OBJECTIVES: Our aim is to define the microstructural organization of the corpus callosum (CC) in people with congenital mirror movements (CMM). We hypothesised that smaller fractional anisotropy (FA) values will be found in our CMM sample compared to the control group.
METHODS: Diffusion tractography (DTI) was applied to quantify CC microstructure in 46 neurologically-normal controls and four individuals with CMM characterised by pathogenic variants known to interact with netrin-1 during embryonic development of axonal pathfinding processes. FA was quantified across 80 nodes within each of 8

BACKGROUND: Lowe syndrome is an extremely rare X-linked genetic condition caused by disruptions in phosphatidylinositol metabolism, primarily impacting the eyes, brain, and kidneys. The condition is driven by over 200 identified mutations in the OCRL1 gene, which codes for an inositol polyphosphate 5-phosphatas. METHODS: This report details an Egyptian family seeking prenatal diagnosis after having a child with Lowe syndrome (LS). The affected child exhibited bilateral congenital cataracts, profound developmental delay, heart abnormalities, hypotonia, and proximal renal tubular dysfunction, an

Monoallelic pathogenic variants consistent with Mendelian inheritance patterns causing congenital heart disease (CHD) have been increasingly identified as genetic testing, including genome sequencing (GS), has become more widely available within the clinical space. Here, we focus on a newly described pathogenic variant in CDH2 resulting in Agenesis of Corpus Callosum (ACC), Cardiac, Ocular, and Genital Syndrome (ACOGS). While previous studies document variable expressivity of the CDH2 variants in unrelated individuals, no cases have described such variable expressivity in twins with the same C

Donnai-Barrow syndrome (DBS) arises from loss-of-function (LoF) variants in the endocytic receptor low-density lipoprotein receptor-related protein 2 (LRP2; or megalin) and is characterized by low-molecular weight proteinuria and developmental abnormalities. Urinary proteomics of 9 patients with DBS revealed that the urinary proteome of a DBS patient with the missense variant LRP2 p.C1400R was indistinguishable from that of patients with splice site, nonsense, or frameshift mutations. A CRISPR mouse model of the variant was generated to determine the mechanism of LoF and proteinuria. The mutan

Copy number variations (CNVs) at the proximal 16p11.2 BP4-BP5 locus are among the most well-characterized genomic regions associated with neurodevelopmental disorders. We aimed to define the clinical and molecular spectrum of these CNVs in a pediatric cohort. A total of 19 patients with pathogenic or likely pathogenic 16p11.2 BP4-BP5 CNVs were identified among 2200 individuals referred for chromosomal microarray analysis (prevalence: 1/116, 0.86%). Deletions were detected in 16/19 (84.2%) patients and duplications in 3/19 (15.8%). Developmental delay/intellectual disability was observed in 12/

The autophagy-tethering factor ectopic P-granule 5 autophagy protein (EPG5) plays a key role in autophagosome-lysosome fusion. Impaired autophagy associated with pathogenic variants in EPG5 causes a rare devastating multisystem disorder known as Vici syndrome, which features neurodevelopmental defects, severe progressive neurodegeneration and immunodeficiency. The pathophysiological mechanisms driving disease presentation and progression are only partially understood. In patient-derived fibroblasts and iPS cells differentiated to cortical neurons, we find that impaired mitophagy leads to mitoc

INTRODUCTION: Congenital fibrosis of the extraocular muscles type 3 (CFEOM3) is a congenital cranial dysinnervation disorder (CCDD) marked by variable ophthalmoplegia and ptosis with considerable phenotypic heterogeneity. We report a large multigenerational Iranian family with autosomal dominant CFEOM3.
METHODS: Affected individuals underwent a comprehensive ophthalmologic evaluation. Whole-exome sequencing (WES) in the proband, followed by Sanger sequencing in 10 affected and 4 unaffected relatives, identified a heterozygous missense variant, c.784C>T (p.Arg262Cys), in exon 4 of TUBB3 (16q24

PURPOSE: Congenital brain malformations are structural anomalies present at birth stemming from underlying genetic mutations or prenatal disruptions. The increased use of advanced genomic sequencing has led to major breakthroughs in pediatric neurogenetics; however, progress in unraveling the genetic basis of many central nervous system (CNS) malformations has lagged. This gap is partly due to the complexity of brain development and challenges like establishing the genetic culprit in somatic mosaicism. This review aims to integrate embryology, genetics, and neuroimaging to provide a practical

BACKGROUND: Sotos syndrome is a rare overgrowth-intellectual disability(OGID) disorder typically associated with developmental delay that stabilizes or improves with early intervention.
CASE PRESENTATION: We report a 17-month-old boy with classic clinical features of Sotos syndrome who presented with severe speech and motor delay. Whole-exome sequencing(WES) revealed a novel de novo nonsense pathogenic variant in NSD1:c.3910 C > T (p.Gln1304Ter), which is predicted to truncate multiple functional domains, including the SET(Su(var)3-9, Enhancer-of-zeste, and Trithorax) catalytic domain. Congenital facial palsy most commonly results from perinatal trauma. Developmental causes are rare and typically occur within syndromic constellations such as Möbius, Goldenhar, or CHARGE (coloboma, heart defects, atresia choanae, retarded growth, genital abnormalities, ear anomalies). Non-syndromic developmental facial palsy (DFP) is exceedingly uncommon and is thought to arise from disturbances in facial motor nucleus development, the intrapontine facial nerve tract, or its vascular supply. Chiari I malformation (CM-I) is characterized by ≥5 mm cerebellar tonsillar descent below the foramen

INTRODUCTION: Holoprosencephaly (HPE) is a rare congenital malformation of the forebrain caused by incomplete midline cleavage, often accompanied by craniofacial abnormalities. The condition arises from multifactorial etiologies, including genetic, environmental, and maternal factors. Early prenatal diagnosis is essential for parental counseling, management decisions, and detection of associated anomalies.
CASE REPORT: We report two cases of alobar HPE diagnosed in the third trimester by ultrasonography. The first case involved a 41-year-old primigravida at 32 weeks of gestation, with ultraso

Activating Signal Cointegrator 1 Complex, Subunit 3 (ASCC3) encodes a DNA helicase that is pivotal in transcriptional regulation, DNA damage response, and ribosomal quality control. Bi-allelic ASCC3 variants have been recently associated with a wide spectrum of non-lethal neuromuscular and neurodevelopmental disorders, with no consistent brain malformations or endocrine dysfunction. We describe a severe lethal form of ASCC3 disease: eight individuals of three consanguineous Bedouin families presented with a consistent phenotype of severe global developmental delay, axial hypotonia, and hypopla
