Domain-Specific Cognition-Sparing Stereotactic Radiosurgery: A Novel Planning Approach for Functional Preservation.

PURPOSE: To introduce a novel cognition-sparing stereotactic radiosurgery (SRS) planning framework that limits dose to the cognitive connectome (10 bilateral white matter tracts, the corpus callosum, and the hippocampi) and to demonstrate its capability to preferentially spare the cognitive domains of memory, language, executive function, attention, and fine motor control. METHODS AND MATERIALS: Three cases representing the spectrum of intracranial disease treated with SRS (intact metastases, postoperative cavity, and arteriovenous malformation) and presenting competing domain-specific priori

Single-cell atlas of the developing Down syndrome brain cortex.

Down syndrome (DS), caused by trisomy of chromosome 21, is the leading genetic cause of intellectual disability, yet the mechanisms disrupting fetal brain development remain unclear. We performed single-cell transcriptomic and chromatin accessibility profiling of approximately 250,000 cells from 15 DS and 15 control human fetal cortices (10-20 weeks postconception). Our analysis revealed a subtype-specific reduction in RORB- and FOXP1-expressing excitatory neurons and widespread disruption of neurodevelopmental transcriptional programs. Chromosome 21 transcription factors BACH1, PKNOX1 and GAB

White matter abnormalities in metabolic syndrome patients with and without mild cognitive impairment: A diffusion tensor imaging study.

BACKGROUND: The presence of metabolic syndrome (MetS) constitutes a significant risk factor for the development of cognitive dysfunction and dementia. MetS can result in damage to the integrity and microstructure of white matter (WM). There may exist certain distinctions in WM between MetS patients with mild cognitive impairment (MCI) and MetS patients with preserved cognitive function. Using diffusion tensor imaging (DTI), we aim to investigate the alterations of the WM in MetS patients with and without MCI. METHODS: 36 MetS patients with MCI (MetS-MCI group), 30 age-, gender-, and education

Failure of endocytic flux in Donnai-Barrow syndrome caused by LRP2 p.C1400R.

Donnai-Barrow syndrome (DBS) arises from loss-of-function (LoF) variants in the endocytic receptor low-density lipoprotein receptor-related protein 2 (LRP2; or megalin) and is characterized by low-molecular weight proteinuria and developmental abnormalities. Urinary proteomics of 9 patients with DBS revealed that the urinary proteome of a DBS patient with the missense variant LRP2 p.C1400R was indistinguishable from that of patients with splice site, nonsense, or frameshift mutations. A CRISPR mouse model of the variant was generated to determine the mechanism of LoF and proteinuria. The mutan

Connexin-43 Restoration Alleviates Desmosomal Arrhythmogenic Cardiomyopathy.

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) is a fatal genetic heart disease primarily caused by mutations in desmosomal genes, leading to impaired cell-cell adhesion, ventricular arrhythmias, and progressive heart failure. Although gene therapy for specific ACM populations shows promise, it remains unclear whether mutation-agnostic pathways dysregulated across desmosomal mutations could be exploited for therapeutic intervention in this genetically broad and severe population. The reduction in expression of the ventricular gap junction protein Cx43 (connexin-43) is a common molecular alter

Distinct Role of γ-Synuclein in the Regulation of Motor Performance and Behavioral Responses in Mice.

Background: The three Synuclein family members (α-, β-, and γ-synuclein) are presynaptic proteins that regulate synaptic vesicle trafficking and thereby influence neurotransmitter release. Synucleins belong to a class of intrinsically disordered proteins and are prone to aggregation into pathological deposits, which may impair their physiological synaptic functions. Knockout (KO) mouse lines, commonly used to model synuclein depletion in the nervous system, reveal a range of phenotypes with different motor and behavioral deficits. However, given the high sequence homology and functional interp

Gestational age-specific DTI templates of the neonatal brain: Application in preterm developmental study.

Due to significant differences in brain volume, morphology, and white matter integrity among neonates of varying gestational ages, using a single full-term template for preterm analysis inevitably introduces analytical errors. To address this, we aimed to develop gestational-age-specific stereotaxic DTI templates using retrospective diffusion MRI scans from 161 neonates acquired between August 2021 and January 2024. The cohort was stratified into four WHO-defined subgroups: extremely preterm (n = 31), very preterm (n = 29), moderate to late preterm (n = 28), and full-term (n = 73). Templates w

A Novel PTEN Frameshift Variant in a Child With Autism Spectrum Disorder and Macrocephaly: A Case Report.

PTEN hamartoma tumor syndrome (PHTS) is a rare genetic condition associated with neurodevelopmental disorders, macrocephaly, and increased cancer risk. We report the case of a four-year-old girl with congenital hypothyroidism, progressive macrocephaly, and global developmental delay, later diagnosed with autism spectrum disorder (ASD). Brain MRI revealed megalencephaly with prominent extra-axial spaces and a diffusely thickened corpus callosum. Genetic testing identified a novel frameshift variant in the PTEN gene. This case highlights clinical findings that should raise suspicion for PHTS and

Clinical and Molecular Spectrum of Proximal 16p11.2 BP4-5 Copy Number Variations: Insights From a Pediatric Cohort.

Copy number variations (CNVs) at the proximal 16p11.2 BP4-BP5 locus are among the most well-characterized genomic regions associated with neurodevelopmental disorders. We aimed to define the clinical and molecular spectrum of these CNVs in a pediatric cohort. A total of 19 patients with pathogenic or likely pathogenic 16p11.2 BP4-BP5 CNVs were identified among 2200 individuals referred for chromosomal microarray analysis (prevalence: 1/116, 0.86%). Deletions were detected in 16/19 (84.2%) patients and duplications in 3/19 (15.8%). Developmental delay/intellectual disability was observed in 12/

The Role of the Hedgehog Pathway in Alcohol-Induced Birth Defects.

In this chapter, we review the current literature that focuses on the role of Sonic hedgehog (Shh) signaling, a major morphogenic pathway regulating embryonic development, in the pathology of fetal alcohol spectrum disorder (FASD), caused by prenatal alcohol exposure (PAE). Ethanol exposure disrupts Shh signaling during development across a range of species including in mice, chick, and zebrafish. Ethanol appears to interfere with Shh signaling through multiple mechanisms, both direct and indirect, including reduced Shh ligand production, impaired post-translational processing, and inducing ap

Identification of a diagnosis-selective neurobiological substrate for bipolar disorder, major depressive disorder, and schizophrenia: a meta-analysis of 57,717 subjects.

BACKGROUND: Neuroimaging studies have consistently revealed neuroanatomical abnormalities in individuals with bipolar disorder (BD), major depressive disorder (MDD), and schizophrenia (SZ). However, it remains unknown whether and to what extent disorder-selective gray matter variations occur in these prominent psychiatric disorders. This study conducted a meta-analysis of 25 years of published voxel-based morphometry (VBM) research to assess the presence of selective and robust neuroanatomical substrates of gray matter variation in BD, MDD, and SZ. METHODS: Peer-reviewed experiments encompass

Rare-variant aggregation highlights disease-linked genes associated with brain volume variation.

Rare brain disorders often present with changes in brain volume, and variation in brain volume is known to be highly heritable. Recent work studying brain volume variation has largely focused on common variants and structural variants. Rare variants often have large effect sizes and clearer connections to biological mechanisms, but the role of rare variants has not been extensively studied. We performed rare-variant gene aggregation analysis for total brain volume and 43 regional brain volume phenotypes (n = 50,061) to identify genes associated with brain volume variation through loss-of-funct

Pathogenic variants in the autophagy-tethering factor EPG5 drive neurodegeneration through mitochondrial dysfunction and innate immune activation.

The autophagy-tethering factor ectopic P-granule 5 autophagy protein (EPG5) plays a key role in autophagosome-lysosome fusion. Impaired autophagy associated with pathogenic variants in EPG5 causes a rare devastating multisystem disorder known as Vici syndrome, which features neurodevelopmental defects, severe progressive neurodegeneration and immunodeficiency. The pathophysiological mechanisms driving disease presentation and progression are only partially understood. In patient-derived fibroblasts and iPS cells differentiated to cortical neurons, we find that impaired mitophagy leads to mitoc

Mediodorsal and pulvinar thalamus reductions across the psychosis spectrum.

BACKGROUND: Abnormalities in thalamic structure and function are a consistent feature of psychosis spectrum disorders including schizophrenia (SZ), schizoaffective (SAD), and bipolar disorder with psychosis (BDP), yet the extent to which specific thalamic nuclei differ across diagnoses, and how these differences relate to cognition and psychopathology, remains unclear. We examined these questions, focusing on two nuclei - the mediodorsal (MD) and pulvinar (Pu), among individuals with psychosis (n = 2160) in the Bipolar-Schizophrenia Network on Intermediate Phenotypes consortium. METHODS: Whol

An Autosomal Dominant TUBB3 Mutation Associated With Congenital Fibrosis of the Extraocular Muscles Type 3 in an Iranian Family.

INTRODUCTION: Congenital fibrosis of the extraocular muscles type 3 (CFEOM3) is a congenital cranial dysinnervation disorder (CCDD) marked by variable ophthalmoplegia and ptosis with considerable phenotypic heterogeneity. We report a large multigenerational Iranian family with autosomal dominant CFEOM3. METHODS: Affected individuals underwent a comprehensive ophthalmologic evaluation. Whole-exome sequencing (WES) in the proband, followed by Sanger sequencing in 10 affected and 4 unaffected relatives, identified a heterozygous missense variant, c.784C>T (p.Arg262Cys), in exon 4 of TUBB3 (16q24

The Clinical Spectrum and Neurodevelopmental Pathogenesis of KPTN-Related Disorder in a Mouse Model.

OBJECTIVE: Pathogenic variants in Kaptin (KPTN) cause KPTN-related disorder (KRD). KPTN modulates mTOR signaling activation within the KICSTOR complex in response to cellular amino acid levels. We define the clinical spectrum and investigate the developmental pathogenesis of KRD. METHODS: We report the genotype and clinical phenotype of 71 KRD individuals (28 female subjects, ages 1 to 55 years) including 48 newly identified KRD individuals. The effects of Kptn knockout on brain development were assayed in vitro and in vivo. RESULTS: We defined 15 novel KPTN variants. Intellectual disability

Radiogenomics of congenital brain malformations: Linking embryology, genetics, and imaging.

PURPOSE: Congenital brain malformations are structural anomalies present at birth stemming from underlying genetic mutations or prenatal disruptions. The increased use of advanced genomic sequencing has led to major breakthroughs in pediatric neurogenetics; however, progress in unraveling the genetic basis of many central nervous system (CNS) malformations has lagged. This gap is partly due to the complexity of brain development and challenges like establishing the genetic culprit in somatic mosaicism. This review aims to integrate embryology, genetics, and neuroimaging to provide a practical

[Exploring the therapeutic potential of PPARα in schizophrenia].

Peroxisome proliferator-activated receptor α (PPARα) is a nuclear receptor that regulates lipid metabolism and inflammatory responses. Recent studies have revealed that PPARα also plays an important role in maintaining neuronal homeostasis through modulation of fatty-acid oxidation and inflammation in the brain. Our previous work using both animal models and human samples has demonstrated that PPARα is involved in the pathophysiology of schizophrenia. Ppara-deficient mice exhibit reduced prepulse inhibition, increased anxiety-like behavior, and decreased dendritic spine density with a reductio
