Alobar holoprosencephaly (HPE) is a severe malformation where the embryonic forebrain fails to divide, characterized by profound midline facial defects and high mortality. We report a case of a term neonate born to a grand multigravida mother from a rural area with no prior antenatal screening. At birth, the neonate presented with severe midline craniofacial anomalies, including arrhinia and anophthalmia. Transcranial ultrasonography demonstrated findings highly suggestive of alobar HPE, including a single monoventricle and absence of key midline structures, while additional findings included

BACKGROUND: Holoprosencephaly (HPE) is a rare congenital condition encompassing incomplete midline cleavage of the prosencephalon and associated craniofacial abnormalities.
CASE PRESENTATION: We report a rare case of a 34-year-old Memon woman, gravida 3 para 2, with no known comorbidities or exposure to teratogens, who presented for her 12-week scan which revealed a single alive fetus having an absent falx cerebri with fused thalami, a single large posterior ventricle, and facial anomalies suggestive of alobar HPE. A follow-up ultrasound showed abnormally fused portions of the brain and a hig

We present a case of a 30-year-old G3P2L1 with a spontaneously conceived monochorionic triamniotic (MCTA) triplet pregnancy complicated by a Twin Reversed Arterial Perfusion (TRAP) sequence in one fetus. Early ultrasound at 10 weeks 4 days showed MCTA triplets with one hydropic fetus with alobar holoprosencephaly. A review scan at 12 weeks 6 days gestation showed that this fetus with holoprosencephaly had no heart. Colour Doppler showed a single artery in the acardiac fetus, suggestive of twin reverse arterial perfusion (TRAP sequence). Ultrasound-guided radiofrequency ablation (RFA) was succe

Defects in ZIC2, a member of the Zinc Finger of the Cerebellum family of transcription factors (TFs), cause holoprosencephaly, a congenital brain malformation characterized by the defective cleavage of cerebral hemispheres. However, the gene regulatory network (GRN) controlled by ZIC2 during neural development remains largely unexplored. Here, we combined a mouse embryonic stem cell (mESC) in vitro differentiation model toward anterior neural progenitors with genome editing and genomic methods to elucidate the ZIC2 GRN. We found that ZIC2 is dispensable in mESC due to compensation by ZIC3. In

FOXA2 (hepatocyte nuclear factor-3β, HNF-3β) encodes a transcriptional activator involved in early embryogenesis, particularly in the patterning and differentiation of midline structures such as the neural tube, foregut, and pituitary gland. Its role in human pathogenesis was first suspected when patients with deletion of chromosome 20p11.2 which encompassed the FOXA2 gene were reported. With improvement in molecular diagnosis, patients with variants in FOXA2 genes have more recently been described with pituitary dysfunction, hypoglycemia, craniofacial, and/or endoderm-derived organ malformati

Holoprosencephaly (HPE) is a rare congenital brain malformation resulting from incomplete separation of the cerebral hemispheres. It has an estimated prevalence of 1 in 10,000 live births. We report a case involving an 18-year-old primigravida at 28 weeks of gestation. Prenatal ultrasound revealed polyhydramnios, marked hydrocephalus, and absence of the cerebral and cerebellar hemispheres, with preservation of the brainstem. Cesarean delivery was performed, and the female neonate exhibited hypotelorism, arhinia, cleft lip and palate, and a single frontal bony plate (neonatal weight, head circu

First-trimester ultrasound has evolved beyond basic screening to permit structured neurosonographic assessment of the developing fetal brain. Historically, detailed evaluation of intracranial anatomy was deferred to the mid-trimester, based on assumptions regarding developmental maturity and technical limitations. However, advances in imaging technology, improved understanding of early neuroembryology, and refinement of standardized scanning planes have shifted this paradigm. Major structural anomalies that disrupt early cranial morphology, including acrania-exencephaly sequence, holoprosencep

OBJECTIVES: To establish gestational age-specific reference ranges for two new indices, the forehead-anterior cerebral artery angle (FACAA) and forehead-anterior cerebral artery distance (FACAD), and to objectively evaluate their diagnostic effectiveness, measured using Doppler technology with microvascular flow imaging, for identifying holoprosencephaly, even the semilobar and lobar types, in early pregnancy.
METHODS: We evaluated FACAA and FACAD in 462 normal fetuses between 12 and 34 gestational weeks (GW) to generate normative reference ranges. Additionally, we evaluated these two indices

BACKGROUND: Median (Tessier 0) cleft lip includes phenotypes ranging from preserved midline dysraphia to hypoplastic midline deficiency associated with primary palate agenesis. Despite these differences, management is often based on principles used for conventional cleft lip repair. The longitudinal effects of reconstruction in hypoplastic variants remain poorly defined.
METHODS: This retrospective descriptive case series included 13 patients with median upper lip cleft treated at a single craniofacial center. Patients were grouped into dysraphic and hypoplastic phenotypes. Clinical presentat

INTRODUCTION: Holoprosencephaly (HPE) is a complex spectrum of cephalic malformations resulting from incomplete prosencephalic division. Semilobar HPE is an intermediate variant, often associated with significant craniofacial dysmorphism and neurological morbidity.
CASE PRESENTATION: We present a preterm female infant born to a mother with unmanaged gestational diabetes. At birth, she exhibited microcephaly, total arhinia, and a median cleft lip and palate. MRI confirmed semilobar HPE. Clinical progression included hypotonia, seizures, and recurrent hyperpyrexia associated with hypothalamic d

Middle interhemispheric variant (MIH) of holoprosencephaly (HPE), also known as syntelencephaly, is a rare subtype of HPE characterized by abnormal midline connection of the posterior parts of the frontal lobes and the anterior parts of the parietal lobes with variable corpus callosum abnormalities. We report the case of a five-year-old girl with syntelencephaly presenting with a cleft lip and palate, developmental delay, cerebral palsy, and intermittent diabetes insipidus. Brain magnetic resonance imaging demonstrated the characteristic features of both syntelencephaly and septo-optic dysplas

BACKGROUND AND AIMS: Phloroglucinol is a spasmolytic drug widely prescribed in France for gastrointestinal or pelvic pain, and frequently used during pregnancy despite limited evidence regarding its safety. We aimed to evaluate the association between first-trimester phloroglucinol exposure and the risk of major congenital malformations (MCMs) in a nationwide cohort.
METHODS: Using data from the French EPI-MERES cohort based on the national health data system (SNDS), we identified all pregnancies ending after 22 weeks between 2010 and 2022, excluding women with phloroglucinol use in the year

OBJECTIVES: More than 20% of rheumatoid arthritis (RA) patients have comorbid fibromyalgia (FM+), which may elevate DAS28-ESR (disease activity score 28-erythrocyte sedimentation rate) and other indices, resulting in challenges to assess inflammatory disease activity. Although several reports indicate that elevated patient global assessment (PATGL) may elevate DAS28 in the absence of inflammatory activity, less information is available concerning the other three components, tender joint count (TJC), swollen joint count (SJC), and erythrocyte sedimentation rate (ESR), to possibly elevate DAS28

Atomoxetine (Strattera(R)) is a selective norepinephrine (noradrenaline) reuptake inhibitor that is not classified as a stimulant, and is indicated for use in patients with attention-deficit hyperactivity disorder (ADHD). Atomoxetine is effective and generally well tolerated. It is significantly more effective than placebo and standard current therapy and does not differ significantly from or is noninferior to immediate-release methylphenidate; however, it is significantly less effective than the extended-release methylphenidate formulation OROS(R) methylphenidate (hereafter referred to as osm

BACKGROUND: Patients with eating disorders (ED) often suffer from compulsive exercise behavior, which is associated with lower short-term response to treatment and poorer long-term outcome. Evidence-based interventions specifically targeting compulsive exercise behavior have been scarce so far. We developed a manualized group therapeutic approach integrating cognitive-behavioral therapy, exercise therapy and exposure with response management to promote healthy exercise behavior. Our objective was to examine the feasibility and acceptance of this new approach as add-on to regular inpatient trea

Transcription factors from the family of Signal Transducers and Activators of Transcription (STAT) are activated by numerous cytokines. Two members of this family, STAT5A and STAT5B (collectively called STAT5), have gained prominence in that they are activated by a wide variety of cytokines such as interleukins, erythropoietin, growth hormone, and prolactin. Furthermore, constitutive STAT5 activation is observed in the majority of leukemias and many solid tumors. Inactivation studies in mice as well as human mutations have provided insight into many of STAT5's functions. Disruption of cytokine
