Despite angiographic occlusion, mass effect may still be clinically significant.
CASE DESCRIPTION: A 67-year-old man presented with headache and was diagnosed with a 27 × 17-mm giant unruptured basilar tip aneurysm. Preoperative angiography demonstrated perforators arising predominantly from the left P1 segment and a well-developed right posterior commun

Obstructive hydrocephalus is a serious and potentially life-threatening condition caused by the temporary blockage of cerebrospinal fluid (CSF) flow. Most cases require urgent intervention as spontaneous resolution is very uncommon. We present the case and brain imaging of a 63-year-old man who presented to the emergency department with complaint of headache, nausea, confusion, and dysarthria. Initial imaging showed a left caudate bleed with intraventricular hemorrhage and mild hydrocephalus secondary to obstruction of CSF in the aqueduct of Sylvius. External ventricular drain (EVD) was deferr

OBJECTIVES: Coagulase-negative staphylococci (CoNS) commonly infect Ommaya reservoirs, but data on intraventricular (IVT) vancomycin in neonates are very limited. We report a very preterm infant with oxacillin-resistant Staphylococcus hominis (ORSH) ventriculitis who was successfully treated with IVT vancomycin after systemic antibiotic failure.
DESIGN: A female infant born at 29+2 weeks (birth weight 1,250 g) developed disseminated Candida albicans infection involving blood, central nervous system, eyes, and urinary tract. Progressive hydrocephalus required Ommaya reservoir placement at post

Intracranial dermoid cysts are rare, benign congenital lesions originating from ectodermal remnants. Though typically asymptomatic, their rupture, although infrequent, can lead to significant neurological complications, such as chemical meningitis, vasospasm, and hydrocephalus. Imaging with computed tomography (CT) and magnetic resonance imaging (MRI) plays a crucial role in diagnosis, providing characteristic features related to the high lipid content of these lesions. An 86-year-old female presented with a 3-day history of headache, nausea, vomiting, and facial cellulitis after an arthropod
