Pearls & Oy-sters: Hereditary Spastic Paraplegia Type 15 Presenting as Juvenile Onset Levodopa-Responsive Parkinsonism.

We report the case of a 27-year-old man with a history of speech delay and chronic, progressive movement disorder. He first developed gait difficulty at the age of 12. Given clinical signs of bradykinesia and resting tremor, he received a clinical diagnosis of childhood-onset parkinsonism. Treatment with oral levodopa initially improved symptoms, but after 2 years, he developed motor fluctuations and dyskinesias. Additional signs of spasticity and brain MRI showing a thin corpus callosum prompted genetic testing that identified a heterozygous pathogenic variant in the PRKN gene. However, he ex

SPG4 and Dementia: Expanding the Clinical Spectrum.

OBJECTIVE: Hereditary spastic paraplegia (HSP) is a group of disorders characterized by progressive spasticity and lower limb weakness, with mutations in SPG4/SPAST being the most common cause. Detailed studies and clinical and molecular comparisons across different populations are missing. We examined the clinical, pathological, and genetic spectrum of the SPG4/SPAST gene in patients with HSP. METHODS: The study involved 726 HSP patients recruited from Italy, Brazil, and Japan between 2001 and 2025, with analysis conducted in collaborative centers. SPG4/SPAST variants were identified using d

Neuroimaging features of incontinentia pigmenti: prompting genetic confirmation.

A term female neonate presented with vesicular skin lesions along Blaschko's lines and refractory seizures in the first week of life. Brain MRI revealed multifocal T2/fluid-attenuated inversion recovery hyperintensities and restricted diffusion in the periventricular and subcortical white matter, basal ganglia, thalami, corpus callosum and cerebellum, with microhaemorrhages on susceptibility-weighted imaging. These imaging findings, along with clinical history, suggested incontinentia pigmenti (IP), prompting genetic testing, which confirmed a heterozygous IKBKG exon 4-10 deletion. Seizures we

Adverse Pregnancy Outcomes and Prevalence Associated With Arboviral Infections (Zika, Dengue, and Chikungunya): An Umbrella Review of Systematic Reviews and Meta-Analyses.

Maternal infection with Zika virus (ZIKV), dengue virus (DENV), or chikungunya virus (CHIKV) may increase the risk of adverse pregnancy and foetal outcomes, but evidence credibility varies. The aim of this study is to evaluate and compare the strength of evidence from systematic reviews and meta-analyses on these arboviruses and pregnancy outcomes. PubMed, Web of Science, and Cochrane Library from inception to December 31, 2025 were searched. Two reviewers independently screened studies, extracted data, and assessed methodological quality using AMSTAR 2. Evidence credibility was graded as high

Fragile X-associated tremor/ataxia syndrome with autoimmune hepatitis requiring liver transplantation.

Fragile X-associated tremor/ataxia syndrome (FXTAS) is a neurodegenerative disorder that develops in some carriers of the FMR1 premutation. It starts with slight tremors or unsteadiness that progresses over time and includes cognitive decline. Here, we present the case of a man in his mid-60s with 88 CGG repeats in FMR1 who was diagnosed with FXTAS in his late 50s. In his early 60s, he was diagnosed with autoimmune hepatitis (AIH), confirmed by biopsy, and his liver function declined, requiring a transplant. Recovery was complicated by a bile duct leak, but he eventually stabilised. Neurologic

Lysosomal metabolism in pancreatic cancer and its crosstalk with host physiology.

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine

CHCHD2 as a prognostic biomarker and therapeutic target in lung adenocarcinoma: Insights from pan‑cancer multi‑omics and functional studies.

Helminths include both parasitic nematodes (roundworms) and platyhelminths (trematode and cestode flatworms) that are abundant, and are of clinical importance. The genetic characterization of parasitic flatworms using advanced molecular tools is central to the diagnosis and control of infections. Although the nuclear genome houses suitable genetic markers (e.g., in ribosomal (r) DNA) for species identification and molecular characterization, the mitochondrial (mt) genome consistently provides a rich source of novel markers for informative systematics and epidemiological studies. Extracorporeal blood filtration leading to tumor growth arrest and reduced analgesic requirements in Stage IV poorly differentiated pancreatic adenocarcinoma: A case report.

BACKGROUND: Despite significant strides in the management of metastatic solid tumors over the past few decades, metastatic disease remains a major clinical challenge, often leading to unfavorable patient outcomes. Circulating tumor cells (CTCs), which shed from the primary tumor, have the potential to disseminate and establish distant metastases, contributing to disease progression and reduced survival rates. Removal of CTCs via extracorporeal blood filtration could have significant therapeutic implications. CASE: A 51-year-old woman was diagnosed with metastatic poorly differentiated adenoca

Fetal MRI Findings, Etiology, and Outcome in Prenatally Diagnosed Schizencephaly.

BACKGROUND AND PURPOSE: Schizencephaly is a rare brain anomaly that is increasingly detected in utero. There are limited data on the etiology and outcomes in fetal schizencephaly to guide work-up and counseling. We aimed to determine the associated imaging findings, etiology, and outcomes in schizencephaly detected in utero. MATERIALS AND METHODS: This retrospective cohort study included 22 fetuses with a total of 34 schizencephaly defects identified by keyword search of fetal MRI reports from 1996 to 2022 followed by image review. Follow-up fetal and postnatal imaging, when available, was re

KIF21A-associated peripheral neuropathy defined by impaired binding with TUBB3.

KIF21A encodes a kinesin motor protein associated with isolated congenital fibrosis of the extraocular muscles (CFEOM), which occurs when the autoinhibitory interaction between its motor and third coiled-coil domains is disrupted. In this study, we describe a female child who is heterozygous for a novel de novo missense variant in KIF21A p.Leu664Pro, located in the second coiled-coil domain that was absent in her unaffected parents and in healthy population cohorts. She presented with progressive peripheral neuropathy, hypoplasia of the corpus callosum and strabismus in the absence of CFEOM. P

Fetal Corpus Callosum Anomalies: A Review of Underlying Genetic Disorders and Prenatal Testing Options.

Anomalies of the corpus callosum (CC) are amongst the most common fetal Central Nervous System (CNS) anomalies detectable on ultrasound. Underlying genetic disease plays an important part in defining prognosis. Associations with aneuploidy and submicroscopic chromosomal deletions or duplications have been well demonstrated using chromosomal microarray analysis. Next-generation sequencing techniques such as exome sequencing, have revolutionized the ability to detect monogenic disease in these fetuses. In the context of important recent publications on exome sequencing in prenatal populations, a

Mice with 16p11.2 Deletion and Duplication Show Alterations in Biological Processes Associated with White Matter.

Deletion and duplication in the human 16p11.2 chromosomal region are closely linked to neurodevelopmental disorders, specifically autism spectrum disorder. Data from neuroimaging studies suggest white matter microstructure aberrations across these conditions. In 16p11.2 deletion and duplication carriers, potential gene dosage effects may impact white matter organisation, contributing to phenotypes including impaired cognition. However, the biological mechanisms underlying this white matter pathology remain unclear. To bridge this knowledge gap, we utilised mouse models of 16p11.2 deletion and

Sequence variants in HECTD1 result in a variable neurodevelopmental disorder.

Dysregulation of genes encoding the homologous to E6AP C-terminus (HECT) E3 ubiquitin ligases has been linked to cancer and structural birth defects. One member of this family, the HECT-domain-containing protein 1 (HECTD1), mediates developmental pathways, including cell signaling, gene expression, and embryogenesis. Through GeneMatcher, we identified 14 unrelated individuals with 15 different variants in HECTD1 (10 missense, 3 frameshift, 1 nonsense, and 1 splicing variant) with neurodevelopmental disorders (NDDs), including autism, attention-deficit/hyperactivity disorder, and epilepsy. Expanded Clinical Phenotype and the Role of Untargeted Metabolomics Analysis in Confirming the Diagnosis of Sodium-Dependent Multivitamin Transporter Deficiency.

The sodium-dependent multivitamin transporter (SMVT) is a ubiquitously expressed sodium-solute symporter that transports pantothenic acid, biotin, and α-lipoic acid across the intestinal epithelia and blood-brain barrier. Severe biallelic loss-of-function variants in SLC5A6 (MIM #604024) lead to SMVT deficiency (SMVTD, MIM #618973), which classically presents with developmental delay, brain atrophy, epilepsy, sensorineural hearing loss, peripheral neuropathy, and gastrointestinal, cutaneous, and immunologic abnormalities. We describe a 25-year-old female with autism spectrum disorder (ASD), in

A Novel Missense Mutation in SLC12A6 Impairs Ion Transport Function of the Protein to Cause Agenesis of the Corpus Callosum With Peripheral Neuropathy.

Agenesis of the corpus callosum with peripheral neuropathy (ACCPN) is a rare autosomal recessive disorder characterized by malformation or absence of the corpus callosum, accompanied by progressive peripheral nerve degeneration. ACCPN is associated with mutations in the SLC12A6 gene, encoding the potassium-chloride cotransporter (also termed KCC3), which plays a crucial role in neuronal ion homeostasis. In this study, we report a novel homozygous missense variant (c.1634A>G, p.H371R) in SLC12A6, identified through exome sequencing in a male proband presenting with ACCPN symptoms, including dev

Fetal Callosal Anomalies: A Narrative Review and Practical Recommendations for Pediatric Neurologists.

Agenesis of the corpus callosum is a common indication for fetal neurology consultation, increasingly identified through advances in fetal sonography and fetal magnetic resonance imaging. Despite improvements in diagnostic accuracy, prognostic counseling is challenging due to highly variable neurodevelopmental outcomes. Several factors contribute to neurodevelopmental outcome variability, including associated anomalies and etiologic considerations such as genetic, acquired, and environmental factors. This narrative review discusses existing literature on prenatal findings, postnatal outcomes,

[A variant of p.Arg1623Gln of the DYNC1H1 gene in a patient with corpus callosum agenesis, polydactyly, mental development disorder, and neuromuscular system disorders].

DYNC1H1 encodes the heavy chain of dynein 1, a protein that plays a critical role in intracellular transport and is also involved in neurogenesis, bipolar spindle apparatus formation, and interaction with certain regulatory proteins. Many variants of the gene are described in various neuromuscular, psychoneurological, congenital abnormalities, and malignancies. In this clinical case, the correlation of clinical manifestations with molecular genetic changes of the DYNC1H1 gene was evaluated. A variant was identified and presented de novo in the linker domain of the protein in a patient with abn
