13q Deletion Syndrome Presenting with Lymphopenia Detected Through Newborn Screening for Primary Immunodeficiencies.

The expanded newborn screening (NBS) program in the Russian Federation, launched in 2023, includes the detection of severe forms of T- and B-cell immunodeficiencies via TREC/KREC quantification. We report a rare case of a male infant having multiple congenital anomalies and lymphopenia identified through this program. Genetic testing revealed a 25.8 Mb terminal deletion spanning 13q31.2-qter, consistent with 13q deletion syndrome. Initial NBS revealed reduced TREC levels, prompting further evaluation. The patient exhibited a complex phenotype, including central nervous system malformation (alo

Recent advances in the diagnosis and molecular pathogenesis of holoprosencephaly: a review.

Holoprosencephaly (HPE) is the most common structural anomaly of developing forebrain, characterized by incomplete separation of the cerebral hemispheres. While mutations in the Sonic Hedgehog (SHH) signaling pathway remain the most established genetic cause, recent studies have identified an expanding list of genes and molecular networks involved in the pathogenesis of HPE. These include modulators of the NODAL, NOTCH, WNT/PCP, FGF, and RAS/ERK1/2 pathways as well as components of ciliary structures and cohesin complexes. Incomplete penetrance, broad phenotypic heterogeneity, and gene-environ

Ophthalmologic Findings in an Induced Model of Holoprosencephaly in Zebrafish.

Holoprosencephaly (HPE) is the most frequent developmental disorder of the forebrain. In this condition, the separation of the early precursor domains is hampered. A spectrum of clinical manifestations can be observed, for example, severe forms like alobar HPE and less severe forms like lobar HPE. Ophthalmologic findings that accompany HPE also occur as a spectrum that ranges from ocular hypotelorism and synophthalmia to cyclopia and anophthalmia. In this brief analysis, we made use of a recently established zebrafish model of HPE. This model is based on experimental BMP ligand induction that

Congenital Syngnathia With Holoprosencephaly: A Case Report of a Fatal Presentation in a Resource-Limited Setting.

Congenital syngnathia, the rare fusion of the maxilla and mandible, poses significant feeding and respiratory challenges. Its management is complicated by rarity and potential syndromic associations. We present a 1-week-old female neonate who presented with left jaw deviation, respiratory distress (SpO2 69%, tachypnea), and fever. Examination revealed near-complete inability to open the mouth except for a small right paramedical gap. A CT scan confirmed left maxilla-mandibular bony fusion (Laster type 2b, Dawson type 2b), mandibular hypoplasia, semilobar holoprosencephaly, corpus callosal dysg

Congenital Anomalies in a Neonate With Partial Monosomy of Chromosome 21 q Arm: A Case Report.

Congenital anomalies are a major cause of infant morbidity and mortality. Whole-genome sequencing provides a potential tool for solving diagnostic dilemmas in such instances. We report an interesting case of a neonate from India with holoprosencephaly and multiple associated anomalies, including dysmorphic features and ventricular septal defect. Karyotype analysis revealed mitotic instability. Both ring chromosome 21 and deletions in the q arm of chromosome 21 were observed in the metaphase spreads. To characterize the genomic abnormality more precisely, whole genome sequencing was performed,

Prenatal imaging diagnosis of iniencephaly apertus associated with heterotaxy syndrome, alobar holoprosencephaly and myelomeningocele: a case report.

Iniencephaly is a predominantly lethal and rare form of neural tube defect characterized by fixed hyperextension of the head, occipital bone abnormalities, and cervical dysraphism. Its estimated incidence ranges from 0.1 to 10 per 10,000 births and is frequently associated with both neurological and non-neurological developmental anomalies. Here, we present the first documented case of iniencephaly apertus in conjunction with heterotaxy syndrome (HS), alobar holoprosencephaly (HPE) and myelomeningocele. Our report describes a 19-year-old primigravida with no significant medical history, whose

Congenital external hydrocephalus: A rare presentation of lobar holoprosencephaly in a neonate.

External hydrocephalus is a rare condition characterized by abnormal cerebrospinal fluid (CSF) accumulation in the subarachnoid space, often associated with developmental anomalies. Lobar holoprosencephaly, a milder form of holoprosencephaly, can manifest with hydrocephalus due to its impact on brain development and CSF dynamics. This case report describes a neonate with congenital external hydrocephalus secondary to lobar holoprosencephaly, highlighting the diagnostic imaging findings and management approach. A neonate presented with progressive macrocephaly, irritability, altered sensorium,

Malformation Pattern and Molecular Findings in the FGFR1-Related Hartsfield Syndrome Phenotype.

BACKGROUND/OBJECTIVES: The Fibroblast Growth Factor Receptor 1 (FGFR1, MIM*136350) is a protein member of the fibroblast growth factor receptor (FGFR) family, with various biological functions, such as the normal development control. It contains an extracellular site for the ligand (three Ig-like domains, IgI, IgII, IgIII), a single transmembrane and a cytoplasmic protein tyrosine kinase (TK) domain. Variants in this gene have been associated with a wide spectrum of genetic disorders, including the clinical entity known as FGFR1-related Hartsfield or Hartsfield syndrome (HRTFDS, MIM#615465), w

Centriolar protein PIBF1 is required for craniofacial and forebrain development.

Primary cilia are microtubule based extensions on the surface of most cells that play a crucial role in cellular signaling during development, tissue homeostasis, and organ function. Defective cilia result in a wide variety of clinical manifestations affecting multiple organ systems, collectively termed ciliopathies. Ciliopathies are rare, exhibit tremendous genetic diversity and an overlap of clinical features, making diagnosis and treatment challenging. Identifying and characterizing novel ciliary variants is critical to gain an improved understanding of ciliopathic pathologies. To address t

Prenatal Diagnosis of Hartsfield Syndrome in the Fetus With Isolated Ectrodactyly Caused by a Novel Variant in FGFR1.

Molecular genetic testing was performed on a fetus with ectrodactyly of the right foot to clarify the pathogenic cause and provide evidence for prenatal counseling. Trio whole-exome sequencing (trio-WES) was performed on the fetus and his parents to identify the underlying genetic cause. Candidate variants were validated by Sanger sequencing, and their molecular effects were analyzed through minigene assays. Trio-WES identified a novel heterozygous variant (c.1977+1G>C) in FGFR1, which is consistent with FGFR1-related Hartsfield syndrome (HS; OMIM#615465). Sanger sequencing confirmed that this

Novel variants in STAG2 and PKD1 associate with multiple congenital malformations and autosomal dominant polycystic kidney disease in a Chinese family: A case report and literature review.

Cohesinopathies are rare multisystem disorders caused by defects in the cohesin complex, which is critical for chromosome segregation, DNA repair, replication, heterochromatin formation and gene transcription regulation. Stromal antigen 2 (STAG2), a key cohesin component, is linked to neurodevelopmental disorders such as X-linked holoprosencephaly 13 and Mullegama-Klein-Martinez syndrome (MKMS). Polycystic kidney disease (PKD), particularly autosomal dominant PKD (ADPKD), is characterized by renal cysts and is commonly associated with variants in the PKD1 gene. In the present study, a Chinese

Origin stories of neural crest roles in craniofacial development: A tale of the meninges.

As developmental biologists, we tell origin stories to explain differences in vertebrate craniofacial form and complexity. Some of these differences are linked to diseases, notably of patients born with congenital malformations, many of them syndromic. These defective processes impede proper neural crest cell migration, differentiation, and patterning of bones, cartilage, joints, muscle and other connective tissues, including of the head. For example, a rare form of holoprosencephaly called cyclopia is a particular quandary in its diverse presentation. This syndromic congenital disease include

Digital Technologies in Diagnosing Solitary Median Maxillary Central Incisor Syndrome.

Solitary Median Maxillary Central Incisor Syndrome is unique congenital developmental defect affecting midline structures of the head and the body. The prevalent symptom is a solitary median incisor of the maxilla in primary and secondary dentition, which is positioned exactly in the midline of the alveolus. Other abnormalities that are characteristic of the syndrome include holoprosencephaly, nasal cavity anomalies, cleft palate-lip, hypotelorism, and microcephaly. It is estimated to occur in 1:50,000 live births, with female gender predilection. The cause of the syndrome is related to midlin

Loss of Asxl1 disrupts telencephalic midline integrity through dysregulation of SIX3 target genes.

Mutations in the epigenetic regulator Additional sex combs like 1 (Asxl1) have been implicated in neurodevelopmental syndromes; however, its role in embryonic brain development remains poorly understood. Here, we report that Asxl1 knockout mice exhibit severe telencephalic midline defects, including agenesis of the corpus callosum, absence of the septum, and formation of a single cerebral ventricle. These phenotypes closely resemble those of Six3-deficient brains, suggesting a functional link between Asxl1 and Six3. Co-immunoprecipitation and domain mapping revealed that ASXL1 directly interac
